학위논문 (석사) -- 서울대학교 대학원 : 사회과학대학 지리학과, 2020. 8. 신혜란. ; 본 연구는 정책이동(policy mobility)의 관점에서 서울로 7017의 조성 과정을 분석하였다. 특히 본 연구는 정책이 이동하는 과정에 있어서 도시 정치가 어떠한 방식으로 개입하게 되는지를 집중적으로 살펴보았다. 정책이동은 도시 연구 분야에서 주요 연구 주제로 떠오르고 있다. 전 세계 도시 간 협력과 교류가 활발해짐에 따라 정책의 이동 - 선진 정책의 벤치마킹 및 수입과 모방, 정책 관련 국제 컨퍼런스 발표 및 수상 등 - 은 활발하게 진행되고 있고, 이러한 현상은 도시정책의 추진 및 의사결정에 있어서도 직접적인 영향을 미치고 있다. 한편 기존 정책이동 관련 연구들은 정책이 이동되는 과정자체에 초점을 맞추어 도시들 간의 지역적 맥락이나 정치구조의 차이가 정책의 적용과 변용 등에 어떠한 영향을 미치는지를 주로 살펴보았다. 이에 반해 정책 이동 현상이 넓은 범위의 도시정치와 거버넌스에 어떠한 영향을 미치는지를 활발하게 연구되지는 못하였다. 본 연구는 이러한 부분에 있어서 기여하고자 서울로 7017이 조성되는 과정에서 나타났던 벤치마킹이나 정책 홍보 등의 정책이동 과정이 서울시 도시정책에 어떠한 영향을 미쳤는지를 분석하였다.연구의 결과는 다음과 같다. 첫째, 서울로 7017 조성과정에서 나타난 정책의 이동 – 뉴욕 하이라인의 벤치마킹 또한 서울로 7017 정책의 홍보와 선전 - 은 서울시가 도시정책을 정당화하는 주요한 방법으로서 활용되었다. 서울로 7017은 별다른 공론화 과정 없이 박원순 시장의 정치적 추진력을 통해 급하게 추진되었기 때문에 서울시는 사업에 대한 정당성을 보여주는 것이 필요했다. 이에 서울시는 이미 진행되었던 뉴욕 하이라인의 성공을 통해서 사업의 필요성과 당위성을 보여주었다. 또한 서울시는 사업의 가치와 성공을 국내와 해외로 홍보하는 과정을 통해서, 서울로 7017 대한 성공적인 이미지를 스스로 다듬어갔다. 이처럼 서울로 7017이 조성되는 과정에서 나타났던 벤치마킹과 정책 홍보는 서울시가 도시 정책을 정당화하기 위한 목적에서 진행되었다. 둘째, 서울로 7017의 조성에 있어서 서울시는 하이라인을 벤치마킹 하였지만, 이는 서울시의 정책적 목표와 필요에 따라 선별적으로 이루어졌다. 또한 이처럼 정책적 필요에 맞추어 벤치마킹과 모방이 진행되는 과정에서, 처음 모방하고자 하였던 하이라인의 운영방식과 내용은 큰 폭으로 변용되었다. 일례로 하이라인의 디자인은 유사한 형태로 모방되었지만, 시민참여를 대표하였던 하이라인의 운영방식은 보수적인 서울시의 행정 구조에 맞추는 과정에서 큰 폭으로 변용되었다. 또한 이와 같은 모방과 변용의 과정에서 서울시의 정책, 행정구조, 공간은 빠르게 변화되었다. 본 연구는 전세계적으로 활발하게 진행되고 있는 정책이동 현상이 도시를 어떻게 변화시키고 있는지를 서울로 7017의 실증사레를 통해서 확인하였다. 한편 본 연구는 정책이동을 통해서 이루어지고 있는 다양한 변화들을, 다소 개괄적인 범위에서 살펴보았다는 점에서 한계를 갖기도 한다. 발전된 후속 연구를 통해서 정책이동이 도시의 변화에 갖는 의미를 더욱 면밀하게 검토하고 구체적인 정책적 시사점이 제시할 수 있기를 기대한다. ; This study analyzes the development process of Seoullo 7017 through the theoretical lens of policy mobility. In the past few decades, transfers, diffusion, and movement of urban policies across national borders have increased rapidly. Accordingly, researches on policy mobility have developed in the fields of geography and urban studies. These studies have critically investigated the processes of adaptation, assemblage, and mutation of a certain policy, but have little focused on how the ordinary urban policymaking and the urban area has changed in the process of policy movement. In this regard, this study explores how the mobilization of certain policy has changed and reconstructed the urban policy and planning system. Especially, this research has focused on the interaction of local decision-making and policy mutation processes. The results of the study are as follows. First, the mobilization of policy that appeared in Seoullo 7017 including benchmarking and policy promotion was strategically used by the Seoul city government. The main purpose of these mobilizations was to legitimize the city's urban policy. Seoullo 7017, which was a mayor's landmark project, has proceeded rapidly without the civic consensus process. In this regard, the New York High Line's success was used as a tool to represent the legitimacy of the Seoullo 7017 project. Also, the branding and promotion of Seoullo 7017, which received great attention from the city government, was used as a self-curation tool to make the success story of Seoul's urban policy. Second, the mobilization and adaptation of the High Line were proceeded selectively according to specific political goals of the Seoul city government. Seoullo 7017 benchmarked both the design and governance of the High Line. Lots of critics pointed out that the design should be differentiated, as the regional context of Seoul and New York was different, However, the design was similarly imitated into Seoullo 7017 because of the symbolic image of the High Line's design itself. Also, the management style which emphasized citizen participation and governance were also imitated, but it ends with instrumental form due to bureaucratic political structure. As such, adaptation, mobilization, and mutation has been carried out in accordance with the process of legitimizing local policy and political authority. The importance of this research is that it investigates the consequences and implications of mobile urban policies through an empirical case at various points. However, it also has its limitations as this study looks into various aspects of mobile policies on a superficial level. 본 논문은 대외정책 분야에서 드러나는 미국 대통령제의 운용원칙인 권력분립을 미국식 민주주의의 사각지대로 접근하는 시각을 제시하여 선행연구와 차별성을 추구한다. 구체적으로 헌법이 천명하는 권력분립 원칙에도 불구하고 오히려 그 권력분립의 기제인 상호간 독자적 선거제에 의해 미국의 유권자 전체가 선출한 대통령은 각 지역구에서 선출된 입법부 구성원이 내세우는 집합적 통치 위임과 확연하게 구별되는 유일한 공공리더십(public leadership)을 정책결정과정에서 독점할 수 있다는 점을 주시하고자 한다. 결국 미국 대통령제의 과제는 미국식 민주주의의 주요 통치원칙인 권력분립을 중시하는 동시에 성공적 통치를 위해 권한공유의 묘(妙)를 찾는 부단한 노력을 경주해야만 가능하다는데 있다. 특히 국내정책 분야보다 대외정책 분야에서 이 난제의 해법을 모색하는데 있어서 법제도적 및 정치적 장애물이 가로막혀 있다. 게다가 선거결과와 무관하게 통치 위임을 생성하는 정치의 사법화가 입헌 대통령제의 근간을 흔들 수 있는 위험성을 목격했다. 이에 본 논문은 대외정책 분야의 미국 대통령제에 대한 평가를 권력분립의 양면성, 즉 권력분립은 권력공유를 전제로 한다는 패러독스에 중점을 두고 대통령의 우위에 대한 주장을 가능하게 하는 헌법적 및 정치적 근거를 대조하면서 미국식 민주주의의 사각지대를 재조명하고자 한다. ; In this paper, I argue that the principle of separation of power in foreign policy under presidentialism invites a blind spot in American democracy. Although the Constitution specifies separation of power both in the conduct of domestic and foreign affairs, independent elections of President and legislators differentiate the kind of collective mandates on Capitol Hill from the unitary mandate bestowed upon President, thus prompting him to proclaim public leadership. The key to a successful governing, then, lies in the art of pursuing "separated institutions sharing powers" persistently. However, the realm of foreign affairs poses a more challenging task, both constitutionally and politically, than its counterpart in domestic policy. Furthermore, we came to witness a modern presidency whose relentless claim on mandate, or what is dubbed as judicialization of politics, was found irrelevant to the electoral outcome itself, thus creating a peril in constitutional presidency. 학위논문(석사)--서울대학교 대학원 :행정대학원 행정학과(정책학전공),2019. 8. 김순은. ; 국문초록 박근혜 전 대통령은 2013년 10월 18일 서울 신라호텔에서 열린 '유라시아 시대의 국제협력' 컨퍼런스 개막식 기조연설에서 다음과 같이 말했다. "내외 귀빈 여러분, 유라시아 대륙은 세계인구의 약 71%가 살고 있고, 동쪽 끝에서 서쪽 끝까지 12개의 시간대에 걸쳐 있는 세계 최대의 단일 대륙입니다. (중략)···저는 이제 유라시아에 새로운 소통의 길을 열어 협력의 잠재력을 끌어내야 할 때라고 생각합니다. 단절과 고립, 긴장과 분쟁을 극복하고 소통과 개방으로 평화롭게 교류하며 함께 번영하는 유라시아를 건설해야 합니다. 이를 위해 저는 유라시아를 '하나의 대륙', '창조의 대륙', '평화의 대륙' 으로 만들어 가는 몇 가지 방향을 제안하고자 합니다." 라고 하며 '유라시아 이니셔티브' 를 주창하였다. 이처럼 유라시아 이니셔티브는 유라시아 국가들 간 물류, 시장, 문화 등을 통합해 나가는 것을 주요 내용으로 하였으며 핵심은 물류 및 교통의 통합을 통해 유라시아 대륙의 공동번영을 이룸과 함께 한국은 유라시아 대륙의 물류 허브로 거듭나는 것이었다. 다수 전문가들은 유라시아 이니셔티브의 추진에 있어 여러 도시 중에서도 세계적인 항만을 보유하고 있고, 한반도의 기점이자 종점이며 세계 전역으로 뻗어나갈 수 있는 지정학적 위치를 갖춘 부산시의 역할에 주목하였다. 이러한 상황에서 당시 부산시장은 부산시의 도시 비전을 '유라시아 관문도시 부산' 으로 정하고 부산이 주도적으로 유라시아 이니셔티브를 이끌어가야 한다고 주장하였다. 이러한 주장에 기반하여 부산시는 2015년부터 본격적으로 유라시아와 관련한 여러 사업을 추진하였다. 하지만, 대부분의 사업은 일회성에 그치거나 큰 효과를 내지 못하였다. 반면, '유라시아 관문도시 부산' 이라는 부산의 비전을 국내외에 전파하고 유라시아 여러 도시들과의 교류협력을 강화하기 위해 추진한 『유라시아 부산원정대』 사업은 정책결정 과정에서는 사업효과에 대한 의문이 다수 제기 되었으며 일회성 행사에 그칠 것으로 우려 하였으나 2019년 현재까지도 지속적으로 추진될 만큼 부산시를 대표하는 유라시아 관련 정책으로 자리매김 하였다. 따라서 본 논문에서는 『유라시아 부산원정대』의 집행과정을 분석해보고, 어떻게 본 정책이 다른 사업들과는 달리 지속될 수 있었는지에 대해 살펴보았으며 이 과정에서 Mazmanian과 Sabatier의 연구를 활용하여 분석을 진행하였다. 따라서 본 연구의 목적은 다음의 세 가지로 나눌 수 있다. 첫째, 『유라시아 부산원정대』의 집행과정을 분석해보고 집행이 잘된 부분과 그렇지 않은 부분을 찾아본다. 둘째, 정책집행과정의 분석을 통해 어떻게 본 사업이 다른 유라시아 관련 정책들과는 달리 일회성에 그치지 않고 지속될 수 있었는지에 대해 알아본다. 마지막으로, 정책집행 연구에서 빈번하게 활용되는 Mazmanian과 Sabatier의 연구를 분석하여 이론이 담고 있는 함의 및 구체적 내용을 알아본다. 이러한 목적의 달성을 위해 본 연구는 우선 부산시 및 여러 유관기관들의 문헌 자료와 함께 집행과정에 참여했던 주요 행위자들을 면담하여 정책집행과정을 면밀히 분석하였다. 다음으로 Mazmanian 과 Sabatier 가 제시한 '정책집행이 효과적으로 이루어지기 위한 조건'을 활용하여 원정대 사업의 정책집행과정이 해당 조건에 부합하는지 여부를 통해 정책집행이 잘 된 부분과 그렇지 못한 부분을 구분하였다. 연구의 결과는 다음과 같다. 첫째, 『유라시아 부산원정대』의 정책목표는 명확하며 일관성을 갖추었다. 특히, 외교부, 경상북도 등 타 기관이 추진한 유사사업들과 비교했을 때 원정대의 정책목표는 상당히 명확하며 구체적이었다. 둘째, 정책목표의 달성을 위해 부산광역시 차원의 개입이 필요하다는 것은 분명하나, 굳이 원정대라는 수단을 선택해야 하는지에 대해서는 원정대 이외에도 본 정책목표를 달성할 수 있는 여러 가지 다른 방법이 있을 수 있기 때문에, 본 정책목표와 원정대라는 정책수단 간의 인과관계는 명확하지 않았다. 셋째, 정책집행 담당기관이었던 부산광역시 통상진흥과 남북경제협력팀 및 부산국제교류재단은 본 정책과 업무관련성이 매우 높고, 조직원들은 본 사업에 대해 상당히 긍정적인 반응을 보였기 때문에 집행기관은 모두 원정대 사업에 대해 상당히 호의적이었던 것으로 나타났다. 넷째, 부산시 및 부산국제교류재단 모두 원정대 사업에 대한 의사결정 과정은 3단계로 이루어져 공식적으로 정해진 절차에 비해 단순화 되었으며, 의사결정 방식 역시 하향식이며 대체로 시장의 지시에 전적으로 순응하는 편이었기 때문에 빠른 정책집행이 가능했다. 다섯째, 부산시 의회 및 부산시 내 관련 부서 등 정책집행 유관기관과 정책결정자인 부산시장은 원정대 사업에 대해 호의적이었으며 지지를 보냈던 것으로 나타났다. 특히, 부산시장의 경우 원정대에 대해 상당히 높은 관심을 보였던 것으로 파악되었다. 마지막으로 원정대 사업의 집행과정에서 동남권 신공항의 유치 문제가 이슈화 되면서 '유라시아 관문도시 부산' 이라는 비전의 홍보 및 국내외 확산이 더욱 중요해졌고 이로 인해 원정대 사업의 정책목표가 갖는 중요성은 정책결정 시에 비해 더욱 높아졌다고 볼 수 있다. 연구결과를 종합해보면, 『유라시아 부산원정대』의 정책집행과정은 Mazmanian 과 Sabatier가 제시한 정책집행이 효과적으로 이루어지기 위한 조건을 대체로 만족하는 것으로 나타났다. 특히, 원정대 사업은 ① 정책목표가 상당히 명확하며, ② 정책집행기관 및 정책결정자가 사업에 대해 상당한 관심 및 지지를 보내었고, ③ 사회·경제적 조건의 변화로 인해 정책집행과정에서 정책목표의 상대적 중요성이 더욱 높아졌기 때문에 다른 유라시아 관련 사업들과는 달리 정책이 일회성에 그치지 않고 지속될 수 있었던 것으로 나타났다. 다만, 정책목표와 정책수단 간의 인과관계가 불명확하여 정책이 지속되고 집행에 따른 성과가 창출되었음에도 정책이 갖는 효과에 대한 의문이 지속되었던 만큼 이에 대한 보완이 필요한 것으로 나타났다. ; Former President Park Geun-hye delivered her keynote address at the opening of the 'Global Cooperation in the Era of Eurasia' conference in Seoul Silla hotel on October 18 ; "Ladies and Gentleman, Eurasia is the biggest continent in the world which contains 71% of the world's population and 12 different time zones. .Now is the time to open a new channel to Eurasia in order to lift its cooperation potential. We need to revive Eurasia, a geographic area disconnected by the Cold War, as a space for communication, openness, creation and fusion. I would like to propose a new vision, to make Eurasia, one continent of creation and peace." and then suggest her new vision 'Eurasia Initiative'. The main concept of Eurasia Initiative is integrating logistics, market and culture among Eurasian countries and key point is by integrating logistics and transportation we'd like to achieve prosperity of Eurasia Countries and at the same time, Korean peninsula should role as a logistic hub and the gate way of Eurasia continent. Many specialists focused the role of Busan Metropolitan city to achieve the goal of Eurasia Initiative because of it's global port facilities and geographical location which is the origin and destination of Korean peninsula. In this situation, former Mayor of Busan set the city vision as 'The Eurasia Gateway Busan' and stressed that Busan should lead the Eurasia Initiative. Based on this opinion, Busan Metropolitan City implemented many policies from 2015, but most of them were just one time event and failed to succeed. However, the policy 'Eurasia Expedition from Busan' which focused on publicizing the vision of Busan (Eurasia Gateway Busan) and strengthening the cooperation with Eurasian cities is still maintaining in 2019 and becoming the most famous and symbolic policy about Eurasia in Busan eventhough there were lots of doubts about being one time event in the policy decision process. Thus in this study, analyzing the policy implementation process of 'Eurasia Expedition from Busan' and finding the reason how this policy can be maintained unlike other policies by using the Mazmanian-Sabatier model. The goals of this study can divided into three things. First, analyzing the policy implementation process of 'Eurasia Expedition from Busan' and finding the good and bad points of implementation. Second, by analyzing the policy implementation process, how this policy can be maintained unlike other Eurasian policies in Busan. Lastly, analyzing the Mazmanian-Sabatier model which is frequently used in the study of policy implementation, and finding the exact meaning of that model. To achieve these goals, first of all, reviewing much data of Busan Metropolitan cities and other related organizations and also interviewing some people who participated the policy implementation process. Next, using the "Conditions of effective policy implementation which is argued by Mazmanian-Sabatier, analyzing the policy implementation process of 'Eurasia Expedition from Busan' whether it is fit to that conditions or not. The results of this study are as follow. First, the objectives of 'Eurasia Expedition from Busan' are clear and consistent. Especially, comparing with the similar policies which was implemented by the ministry of diplomacy and Gyeongsangbuk-do, the objectives of these policies are very obvious and concrete. Second, to achieve this objectives, it's clear that the intervention of government is necessary, but the causal theory between this policy and the objectives is not clear. Third, both Inter-Korean cooperation team in Trade promotion division of Busan Metropolitan City government and Busan Foundation for International Cooperation which were in charge of implementing 'Eurasia Expedition from Busan' were quite related to this policy and all of members were so much positive to this policy. Thus both organizations are so much favorable to this policy. Fourth, the decision rules of both organizations were in a three stages which was quite simplified than formal steps, and quite top-down ways especially complying the order of mayor thus it can simplify policy implementation process and facilitate effective implementation. Fifth, the related organizations of policy implementation like Busan city council and other divisions of Busan Metropolitan City and the mayor of Busan who is a policy decision maker were favorable and supportive to 'Eurasia Expedition from Busan'. Especially, mayor of Busan gave considerable support to this policy. 학위논문 (석사)-- 서울대학교 대학원 : 외교학과, 2012. 2. 신욱희. ; 본 연구는 구조적 환경과 행위자의 선택, 그리고 구조와 선택을 매개한 정책결정자의 인식의 상호작용을 통해 1960년대 중반 북한 자주외교노선의 결정요인을 분석하고자 한다. 정권수립 이후 북한은 동아시아에 형성된 냉전 구조 속에서 사회주의 대국이었던 소련과 중국에게 편승함으로써 상대적으로 "밖"의 문제보다 "안"의 문제에 집중할 수 있었다. 김일성은 경쟁세력을 차례로 숙청하며 권력을 집중화할 수 있었고, 단일지도체제를 구축할 수 있었다. 하지만 김일성은 1956년 소련공산당 제20차 대회 이후 전개된 흐루시초프의 신노선에 소극적으로 대응하였고, 이에 정권의 중심에서 밀려나 있던 소련파와 연안파의 불만을 촉진시켰다. 김일성에 대한 개인숭배 심화와 당 지도부의 경제, 인사 정책에 불만을 가졌던 경쟁 세력은 외부의 힘을 이용하여 당의 노선 변화를 도모하고자 하였고, 이러한 계획은 1956년 8월 전원회의를 통해 현실화되었다. 하지만 사전에 계획을 인지하고 있었던 김일성의 방해로 인해 연안파와 소련파의 계획은 실패하였고, 주도자들은 종파주의자로 낙인 찍혀 대규모 숙청의 대상이 되었다. 하지만 김일성의 과도한 권력집중을 일탈로 우려한 소련과 중국은 미코얀과 펑더화이를 파견하여 북한의 내정에 간섭하였다. 이로써 김일성의 반(反)대국주의 의식은 심화되었고, 이후 중소분쟁이 본격적으로 시작하면서 국내정치에서 외부의 힘을 제거하고 단일지도체제를 구축하였다. 1960년대가 시작되면서 중국과 소련의 갈등은 이념적 차원을 넘어 정책적 차원으로 확대되었으며 북한은 국내외의 문제에 사로잡힌 양국의 관심에서 벗어나 본격적으로 대외관계를 주도할 수 있었다. 이전까지 북한의 대외관계는 소련과 중국의 압도적인 영향력 아래 있었지만 상호방위조약을 통해 양국과 대등한 관계를 형성함은 물론 제3세계 국가들과의 대외관계를 적극적으로 확대하면서 자주외교노선의 기틀을 마련하였다. 소련공산당 20차 대회와 중소분쟁으로 이어지는 사회주의 진영의 구조적 환경의 변화 속에서 김일성을 비롯한 당 지도부는 역사적으로 형성된 반(反)대국주의 인식으로 인해 어느 한 국가에 대한 일방적인 편승을 배제한 채 1960년대 중반 대외적으로 자주외교노선을 공식화할 수 있었다. ; The purpose of this paper is to analyse North Korea's foreign policy, called the self-reliance foreign policy, which was implemented in the mid-1960s. Among the various factors that can influence foreign policy, this paper concentrates on the interaction of structure, decision maker's intention, and perception or values according to Carlsnaes' dynamic model of analysing foreign policy change. After nation building, there have been two critical structural diversions in the Communist bloc—the Twentieth Party Congress of the CPUS in February 1956 and the Sino-Soviet conflict that began in the late 1950s. In each event, Kim Il-sung attempted to increase DPRK's policy-making autonomy and exclude the political leverage of two brotherhood countries, Soviet and China, in the domestic politics. Kim's strong anti-great power chauvinism was strengthened from his past experiences, and it was reinforced by two countries' intervention in 1956. These two primary elements had influenced Kim Il-sung's foreign policy decision making during his reign. In the late 1950s, Kim and his partisans concluded the intra-party conflict with two rival groups, the Soviet Korean and Yan'an groups. These events completely monopolized power in domestic politics. As the dispute between China and Soviet aggravated in the 1960s, North Korea could secure its policy-making autonomy from these two countries. It had become more equal that North Korea extended its diplomatic relations to the Third World nations in this period. On the basis of the consecutive policies, North Korea could implement the self-reliance foreign policy in around 1965. ; Master

Background Low-quality services of emergency medical institutions, unclear function of institution classification, and overcrowding of emergency departments (ED) of regional emergency medical centers (regional centers) have been major problems in the Korean emergency medical services system (EMSS). In 2015, the Middle East Respiratory Syndrome (MERS) outbreak spread from patients waiting for hospitalization in the overcrowded ED. The MERS outbreak was an event that revealed the vulnerability of EMSS and opened the policy window to solve the problem of EMSS in Korea. Several policies for EMSS improvement were implemented after the 2015 MERS outbreak focusing the improvement of emergency medical services (EMS) delivery system and ED infection control. This study investigates the policy processes for improving emergency EMSS and infection control program after the MERS outbreak. The purpose of this study was to examine the policy processes regarding EMSS from the view of policy actors and draw lessons for improving the EMSS in Korea. 2. Methods A mixed method was utilized to identify and explain the complex subject of the policy processes of Korean EMSS during and after the MERS outbreak in 2015. As a quantitative study, changes in EMSS after MERS were investigated according to the WHO's model of a national health system infrastructure. Literature review and data analysis of Korean Health Insurance Service (NHIS) database were conducted. In Korea, the NHIS covers all citizens. A quantitative analysis was performed on 16,965,755 cases using ED during 2014-2016. In-depth interviews were conducted on 30 experts, who were involved in the emergency medical policy process, including clinicians, policy experts, and hospital management experts. The total interview time was 55 hours and 4 minutes. Changes in EMSS and the policy process after the MERS outbreak were evaluated from the view of policy actors. In this study, quantitative data was interpreted qualitatively, and independent qualitative evaluation was conducted, as well. Therefore, this research is a mixed-methods study focusing on qualitative research. SAS version 9.4 was used for the quantitative analysis and NVivo 12.0 was used for the qualitative analysis. 3. Results The quantitative analysis results showed that in the area of EMS delivery system, the human resource of ED and number of regional centers have been increased and facility, equipment, and manpower standards have been revised in order to expand emergency medical resources. As a result, the number of medical personnel and emergency intensive care units in regional centers increased and profitability of ED improved as well. The number of monthly ED visits temporarily decreased during the MERS outbreak but then recovered afterward. As ED overcrowding indicators were emphasized in the evaluation of EMS institutions, monthly mean ED length of stay of regional centers was decreased. However, despite the emphasis on providing critical care, transfer rate of severe emergency patients was not decreased. In the policy processes of EMS, experts evaluated that EMS delivery system was set up as a pseudo-agenda and exploration and comparison of policy alternatives were not sufficiently carried out. As a result, the physicians did not comply with the policy implementation and policy goals were not achieved. In the area of ED infection control program, the human resources for infection control and the number of isolation unit of ED were increased. Budgets for emerging infectious disease management were increased and profitability of infection control was also improved. The EMS Act was revised to respond to emerging infectious diseases and indicators related to infection control were strengthened in evaluation of EMS institutions. In the qualitative research, experts said that since the ED infection control program was such an intuitive and obvious problem, it was easy to become a policy-agenda. Despite the resistance of small hospitals, ED infection control program was implemented by powerful policy measures such as the revision of the Law. As a result, policy goals of expanding the infection control resources and improving the infection management environment were able to be achieved. From results of observing EMS policy process, the following characteristics of policy actors were found. First, there was difference of opinions within the physician subgroup as well as between government and medical professionals. ED staff, other department staff, and hospital executives, who have different views on policy alternatives could have all served as veto points in the policy process. Second, while the EMSS expert organization, which was an unofficial participant in the emergency medical policy process, could have played both roles of cause group and sectional group in the policy process, their sectional position has been emphasized, thereby weakening their insider status during policy process. Third, policy actors understood the publicness of EMS differently in terms of its ownership and function. The ED staff insisted that government support is needed because emergency medical care is based on public interest, however, some staffs were unwilling to provide less profitable cares, leading to conflict. Main characteristics of the emergency medical policy process that made it difficult to successfully implement are as follows: First, there was lack of consideration regarding variation of EMSS in Korea, such as distribution of medical resources among different regions, incentives to hospitals, and understanding of functional variations in ED. Second, the problem of the EMS delivery system was a problem of the low quality of emergency medical care. Third, the lack of professionalism in some physicians has lowered the capacity of emergency medical care, as well as trust between patient and doctor, therefore made the problem more difficult to solve. 4. Discussion Based on the analysis of the policy processes of the EMSS after the MERS outbreak in Korea, following lessons are suggested. First, setting the goals of EMS from the perspectives of citizen is warranted. The final performance goal should focus on improving health status of citizen and satisfaction with emergency medical care. Second, to close the gaps among policy actors in understanding policy content and policy process, actors should work together to explore the root causes of the problems and establish a policy agenda together. Third, a policy alternative should be causal in solving the problems in EMSS considering variation of EMS. Fourth, compliance with policies could be secured by internal persuasion among policy actors with proper incentives and reasonable punishment, especially EMSS expert organization. Fifth, policy outcomes should be evaluated based on both policy output and side effects together to identify policy impact. Sixth, policy governance regarding EMSS should be established and the roles of EMSS expert organization should be strengthened as the cause group rather than sectional group. This study examined the emergency medical policy processes from the view of policy actors through the 2015 MERS outbreak in Korea. The improvement of EMSS should be based on understanding of the emergency medical policy process and policy actors. ; 1. 서론 응급의료기관의 응급환자 진료 역량부족과 불명확한 종별 기능 구분, 일부 응급실의 과밀화는 한국 응급의료체계(emergency medical services system, EMSS)의 고질적인 문제였다. 2015년 과밀화된 상급종합병원 응급실에서 입원대기 중이던 환자로부터 메르스(Middle East Respiratory Syndrome, MERS) 유행이 확산되었다. 메르스 유행은 우리나라 응급의료체계의 취약성이 드러난 사건이자 응급의료 문제를 해결할 수 있는 정책의 창이 열린 사건이었다. 정부는 메르스 유행 이후 여러 가지 응급의료 개선정책을 추진하였다. 이 연구는 취약한 응급의료 전달체계 하에서 응급환자 감염관리의 실패로 인해 메르스 유행이 발생했다는 점에 문제의식을 두었다. 그래서 응급의료 전달체계와 응급환자 감염관리 영역에서 메르스 유행 이후 이루어진 정책과정과 그로 인한 변화를 분석하였다. 응급의료 정책행위자의 관점에서 정책과정과 정책결과를 분석함으로써, 향후 응급의료 개선을 위한 정책과정에서 고려해야 할 점을 도출하는 것이 이 연구의 목적이다. 2. 연구방법 메르스 유행 이후 응급의료 정책과정 분석이라는 복잡한 주제를 파악하고 설명하기 위하여 혼합연구방법(Mixed methods research)을 사용하였다. 양적연구로 문헌고찰과 국민건강보험공단 자료 분석을 통해 메르스 유행 이후 응급의료체계의 변화를 보건의료체계의 구성요소별로 조사하였다. 한국은 1989년 전국민 의료보험제도를 도입하여 전 국민이 국민건강보험에 가입되어 있는 국가이다. 메르스 유행 전후 응급실 이용 변화를 조사하기 위하여 국민건강보험공단 DB의 2014-2016년 응급실 이용 진료건 16,965,755건을 분석하였다. 질적연구로 응급실 의료진, 정책 전문가, 병원 경영진 등 응급의료 정책과정의 관련 전문가 30명을 대상으로 심층면접을 수행하였다 전체 심층면접 분량은 55시간 4분이었다. 심층면접을 통해 정책 행위자의 관점에서 메르스 이후 이루어진 응급의료 정책과정을 고찰하고, 응급의료체계의 변화를 조사하였다. 양적연구 결과를 질적연구로 해석하고 질적연구로 독자적인 분석을 수행하여, 이 연구는 질적연구가 중심이 된 혼합연구로 볼 수 있다. 양적연구의 분석에는 SAS version 9.4를 사용하였고, 질적연구 분석은 NVivo 12.0을 사용하였다. 3. 연구결과 응급의료 전달체계 영역에서 응급의료 자원을 확대하기 위해 권역응급의료센터(권역센터)가 확대되고 시설·장비·인력기준과 관련 수가가 개정되었다. 그 결과 권역센터의 의료인력과 응급중환자실 병상 수가 증가하고 수익성이 개선되었다. 달빛어린이병원 사업을 확장한 결과 달빛어린이병원 수도 증가하였다. 응급실 과밀화를 완화하기 위해 응급의료기관 평가에 재실시간 지표를 수가연동지표로 도입하고 응급의료에 관한 법률에 응급의료센터 24시간 이상 재실환자 관리 규정을 넣었다. 그 결과 응급실 월별 방문환자 수는 2015년 메르스 유행 당시 일시적으로 감소한 후 지속 증가 추세이나, 권역센터 응급환자의 월별 재실시간 평균은 2014-2016년 기간 동안 감소 추세를 보였다. 응급의료기관의 응급환자 진료역량을 강화하기 위해 응급의료기관 평가에 중증해당환자 최종치료 제공 관련 지표를 강화하였으나 2017-2018년 기간 중증응급환자의 전원율은 개선되지 않았다. 전문가들은 응급의료 전달체계 개선은 위장의제(pseudo agenda)로 설정되어 정책대안 탐색과 비교평가가 충분히 이루어지지 않았으며 점중주의적으로 쓰레기통 모형의 정책결정이 이루어졌다고 평가하였다. 그 결과 정책대상집단인 의사들은 정책집행에 순응하지 않았고, 정책은 집행 후에도 정책목표를 달성하지 못했다. 응급환자의 감염관리 영역에서 권역센터 시설·장비·인력기준과 감염관리실 설치기준 및 인력기준이 개정되어 응급실 감염관리 자원이 확대되었다. 정부는 응급실 방문객 관리를 도입하고, 감염예방관리료 등 감염관리 관련 건강보험 수가를 신설하였으며, 신종감염병 대응 관련 법률 등 감염관리 관련 법률을 개정하여 응급실 감염관리 여건을 개선하였다. 전문가들은 응급환자 감염관리는 직관적이고 명확한 문제라는 특성이 있어 병원 내 감염관리 강화로 의제화되기 쉬웠다고 평가하였다. 감염관리 개선정책은 보호적 성격을 가진 규제정책으로 집행 과정에서 중소병원을 중심으로 일부 병원의 반발이 있었다. 그러나 응급의료에 관한 법률 개정과 같은 강력한 정책수단을 사용하여 집행된 결과, 응급실 감염관리 자원확대와 감염관리 환경개선이라는 정책목표를 달성하였다. 응급의료 정책행위자들의 관점에서 응급의료 정책과정을 관찰한 결과 다음과 같은 정책행위자의 특징을 발견하였다. 첫번째, 정부 전문가와 의료진 전문가 사이뿐 아니라 의사 하위집단 내에서도 견해 차이가 존재하였다. 정책대안에 대해 다른 견해를 가지고 있는 응급실 의료진, 배후진료과 의료진, 병원 경영진은 정책과정에서 모두 거부점(veto point)으로 작용할 수 있었다. 두번째, 응급의료 정책과정의 비공식적 참여자인 응급의료 관련 전문학회는 정책과정에서 대의집단이자 파벌집단의 역할을 동시에 수행할 수 있으나, 정책과정에서 파벌집단의 역할이 강조됨으로써 내부자의 지위가 약화되었다. 세번째, 응급의료의 공공성에 대하여 소유주체 기준에 따른 공공성과 기능에 따른 공공성 개념이 혼재되어 사용되며 서로 다른 공공성 개념에 따른 정책행위자간 갈등이 발생하였다. 응급실 의료진은 응급의료가 공공의 이익을 실현하는 의료영역이므로 정부 지원이 필요하다고 주장했으나 일부 의료진은 수익성이 낮은 진료를 기피하는 모습을 보여 정부 전문가와 의견 차이를 보였다. 응급의료 정책과정의 성공적 추진을 어렵게 한 정책과정의 특징은 다음과 같다. 첫번째, 응급의료 정책과정에서 지역간 의료자원 분포, 병원에 작용하는 유인, 병원 내 응급실의 역할과 같은 응급의료에 존재하는 변이에 대한 고려가 부족했다. 두번째, 응급의료 전달체계 문제는 곧 응급의료기관의 취약한 응급환자 진료 역량의 문제였다. 세번째, 일부 의사들의 전문직업성(professionalism) 부족이 응급의료기관의 응급환자 진료 역량을 취약하게 하고, 환자-의사 사이 신뢰를 저하시켜 대형병원 쏠림현상을 악화시켰다. 4. 고찰 메르스 유행 이후 이루어진 응급의료 개선정책의 정책결과 분석과 정책과정에 대한 연구 결과를 근거로 하여 향후 응급의료 정책과정에 대하여 다음과 같이 제언한다. 첫번째, 국민의 관점에서 응급의료체계의 개선목표를 설정할 필요가 있다. 건강수준 향상과 응급의료 서비스에 대한 만족도 향상이라는 최종 성과목표가 개선정책의 목표로 설정되어야 한다. 두번째, 정책행위자의 인식 차이를 좁히기 위하여 여러 행위자가 함께 문제의 원인을 탐구하고 정책의제를 설정하는 과정이 필요하다. 세번째, 문제 해결에 인과성 있는 정책대안이 결정되어야 한다. 인과성 없는 정책대안은 정책이 집행되어도 정책문제를 해결하지 못한다. 네번째, 정책대상자의 순응을 확보할 필요가 있다. 유인과 처벌 이전에 전문가이자 정책대상자인 의사들에 대한 내면적 설득 과정이 필요하다. 다섯번째, 정책산물과 부수작용을 포괄하는 정책결과 평가가 필요하다. 여섯번째, 응급의료 정책 거버넌스를 확립하고 응급의료 관련 전문학회를 정책과정에 참여시켜 대의집단의 성격을 강화할 필요가 있다. 이 연구는 2015년 메르스 유행이라는 사건을 통해 응급의료 정책행위자들의 관점에서 우리나라 응급의료 정책과정을 고찰하였다. 응급의료 정책과정과 정책행위자에 대한 이해를 바탕으로 응급의료체계 개선을 추진해나가야 한다. ; I. 서 론 1 1. 연구 배경과 필요성 1 1) 한국 응급의료체계의 문제 1 2) 2015년 메르스 유행과 응급의료체계 4 3) 연구의 필요성 6 2. 연구 목적 10 II. 이론적 배경 12 1. 보건의료체계의 평가 12 1) WHO 보건의료체계의 구성요소 12 2) 보건의료체계 개혁을 위한 조종손잡이 14 2. 정책론 18 1) 정책과정의 이해 18 2) 응급의료 정책과정의 이해 25 III. 연구방법 35 1. 혼합방법 연구설계 35 2. 양적연구 방법 43 1) 양적 연구의 자료 수집 43 2) 양적 연구의 자료 분석 46 3. 질적연구 방법 54 1) 연구팀 구성 54 2) 심층면접 설계 55 3) 분석 방법 67 4. 혼합방법 연구의 통합 70 IV. 연구 결과 72 1. 양적연구 결과 72 1) 응급의료 전달체계 72 2) 응급환자의 감염관리 121 2. 질적연구 결과 134 1) 심층면접 참여자의 특성 134 2) 응급의료 전달체계 137 3) 응급환자의 감염관리 249 3. 연구결과의 종합 297 1) 2015 메르스 유행의 응급의료체계에 대한 의의 297 2) 메르스 유행 이후 응급의료 정책과정 분석 298 3) 응급의료 정책과정의 특징 340 V. 논의 363 1. 응급의료 정책과정 고찰 363 2. 연구의 한계 380 3. 연구의 의의 383 1) 방법론적 의의 383 2) 정책적 의의 384 4. 연구의 결론 385 1) 메르스 유행 이후 응급의료 전달체계와 응급환자 감염관리의 변화 385 2) 응급의료 정책과정 고찰 386 3) 응급의료체계 개선을 위한 정책과정에의 시사점 388 VI. 참고문헌 390 ; Doctor

This study examined the influence of the separation of prescribing and dispensing roles(SPD) policy implemented in Korea in July 2000, especially on the change in the net profit of medical institutions. Using the data set from the Korea's National Health Insurance and the previous research, this study elicited the following main results. First, tertiary care institutions was estimated to lose about 631 billion won after the SPD policy. Second, general hospitals and hospitals gained about 557 billion and 564 billion won, respectively. Third, it is shown that clinics also gained 389-659 billion won. Finally, however, the change in net profit of medical institutions after the SPD policy largely depends on different estimation models. Moreover, it also varies from the assumptions on the price differential of a reimbursable drug which worked as cross-subsidy to insufficient physician's fee before the SPD policy. Despite such limitations as lack of data outside of the National Health Insurance's coverage, this study differs from others. This is the first research to explore the effect of the SPD policy on different types of medical institutions and to attempt to purely focus on the SPD policy. In this study, we can draw the policy implication that preparing for a policy change, the government should set up the policy evaluation system to collect the concerned data and develop the methodologies in advance to the policy implementation. ; open
