Connaître ou informer : la carrière sociale des tests ELISA/VIH dans deux enquêtes épidémiologiques en France et aux États-Unis

Abstract

Between knowing and informing: the social career of the HIV tests in two epidemiological surveys in France and in the United-StatesEmbedded like epidemiologic instrument of evaluation of the HIV epidemic in surveys with primarily statistical aiming (EANR), the ELISA/HIV tests were widely diffused in France and in the United States. The concern caused by the extension of the epidemic apart from the first « risk groups » generated a keen demand of expertise for the public management of the collective risks related to the epidemic. The development of derogatory modes allowed the conduct of EANR on the newborns (the United States) and the pregnant women (France). To the beginning of the Nineties, under the combined pressure of the first treatments and the mobilization of actors and standards protecting the private life, these EANR are called into question. According to countries, the public regulation took different ways. In the United States, a process of politization started in the State of New York leads the federal State to suspend these surveys in 1996. In France, on the other hand, the stop of the PREVAGEST survey answers economic and ethical considerations primarily. ; Appropriés comme instrument d'évaluation épidémiologique de l'épidémie à VIH dans des enquêtes à visée essentiellement statistique (enquêtes anonymes non reliées – EANR), les tests ELISA/VIH se sont largement diffusés en France et aux États-Unis. L'inquiétude suscitée par l'extension de l'épidémie en dehors des premiers groupes « à risque » a généré une forte demande d'expertise pour la gestion publique des risques collectifs liés à l'épidémie. L'élaboration de régimes dérogatoires a permis la conduite d'EANR sur les nouveau-nés (États-Unis) et les femmes enceintes (France).
