Objective To determine whether inadequate housing is the main barrier to the provision of home dialysis treatment. Design Prospective observational study. Participants All patients attending a predialysis clinic between 2006 and 2009 deemed medically suitable for home dialysis and not active on the preemptive transplant list. Setting A predialysis clinic in a London teaching hospital. Main outcome measure Assessment of patient9s accommodation for suitability for home-based dialysis using departmental guidelines and the Government9s Housing Health and Safety Rating System regulations 2005. Results A lack of adequate housing prohibited the provision of home haemodialysis to all but one of these patients. Moreover, only 29% of homes assessed were suitable for peritoneal dialysis, despite the lower spatial demands of this form of renal replacement therapy. In addition to the specific requirements of dialysis, we also found that only 33% of the homes visited fulfilled the minimum standard of housing as defined in the Government9s Decent Homes Standard, with multiple specific hazards identified across the properties. Conclusions This study illustrates that the lack of suitable housing is a major barrier to the provision of home-based dialysis and underscores the need for this to be addressed urgently at both the central government and local authority levels. We suggest that it should be considered as a major priority to rehouse medically suitable patients with a view to enabling home-based therapy.
This CRC research paper of MINES ParisTech is setting the scene for an ambitious study programme to study films (documentaries, fictions, advertisements, etc.) that deal with civil nuclear in France and internationally. The ambition is to analyse the myths and representations conveyed by these works on a controversial technology. This report is made up of five contributions which present very briefly ongoing work and which prepare for future results which will be the subject of more elaborate publications. The first is the testimony of Aurélien Portelli, a historian of science and technology, who explains and motivates the interests in film research. The second is a quick, but not exhaustive, panorama of the films that have marked the history of both civilian and military nuclear since the 1970s. The third is the report by The Land of Hope, a film produced by Sono Sion in 2012, which discusses the consequences of a nuclear accident on the inhabitants of a small city in Japan. The fourth illustrates the documentary film, taking the example of one of them, produced in 2013, which deals with the issue of decommissioning of nuclear power plants. On 11 February 1975 Heath lost the election as leader of the Conservative Party and transferred his place to Margaret Thatcher. This date marks the anchor of a process of voluntary marginalisation through which Heath opposed Thatcher and rejected thatcherism. During the fifteen years of the Thatcher era, Heath took on the role of the revolted dissident and, according to John Campbell, became "the embodiment of anti-Thatcherism." We will see on this occasion that if his dissent company follows traditional mechanisms — denial of leadership, denial of legitimacy, systematic criticism of his action — it nevertheless offers an interesting reversal of the codes. Her entry into dissent was much more than an opposition to a political adversary, she was a means of life and renaissance. Power muted Heath, the dissent released him. Heath opposed himself and turned out to rebel. His dissent experience thus provides original insights into a concept too often associated with the concepts of oppression or clandestine. Heath was the only real resistance to Thatcher and there is still a key feature of dissent and rebellion in the British political landscape. International audience The French Atlases of landscapes or the difficulties of reconciling the sensitive and scientific approach. - About fifty landscape atlases cover two thirds of the national French territory today. Initiated by the Minister of the environment in its national political setting for landscape and in the application of article 6c of the European Convention on Landscape, these documents are drawn up on the departmental or regional scale (depending on the case). These are above all tools of knowledge (they do not have any statutory value) and for the awareness of local actors at the stakes of landscape. The purpose of this article is, drawing from a specific atlas example in the construction of which we have participated, to expose the methodological difficulties encountered in reconciling the sensitive and the scientific approach. "Der Beitrag skizziert am Beispiel der Urheberrechtsdiskussionen um die Musikin-dustrie und das wissenschaftliche Publizieren die sozialen Prozesse, in denen die Grenzen zwischen Märkten und Öffentlichkeiten, zwischen Recht, Wirtschaft, Technik und Kultur gezogen und verhandelt werden. Dies geschieht zur Zeit bei der Novellierung des Urheberrechts, aber auch in den globalen öffentlichen Debatten um Patente auf "Leben" oder auf Software, um Privatkopien und Piraterie. In Prozessen der Regulierung geistigen Eigentums handeln Wissensgesellschaften ihren Umgang mit den mutmaßlich zentralen Ressourcen Wissen, Kultur, Innovation und Innovativität aus. In der hier vertretenen Perspektive geht es darum, diese Prozesse nicht von vornherein als marktlich, als strukturiert durch Interessen und Eigentumsrechte zu begreifen. Wissenssoziologisch und sozialkonstruktivistisch betrachtet, spezifizieren die Akteure diese Ressourcen im Prozess der Aushandlung erst als solche – und sie spezifizieren nicht nur die Ressourcen, sondern unterschiedliche Akteure konfigurieren Ensembles aus Regulierungen und Praxen des Schaffens und Konsumierens aus sozialen und ökonomischen Tauschbeziehungen, aus Produkten, Diensten und Leistungen. Damit befassen sich eher JuristInnen, Rechts- und WirtschaftswissenschaftlerInnen, aber auch AutorInnen aus den Informations- und Kommunikationswissenschaften (Kuhlen 1995; 2002a; 2002b; Grassmuck 2000; Lutterbeck 2002). In der Soziologie findet sich das Thema bislang verstreut. In der sozialwissenschaftlichen Wissenschafts- und Technikforschung gibt es eine Diskussion über "commodification of knowledge" (zum Beispiel die Beiträge in den Science Studies 2/2001, prokla 126; Nentwich 2003) in kritischer Abgrenzung zur These eines vernetzten und konextoffenen "mode 2" der Wissensproduktion (Gibbons u.a. 1994; Nowotny u.a. 2001). In der Untersuchung von Märkten, Institutionen und Governancebeziehungen um das Internet herum (Hofmann 2002; Lieckweg 2002) taucht es auf, in Untersuchungen von Interessen, Diskursen und Verhandlungsprozessen (Döbert/van den Daele 2002) und bei der Analyse wissens-intensiver Industrien (Giesecke 2001), in denen geistiges Eigentum zum Produkt wird. Das hat zur Folge, dass diese Diskussion überwiegend in Begriffen von Gütern stattfindet, also von "Dingen", auf die sich dann Verfügungsrechte beziehen können. Die These dieses Beitrags ist dabei: "Die" public domain löst sich, soziologisch betrachtet, auf in heterogene Ensembles von marktlichen, staatlichen, zivilgesellschaftlichen und professionellen Akteuren, Praxen und Orientierungen. Sie stellen einstweilige Resultate technisch-institutioneller Einklammerungen dar, die im Fluss von Wissen, Kultur und Sinn immer wieder de- und rekontextuiert werden. Geistige Eigentumsrechte bilden jedoch nicht einfach einen institutionellen Rahmen, sondern greifen tief in die Produktion und Nutzung von Wissen und Kultur ein.
