Envelhecimento ativo e a indústria da perfeição
In the last two decades, Active Ageing acquired significant importance in scientific and political forums as a new paradigm of public policies to meet challenges posed by the ageing population. Defined by the WHO as to emphasize society's responsibility regarding quality of life as people age, the Active Ageing paradigm represents, nowadays, a progressive narrowing of action scope and accountability. In recent years, assuming the logic of individual accountability, it serves as basis for the accusation of individual negligence in not adopting a healthy lifestyle. This study analyses the websites of several Portuguese medical clinics specialized in anti-ageing medicine, where one can find advertisements of the services provided, and, in some cases, opinion articles drawn up by doctors of this medical specialty, for a lay audience. Considering the increasing international dimension of the aesthetic and hormonal enhancement biotechnologies market, the ideal of Active ageing can, today, be at the service of the culture of perfection. ; info:eu-repo/semantics/publishedVersion

Nas últimas duas décadas, o envelhecimento ativo adquiriu expressão relevante enquanto paradigma de políticas públicas para fazer face aos desafios colocados pelo envelhecimento demográfico. Definido, primeiramente pela Organização Mundial da Saúde (OMS), de forma a enfatizar a responsabilidade da sociedade no seu conjunto relativamente à qualidade de vida ao longo do processo de envelhecimento, o paradigma do envelhecimento ativo, atualmente, tem vindo a concentrar-se, sobretudo, no adiamento da idade da reforma e na redução dos custos com a saúde nos últimos anos de vida, a partir de uma lógica de responsabilização individual. Para a elaboração deste artigo, foram consultados vários sítios on-line de clínicas médicas portuguesas nas quais se praticam consultas/tratamentos de medicina antienvelhecimento, analisando os anúncios publicitários dos serviços prestados, bem como artigos de opinião redigidos pelos próprios médicos dessa especialidade, dirigidos a um público leigo. Conclui-se que, a par da responsabilização individual pela saúde e, de forma mais geral, pela forma como se envelhece, assistimos hoje à expansão de um amplo mercado de produtos e serviços antienvelhecimento que nos permitem argumentar que o ideal de envelhecimento ativo poderá estar ao serviço de uma indústria da perfeição. ; In the last two decades, Active Ageing acquired significant importance in scientific and political forums as a new paradigm of public policies to meet challenges posed by the ageing population. Prevention of falls in the elderly in the home: promoting active ageing

Objective: To determine the frequency of falls and identify risk factors in the homes of the elderly under the Home Care Service of a village in Alentejo (Portugal). Method: Exploratory, descriptive study. The target group were elderly persons under the Home Care Service (23). The questionnaire consists of open and closed questions, and was based on the Jefferson Area Board for Aging Safety in the Home Assessment; Instrument to Assess the Risk of Falls and Adaptations to Prevent Falls at Home. Results: Of the 23 seniors, 13 were men; the mean age was 85; 10 widowers; 11 live alone; 12 cannot read or write; 17 have experienced falls, loss of balance being the main cause. They report changes in vision (21), hearing (14) and rheumatic diseases (14); hypertension (19); they use 4 or more drugs on a daily basis (16). Conclusion: The physiological changes associated with ageing may increase the risk of falls. Due to the consequences, it is a priority field in community intervention.
The active ageing as life experience: narratives of elderly people

Objective: Analyze the experience of active aging in the life's history of old people in the city of Franca (SP). Method: Study bibliographical, documentary and field research. Use the qualitative approach, we have opted for the oral history method, because it searches to explain the meanings of daily has grown significantly through the stories of lives, gathering information through interview. Results: The theoretical-methodology referential has found in following categories: 1) identity in old age: person, sociability and labor, 2) identity and active ageing. Discussion: The analyses of the data comprised three distinct phases: organizing and structuring of data, classification and interpretation of data. Conclusion: The challenge of the XXI century is to ensure the active and dignified ageing of the population, it seeks a society for all ages, so that older people have a participation in social life, to fight the determinants that compromise the active aging.
Health promotion in groups of elderly: reflections for active ageing

Objectives: To analyze the perceptions of the elderly on health promotion in favor of an active aging; reflect on viable health promotion among elderly groups aiming to reach active aging strategies. Method: A descriptive, exploratory study with a qualitative approach, conducted with 45 subjects belonging to a group of conviviality. The project was approved by Brazil under the Platform CAAE 07243812.6.0000.5187. Results: Most elderly people belonging to this group had access to information about their pattern of aging. When inquiring about health promotion and active aging, possible to list out two thematic categories: I) Perceptions of the elderly on health and II) Perceptions of people on strategies for health promotion and empowerment of active aging. Conclusion: Spaces of experiences and elderly group allow the preparation of proposals for promoting the health and empowerment of active aging.

A Critical Approach to the Demographics of Ageing: The Case of Oslo ; Uma abordagem crítica à demografia do envelhecimento: o caso de Oslo
Demographic analysis is on the basis of every city plan. It is often regarded as a technical and indisputable tool to inform policy making. In a strongly contested political environment, information on population is crucial to set priorities and define goals. However, the frequently miss interpretation, or over interpretation, of demographic information, might also support inadequate public policy design. The increased interest on ageing in the urban context has already resulted in various initiatives all-over the world. The idea of an unsustainable "elderly boom" is a powerful narrative that is frequently used to argue for the need of improved policies for older persons. The article argues that city planners and urbanists inadequate use of demographic assessment of ageing might generate policies that can ultimately work against the interests of the older persons. I use the city of Oslo as an example of the varieties of approaches that can be used to assess ageing on a specific population. Contrary to the general assumption that Oslo will face an "elderly boom", the study shows that the city's ageing process today is less intense than the observed in the past. A particular attention is paid to the varieties of demographic trends observed within Oslo's different scales.

La creciente conciencia de la dimensión de los desafíos que representa para las sociedades actuales el envejecimiento de la población, se traduce en iniciativas políticas para encontrar soluciones integrales a este complejo problema. El artículo analiza algunas de estas iniciativas y sugiere áreas de oportunidad donde la investigación en enfermería y debe movilizarse para formar parte integral de las iniciativas y programas que se pondrán en marcha. ; The growing awareness of the challenge that aging represents for modern societies translates into political initiatives aimed to find comprehensive solutions for this complex issue. This paper analyses some of these initiatives and suggests areas of opportunity where nursing research must be mobilized to take integral part of the initiatives and programmes that are to be implemented.

Geriatrics, a centenarian medical specialty [Abstract in English] ; Geriatria, uma especialidade centenária
Aims: To commemorate the centennial of the geriatrics, the authors present a review of the population aging, the elderly individual, the characteristics of the geriatric medicine, and its history. Source of data: A throughout review of the literature found on MedLine and LILACS, as well as textbooks, dissertations, thesis, and government directives was performed. Summary of the findings: Geriatrics is the area of medicine that cares for the health and treatment of diseases in old age, which works with physical, mental, functional and social aspects in the acute, chronic, rehabilitative, preventive and palliative care of the elderly. Geriatrics goes beyond "medicine centered on organs and systems", offering holistic treatment by interdisciplinary teams with the principal objective of optimizing functional capacity and improvement of quality of life and autonomy of the elderly, which make up the core of this medical specialty. Geriatrics, in daily assistance practice, works with particular questions such as: the heterogeneity of patients and of scenarios of care; concomitance of multiple diseases/disorders (plurimorbidity), including geriatric syndromes ("the giants of geriatrics"); difficulty of identifying all the problems (iceberg phenomenon); polypharmacy; fragility; vulnerability; the various losses that the elderly show; and terminality. Conclusions: Geriatrics refers to the medical specialty responsible for the clinical aspects of aging and for the various forms of health care necessary for the elderly. Because of the complexity allied to the aging process, geriatrics has become an intriguing, challenging and contemporaneous specialty. ; Objetivos: para comemorar o centenário da Geriatria, os autores apresentam uma revisão sobre o fenômeno do envelhecimento, o indivíduo idoso, as características da medicina geriátrica e um resumo da sua história. International audience ; An ageing population is a challenge for every country as the " demographic transition " is about to be an universal phenomenon. In this paper are first analyzed the two main dimensions of this dynamic. The first one is the demographic transition as such. Traditionally stages are defined that are placed in a chronological way. Each country being at different stage. The second dimension is the financing one, as it is necessary to allocate resources in a more efficient way that market forces would do. In a second part, the controversial impact on health care cost will be discussed. It will be demonstrated that the " last wagon " syndrome is nowadays proven. As a result, the rise of the life expectancy will not be such a burden for health insurance, whatever the models (Beveridge or Bismarck) are. To go further in the analysis of policies oriented toward caring elderly, some hints of European policy experience will be given. Beyond the European model ideology, studies show a double divide between South and North and between old and new European Union members. To conclude, the Brazilian situation will be shortly described in the light of the on going dynamic. ; El envejecimiento de la población es un reto para todos los países desde el momento en que "transición demográfica" está a punto de ser un fenómeno universal.
