REFORMING NATIONAL HEALTH SYSTEMS: CURRENT ASPECTS OF FOREIGN EXPERIENCE ; РЕФОРМУВАННЯ НАЦІОНАЛЬНИХ СИСТЕМ ОХОРОНИ ЗДОРОВ'Я: АКТУАЛЬНІ АСПЕКТИ ЗАКОРДОННОГО ДОСВІДУ

Abstract

Ukraine's medical policy is one of the most important tools for ensuring the proper preservation, functioning and development of domestic society. This policy is socially significant, relevant for statesmen, politicians, public servants, doctors, and public figures. For its successful implementation it is useful to take into account the positives and negatives of foreign practices. This experience is valuable for Ukraine, ensuring the construction and successful implementation of a rational model of the health care system. The article describes the aspects of foreign experience of medical reforms that are attractive for Ukraine, namely the following issues: financing of health care from the budget and health insurance; decentralization of the medical industry, autonomy of medical institutions, liberalization of the pharmaceutical market; substantiation of the advantages of the "state" model and state regulation of pricing, implementation of personnel medical policy; strengthening the strategic capacity of public administration through the introduction of Concepts, State programs, innovation strategies, tools of public-private partnership in the formation and implementation of health policy. The formation of medical care began with its primary link in the V – X centuries. Its development was accompanied by the adoption of three main models of financing medicine (insurance, state (budget) and paid) in the twentieth century. This path was ensured by: broad approval of institutional mechanisms at the global level; implementation of the Health for All in the 21st Century Program, aimed at improving access to primary care in the EU. In Ukraine, the state (budget) model has been chosen. It is used in Australia, Great Britain (founder), Greece, Spain, Canada, Sweden, Finland, as well as in the CIS countries. The peculiarity of this model is: the joint responsibility of the state and society for the implementation of personnel policy; contract-competitive approach to the appointment of heads of medical institutions; availability of management education for candidates for the head of this institution; training and retraining of family doctors (general practitioners who should treat 80% of diseases); the workload of the family doctor should be approximately 2000 patients; providing doctors with housing in the area where patients live. ; Розглянуто досвід здійснення медичної політики за кордоном. Ця політика є одним із найважливіших пріоритетів збереження, функціонування і розвитку суспільства. Його цінність полягає у сприянні покращенню побудови «державної» моделі надання медичної допомоги в Україні. Привабливими аспектами для її побудови є: обґрунтування переваг «державної» моделі, особливості її фінансування з бюджету країни і накопичень медичного страхування, регулювання державою ціноутворення медичних послуг; пріоритети кадрової політики; децентралізації медичної галузі, автономізації медичних закладів, лібералізації фармацевтичного ринку; посилення стратегічної спроможності державного управління шляхом запровадження Концепцій, Державних програм, інноваційних стратегій, інструментів державно-приватного партнерства у формуванні і реалізації медичної політики в Україні.
