La aceptabilidad de la vacuna contra el virus del papiloma humano : consideraciones desde el enfoque de precaución

En décadas recientes la vacunación infantil ha sido objeto de controversias. Tal es el caso de la vacuna contra el VPH. En esta tesis se plantea la relación entre riesgo y tecnología desde la sociedad del riesgo, la construcción social del riesgo y el análisis de la incertidumbre en procesos y sistemas complejos de toma de decisiones. Se describe la importancia de la construcción social del riesgo y su proceso de construcción cultural. La incertidumbre y controversia relacionada con el riesgo de la vacunación contra el VPH involucra diferentes posturas. Dicha controversia enfrenta frecuentamente un abordaje reduccionista y falta de inclusión de expertos y no expertos que pudiesen enriquecer el proceso. Varios organismos internacionales defienden la vacunación contra VPH, dejando de lado puntos de vista de diferentes actores. Se propone desde el enfoque de precaución que la sociedad exponga su posición ante acciones o decisiones cuyas consecuencias puedan resultar adversas, y que permita así resolver las disputas sobre la aceptabilidad de la vacuna contra el VPH . Se incluyen elementos de la ciencia posnormal que se articulan con el enfoque de precaución y que facilitan un diálogo democrático. Como consecuencia práctica se deriva la propuesta de utilizar una metodología viable en Colombia, una Conferencia de Consenso. Se expone entonces la necesidad de que la institucionalidad en Colombia esté en capacidad de resolver disputas en relación con la aceptabilidad del riesgo, evitando que los actores más hegemónicos filtren su punto de vista y lo posicionen como el único válido. ; In recent decades, childhood vaccination has been controversial. Such is the case of the HPV vaccine. This thesis proposes the relationship between risk and technology based on the risk society, the social construction of risk and the analysis of uncertainty in complex decision making processes and systems. The importance of the social construction of risk and its process of cultural construction is described. The uncertainty and controversy related to the risk of HPV vaccination involves different positions. This controversy frequently faces a reductive approach and lack of inclusion of experts and non-experts that could enrich the process. Several international organizations defend vaccination against HPV, leaving aside the views of different actors. It is proposed from the precautionary approach that society exposes its position to actions or decisions whose consequences may be adverse, and that allows to resolve disputes about the acceptability of the vaccine against HPV. It includes elements of post-normal science that are articulated with the precautionary approach and that facilitate a democratic dialogue. As a practical consequence, the proposal to implement a viable methodology in Colombia, a Consensus Conference is derived. It is then exposed the need for institutions in Colombia to be able to resolve disputes regarding the acceptability of risk, preventing the most hegemonic actors from filtering their point of view and positioning it as the only valid one.
