Dans le corpus épigraphique crétois, quatre inscriptions d'époque hellénistique relatives à des médecins sont adressées à des cités dont ces hommes étaient originaires (Cos, Kasos). L'analyse de ces textes met en lumière leur contexte historique ; la comparaison linguistique des documents crétois avec des inscriptions de Cos et de Rhodes (cette cité contrôlait politiquement la petite île de Kasos) prouve que la koina dorienne et surtout la koinè ionienne-attique avaient pénétré dans ces dialectes. ; In the epigraphical Cretan corpus, four Hellenistic inscriptions relating to physicians are addressed to the cities of which these men were native (Kos, Kasos). The analysis of these texts throws light on their historical context; the linguistic comparison of the Cretan documents with Koan and Rhodian inscriptions (Rhodes controled politically the small island of Kasos), makes evident that Dorian koina and above all Attic-Ionic koine had penetrated into these dialects.

This thesis study the relations between medical world and local political life. With the results of the local elections between 1959 and 1989, it tries to show that the research of a general pattern is not available. Physicians identities in the context of local election come from a double process of differenciation. The professional status give to each physician the symbolic power of the profession. The professional role eases the knowledge of the personality. But the elective identity construction has to deal with political realities. When physicians want to reach several elective positions, they have to find an agreement between their individual perspective and the collective frame of the political life. Their investment in the public institutions life may be read according differenciation and no differenciation. Some of them are involved in specialized institutional sector like helthe and social policies. In this case, we can say that thre is the continuity between the professional habitus and the local institutional frame. Others occupy positions which need perspective and knowledge-ability quite different. It's why physicians local representant may be more representan than medicals. ; À partir des résultats des éléctions cantonales et municipales dans les cinq départements d'Aquitaine, entre 1959 et 1989, la thèse propose une réflexion sur l'identité électorale des médecins dans les situations de compétition politique et sur la compétence qu'ils peuvent mettre en œuvre lorsqu'ils accèdent aux positions institutionnelles locales. Considérant qu'il n'est pas pertinent de chercher un modèle général d'analyse pour un ensemble d'agents, de trajectoires et de contextes marqué par une grande hétérogénéité, elle fait le choix d'observer les différents espaces de reconnaissance que doit franchir le médecin quand il se trouve engagé dans ce type de situation. Introduction: Patients increasingly want to be treated at home and the government wants to develop Hospital at home (HH).Objective: We aim at determining knowledge of practitioners in Guadeloupe about this new form of hospitalization and the reservations, which could reduce HH use. Methods: A transversal study was conducted in Guadeloupe from the 1st of June 2019 and the 7th of July 2019 on general practitioners, hospital practitioners and medicine students, except physicians who had already worked in a service of HH. Data was collected anonymously. The following were explored: HH use, knowledge on HH, positive and negative reservations of physicians, the benefits of HH. We have looked for a link between the number of negative concerns and the level of knowledge or the lack of HH's use. We have studied possible differences between the three groups of physicians regarding the knowledge, negative perceptions and the benefits of HH.Results: Out of 843 physicians surveyed, 99 were included. 79.8% of the physicians had already asked for an HH. The level of knowledge was insufficient with an average < 75% of correct answers and no difference between the groups. The main positive reservations were the respect for the patient's wish to stay at home, improvement of their quality of life and the presence of nurses who have been prepared for the management of difficult situations. The main concerns were the risk of family being exhausted, the very different management according to HH services and the fact that the patient was sometimes kept at home, which was sometimes inappropriate. The physicians with less knowledge, had a more reservations (p = 0.041) but there was no link between the number of negative perceptions and the lack of request (p = 0.19). General practitioners had more concerns than other physicians (p = 0.035) and they considered that the benefit of HH was lower (p = 0.011).Conclusion: Our study showed the interest of informing physicians in order to develop the use of HH in Guadeloupe and to decrease the reluctances. ; Introduction : Les patients souhaitent de plus en plus être soignés à leur domicile et les pouvoirs publics veulent développer l'Hospitalisation à Domicile (HAD). Decision-making in a public hospital is complex and subject to frequent legislative and regulatory changes. The rules of decision-making – an essential determinant of service quality – are defined not only by Law and regulations, but also by actual practices.This Dissertation analyses the relationships between the two types of actors predominantly involved in decision-making: managers and physicians.These actors hold a major and complementary role in the two phases of the decision-making process. First, they take charge of a large part of the preparation of decisions, thanks to informal exchanges and formal committee meetings. Managers and physicians thus have an important role in the preliminary discussions preceding the decision. Second, they also play a major role in the decision-taking stage itself. Beyond the decision power – that belongs to the manager. Managers and physicians have a reciprocal influence in the final decision-making process.This study aims at demonstrating that the relationships between managers and physicians in the process of decision-making in a public hospital are based on dialogue, in particular in the preparation of decisions. They are also power relationships, in particular in the final phase of decision-making.Last, this Dissertation offers suggestions to improve the quality of the decision-making process in public hospitals. The thesis deals with Direct-To-Consumers Genetic Testing (DTCGTs) sold on the Internet, and more specifically with the ones for health, physiology, performance and behaviour. This market is booming since the end of the 90's, and is sparking off controversies which are rooted in multiples scientific and medical uncertainties ("missing heritability", informed consent, third parties, etc.). It doesn't require for medical prescription, and health professional consultancy for test data and results is not systematically needed, which is not allowed by some national legislations. For all that, is it a medicalisation phenomenon without physician ?To understand the socio-economic dynamic of this market, we had listed 130 websites (for 60 in the academic literature) and identified 155 entrepreneurs, then we studied the evolution of theirs characteristics. The territorial localisation of the market changed, and european websites are finally as numerous as theirs north-american counterparts. Either in technical devices conception, or in commercialisation, DTCGTs are a sign of the autonomy progression of outsiders from the medical field, if not of their intrusion in this field. Medical dispensing systems are growing alongside of DTC selling, notably through the physician hiring by the DTCGT companies (i.e. hotline selling). Furthermore, some outsiders are relatively new, for they are related to investment funds which are specialized in science and more or less independent from industrial groups. Furthermore, one must notice the presence of informatics and Internet companies which, till now, didn't really explore the health field. Ce travail vise à comparer les archives du Conseil National de l'Ordre des médecins (CNOM) et ceux du conseil départemental (CD) d'Ille et- Vilaine pendant la Seconde Guerre mondiale. Nous avons eu accès aux documents du CNOM envoyés au CD d'Ille et Vilaine, pour qui nous ne pouvons pas exclure un « blanchiment » des archives à la fin de la guerre. Le Conseil National était probablement plus impliqué politiquement, et sa direction a suivi les lignes directrices de l' « Etat français » et les demandes répétées pour l'identification des médecins juifs et étrangers sous la pression du Commissariat général aux questions juives, dirigé initialement par Xavier Vallat. En ce qui concerne le CD d''Ille et Vilaine, il est difficile d'avoir une idée claire des opinions de ses membres en relation avec les événements qu'ils ont vécus. Ils ont tout simplement comme il apparaît, suivi, comme la majorité des Français, l'évolution que nous connaissons maintenant, à partir d'un découragement presque totale à une résistance relative. Nous espérons que cette histoire va rejoindre celle de l'enseignement de l'éthique médicale, en particulier pour les jeunes étudiants en médecine et que cela débouche sur un inventaire complet de protection et un vrai travail sur les documents archivés mais fragiles. ; This work aimed at comparing the archives of the Supreme Council of the Physicians Order (SCO) and those of the County Council (CD) Ille et Vilaine in France during the second World War. We had access to documents from the SCO but only those sent to the CD of Ille et Vilaine, for which, indeed, we cannot frankly exclude an archives laundering at the end of the war. The national system i.e. the SCO, was probably more politically implicated, and followed the government guidelines of the French State and repeated requests for identification of Jewish and foreign doctors under the pressure from the General Commissariat for Jewish Questions, first directed by Xavier Vallat. Regarding the Ille et Vilaine CD, it is difficult to get a clear idea regarding the views of its members in relation to the events that they lived through. They appear simply have followed, as for the majority of French people, the evolution that we now know, from an almost total despondency to a relative resistance. We hope this story will join that of the teaching of medical ethics, especially to young medical students and overall that there will be a real work of protection and a more comprehensive inventory of documents.

International audience The National College of Teachers in Family Medicine developed a competence-based teaching model for family physicians' early curriculum, the «competence daisy » (CD).The CD model is a graphic representation of six core competencies necessary to a future practicing family physician.These competencies are implemented in primary care situations described in 11 categories of prototypic and exemplary situations. Patient-centered care (PCC) is an internationally validated systemic care model based on the work of M. Balint and C. Rogers. This pragmatic and eclectic model encompass both physician's and patient's perspectives. It is applicable by every physician but seems especially relevant to primary care. These two models, CD and PCC, are complementary and interact wich each other. The CD model is a pedagogical model: it gives meaning and coherence to competence acquisition for the health care role of future family physicians. The purpose of the PCC model is to give meaning to this health care role by putting the patient and his or her life plan (or personal health plan) at the center of the patient-doctor relationship. We hereby propose a reconfiguration of the systemic architecture and the dimensions of the PCC model that integrates the two models. Creating connexions between CD and PCC would link the CD model to the internationally validated PCC model, thus opening a field for research in academic family medicine. ; Le Collège national des généralistes enseignants (CNGE), a développé un modèle pédagogique d'apprentissage centré sur l'acquisition et le développement de compétences dans la formation initiale des médecins généralistes. Introduction: an inventory of the vaccination practices of general practitioners (GPs) is required before the COVID-19 vaccine campaign in the Windward Islands of French Polynesia. Materials & methods: an online questionnaire is sent to private and public registered physicians in primary health care in the Windward Islands from January 4 to 14, 2021. The aim is to analyze their attitudes and practices about recommended vaccines from childhood to adulthood. GPs' positions on COVID-19 vaccination are also studied. Results: response rate is 61 %. More than 9 GPs out of 10 recommend the entire vaccine program to patients, which they recommend to their relatives. More than 9 GPs out of 10 do not recommend the entire vaccine program to relatives, which they do not recommend to patients. GPs are in favor of vaccination (99 %). Half consider that they have insufficient initial medical training in vaccinology. An "authorized" vaccine is safe for 89 %. GPs are less confident in explaining vaccine safety (83%) and adjuvants' role (50%) with patients. For COVID-19 vaccination, 18% do not know, due to lack of information whether they will vaccinate their patients. Among them, two thirds cite insufficient initial medical training and 92% believe that pharmaceutical industry influences health authorities. An "authorized" vaccine is safe for 71 % of these GPs. Nearly 3 GPs in 10 do not trust the Polynesian and French health ministries and 2 GPs in 10 the European Medicines Agency. Conclusion: these results should be contextualized in January 2021, before the vaccine campaign in French Polynesia. Vaccinated GPs readily offer vaccines to their patients and are mostly young with sufficient initial studies about vaccines. GPs who do not vaccinate their patients do not vaccinate themselves, they are represented by older doctors with insufficient initial studies. Confidence in health policies and vaccine safety hamper the implementation of recommendations. For those who are reluctant to vaccinate patients against COVID-19, the impediments are similar but pharmaceutical industry's influence is stronger. It seems urgent to develop continuing medical training In Polynesia on vaccinology to avoid a drop in vaccination coverage and a risk of emergence of infections in insufficient vaccinated patients in the territory. ; Introduction : un état des lieux des pratiques vaccinales des médecins de soins de santé primaire s'impose à l'approche de la campagne vaccinale contre la COVID-19 dans les Îles du Vent de la Polynésie Française. Matériels & méthodes : un questionnaire en ligne est adressé aux médecins recensés des soins de santé primaire des Îles du Vent des secteurs privé et public du 4 au 14 janvier 2021. La moitié des médecins considère avoir eu une formation médicale initiale insuffisante sur la vaccinologie. Un vaccin « autorisé » est sûr pour 89 %. Les répondants ont moins confiance en eux pour aborder avec les patients la sécurité des vaccins (83 %) et le rôle des adjuvants (50 %). Concernant la vaccination contre la COVID-19, 18 % ne savent pas, par manque d'informations, s'ils vont vacciner leurs patients. Parmi eux, deux tiers évoquent une formation médicale initiale insuffisante. 92 % pensent que l'industrie pharmaceutique influence les autorités de santé. 71 % considèrent un vaccin « autorisé » comme sûr. Près de 3 médecins sur 10 n'ont pas confiance dans les ministères de la santé polynésien et français et près de 2 sur 10 dans l'agence européenne du médicament. Conclusion : ces résultats sont à contextualiser à janvier 2021, avant la campagne vaccinale en Polynésie. Les médecins vaccinés proposent volontiers les vaccins à leurs patients, et sont majoritairement jeunes avec une formation initiale suffisante. Les médecins ne vaccinant pas leurs patients ne se vaccinent pas, ils sont représentés par des médecins plus âgés avec une formation initiale insuffisante. La confiance dans les politiques de santé et dans la sécurité des vaccins sont un frein à l'application des recommandations. Concernant ceux qui hésitent à vacciner les patients contre la COVID-19, les freins sont similaires mais l'influence de l'industrie pharmaceutique est plus forte. Il parait urgent de développer la formation médicale continue en Polynésie sur la vaccinologie pour éviter une baisse de couverture vaccinale et un risque d'émergence d'infections chez des patients insuffisamment vaccinés sur le territoire.

The strategic medical evacuation (MEDEVAC) allows the airborne repatriation to the metropolis of soldiers injured or sick on missions. The aim of the study was to describe the epidemiology of strategic MEDEVACs and to analyze the role of the anesthetist-intensive care physician (MAR) in the management of critical situations in flight. All soldiers who benefited from a strategic MEDEVAC with a MAR on board between 1 January 2001 and 30 November 2017 were included. A total of 452 soldiers were repatriated, 245 (54%) for traumatic reasons and 207 (46%) for medical reasons, with 276 (61%) evacuations within 48 hours of the event. The median annual number of strategic MEDEVACs with MAR was 26 [20-32]. One hundred and fifty-five (34%) patients were intubated, 103 (23%) on catecholamines and 188 (42%) in an early postoperative setting. The median IGS-2 score was 13 [8-24]. One hundred and seventy-eight adverse events were identified, 123 (69%) related to a worsening of the patient's clinical condition and 30 (20%) related to a technical problem. Forty-seven (20%) patients without apparent initial failure worsened in flight. No deaths occurred on board and no flights had to be diverted for an uncontrolled situation. The strategic early MEDEVAC is a determining element of the potential for the engagement of the armed forces in operations avoiding the saturation of the medical structures on the spot. The MAR ensures the continuity of care for serious traumatic and medical patients, based on medical and aeronautical expertise requiring theoretical and practical training that should be based on these lessons learned. ; L'évacuation médicale (MEDEVAC) stratégique permet le rapatriement aéroporté vers la métropole de militaires blessés ou malades en missions. Le but de l'étude était de décrire l'épidémiologie des MEDEVAC stratégiques et d'analyser le rôle du médecin anesthésiste-réanimateur (MAR) dans la gestion des situations critiques en vol. Tous les militaires ayant bénéficié d'une MEDEVAC stratégique avec un MAR à bord entre le 1er janvier 2001 et le 30 novembre 2017 ont été inclus. Au total, 452 militaires ont été rapatriés, 245 (54%) pour cause traumatique et 207 (46%) pour cause médicale, avec 276 (61%) évacuations dans les 48 heures suivant l'évènement. Le nombre médian annuel de MEDEVAC stratégiques avec MAR était de 26 [20-32]. Cent cinquante-cinq (34%) patients étaient intubés, 103 (23%) sous catécholamines et 188 (42%) en contexte post-opératoire précoce. Le score IGS-2 médian était de 13 [8-24]. Cent soixante-dix-huit effets indésirables ont été recensés, 123 (69%) liés à une aggravation de l'état clinique du patient et 30 (20%) liés à un problème technique. Quarante-sept (20%) patients sans défaillance initiale apparente se sont aggravés en vol. Aucun décès n'est survenu à bord et aucun vol n'a dû être détourné pour situation incontrôlée. La MEDEVAC stratégique précoce est un élément déterminant du potentiel d'engagement des forcesarmées en opérations évitant la saturation des structures médicales sur place. Le MAR y assure la continuité des soins des patients graves traumatiques et médicaux, reposant sur une expertise médicale et aéronautique nécessitant une formation théorique et pratique continue qui devrait être basée sur ces retours d'expérience.
