Obesity is a growing problem. The Dutch government has formulated different objectives in the last decades, aimed at diminishing the problem, but none of these objectives have been reached. Obesity among adults doesn't decline, obesity among children is increasing. Compared with the government policy on alcohol and tobacco, the government has less policy instruments available for the domains nutrition and moving. The emphasis is on stimulating healthy behavior by influencing individual and collective behavior. This influence can be directly or indirectly, by influencing the social and physical environment of the individual. The national government defines a big role for the local government (because of the possibilities on the local level for influencing behavior), but also gives the local authorities a lot of policy freedom. National policy itself is focused on supporting the execution of the policy by local authorities. The policy instruments on national level are used at a suboptimal level, cooperation between departments could me much more intensive. Various projects abroad (Finland, Epode in France) and in the Netherlands (Hartbeat Limburg, Healthy Weight Overvecht) lead to the conclusion that an integrated approach, with strong involvement of the population and a large variety of organizations, can diminish the problem of obesity. In the local public health reports, which each municipality has to formulate once in every four year, the elaboration of such an integrated approach is almost absent. As reasons for this phenomenon a lack of time, staff and budget are mentioned frequently, as well as a lack of commitment from the municipal board and other municipal departments. An important underlying cause seems to be a low sense of urgency by the aldermen, who are responsible for the local policy regarding the problem of overweight. If such a low sense of urgency coincides with other determinants in the theory of Kingdon, such as complex, obdurate solutions, the absence of focusing events and the absence of policy entrepreneurs, effective policy will be absent. Recommendations are therefore formulated for all these aspects.

The protozoan parasites Cryptosporidium parvum and Giardia intestinalis have emerged as significant waterborne pathogens over the past decades. Many outbreaks of waterborne cryptosporidiosis and giardiasis have been recorded,primarily in the United States and the United Kingdom.Chapter 1 gives an overview on the currently available knowledge on the parasites, the disease, the transmission through drinking water and the measures to prevent waterborne transmission. The disease caused by Cryptosporidium and Giardia consists of a self-limiting diarrhoea that lasts for several days in the majority of cases, but the burden of disease and the mortality are high in the immunocompromised part of the infected population. Several characteristics of the parasites facilitate their waterborne transmission: they are very resistant to environmental stress and to chemical disinfection, they can be transmitted from livestock and wildlife to man and their infectivity is high, so even a dose of 1 (oo)cyst gives a discrete probability of infection. The abundance and size of drinking waterborne outbreaks in developed countries show that transmission of Giardia and Cryptosporidium by drinking water is a significant risk. In the case of Cryptosporidium, the absence of an adequate cure for immunocompromised patients increases the problem. Although the outbreaks receive most attention, low-level transmission of these protozoa through drinking water is very likely to occur. Cysts and oocysts are regularly found in drinking water, although only a small proportion may be viable and infectious to man. A major drawback for the determination of the health significance of (oo)cysts in (drinking) water is that methods for a sensitive and specific detection of infectious (oo)cysts, with a consistently high recovery are not available. The cause of drinking water contamination with these parasites that led to the reported outbreaks was not limited to obvious treatment inadequacies or post treatment contamination, but also occurred in apparently well-treated water. Moreover, in several outbreaks, the coliforms, the parameter that was used to demonstrate the microbiological safety of drinking water did not warn against parasite breakthrough through the treatment, particularly because the coliforms were more efficiently eliminated by disinfection than both parasites. Surveys of surface water show that these parasites are ubiquitously present in the aquatic environment, even in pristine environments. Hence, all surface water treatment systems have to deal with these protozoa. These developments raised concern over the safety of Dutch drinking water with regard to Cryptosporidium and Giardia. Considering this situation, the Dutch drinking water companies and government initiated a research programme to determine the (im)probability of transmission of Cryptosporidium and Giardia through drinking water (Chapter 2). The protozoa have changed the philosophy in the developed countries towards safe-guarding of drinking water from monitoring of the 'end-product' drinking water to monitoring raw water and the efficiency of the treatment. Furthermore, the extreme resistance of these organisms implies that a "zero-risk" is no longer achievable. Treatments should be designed to reduce the (oo)cyst concentrations in the raw water as far as possible and preferably include filtration step(s). This implies that information on the parasite concentrations in the raw water is necessary, as well as information on the removal efficiency of the treatment. Quantitative risk assessment provides a tool for the combination of information on raw water quality (concentrations detected, recovery of the detection method, viability) and treatment efficiency (removal by different steps in the treatment).

De ideeën van Paracelsus over gezondheid vanuit een sociaal-economische visie zijn al 500 jaar oud, maar werden eerst recent teruggevonden en toegankelijk gemaakt door een nieuwe editie van zijn sociaal-economisch werk. Ook de andere hier weergegeven inzichten over gezondheid en gezondheidszorg vanuit een economisch, sociaal en politiek perspectief zijn weinig bekend. In dit boek gaat het over de meest belangrijke ideeën van sociale wetenschappers en economen over gezondheid en gezondheidszorg, beginnend met Paracelsus en eindigend met Schumpeter. Behandeld worden grondleggende ideeën en concepten van Wolff, Justi, Roscher, Menger, Schmoller, Bücher, Oppenheimer en Althoff. De conclusie luidt dat hun bijdragen wel deels zijn te herkennen in de moderne gezondheidseconomie, maar dat wij toch ook belangrijke invalshoeken, die zouden voortvlooeien uit hun werk, vandaag missen. Daarom kan een geschiedenis van de theoretische ontwikkeling van de gezondheidseconomie ook geen Whig history zijn. Volgens deze richting is de huidige stand van de ontwikkeling het logische gevolg van werk dat vroeger al is gedaan. Om een aantal redenen hoeft dit niet per se zo te zijn. De nieuwe economie van de gezondheidszorg heeft andere wortels, met name de Engelse National Health Service. Verder staan de hier besproken bijdragen niet in het Engels, vandaag de lingua franca van de sociale wetenschappen, ter beschikking. In de dissertatie zijn, naast één voorbeeld van wetenschapspolitiek (Althoff), uitsluitend auteurs uit het Duitse taalgebied die klassieke bijdragen hebben geleverd voor een bespreking opgenomen. De hier gekozen auteurs zien gezondheid als onderdeel van menselijk handelen en gezondheidszorg als onderdeel van cultuur. De nadruk ligt op culturele ontwikkeling en gezondheid, op het subsidiariteitsbeginsel en op een minimale rol van de overheid in de gezondheidszorg. De weergave van de inzichten van de behandelnde auteurs geeft ook een antwoord op de vraag in hoeverre zij een bijdrage leveren over aspecten zoals vraag naar en aanbod van de gezondheidszorg, informatie, optimale contracten, de markt voor geneesmiddelen, verzekeringen, technologie, instituties zoals ziekenhuizen en bejaardenhuizen, waardering van levens en levenskansen, sociale verzekeringen, en beleid en vraagstukken in de toekomst. Aangegeven wordt dat hun voorstellen politieke relevant waren in hun eigen tijd

Providing optimal care for a sudden, unexpected large amount of victims from a disaster or major incident is challenging. It requires an approach different from regular traumacare. The population as a whole, rather than the individual, should be the focus of management. This thesis focuses on medical preparedness, care and follow-up for victims of a disaster or major incident, and explores new opportunities for improvement of disaster relief using the Internet as a supportive tool. The Major Incident Hospital (MIH) , the Netherlands, is a unique facility that offers permanent, reserved facilities to provide acute , structured care for groups of patients after a disaster or major incident. Its contribution to preparedness is not limited to providing directly available resources without impact on regular care. It also offers expertise, infrastructure, organisation, support systems, training, research and systematic working methods adapted to provide the greatest good for the greatest amount of people in case of multiple simultaneous victims. Pre-arranged cooperation, between the military, a trauma center and the National Poison and Information Center add to its value. Group-wise treatment relief for incident victims showed to be advantageous for overview, logistics, registration, quarantine, emotional support and (pre)arrangements for family, media and security. The evaluation of all MIH deployments since its foundation in 1991 resulted in five concluding recommendations: 1) improvement of embedding of the MIH in regional and national procedures; 2) continued dedicated time and staff for training, research and development; 3) improvement of nuclear/biological/chemical decontamination facilities and preparedness; 4) implementation of standardized scoring systems; and 5) expansion of registration systems to the pre-hospital setting. Decision-making refers to assessing costs and benefits of competing actions, with either a known outcome or an uncertain result. Decision-making depends on several abilities, such as behavioural flexibility and inhibiting risky responses. Several factors affect decision-making, causing differences in the outcome of decision-making processes. The overall aim was to improve our understanding of effects of stress and gender on decision-making, in rodents and in humans. First, impulsive decision-making was examined by testing male and female mice in a delay-discounting task. The data show that female mice shift more quickly from a large-late to a small-soon reward than male mice. While this suggests that female mice are more impulsive than male mice, an alternative explanation is that female mice are more exploratory than male mice and thus detect changes earlier. Factors like gender and internal state, but also handling-induced stress may affect task outcome. Therefore, home-cage testing has become popular. Here, a pilot study on home-cage testing of delay-discounting was conducted in rats. No differences were found in task-performance compared to traditional stand-alone tasks. Following from this, rats were tested in a home-cage setting in a more complex decision-making task: a new protocol to run a rodent version of the Iowa Gambling Task (IGT), a task for decision-making in humans. To validate this protocol, brain serotonin levels in rats were manipulated prior to testing their decision-making. As expected, lowering serotonin levels led to both poor decision-making and to gambling proneness. As increases in the stress hormone cortisol have been shown to disrupt IGT decision-making in men, time-dependent effects of corticosterone - the rodent equivalent of cortisol - on decision-making were addressed by treating male rats at two different time-points prior to testing in the rodent version of the IGT. Administration of corticosterone 30 min prior to testing disrupted reward-based decision-making. This was associated with increased c-Fos expression in a number of areas, i.e. the lateral orbitofrontal cortex, infralimbic cortex and insular cortex. Administration of corticosterone 3 hours prior to testing had no effect on task-performance. In line with systemic injections, infusions of corticosterone in the infralimbic cortex disrupted reward-based decision-making. The data for the lateral orbitofrontal cortex were inconclusive, possibly due to (unintended) damage, preventing treatment differences to be observed. Finally, time-dependent effects of stress on social decision-making were addressed. Earlier studies showed that experimentally induced stress in men is associated with lowered generosity, and altered altruistic punishment in a time-dependent manner. In contrast, women stress altruistic punishment or general altruism in women was not affected in a time-dependent manner by stress. This may be due to, among other things, the use of hormonal contraceptives, a condition that is associated with a more blunted stress-induced cortisol response. In professional environments, such as in health care, financial business, police and military, decisions are made under stress. Taking wrong decisions may have great personal and societal consequences. Therefore, increasing our knowledge of the differential effects of stress on decision-making in men and women, including the underlying brain circuits, is of great societal relevance.

Hedendaags is sprake van een bijna universele onderschrijving van het recht op gezondheid (en het recht op gezondheidszorg als onderdeel daarvan). Ondanks de groeiende wettelijke erkenning van het recht op gezondheidszorg, blijkt uit empirisch onderzoek dat de implementatie van dit recht grotendeels retorisch blijft op het nationale niveau. Hoewel China bijvoorbeeld het IVESCR in 2001 heeft geratificeerd, is weinig aandacht besteed aan de tenuitvoerlegging van het recht op gezondheidszorg op nationaal niveau. Schendingen van dit recht zijn dan ook vastgesteld. Aangezien de hervorming van de gezondheidszorg in China de zogenaamde 'diepwaterzone' ingaat, is het van cruciaal belang voor de Chinese overheid om te onderzoeken hoe iedereen gelijke toegang tot gezondheidszorg te garanderen. Het doel van deze studie is om bestaande tekortkomingen in China's tenuitvoerlegging van het recht op gezondheidszorg te identificeren. Om vervolgens de resterende uitdagingen te onderzoeken in het kader van het mensenrechten-concept accountability (de plicht van overheden om verantwoording af te leggen). En daaropvolgend aanbevelingen te doen aan Chinese wet- en beleidsmakers voor het implementeren van de Chinese verplichtingen onder het recht op gezondheidszorg middels de conceptwet Basic Health Law. De centrale onderzoeksvraag is daarom of en hoe het 'verantwoording afleggen' (accountability) het recht op gezondheidszorg kan bevorderen in het licht van China's unieke politieke, juridische en sociaal-maatschappelijke context. Op deze wijze worden in deze studie twee verschillende concepten gesynthetiseerd: (1) het recht op gezondheid; en (2) accountability. Beide concepten worden zo in een analytisch raamwerk voor 'het recht op gezondheid-gerelateerde accountability' geïntegreerd. Op basis hiervan beoogt de studie een constructief verantwoordingsmodel te bieden dat toegepast kan worden op specifieke gezondheidsproblemen, zowel in China als in andere landen, met name landen met niet-electorale regimes. ; Currently, there is an almost universal commitment to the right to health (care). However, despite the growing legal recognition of this right, empirical evidence suggests that the domestic implementation of the right to health (care) around the globe remains largely rhetorical. For example, although China ratified the International Covenant on Economic, Social and Cultural Rights in 2001, relatively little attention was paid to the domestic implementation of the right to health care. Violations of this right were also identified in reality. Given that China's health care reform is entering into the so-called 'deep-water' zone, it is of vital importance for the Chinese government to guarantee everyone equal access to health care. This study aims to identify existing shortcomings in China's domestic implementation of the right to health care, and to address the remaining challenges through the lens of accountability. Having done so, this study offers recommendations for Chinese law- and policy-makers for implementing China's obligations under the right to health care through the draft Basic Health Law. Therefore, the central research question is whether and how 'accountability' could advance the right to health care in light of China's unique political, legal and social background. In doing so, this study synthesises two different concepts: (1) the right to health; and (2) accountability, and integrates them into an analytical framework for 'right to health-based accountability'. The study is expected to establish a constructive accountability model that can be applied to specific health concerns in China, as well as in other countries, particularly those with non-electoral regimes.

Neighbourhood deprivation monitoring in Rotterdam and London: exploring barriers to evidence-based policy and practice There is ample evidence that area-based approaches to tackling health inequalities, as part of a wider policy of community regeneration, are effective. Nevertheless, embedding this evidence in the routine practice of health professionals has not followed automatically. One of the barriers to the uptake of research is the process by which evidence is generated and its usability, or "stickiness". This paper draws on the concept of stickiness to explore the role of deprivation monitoring data in creating an evidence base for neighbourhood health policies and intervention. The study was undertaken as part of a Knowledge Exchange Programme aimed at sharing learning to improve the participation and health of disadvantaged people in deprived neighbourhoods in Rotterdam and London. The two cities are similar in that they both have highly diverse populations and government health and social policies that employ area-based approaches to tackle deprivation. Documentary analysis and in-depth interviews with health professionals and policymakers in the two cities have explored the construction of health policy, the congruence between data on deprivation and the contextual experience of practitioners, and the factors that influenced the usability of the data. Monitoren van achterstand op wijkniveau in Rotterdam en Londen: een verkenning van obstakels binnen "evidence-based" beleid en praktijk Een gebiedsgerichte aanpak om achterstandswijken te vernieuwen is effectief gebleken om ongelijkheid op het gebied van gezondheid aan te pakken. Toch wordt het bewijs voor de werkzaamheid van deze aanpak niet automatisch ingebed in de dagelijkse werkpraktijk van professionals in de gezondheidssector. Een van de factoren waardoor kennis uit onderzoek niet wordt opgenomen ligt besloten in het proces van kennisverwerving en de bruikbaarheid van de kennis, ofwel de "kleeffactor" van onderzoeksresultaten. Dit artikel bouwt voort op het concept van de kleeffactor binnen de context van het monitoren van achterstand en de rol die het monitoren kan spelen in het vinden en toepassen van evidence-based gezondheidsbeleid en interventies op wijkniveau. Het onderzoek is uitgevoerd binnen een breder project, Everybody on Board, dat zich richt op vergroting van de participatie en verbetering van de gezondheid van groepen mensen met een achterstand in een aantal geselecteerde wijken in Rotterdam en Londen. In beide steden zijn interviews afgenomen bij professionals en beleidsmakers in de gezondheidssector om zo het beleid, de congruentie tussen statistische data en het beeld dat de professional van de werkelijkheid heeft, en factoren die hun gebruik van deze data beïnvloeden, in kaart te brengen.

Nearly a quarter of the global disease burden and premature mortality is expected to be caused by environmental exposures, and the environmental health burden in children is even larger. Among the most hazardous environmental risk factors are tobacco smoke and ambient air pollution. Also temperature extremes have been linked to different adverse health outcomes, which is a concern because of the expected increase in extreme weather events due to climate change. It is well accepted that prenatal and postnatal exposure to second-hand tobacco smoke has detrimental effects on fetal and child health. However, evidence that smoke-free legislations are associated with health benefits early in life was limited before 2013. Also early life health effects of ambient temperature and air pollution are a relatively novel research domain. The number of studies investigating the association between temperature and preterm birth is limited, and results are inconclusive. Concerning fetal growth, evidence for adverse effects of air pollution is cumulating, but results are inconsistent with respect to the exposure window. Finally, although animals may serve as useful sentinels for the effects of environmental exposures on human health, epidemiological findings for human have seldom been replicated in animal populations. In this dissertation, we used population-level registry data to investigate health effects of different environmental exposures and to evaluate the health impact of preventive public health measures, including health outcomes in newborns and in the older segment of the population. The specific objectives of this dissertation were: 1. The assessment of potential public health benefits of smoking policies, focusing on health outcomes in early and in later life. For this purpose, we studied the impact of the stepwise implementation of smoke-free legislation in Belgium on the risk of preterm birth and on the risk of mortality due to acute myocardial infarction. 2. The investigation of the association between environmental exposures and adverse birth outcomes. First, we examined the risk of preterm birth in relation to ambient temperature by investigating potential heat and cold effects. Second, we studied fetal growth in relation to maternal exposure to PM10. For the latter, we focused on birth weight and small-for-gestational-age and we assessed potential effect modification by pregnancy duration and potential non-linearity in the exposure-response shape. 3. The exploration of the use of dairy cattle populations as comparative models of human health risks. For this purpose, we investigated the association between dairy cow mortality and ambient O3, PM10, NO2 and temperature, accounting for harvesting and delayed effects. ; Men schat dat bijna een kwart van de globale ziektelast en vroegtijdige sterfte veroorzaakt wordt door blootstelling aan omgevingsfactoren, en de impact van het milieu op de gezondheid van kinderen is zelfs nog groter. Tabaksrook en luchtverontreiniging behoren tot de meest schadelijke omgevingsblootstellingen. Ook blootstelling aan zeer lage of hoge buitentemperaturen kan leiden tot verscheidene nadelige gezondheidseffecten, wat een probleem vormt in de context van de verwachte toename in extreme weersomstandigheden door de klimaatsverandering. Het is algemeen aanvaard dat prenatale en postnatale blootstelling aan tabaksrook schadelijk zijn voor de gezondheid van de foetus en het kind. Toch was er vóór 2013 nog bijna geen wetenschappelijk bewijs dat rookvrije wetgevingen al vroeg in het leven gezondheidswinst opleveren. Ook het onderzoek naar perinatale gezondheidseffecten van omgevingstemperatuur en luchtverontreiniging is relatief nieuw. Slechts een beperkt aantal studies onderzocht het verband tussen temperatuur en vroeggeboortes, en de resultaten zijn niet eenduidig. Hoewel er groeiend wetenschappelijk bewijs is dat blootstelling aan luchtverontreiniging schadelijk is voor de groei van de foetus, zijn de resultaten van studies inconsistent wat betreft de periode van de zwangerschap waarin de foetus het meest gevoelig is aan zulke blootstelling. Hoewel dieren kunnen gebruikt worden als rolmodellen voor de effecten van milieufactoren op de gezondheid van de mens, werden epidemiologische bevindingen bij de mens slechts zelden bevestigd in dierenpopulaties. In dit proefschrift gebruikten we registergegevens op populatieniveau om de gezondheidseffecten van verscheidende milieufactoren te onderzoeken en om de impact van preventieve volksgezondheidmaatregelen te evalueren. Hiervoor keken we naar gezondheidsuitkomsten in pasgeborenen en in een ouder deel van de populatie. De specifieke doelstellingen van dit proefschrift waren: 1. Een inschatting maken van mogelijke gezondheidsvoordelen ten gevolge van rookwetgevingen, gericht op aandoeningen zowel vroeg als later in het leven. Hiervoor onderzochten we de impact van de stapsgewijze invoering van het rookverbod in België op het risico op vroeggeboorte en op het risico op sterfte door een hartaanval. 2. Het onderzoeken van het verband tussen omgevingsblootstellingen en negatieve zwangerschapsuitkomsten. Om te beginnen bestudeerden we de effecten van lage en hoge buitentemperaturen op het risico op vroeggeboorte. Daarnaast gingen we na of er een verband is tussen foetale groei en blootstelling van de moeder aan luchtverontreiniging. Hiervoor keken we naar geboortegewicht en naar het risico om te licht te zijn rekening houdend met de zwangerschapsduur ("small-for-gestational-age"). We keken ook naar mogelijke effect modificatie door zwangerschapsduur en mogelijke nietlineariteit in het verband tussen blootstelling en uitkomst. 3. De exploratie van het gebruik van melkkoeien als vergelijkende modellen voor menselijke gezondheidsrisico's. Hiervoor onderzochten we de associatie tussen sterfte bij melkkoeien en concentraties van ozon (O3), fijn stof (PM10) en stikstofdioxide (NO2) in de buitenlucht. Daarnaast keken we ook naar de effecten van extreme buitentemperaturen. In de analyses hielden we rekening met mogelijke "harvesting" en vertraagde blootstellingseffecten.

Deze richtlijn gaat over agressie en geweld in de werkomgeving. De problematiek van agressie en geweld krijgt met grote regelmaat aandacht in de media en de politiek. Zo heeft het Nederlandse ministerie van Binnenlandse Zaken en Koninkrijksrelaties (BZK) in 2007 het programma Veilige Publieke Taak gestart om agressie en geweld in de publieke sector terug te dringen. Werknemers in de publieke sector staan in nauw contact met klanten en burgers. Maar werknemers in de publieke sector zijn niet de enigen die geconfronteerd kunnen worden met agressie en geweld in de werkomgeving. In iedere sector kunnen werknemers geconfronteerd worden met agressie en geweld door personen in leidinggevende posities of door directe collega's. Volgens recente peilingen worden vooral mensen werkzaam in het openbaar vervoer, uitkeringsinstanties, zorg en welzijn, onderwijs, politie, horeca, gerechts-deurwaarders, vrachtwagenchauffeurs en tankstations regelmatig geconfronteerd met agressief gedrag, bedreigingen en fysiek geweld (Inspectie SZW, 2012). De Arbeidsomstandighedenwet 2009 (artikel 3, lid 2) verplicht werkgevers om beleid te voeren dat "… gericht is op voorkoming en indien dat niet mogelijk is beperking van psycho-sociale arbeidsbelasting". Agressie en geweld worden in de wet (artikel 1, lid 3e) expliciet genoemd als belangrijke aspecten van de psychosociale arbeidsbelasting. Een en ander betekent dat werkgevers beleid zullen moeten voeren om te voorkomen dat hun werknemers tijdens de uitoefening van hun werkzaamheden te maken krijgen met agressie en geweld, alsmede dat ze de gevolgen daarvan voor de gezondheid en het welzijn van deze werk-nemers minimaliseren. Daarom is deze richtlijn ontwikkeld met als doel aanbevelingen te geven voor interventies en oplossingen die een bijdrage leveren aan het voorkomen van en omgaan met agressie en geweld in het werk.

Curaҫao is a small island nation approximately 70 kilometers north of Venezuela in the Caribbean Sea. The Waitt Institute's Blue Halo Curaҫao program encourages the sustainable use and protection of Curaҫao's marine resources and a priority has been to designate more protected areas. In September of 2018, Klein Curaҫao, a small uninhabited island off the coast of Curaҫao, was designated a Ramsar Wetland of International Importance. Although sparsely vegetated, Klein Curaҫao serves as an important nesting and breeding ground for various species of sea birds, most notably the least tern (Sternula antillarum), as well as the endangered green sea turtle (Chelonia mydas). In addition, the fringing coral reefs found off the coast are some of the healthiest coral reefs in Curaҫao, with high percent of coral cover, fish biomass, and fish and coral biodiversity. However, the island has become a hotspot for tourists, and unregulated tourism is one of the principal threats to the Klein Curaҫao's ecological character. The designation of the island as a Ramsar Wetland of International Importance requires a management plan to be drafted that ensures the conservation of the ecological character of the island while also promoting the sustainable use of its resources for the users of the island. With the help of the Waitt Institute, CARMABI, Scripps Institution of Oceanography, the Curaҫaoan Ministerio di Tráfiko, Transporte I Planifikashon Urbano (Ministry of Traffic, Transport & Urban Planning), or VVRP, and the Ministerie van Gezondheid Milieu en Natuur (Ministry of Health & the Environment), or GMN, I wrote a management plan that encompasses the goals of the Ramsar Convention: to protect Klein Curaҫao's marine and terrestrial environments and allow the sustainable use of those environments. The methodology consisted of a preliminary literature review, a site visit to Klein Curaҫao, and meetings with local stakeholders, as well as government officials from VVRP and GMN. The management plan has been revised to take into account feedback given through the stakeholder engagement and meetings with the ministry officials and will be submitted to the government of Curaҫao for review and implementation.

Background: Particulate matter (PM) exposure leads to premature death, mainly due to respiratory and cardiovascular diseases. Objectives: Identification of transcriptomic biomarkers of air pollution exposure and effect in a healthy adult population. Methods: Microarray analyses were performed in 98 healthy volunteers (48 men, 50 women). The expression of 8 sex-specific candidate biomarker genes (significantly associated with PM10 in the discovery cohort and with a reported link to air pollution-related disease) was measured with qPCR in an independent validation cohort (75 men, 94 women). Pathway analysis was performed using Gene Set Enrichment Analysis. Average daily PM2.5 and PM10 exposures over 2-years were estimated for each participant's residential address using spatiotemporal interpolation in combination with a dispersion model Results: Average long-term PM10 was 25.9 (± 5.4) and 23.7 (±2.3) µg/m3 in the discovery and validation cohorts, respectively. In discovery analysis, associations between PM10 and the expression of individual genes differed by sex. In the validation cohort, long-term PM10 was associated with the expression of DNAJB5 and EAPP in men and ARHGAP4 (p=0.053) in women. AKAP6 and LIMK1 were significantly associated with PM10 in women, although associations differed in direction between the discovery and validation cohorts. Expression of the 8 candidate genes in the discovery cohort differentiated between validation cohort participants with high vs low PM10 exposure (area under the receiver operating curve = 0.92; 95% CI: 0.85, 1.00; p=0.0002) in men, 0.86; 95% CI: 0.76, 0.96; p=0.004 in women). Conclusions: Expression of the sex-specific candidate genes identified in the discovery population predicted PM10 exposure in an independent cohort of adults from the same area. The ubiquitous use of organophosphate flame retardants and plasticizers (PFRs) in a variety of consumer products has led to widespread human exposure. Since certain PFRs are developmental and carcinogenic toxicants, detailed exposure assessments are essential to investigate the risk associated with environmental exposure levels. However, such data are still lacking for European countries. In this study, concentrations of thirteen PFR metabolites were measured in urine samples from 600 adolescents from Flanders, Belgium. 1-Hydroxy-2-propyl bis(1-chloro-2-propyl) phosphate (BCIPHIPP), diphenyl phosphate (DPHP), bis(1,3-dichloro-isopropyl) phosphate (BDCIPP), 2-hydroxyethyl bis(2-butoxyethyl) phosphate (BBOEHEP), 2-ethylhexyl phenyl phosphate (EHPHP) and 2-ethyl-5-hydroxyhexyl diphenyl phosphate (5-HO-EHDPHP) were frequently detected (>83%) in all participants. Comparisons with study populations from outside the EU showed that urinary levels of DPHP, BDCIPP and BCIPHIPP were generally within the same range. Only exposure to 2-ethylhexyl diphenyl phosphate (EHDPHP) was presumably higher in Flemish adolescents. However, determinants analysis through multivariate regression analyses did not reveal significant predictors that may explain this finding. Significantly higher levels of BDCIPP were observed in participants with new decorations at home, while adolescents with highly educated parents had higher levels of BBOEHEP and BDCIPP. Furthermore, multiple PFR metabolite concentrations followed a seasonal pattern. Estimated daily intakes (EDIs) were calculated from the internal dose by including fractions of urinary excretion (FUE) estimated in in vitro metabolism studies. EDIs ranged from 6.3 ng/kg bw/day for TBOEP to 567.7 ng/kg bw/day for EHDPHP, which were well below the available oral reference doses for all investigated PFRs. This suggests that the associated risk is low at present. This is the first report on internal exposure to seven commonly used PFRs in a European population. ; We thank the participating adolescents and their families. Sociale relaties tussen sociale actoren, zoals mensen, bedrijven en landen, en de eigenschappen (gedrag, mening, etc.) van deze actoren zijn niet statisch: ze veranderen over de tijd in wederzijdse samenhang. Iemand kan bevriend raken met een ander vanwege een gedeelde politieke voorkeur, of zijn eigen politieke kleur veranderen naar aanleiding van interacties met zijn vrienden. Het eerste is een voorbeeld van sociale selectie, een proces waarbij actoren hun relaties veranderen op basis van de eigenschappen van andere actoren. Het tweede is een voorbeeld van sociale beïnvloeding, een proces waarbij het gedrag van actoren beïnvloed wordt door de actoren met wie zij een relatie hebben. Het stochastisch actor-georiënteerde model is ontwikkeld om dergelijke dynamiek statistisch te modelleren, gebaseerd op herhaalde metingen van netwerken en actoreigenschappen. In de oorspronkelijke definitie van het model wordt aangenomen dat actoreigenschappen gemeten zijn op een ordinaal categorische schaal. Ten gevolg hiervan hebben meerdere onderzoekers hun continue variabelen moeten discretiseren om deze te kunnen analyseren in het model. Door te discretiseren raakt informatie verloren. In ons onderzoek hebben wij het stochastisch actor-georiënteerde model daarom uitgebreid voor continue actoreigenschappen. Dit nieuwe model is breed toepasbaarheid, omdat in de gezondheids-, onderwijs- en organisatiesociologie, waar men geïnteresseerd in het effect van sociale netwerken op individueel gedrag, veel actoreigenschappen op continue schaal gemeten worden. ; Social relations between social actors, such as people, companies and countries, and the attributes (behavior, opinion, etc.) of these actors are not static: they evolve interdependently over time. People may befriend others with similar political opinions or change their own opinion based on those of their friends. The further is an example of social selection, a process where actors change their relations based on the attributes of other actors. The latter is an example of social influence, a process where the behavior of actors is affected by the actors with whom they have a certain relation. The stochastic actor-oriented model has been developed to statistically analyze this type of dynamics, based on repeated measures of networks and actor attributes. In the original definition of the model, it is assumed that actor attributes are measured on an ordinal categorical scale. As a consequence, multiple researchers had to discretize their continuous actor variables to fit them in the modeling framework. Discretization leads to a loss of information. Therefore, we have extended the stochastic actor-oriented model for continuous actor attributes. This new model is widely applicable, as in health, educational and organizational sociology, where one is interested in the effects of social networks on individual behavior, many actor attributes are measured on a continuous scale.

Slechts een op de drie mensen met een lichamelijke beperking sport regelmatig, terwijl tweederde van de mensen zonder lichamelijke beperking actief is in sport. Dit verschil is spijtig omdat sportdeelname juist ook voor mensen met een lichamelijke beperking vele gezondheidsvoordelen heeft, zoals een verminderde kans op hart- en vaatziekten en overgewicht. Om sportdeelname van mensen met een lichamelijke beperking te stimuleren, is het belangrijk te achterhalen waarom zij niet sporten, om vervolgens gerichte adviezen en maatregelen te nemen. Uit door ons uitgevoerde onderzoeken bij verschillende groepen mensen met een lichamelijke beperking, zowel sportende als niet sportende mensen, blijkt dat stimulansen vooral plezier en gezondheid zijn, naast steun vanuit de omgeving en sociale contacten. Belemmeringen om te sporten zijn vaak gerelateerd aan de onderliggende beperking of zijn van logistieke aard (sportfaciliteiten, transport, etc). Om mensen met een lichamelijke beperking meer te laten sporten moeten ze inzien en bij voorkeur ook ervaren hoe goed en leuk sporten kan zijn. Sport is een onderdeel van de revalidatiebehandeling en het revalidatieteam helpt mensen door informatie te geven over verschillende sportmogelijkheden en helpt bij het zoeken naar een geschikte sport. Ook de politiek zal haar verantwoordelijkheid voor het verbeteren van de gehandicaptensport moeten nemen door betere sportcondities te creëren, zoals het verbeteren van de sportfaciliteiten en transport en vergoedingen voor sporthulpmiddelen. Gemeentes zouden sportclubs moeten stimuleren om mensen met en zonder lichamelijke beperking samen te laten sporten. Want sport is er voor iedereen. ; Only one-third of people with physical disabilities regularly participate in sports, compared to two-thirds of people without physical disabilities. Benefits of sports participation for people with physical disabilities include reducing chances of heart disease and obesity. In order to try to increase sports participation for people with physical disabilities, it is important to understand what withholds them from sports and how they can be stimulated to become active in sports. Results from this research amongst active and inactive people with a physical disability show that facilitators are fun and health, as well as support from family and friends and social contacts. Barriers are mostly related to the underlying disability or related to environment such as adaptation of facilities or lack of transport. In order to increase sports participation of people with physical disabilities, they have to see and experience how much fun sports are and how they physically benefit from sports. Sports is part of the rehabilitation process and rehabilitation professionals help to get people motivated by providing information about possibilities of disability sports or other suitable sports. Politics should also take responsibility for improving disability sports by enhancing infrastructure and providing funding for assistive (sport) devices. Municipalities have to integrate sports clubs for people with and without physical disabilities, to increase sports possibilities for people with physical disabilities. Because sports participation is for everybody.

Er is een analyse van het beleid uitgevoerd; een enquête onder verschillende betrokken actoren geeft aan hoe zij het beleid beoordelen op grond van hun ervaringen. De respondenten zijn verdeeld in vier subgroepen, te weten adviesbureaus, overheid, industrie en burgerorganisaties. Daarnaast geeft een beschrijving van de fysiologie van geurwaarneming en geurbeleving inzicht in de biologische achtergrond van hinderklachten. Het Nederlandse geurbeleid, dat voornamelijk is toegespitst op industrie en landbouw, berust op het vaststellen van een acceptabel hinderniveau per geurbron. Dit hinderniveau wordt vastgelegd in de milieuvergunning van de betrokken inrichting. Lokale overheden zijn verantwoordelijk voor het verlenen van milieuvergunningen en ook voor het bepalen van het acceptabel hinderniveau. De doelstellingen van het landelijk beleid (1995) zijn dat in 2000 maximaal 12% van de Nederlandse bevolking geurhinder mag ondervinden en dat in 2010 geen ernstige hinder meer mag voorkomen. Om dit te realiseren zijn er regels en richtlijnen die voorschrijven hoe het acceptabel hinderniveau moet worden vastgesteld, zoals de Nederlandse emissierichtlijn (NeR) en de Wet geurhinder en veehouderij (Wgv). Dit onderzoek heeft geleid tot een aantal conclusies over de aanpak van geurproblematiek in Nederland. • Geurhinder is in de afgelopen 10 jaar afgenomen. Het percentage geurgehinderden daalde van 46% naar 30%. In 1995 waren landbouw en industrie de grootste bronnen van geurhinder in Nederland; op dit moment zijn dat de huishoudens. In het beleid is aandacht nodig voor deze verandering in de bronnen van geurhinder. • Het huidige geurbeleid wordt als redelijk succesvol beschouwd door een groot deel van de respondenten van de enquête. Vooral de lokale aanpak van geurproblematiek wordt gewaardeerd en moet behouden blijven. • Het ontbreekt aan een goede handreiking om het acceptabel hinderniveau vast te stellen. Respondenten van de enquête geven aan behoefte te hebben aan een eenduidige methode om het acceptabel hinderniveau vast te stellen. • Een landelijk toetsingskader zou meer houvast kunnen bieden voor het bepalen van het acceptabel hinderniveau. Dit toetsingskader moet genoeg ruimte bieden voor een lokale aanpak van de geurproblematiek. • Het instrumentarium voor de analyse van geur in de omgeving schiet tekort. Er is geen uniforme methode voor het meten van geurhinder als basis voor het vaststellen van het acceptabel hinderniveau. • Er is geen ruimte in het beleid om gezondheidsrisico's te betrekken in de beoordeling van een geursituatie. Respondenten van de enquête vinden dat behalve de geurverspreidende inrichting, de overheid ook verantwoordelijkheid moet nemen op dit gebied. Meer onderzoek naar de effecten van geur op de gezondheid zou richting kunnen geven aan het geurbeleid. • Hinderklachten kunnen mogelijk worden verminderd door betere communicatie tussen de verschillende betrokken partijen. Stress kan vermeden worden als een situatie beter voorspelbaar en controleerbaar wordt. Een goede klachtenafhandeling kan bijvoorbeeld irritatie bij gehinderden verminderen en zorgen voor meer draagvlak en begrip bij alle partijen. Ook informatie over het productieproces en de vrijgekomen stoffen kunnen hindergevoelens doen afnemen. Samengevat, een landelijk toetsingskader kan meer houvast bieden bij het vaststellen van het acceptabel hinderniveau, maar er moet ruimte blijven voor een lokale beoordeling. In de toekomst moet er aandacht zijn voor de ontwikkeling van nieuwe meetmethoden en onderzoek naar de effecten van geur op de gezondheid. Op de korte termijn kan betere communicatie tussen inrichtingen, overheid en burgers leiden tot meer draagvlak voor genomen beslissingen en afname van hinder. Daarnaast moeten beleidsmakers anticiperen op de toenemende mate van hinder vanuit huishoudens. Abstract This report presents an evaluation of the Dutch odour policy. An analysis of the policy is supported by a questionnaire among several actors involved. The result of this questionnaire is an overview of the way that policymakers, citizens and others evaluate the current odour policy. A review of the biological systems involved in odour perception shows how health complaints originate. The Dutch odour policy focuses on industrial and agricultural odours and states an acceptable level of odour for each situation. This level of accepted odour is laid down in law by the environmental permit of the source. Local governmental institutions are responsible for granting the permits and controlling the odour emissions. The goals for the Dutch odour policy are a maximum of 12% nuisance in 2000 and no severe nuisance in 2010. To realise these goals several guidelines and rules are established, for example the NeR (Dutch emission guidelines) and the Wgv (Law for odour nuisance and livestock farming). The evaluation leads to several conclusions on the handling of odour problems in the Netherlands. • Odour nuisance has decreased over the past 15 years. The percentage nuisance decreased from 46% to 30% (annoyed persons in the Netherlands). In 1995 industrial and agricultural sources were the biggest cause of odour nuisance. Today the main source are households. Policymakers have to realise there is a change in the cause of odour nuisance. • The current policy is considered reasonably successful. Especially the local approach of the policy is highly valued by the respondents of the questionnaire. • Determining an acceptable level of odour emission is difficult, more unambiguous guidelines can help to make this more explicit. A national framework can help determine the acceptable level; this has to include an aspect of local decision making. • The tools to determine odour emissions need improvement. The respondents point out that there is no uniform method available to determine odour nuisance. Several methods have to be combined to get a good estimation of the odour nuisance. The electronic nose is promising for the future of odour determination. It might even be possible to combine odour concentrations and annoyance in this method; it is also suitable to monitor a situation 24 hours a day. • There is no method available to include health risks in the determination of the acceptable level of odour. Respondents of the questionnaire state that the government as well as the odour emitting company has to take responsibility in decreasing health risks as a result of odour nuisance. • Nuisance might decrease when there is sufficient communication between sources, government and citizens. Stress is one of the main causes of nuisance. Stress originates from uncontrollable situations; the situation becomes more controllable when risks are communicated. Information on the process is essential to decrease stress in citizens. Concluding, a national framework might improve the way to determine acceptable odour levels. According to the respondents of the questionnaire there has to be room for a local evaluation of the situation. In the future more tools to determine odour emissions have to be developed. Communication between industrial and agricultural plants, policymakers and citizens has to be improved to minimize stress as a result of odour nuisance. Policymakers also have to anticipate on the shift in the cause of odour nuisance. Households are the main source of nuisance nowadays, but there is no sufficient policy for this group.
