ABSTRACT Objective: to present the experiences of a medicine student in the second extension training in communities and its implications in the training of health professionals. Method: descriptive study, of experience report type, with qualitative approach. Observation and dialogues with residents, workers and other people related to the rural community were used through the Methodology for Collective and Individual Mobilization. Results: the Methodology for Collective and Individual Mobilization enabled understanding the Itapecerica community, at the municipality of Mamanguape (PB), Brazil, under various aspects: historical, geographic, economic, political, social, cultural, religious and environmental. Conclusion: the experience of living in a community is a modulating activity from the perspective of the health professional under training. Female sterilization, or tubal ligation, is the most prevalent form of contraception in rural India. The paperwork that surrounds this procedure provides an interesting lens to investigate the state, its institutions, and their material conditions. The production and circulation of this paperwork uncover the state as an illegible and unpredictable entity that materializes in people's everyday lives through bureaucratically futile certificates. In this article, I look at sterilization paperwork as a tool to tell stories about the state and its institutions in the context of seeking and undergoing tubal ligation in a government facility in rural India.

In many rural areas of Africa traditional medicine is the only way for providing basic health care, despite all scientific and technological development recorded at the end of last century. However, traditional medicine continues in the secondary policy plans of African governments, despite that some countries already have institutes or centers of traditional medicine and have formulated policies and legislation. The author asks why African countries do not integrate traditional medicine into their national health systems and suggests that thinking in terms of "integrating" traditional medicine might not be helpful: traditional medicine should be regarded as an independent and parallel system with equal dignity. She argues that African countries must decide if this valuable resource should continue to be used to promote the well being of their people. They should be proud of their traditional knowledge and invest in it, stimulating research, development of technologies, infrastructures and policies.

Reading product labels is a common and important use of literacy and numeracy skills in daily life. Instructions for commercial oral-rehydration-therapy salt solutions were examined. The products are widely available in rural Kenya, where they are purchased for alleviating the symptoms of life-threatening diarrheal dehydration. Many features of these texts do not facilitate comprehension. Information about preparation, dosage, administration, storage, and precautions is difficult to locate, few discourse conventions are used to enable readers to efficiently process information, and much tacit knowledge is presumed—including knowledge of English even when a Kiswahili translation is provided. Numeracy tasks associated with compliance with the treatment regime cannot be easily performed with school-learned mathematical algorithms. Measuring and teaching functional literacy and numeracy skills necessitates a better understanding of the kinds of texts that readers will encounter and analyzing the comprehension and problem-solving skills they require.

It is often suggested that, in the past 50 years, Vietnam has experienced a traditional medicine 'revival' that can be traced back to late President Ho Chi Minh's 1955 appeal 'to study means of uniting the effects of oriental remedies with those of Europe'. In this article, I demonstrate how traditional herbal medicine came to be recruited as an important component of national efforts to promote the public health of urban and rural populations in Vietnam. Importantly, this has entailed a rejection of a colonial biopolitics that sought to marginalize 'quackery' in favour of a postcolonial bio-politics that aims to promote the 'appropriate' use of traditional herbal medicines. While the Vietnamese case bears many parallels to other countries in this respect, notably China, Vietnam's ancient history of medicine, postcolonial isolation and extensive health delivery network have resulted in a unique strategy that encourages rural populations to become self-sufficient in the herbal treatment of their most common illnesses.

BACKGROUND: Widening access to medicine in the UK is a recalcitrant problem of increasing political importance, with associated strong social justice arguments but without clear evidence of impact on service delivery. Evidence from the United States suggests that widening access may enhance care to underserved communities. Additionally, rural origin has been demonstrated to be the factor most strongly associated with rural practice. However the evidence regarding socio-economic and rural background and subsequent practice locations in the UK has not been explored. The aim of this study was to investigate the association between general practitioners' (GPs) socio-economic and rural background at application to medical school and demographic characteristics of their current practice. METHOD: The study design was a cross-sectional email survey of general practitioners practising in Scotland. Socio-economic status of GPs at application to medical school was assessed using the self-coded National Statistics Socio-Economic Classification. UK postcode at application was used to define urban-rural location. Current practice deprivation and remoteness was measured using NHS Scotland defined measures based on registered patients' postcodes. RESULTS: A survey was sent to 2050 Scottish GPs with a valid accessible email address, with 801 (41.5 %) responding. GPs whose parents had semi-routine or routine occupations had 4.3 times the odds of working in a deprived practice compared to those with parents from managerial and professional occupations (95 % CI 1.8-10.2, p = 0.001). GPs from remote and rural Scottish backgrounds were more likely to work in remote Scottish practices, as were GPs originating from other UK countries. CONCLUSION: This study showed that childhood background is associated with the population GPs subsequently serve, implying that widening access may positively affect service delivery in addition to any social justice rationale. Longitudinal research is needed to explore this association and the impact of widening access on service delivery more broadly.
