An increasing number of people live near airports with considerable noise and air pollution. The Hypertension and Exposure to Noise near Airports (HYENA) project aims to assess the impact of airport-related noise exposure on blood pressure (BP) and cardiovascular disease using a cross-sectional study design. We selected 6,000 persons (45-70 years of age) who had lived at least 5 years near one of six major European airports. We used modeled aircraft noise contours, aiming to maximize exposure contrast. Automated BP instruments are used to reduce observer error. We designed a standardized questionnaire to collect data on annoyance, noise disturbance, and major confounders. Cortisol in saliva was collected in a subsample of the study population (n = 500) stratified by noise exposure level. To investigate short-term noise effects on BP and possible effects on nighttime BP dipping, we measured 24-hr BP and assessed continuous night noise in another subsample (n = 200). To ensure comparability between countries, we used common noise models to assess individual noise exposure, with a resolution of 1 dB(A). Modifiers of individual exposure, such as the orientation of living and bedroom toward roads, window-opening habits, and sound insulation, were assessed by the questionnaire. For four airports, we estimated exposure to air pollution to explore modifying effects of air pollution on cardiovascular disease. The project assesses exposure to traffic-related air pollutants, primarily using data from another project funded by the European Union (APMoSPHERE, Air Pollution Modelling for Support to Policy on Health and Environmental Risks in Europe).

A renewed AUDITIO! World Hearing Day is coming and we couldn't have chosen a better time to welcome the new AUDITIO. After many months of work, AEDA has managed to renew its journal in a unique way. A team, led by Dr. Raul Sánchez, brand new Editor-in-Chief of the journal, began to work with the aim of renovating, modernizing and adapting the journal to the needs of the 21st century. It may surprise you but AUDITIO is a pioneering journal in audiology as it was designed, by the Fundación Canaria Dr. Barajas in 2001, as a free access electronic journal. I assure you that in 2001 this format was quite a novelty and this fact has marked its existence. The distribution of AUDITIO through the internet among the Spanish-speaking community made it the most widely distributed Audiology journal in the Spanish language and therefore one of the most widely read journals. AUDITIO has always maintained its academic orientation and has successfully fulfilled its goal: the free dissemination of Audiology. So why did it have to be renewed? It was a demand from our associates and a thorn in many of us. So much so that it became a goal of my action program for the legislature of the Presidency in the 2020 elections. The reason for this demand was clear: its lack of periodicity. In the last five years AUDITIO has suffered ups and downs and a lack of regularity, due to a lower reception of publications. It has not been due to lack of work from its Editor-in-Chief, Dr. Bartolomé, who knew how to surround herself with a great team of reviewers, was able to encourage university students and masterfully handled her contacts with prestigious researchers to maintain the greatest possible regularity. We should all thank her for her effort, dedication and results, but AUDITIO was slowly getting sick and we were unable to detect it and we were unable to find solutions. And in the face of illness, renew or die. It was the cruel reality that AEDA had to face. And after delving into the reasons, we discovered that AUDITIO still had possibilities and we decided to bet on it and mark a turning point. AUDITIO and audiology in Spanish deserve it. Because audiology is one of the disciplines of science with the greatest scientific production and we must base its future development on it. AUDITIO must maintain its unique role, as a means of expression within audiology in the Spanish language. A key element in the promotion of clinical practice based on scientific evidence and the speaker of knowledge in audiology in the Spanish language where to publish new research, projects, clinical practice guides, conference summaries, etc. An agile, young and dynamic communication medium, adjusted to the highest scientific rigor. For this, we aspire strongly and adopt the Open Journal System (for content management, applications, reviews and production), we register all content with Digital Object Identifiers (DOI), we adapt the journal to FECYT quality standards and adjust the contents for a better indexing of Google Scholar, RedIB, PubMed, WoS, Scielo, etc. A new time, new standards, an objective within the reach of this Board of Directors, an effort by AEDA associates and a brilliant job, catalyzed by its member and editor-in-chief/managing editor of the journal, Raul Sánchez-López. Congratulations to all! ; ¡AUDITIO se renueva! Llega el Día Mundial de la audición y no podíamos haber elegido mejor momento para dar la bienvenida a la nueva AUDITIO. Tras muchos meses de trabajo, AEDA ha conseguido renovar de manera única su revista científica. Un equipo, liderado el Dr. Raul Sánchez, flamante Editor Jefe de la revista, empezó a trabajar con el objetivo de renovar, modernizar y adaptar la revista a las necesidades del siglo XXI. Puede que les sorprenda pero AUDITIO es una revista pionera en audiología pues fue diseñada, por la Fundación Canaria Dr. Barajas en 2001, como una revista electrónica de acceso libre. Les aseguro que en 2001 este formato era toda una novedad y este hecho ha marcado su existencia. La distribución de AUDITIO a través de internet entre la comunidad hispanohablante la convirtió en la revista de Audiología de mayor difusión en lengua española y por tanto, una de las revistas más leídas. AUDITIO siempre ha mantenido su orientación académica y ha cumplido con éxito su objetivo: la divulgación gratuita de la Audiología. Entonces, ¿por qué tenía que ser renovada?. Era una demanda de nuestros asociados y una espina clavada en muchos de nosotros. Tanto que se convirtió en un objetivo de mi programa de actuación para la legislatura de Presidencia en las elecciones del 2020. El motivo de esa demanda era claro: su falta de periodicidad. En el último lustro AUDITIO ha sufrido altibajos y la falta de regularidad, por una menor recepción de publicaciones. No ha sido por falta de trabajo de su Editora Jefe, la Dra. Bartolomé, que supo rodearse de un gran equipo de revisores, supo incentivar a los universitarios y manejó con maestría sus contactos con prestigiosos investigadores para mantener la mayor regularidad posible. Todos debemos agradecerle su esfuerzo, dedicación y resultados pero AUDITIO enfermaba lentamente y no supimos detectarlo ni fuimos capaces de encontrar soluciones. Y ante la enfermedad, renovar o morir. Era la cruel realidad que AEDA tenía que afrontar. Y tras profundizar en los motivos, descubrimos que AUDITIO aun tenía posibilidades y decidimos apostar por ella y marcar un punto de inflexión. AUDITIO y la audiologia en español lo merece. Porque la audiología es una de las disciplinas de la ciencia con mayor producción científica y en ella debemos fundamentar su desarrollo futuro. AUDITIO debe mantener su papel único, como medio de expresión dentro de la audiología en lengua española. Un elemento clave en la promoción de la práctica clínica basada en la evidencia científica y el altavoz del conocimiento en audiología en lengua española donde publicar nuevas investigaciones, proyectos, guías de la práctica clínica, resúmenes de congresos etc. Un medio de comunicación ágil, joven y dinámico, ajustado al mayor rigor científico. AUDITIO merece la pena y debe tener su espacio en nuestra disciplina y nuestra lengua (la 2ª lengua materna más hablada en el mundo). Hoy, Día Mundial de la Audición, empieza una nueva era para AUDITIO. Una revista cuyas publicaciones serán fácilmente localizables por los lectores, una revista moderna, acorde a las necesidades de autores y lectores. This is the corrected version of the retracted article under the same title, which was published with the following DOI: https://doi.org/10.11649/slh.1517A Triumphant Gate of the Polish Narrative: The Symbolic Reconstruction of the Bridge over Chłodna Street in Warsaw vis-à-vis the Crisis of the Dominant Polish Holocaust NarrativeThe article is a study of the symbolic topography of Warsaw's Chłodna Street. In 1940–1942, Chłodna Street was an "Aryan" border strip, dividing the small and large ghettos. For the past quarter-century, the symbolic dominance of this space has been made manifest by the subsequent commemorations of the ghetto's wooden footbridge that existed here between January and August 1942. These commemorative artefacts evolved from a counter commemoration (1996), into a commemoration through the use of painting (2007) and photography (2008), into a symbolic reconstruction of the bridge (2011). The analysis of this dynamic, from representation of an object to its materialization, is complemented by a reconstruction of the patterns and stakes of narratives that make up the subsequent layers of the Chłodna Street narrative palimpsest. An important context for the analysis, apart from the history of the site, is the fact that today, the means of control over the space of the former ghetto are in the exclusive possession of the non-Jewish majority.A turning point in the commemoration process was Roman Polański's film The Pianist (2002). The bridge over Chłodna was reconstructed "one to one" – in Warsaw but outside the space of the former Warsaw ghetto. The film was shot in the midst of a nationwide debate taking place in Poland about Jan Tomasz Gross' book Neighbors (2000). The debate concerned the Poles' participation in the Holocaust and the socio-cultural determinants of their attitudes and behavior. Within Polish dominant culture, the narrative shock was so immense that a return to the old Polish tale of heroism and martyrdom seemed impossible. Meanwhile, the mainstream journalistic discourse cast Gross and Polański on two sides of a narrative opposition. The former supposedly represented subjectivism and rash generalizations, while the latter was supposed to embody objectivism and fair judgment. The silhouette of the bridge over Chłodna Street merged at that point with a counterfactual vision of the Polish context of the Holocaust. The international success of The Pianist (Palme d'Or and Oscar) sealed the evolution of the bridge as a figure: from an abject to an object of desire, a tourist attraction, and an export commodity. (A separate issue, also addressed in the article, is how the events depicted in the film relate to the story of Władysław Szpilman, mediated originally by Szpilman's ghostwriter Jerzy Waldorff and then by the scriptwriters, Ronald Harwood and Roman Polański. In both cases, the most serious discrepancies concern depictions of the Poles' attitudes toward the Jews during, but also before, the Holocaust).The success of the film was the determining factor in a decision made to invest public funds in another, and up to now the last, commemoration of the bridge over Chłodna: its symbolic reconstruction in situ. What was reconstructed was not so much the bridge as it was in 1942 but the bridge as depicted in the film, and with it the film's idealized portrayal of the Polish context of the Holocaust. The figure of the bridge suggests tight isolation of Jews and Poles – an isolation instituted by the Germans. By so doing, it feeds into the image of Poles as helpless bystanders (onlookers/gawkers, at times even witnesses) in the face of the Holocaust. The figure of the Polish bystander/witness to the Holocaust is in turn a key figure of Polish innocence. In addition, the symbolic reconstruction of the bridge draws the attention of the users of this space away from another site on Chłodna Street, where Jews and Poles came into direct contact with each other throughout the entire existence of the Warsaw ghetto. In obscuring the factual state of affairs uncovered as a result of the Jedwabne debate, the symbolic reconstruction of the bridge over Chłodna Street functions as a discourse-reproducing machine of sorts, alowing the dominant majority to "indulge inretrospective hallucination" (Baudrillard).The article uncovers the workings of a roly-poly narrative mechanism that renders impossible any successful reassessment and rejection of the Polish dominant culture and prevents common knowledge about facts from bearing in any way on the collective consciousness. In 1940–1942 Chłodna Street was an "Aryan" border strip, dividing the Small and Large Ghettos. For the past quarter of century, the symbolic landmark dominating within this space have been the subsequent commemorations of the Ghetto's wooden footbridge that existed here between January and August 1942. These commemorative artefacts evolved from a counter-commemoration (1996), through a commemoration with the use of painting (2007) and photography (2008), to a symbolic reconstruction of the bridge (2011). The analysis of this dynamics, from representation of an object to its materialization, is complemented by a reconstruction of the patterns and stakes of narratives that make up the subsequent layers of the Chłodna Street narrative palimpsest. An important context of the analysis, apart from the history of the site, is the fact that today, the means of control over the space of the former Ghetto are in exclusive possession of the non-Jewish majority.A breaking point in the commemoration process was Roman Polański's film The Pianist (2002). The bridge over Chłodna was reconstructed "one to one" – in Warsaw but outside the space of the former Warsaw Ghetto. The film was shot in the midst of a nationwide debate taking place in Poland over Jan Tomasz Gross's book Neighbors (2000). The debate concerned the Poles' participation in the Holocaust and the socio-cultural determinants of their attitudes and behaviors. Within the Polish dominant culture, the narrative shock was so immense that a return to the old Polish tale of heroism and martyrdom seemed impossible. Meanwhile the mainstream journalistic discourse cast Gross and Polański on the two sides of a narrative opposition. The former supposedly represented subjectivism and rash generalizations, while the latter was supposed to embody objectivism and fair judgment. The silhouette of the bridge over Chłodna Street merged at that point with a counterfactual vision of the Polish context of the Holocaust. The international success of The Pianist (Palme d'Or and Oscar) sealed the evolution of the bridge as a figure: from an abject to an object of desire, a tourist attraction, and an export. (A separate issue, also tackled in the article, is how the story told in the film relates to the story of Władysław Szpilman, mediated originally by Szpilman's ghostwriter Jerzy Walfdorff and then by the scriptwriters, Ronald Harwood and Roman Polański. In both cases the most serious discrepancies concern depictions of the Poles' attitudes toward the Jews during, but also before, the Holocaust).The success of the film was the determining factor in a decision made to invest public funds in another, and thus far last, commemoration of the bridge over Chłodna: its symbolic reconstruction in situ. What was reconstructed was not so much the bridge as it was in 1942 but the bridge from the film, and with it – the film's idealized portrayal of the Polish context of the Holocaust. The figure of the bridge suggests tight isolation of Jews and Poles – an isolation instituted by the Germans. By so doing, it feeds into the image of Poles as helpless bystanders (onlookers/gawkers, at times even witnesses) in the face of the Holocaust. The figure of the Polish bystander/witness to the Holocaust, in its turn, is a key figure of Polish innocence. In addition, the symbolic reconstruction of the bridge draws the attention of the users of this space away from another site on Chłodna Street, where Jews and Poles came in direct contact throughout the entire existence of the Warsaw Ghetto. By obscuring the factual state of affairs uncovered as a result of the Jedwabne debate, the symbolic reconstruction of the bridge over Chłodna Street functions as a discourse-reproducing machine of sorts, allowing the dominant majority to "indulge in retrospective hallucination."The article uncovers the workings of a roly-poly narrative mechanism that renders impossible any successful reassessment and rejection of the Polish dominant culture and prevents common knowledge about facts from bearing in any way on the collective consciousness. An overview of the stylistic resources of the Ukrainian language would be incomplete without the texts of the main ideologist of the Ukrainian nationalism, Dmytro Dontsov, since some terms-notions in Dmitriy Dontsov's journalistic texts receive new connotative coloring due to the author's definitions. The article analyzes the phenomenon of transition of the linguistic units of one style to another in the linguistic reality. It has been revealed that the terms in the journalistic texts of Dmytro Dontsov are emotionally colored due to the author's definitions and the use of numerous tropes (metaphors, antitheses, comparisons). New semantic shades of terms in the works of D.Dontsov contribute to better ideological understanding of the basic political realities. The case study proves that the terms chauvinism and fanaticism are considered in all sources as negative only, so it is worth analyzing the interpretation of these concepts not only in the scientific literature, but also in journalism, which influences the formation of the reader's position and is an effective factor in the creation of political thinking. In addition, it is impossible to give definitely negative assessment of the terms that are still functioning in the Ukrainian politics. The study has revealed that the attributive modifiers for the terms chauvinism and fanaticism used in the texts of the Tribune of the Nation do not coincide with the attributive modifiers fixed in the dictionaries and the political science and philosophy research papers. The author argues that the retrospective review of socio-political terms, which after the collapse of the USSR Empire returned to the cultural heritage of an independent state, is obviously an important issue. It is proved that the emotional texts of Dmytro Dontsov are the basis for the formation of a state-building outlook of the citizens and the basis for the theoretical understanding of the contemporary socio-political problems. Transformative strategies for the development of higher education, pointed out in the list of key educational competencies of the 21st century by international organizations of various levels, are associated with the processes of becoming and transformation of individuals as integral subjects of cognition and action within the framework of communicative strategies for the formation of a global civil society. The implementation of transformative educational strategies in a meaningful aspect requires inter- and transdisciplinary methodologies to research the process of transformative adult learning aimed at developing an "integral person", and not just its rational-cognitive aspects. Within this holistic approach, questions arise about the nature of the transformative changes in the basic ontological and anthropological components of the educational process in the context of the transition epoch, namely, the becoming a global transcultural civil society. In order to clarify the features of such transformations, the dynamics of the phenomena and concepts of identity and self is examined in terms of a change in the conceptual landscape of culture. Various disciplinary and transdisciplinary approaches to their definition are analyzed. The relevance of the concepts of multiple identity (as an open permanent identification process) and the transversal self (as a process-dialogue unity of differences) in the liminal space at the global and individual levels are substantiated. ; Трансформативні стратегії розвитку вищої освіти, артикульовані в переліку ключових освітніх компетентностей ХХІ століття міжнародними організаціями різного рівня, пов'язані з процесами становлення і трансформації індивідів як цілісних суб'єктів пізнання і дії в межах комунікативних стратегій формування глобального громадянського суспільства. Реалізація трансформативних стратегій вищої освіти в змістовному аспекті вимагає інтер- і трансдисциплінарних методологій дослідження процесу трансформативного навчання дорослих, спрямованого на розвиток «цілісної людини», а не тільки його раціонально-когнітивних складових. В рамках такого холістичного підходу постають питання про природу трансформативних змін основних онто-антропологічних складових освітнього процесу в контексті епохи переходу, а саме становлення глобального транскультурного громадянського суспільства. З метою прояснення особливостей таких трансформацій розглядається динаміка феноменів і концептів ідентичності і самості на тлі зміни концептуального ландшафту культури. Аналізуються різні дисциплінарні і трансдисциплінарні підходи до їх визначення і обґрунтовується релевантність концептів множинної ідентичності як відкритого перманентного процесу ідентифікації та трансверсальної самості як процесуально-діалогової єдності відмінностей у просторі лімінальності на глобальному та індивідуальному рівнях. Background: As a response to evidence that mental health service users and carers expect greater involvement in decisions about antipsychotic medication choice and prescribing, shared decision-making (SDM) has increasingly come to be viewed as an essential element of person-centred care and practice. However, this aspiration has yet to be realised in practice, as service users and carers continue to feel alienated from healthcare services. Existing understanding of the factors affecting the use of tools to support SDM is limited to inter-individual influences and wider factors affecting potential implementation are underexplored. Aim: To explore the potential use of a tool designed to enhance collaborative antipsychotic prescribing from the perspectives of secondary mental health service users, carers and professionals. Methods: We conducted a qualitative study (semi-structured interviews and focus groups) using a purposive sample of 33 participants (10 mental health service users, 10 carers and 13 professionals) involved in antipsychotic prescribing in one Trust in the North of England. Participants were asked about the potential implementation of a tool to support SDM within secondary mental health services. Framework analysis incorporating the use of constant comparative method was used to analyse the data. Results: The study identified a divergence in the views of service users and professionals, including a previously undocumented tendency to blame each other for implementation failure. This dissonance is shaped by meso and macro level influences relating to paternalism, legislative frameworks, accountability and lack of resources. Participants did not identify macro level (policy or structural) facilitators to the use of the tool highlighting the negative impact of mental health contexts. Our study indicated that inter-individual factors are likely to be most important to implementation, given their potential to transcend meso and macro level barriers by disrupting paternalism. Conclusions: Consideration of the meso and macro level influences identified areas for potential intervention, including challenging professionals and service users' perceptions of each other, rebalancing the notion of accountability within services and introducing new means for service user feedback on the quality of SDM. Multi-level strategies for facilitating the implementation of tools to support SDM are also presented.

The European Multicentre Bronchiectasis Audit and Research Collaboration (EMBARC) is a European Respiratory Society (ERS) Clinical Research Collaboration dedicated to improving research and clinical care for people with bronchiectasis. EMBARC has created a European Bronchiectasis Registry, funded by the ERS and by the European Union (EU) Innovative Medicines Initiative Programme. From the outset, EMBARC had the ambition to be a patient-focussed project. In contrast to many respiratory diseases, however, there are no specific patient charities or European patient organisations for patients with bronchiectasis and no existing infrastructure for patient engagement. This article describes the experience of EMBARC and the European Lung Foundation in establishing a patient advisory group and then engaging this group in European guidelines, an international registry and a series of research studies. Patient involvement in research, clinical guidelines and educational activities is increasingly advocated and increasingly important. Genuine patient engagement can achieve a number of goals that are critical to the success of an EU project, including focussing activities on patient priorities, allowing patients to direct the clinical and research agenda, and dissemination of guidelines and research findings to patients and the general public. Here, we review lessons learned and provide guidance for future ERS task forces, EU-funded projects or clinical research collaborations that are considering patient involvement. Objective: Combat Stress, a UK national charity for veterans with mental health problems, has been funded by the National Health Service (NHS) to provide a national specialist service to deliver treatment for post-traumatic stress disorder (PTSD). This paper reports the efficacy of a PTSD treatment programme for UK veterans at 6 months follow-up. Design: A within subject design. Setting: UK veterans with a diagnosis of PTSD who accessed Combat Stress. Participants: 246 veterans who received treatment between late 2012 and early 2014. Intervention: An intensive 6-week residential treatment programme, consisting of a mixture of individual and group sessions. Participants were offered a minimum of 15 individual trauma-focused cognitive behavioural therapy sessions. In addition, participants were offered 55 group sessions focusing on psychoeducational material and emotional regulation. Main outcome measures: Clinicians completed measures of PTSD and functional impairment and participants completed measures of PTSD, depression, anger and functional impairment. Results: We observed significant reductions in PTSD scores following treatment on both clinician completed measures (PSS-I: -13.0, 95% CI -14.5 to -11.5) and self-reported measures (Revised Impact of Events Scale (IES-R): -16.5, 95% CI -19.0 to -14.0). Significant improvements in functional impairment were also observed (eg, Health of the Nation Outcome Scales (HONOS): -6.85, 95% CI -7.98 to -5.72). There were no differences in baseline outcomes between those who completed and those who did not complete the programme, or post-treatment outcomes between those we were able to follow-up at 6 months and those lost to follow-up. Conclusions: In a naturalistic study we observed a significant reduction in PTSD scores and functional impairment following treatment. These improvements were maintained at 6 month follow-up. Our findings suggest it may be helpful to take a closer look at combining individual trauma-focused cognitive behaviour therapy and group sessions when treating veterans with PTSD. This is the first UK study of its kind, but requires further evaluation.
