Araştırma, kurumsal mantıkların bireylerin günlük faaliyetlerinde nasıl tezahür ettiğini incelemek amacıyla Türk sağlık alanında SGK anlaşmalı özel hastanede çalışan hekimler üzerinden tasarlanmıştır. Bu kapsamda bazı varsayımlar yapılmıştır. Araştırma; (a) Türk sağlık alanında çoklu kurumsal mantıkların mevcut olduğu (Meydan ve Yasit, 2015; Gürses ve Danışman, 2017; Özseven, 2020); (b) bu mantıkların hekimin davranış repertuarını (Friedland ve Alford, 1991; Thronton vd., 2012; Reay vd., 2017) genişlettiği; (c) hekimin profesyonel mantığın yanı sıra alanda mevcut olan diğer kurumsal mantıkları da içeren bir müdahale kitine (Swidler, 1986; McPherson ve Sauder, 2013) sahip olduğu; (d) bazı koşullar altında beklendiği üzere profesyonel mantığın değil, diğer kurumsal mantıkların reçete ettiği zihni şablonlardan birini seçerek günlük faaliyetlerini yürütmeye muktedir olduğu (McPherson ve Sauder, 2013; Currie ve Spyridonidis, 2016) ön kabulleri üzerine inşa edilmiştir. Böylelikle her bir kurumsal mantığın hekime tanı-tedavi planlama karar sürecinde bir davranışı reçete edeceği; hekimin bu reçetelerden hangisini seçeceğinin ise durumsal olduğu varsayılmaktadır. Meydan vd. (2020) bildirisinde, hekimin bu seçiminin; (a) hekimin örgüte afiliye olma biçimi (b) ücret rejimi ve (c) hastanın sağlık harcamalarını karşılama açısından statüsü olmak üzere üç durumsal faktör ile açıklanabileceği ifade edilmektedir. Böylece bu çalışmada hekimin mevcut 'müdahale kiti' içerisinden bir kurumsal mantığı durumsal faktörlere bağlı olarak hangi koşullar altında seçerek karar verebileceğine dair kavramsal bir model önerisi geliştirilmiştir. Hekimin davranış repertuarındaki kurumsal mantıklar, Türk sağlık alanında literatür incelemesi yapılarak; durumsal değişkenler ise her biri için ayrı ayrı aşamalar halinde olmak üzere doküman incelemesi ve mülakatlar gerçekleştirilmek suretiyle belirlenmiştir. Buna göre, durumsal değişkenlerin çeşitli varyasyonları gruplandırılarak hekim davranışına yön veren kurumsal mantıkların bir kurumsal mantık takımyıldızındaki görece konumlarının ne olduğu/olabileceği konusunda önermeler geliştirilmiştir. ; The research was designed on the physicians working in private hospitals contracted by SGK in Turkish healthcare field in order to investigate how institutional logics enacted in the daily activities of individuals. In this context, some assumptions have been made. (a) There are multiple institutional logics in the Turkish healthcare field (Meydan and Yasit, 2015; Gürses and Danışman, 2017; Özseven, 2020); (b) these instutional logics expand the physician's behavioral repertoire (Friedland & Alford, 1991; Thronton et al., 2012; Reay et al., 2017); (c) the physician has a "toolkit" (Swidler, 1986; McPherson & Sauder, 2013) that includes professional logic as well as other institutional logics available in the field; (d) physician is able to carry out its daily activities by choosing one of the cognitive frames prescribed by non-home institutional logics, not always professional logic as home logic (Godrick and Reay, 2011; McPherson and Sauder, 2013; Currie and Spyridonidis (2016)). Thus, it is assumed that each institutional logic will prescribe a behavior to the physician in the diagnosis-treatment planning decision process. It is assumed that which of these prescriptions the physician chooses is situational. It is stated that it in the research of Meydan et al. (2020); this choice of the physician can be explained by three situational factors: (a) the way the physician is affiliated with the organization, (b) the wage regime and (c) the status of the patient in terms of meeting health expenses. Thus, in this sudy, a conceptual model proposal has been developed regarding the conditions under which the physician can pick an istutional logic from the existing "toolkit" depending on situational factors. The institutional logics in the behavioral repertoire of the physician are determined by a literature review in the Turkish healthcare area; situational factors are determined by conducting a documentation analysis and interviews in separate stages for each. Accordingly, by grouping various variations of situational factors, suggestions have been developed about the relative positions of institutional logics in an institutional logic constellation that guide physician behavior.

Bu makalede sağlık hizmet anlayışındaki değişmelerin toplumun içinde bulunduğu sosyo-ekonomik ve kültürel yapının yanı sıra bilimsel gelişmelerle de yakından ilişkisi bulunduğu varsayımından hareket edilmiştir. "Sağlık Emek Sürecinin Dönüşümü" olarak da adlandırılan süreci anlayabilmek için mevcut çalışmalar Weed'in (2005) "sistematik tarama" (systematic review) ve Elias'ın (1977; 2000) "yöntembilimsel ilişkiselcilik" (methodological relationalism) olarak belirlediği tarzda değerlendirilmiştir. Araştırma bulguları Türkiye'de 1980 sonrasında egemen olan rejimin "esnek sermaye birikimi" ve buna bağlı olarak sanayisizleşme ve özelleştirme olduğunu göstermiştir. Türkiye'de Neo-liberal politikalarla Güneydoğu Avrupa Modeli uygulanırken temel ilke özellikle sağlık alanında hizmet sunumu ile bunun finansmanının birbirinden ayrılmasıdır. İkinci temel özellik ize merkez-dışı (adem-i merkeziyetçi) yönetime geçilmesidir. Kuşkusuz tüm değişmeler, bazı ilkelere bağlanmışsa da hizmetin finansmandan ayrılması hizmet kalitesini olumsuz etkilemiştir. Çünkü harcamalarda kısıntı yapılması ve işletme mantığı ile maliyetin düşürülmesi beklenmektedir. Ancak hastanelerin bazı gerekliler yerine daha fazla karlı olan işlemlerde yoğunlaşmaları söz konusudur. Ayrıca teşhis ve tedavi sürecindeki denetleyici kısıtlamalar da hekimlik mesleğinin icrası kadar kalitesini de olumsuz etkileyebilmektedir. Hekimlerin gerekli gördüğü tetkikler fiyatlandırma yönetmeliği uyarınca kısıtlandığında ortaya çıkan sonuç hekimin mesleki özerkliğinin kısıtlanmasından başka bir şey değildir. ; In this paper it is assumed that there is a close relationship between the health policy changes and socioeconomic structural changes of the Turkish society. Certainly scientific developments and globalization process are also important on social transformations of any society. Therefore, at the beginning of this paper in order to understand the process that is called " Transformation of Health Work Process" , historical background of health system in Turkey is reviewed based on relevant literature. " Systematic review" method (Weed,2007) and process based sociology namely " methodological relationalism" (Elias,1977) are used to achieve study purposes. Findings revealed that we live in a period that is called " flexible" period in terms of capital accumulation. This period starting from 1990's is characterized by non-industrialization and privatization. In Turkey Southern European Model is applied based on Neoliberal Policies which mainly separates finance from health services. Second characteristic future is application of decentralization of administrative system. Majority of hospitals are transformed from social to economic organizations who run after profits by applying high numbers of operations to get money from insurances. Although there are some regulations to be followed, separation of services from finance also reduced the quality of health care. Besides some malpractices are observed regarding expensive unnecessary operations. Furthermore, some supervisory limitations on diagnostic and treatment processes can be interpreted lack of professional freedom of physicians

Amaç: Bu çalışmada perioperatif antimikrobiyal profilaksi (PAP) uygulamalarında güncel rehberlere uyum oranlarını ve bu oranları etkileyen faktörleri belirlemeyi amaçladık.Gereç ve Yöntemler: 30 Mayıs - 30 Haziran 2013 tarihleri arasında 15 farklı merkezde uygulanan ankete yedi farklı branştan 410 cerrah katıldı. Çoktan seçmeli ve açık uçlu 40 soru içeren anketler yüz yüze görüşme yöntemiyle uygulandı. Bulgular: Katılımcıların ortalama yaşı 38,01±9,1, ,4'ü erkek idi. Ankete katılan cerrahların ,2'si "kurumlarında cerrahi profilaksi rehberi varlığı hakkında bilgisi olmadığını," 'ü ise "rehberin bulunduğunu ve profilaksi uygulamalarının rehbere uygun olduğunu" belirtti. Ankete katılan cerrahların ,1'inin kurum içinde cerrahi profilaksi konusunda herhangi bir eğitim toplantısına katılmadığı, son üç yıl içinde cerrahi profilaksi eğitimi alanlarda rehbere uyumun istatistiksel olarak daha yüksek olduğu belirlendi (p <0,001). Kardiyovasküler cerrahlarda uyum diğer branşlardan cerrahlara kıyasla anlamlı olarak daha yüksek saptandı (p=0,012). Uygulanan profilaksinin süresi katılımcıların 'sında 24 saatten daha uzun idi. Dren kullanılan girişimlerde cerrahların ,7'sinin cerrahi profilaksiyi dren çekildikten sonra sonlandırdığı belirlendi. Ürologların ikinci kuşak ve üçüncü kuşak sefalosporinleri anlamlı olarak (p<0,001; p=0,002) daha sık kullandığı belirlendi. Cerrahların ,6'sı cerrahi profilakside kullanılan antibiyotikte rotasyonel değişiklik yapmadığını ifade etti. Merkezlerin 'ünde 24 saat enfeksiyon konsültasyonu ve mikrobiyoloji laboratuvarı olanağı bulunmadığı belirtildi. Katılımcıların 'si "Cerrahi profilaksi rehberlerine uyumu engelleyen en önemli nedenler nelerdir?" sorusunu yanıtsız bıraktı. En önemli nedenler; "çalışılan kurumdaki hastane enfeksiyonları ve etken mikroorganizmalar hakkında düzenli bilgi verilmemesi" (), "profilaktik ilacın sağlık personeli tarafından planlanan zaman ve dozda uygulanmaması" () ve kurum içi PAP rehberinin hekim tarafından yetersiz bulunması () şeklinde belirlendi.Tartışma ve Sonuç: Kanıta dayalı PAP uygulamalarının yerleşebilmesi için bilimsel rehberler ve kurum içi kılavuzların varlığı kadar cerrahi birimlerin bu kılavuzların hazırlık aşamasına etkin katılımı ve ayrıca düzenli eğitim ve geri bildirim toplantıları ile branşlar arası aktif iletişimin sürdürülmesi de son derece önemlidir. ; Aim: We aimed to determine the rates of compliance with current guidelines for practices of perioperative antimicrobial prophylaxis (PAP) and the factors affecting these rates.Materials and Methods: Four hundred and ten surgeons from seven different branches attended the survey conducted between May 30 and June 30, 2013, in fifteen different centers. A 40-question questionnaire consisting of multiple choice and open-ended questions was prepared and applied by interviewing the surgeons. Results: The mean age was 38.01 ± 9.1 years; and 83.4% of the participants were male. Of the surveyed surgeons 46.2% stated that they had no information about the existence of any guidelines for surgical prophylaxis in their institutions and 34% stated that institutional guidelines were available and their prophylaxis practices were in accordance with them. Surgeons who were trained on surgical prophylaxis within the last three years were found to have a statistically higher (p<0.001) rate of compliance with the guidelines. Compared to surgeons from other branches, compliance rates in cardiovascular surgeons were statistically significantly higher (p=0.012). The duration of prophylaxis applied was longer than 24 hours in 56% of the participants. In procedures involving drains, 63.7% of the surgeons stated that they preferred to terminate surgical prophylaxis after removing the drainage tube. It was found that urologists used second and third generation cephalosporins statistically significantly more frequently (p<0.001; p=0.002). Of the surgeons 87.6% stated that they did not rotationally change the antibiotics they used for surgical prophylaxis. It was found that 33% of the centers did not have 24-hour infection consultation and microbiology laboratory facilities. The question "What are the most important reasons for failure in compliance with surgical prophylaxis guidelines?" was left unanswered by 50% of the participants. The most important reasons of incompliance were found to be "lack of regular feedback about hospital infections and the pathogenic microorganisms" (30%), "failure in application of the prophylactic agent in accordance with the timing and dosing determined by the healthcare personnel" (27%), and physicians' view that institutional PAP guidelines were insufficient (17%).Discussion and Conclusion: Besides the existence of institutional guidelines for the establishment of evidence-based PAP practices, it is also crucial to provide active participation of surgical healthcare units in the preparation of these guidelines and to maintain regular training and feedback meetings and active communication between medical branches.
