ABSTRACT
ObjectivesRoots of Empathy is a school-based mental health program with demonstrated effectiveness in reducing bullying and aggression, and raising children's social/emotional competence and empathy. This study examines the program's impact on several longer-term health and social outcomes that are related to the program's goals, but not specifically targeted by the program's design or objectives.
ApproachThis study used linked, de-identified administrative records of health, education, social service use and justice system involvement for children and youth in Manitoba who participated in the ROE program, along with a carefully matched comparison group. Propensity scores were used along with hard matching on key variables to ensure comparability of groups. Outcome measures encompass a broad array of health, educational, and social indicators including hospitalization for injury, teen pregnancy, high school graduation, involvement with the criminal justice system, and suicide attempts. Analyses were performed at the Manitoba Centre for Health Policy (MCHP), using SAS 9.4.
ResultsFindings from analyses of data from the pilot study (N=688) reveal several 'collateral benefits' that may be related to the program, though most differences did not reach statistical significance in this initial sample. (Results from the full sample will be available May 2016; N ~ 8000.) Compared to matched counterparts who did not receive the program, ROE participants had lower rates of teen pregnancy (6.6% vs 9.6%; p=0.19), and school grade repetition (2.4% vs 3.8%, p=0.09), and higher rates of high school graduation (77.7% vs 73.5%, p=.36). However, they also had higher rates of injury-related hospitalization (6.4% vs 5.0%, p=0.14). Even in this small pilot, sub-group analyses for some outcomes showed significant differences for some strata but not others. Analyses of the full study group (underway) will provide more robust results.
ConclusionThese findings provide intriguing evidence suggesting beneficial impacts in several longer-term health and social outcomes that could feasibly be related to participation in the Roots of Empathy program. The multi-variable propensity scores and hard-matching algorithms used on this large group provide considerable confidence in attributing group differences to program participation. Results from the full sample will provide more conclusive results, and allow sub-group analyses. The linkage of databases from health, education, social service and justice systems provides a unique opportunity to examine the truly multi-dimensional long-term impacts of a program already proven to provide dramatic improvements in short and medium-term outcomes.

ABSTRACT
ObjectivesOur objective was to develop a comprehensive longitudinal data resource, which population health research scientists could use to study the social determinants of child health and health equity.
MethodsThe PATHS Resource was created from data holdings within the Manitoba Population Health Research Data Repository. The Manitoba Health Registry sits at the centre of the Repository and includes information – including a scrambled personal health identification number (PHIN) and date when coverage commenced and expired – on every individual registered with the province's universal healthcare system. The Repository also includes administrative data spanning several sectors including health, social services, justice, and education. We used individuals' scrambled PHINs to link children's administrative records across sectors to build a holistic picture of their health and development. We developed metadata, including routinized SAS algorithms and variable definitions, to ensure consistent operationalization of variables across studies. The longitudinal nature of these data allowed us to construct individual-level health and development trajectories from birth through adolescence for children born from 1984-2014. We used income data from the Canadian Census to develop both indicators of socioeconomic status (average neighbourhood level income) and provincial measures of income inequality (the Gini coefficient).
ResultsThe PATHS Resource includes data on the social determinants of health as well as health and development for children born 1984 to 2014 (n=608,007). We are able to follow children for a median observation period of 15.4 years. Income inequality – measured using the Gini coefficient – increased from 1984 to 2014: 0.16 to 0.21 (p<0.01). The proportion of children born to women from the bottom income quintile (i.e., the poorest 20% of families) also grew from 23.2% in 1984 to 27.2% in 2014 (p<0.01). When we followed children over their life course, we found that they were most likely to experience poverty (measured by family receipt of income assistance) at 2 years of age (p<0.01). Many studies from a variety of researchers have utilized the PATHS metadata to conduct child health and development research, ensuring consistent variable operationalization. These data have been used to identify policy levers for improving child health and reducing health inequalities.
ConclusionA resource such as the PATHS Resource can facilitate research into the health and development of children. Having data on the entire population allows investigators to both monitor trends in health inequities and identify strategies for improving health. Metadata ensure variable consistency and comparability across studies.

ABSTRACT
ObjectivesThere is increased interest in identifying strategies the reduce health inequities. With this focus, population health scientists have applied equity measures first developed in other disciplines to health equity research. The objective of this study is to illustrate the application of these measures in research using linkable administrative databases. This presentation will provide a brief description of some commonly-used equity measures and issues investigators face when applying them in their own health equity research.
MethodsAnalyses focused on children born in Manitoba, 1984 to 2014. We used linkable administrative data from health, social services, and education to develop indicators of health and the social determinants of health. Income data from the Canadian Census were used to stratify children by socioeconomic status. Our study considered the distribution of several child outcomes: breastfeeding initiation, mortality, complete immunization rates at age 2, Grade 9 completion, and high school completion. We examined several measures often used to capture income-related health inequities: rate ratios and rate differences comparing children from high-income neighbourhoods with children from low-income neighbourhoods; the concentration index which quantifies the equity in the distribution of outcomes across the entire socioeconomic gradient; and the relative and absolute indices of inequality which compare the most advantaged individuals with the least advantaged individuals in the population while accounting for the distribution of health across the population.
ResultsWhen these measures are applied to health equity, they can be affected by factors not initially considered by investigators. The application of Concentration measures using health outcomes that are frequently dichotomized, and the prevalence of the health outcome can affect the degree of inequity that is possible, with highly prevalent outcomes showing very little divergence from the line of equity. Comparing concentration measures to the inequality indices can produce contradictory and seemingly incompatible results. Sample selection that alters the distribution of income from the population can also change the apparent equity of health outcomes. These matters are complicated when monitoring changes in health equity, over time.
ConclusionsSummary measures of equity can be useful but come with limitations that need to be considered when interpreting and applying study findings. We offer some suggestions to consider when applying these measures in health equity research.

ABSTRACT
ObjectivesThe objective of this study was to identify whether breastfeeding inequalities have increased between 1984 and 2014 and to examine whether trends in income inequality are related to breastfeeding inequalities.
MethodsWe used linkable administrative data from the Population Health Research Data Repository. Our sample included all infants born in Manitoba, 1984 to 2014. We used area-level income – derived from the Canadian Census – to stratify infants into income quintiles. Canadian Census income data were also used to quantify provincial level income inequality for each fiscal year in our study period. Data from the hospital discharge abstract database were used to classify infants according to whether or not they had initiated breastfeeding. We linked infant data to maternal data using the Manitoba health insurance registry to capture maternal characteristics – including the mother's postal code of residence and her age at first birth. We used generalized linear models to calculate income quintile-specific breastfeeding rates for each fiscal year in our observation period for all of Manitoba. We also calculated age-adjusted breastfeeding rates to account for the changing age distribution in Manitoba mothers, over time. We measured breastfeeding inequities using the concentration index as well as the rate ratio and rate difference (comparing the breastfeeding rate between the highest and lowest income quintiles). We quantified income inequality using the Gini coefficient on income. Trend analyses and two-sided Z-tests tested for changes, over time. Time by income-quintile interactions tested whether breastfeeding rates were statistically significantly different, across socioeconomic groups.
ResultsBreastfeeding rates increased from 1985 to 2014, from 72% to 81% (p<0.01). The Gini coefficient increased from 0.16 to 0.21; a linear trend test of the Gini coefficient showed income inequality increased over the study period (p<0.05). Rate differences, rate ratios, and the Concentration index showed that significant breastfeeding inequalities existed throughout the study period. Trend tests revealed that breastfeeding inequalities did not increase, over time.
ConclusionsAggregate analyses may suggest overall improvement when inequality persists. Although there was improvement in breastfeeding initiation rates, children from lower socioeconomic status continue to lag behind their counterparts. Policy-focused health equity research needs to measure outcomes, overall, and inequity across time.

IntroductionAdolescent girls in care of child protection services are more likely to become pregnant than adolescent girls not in care, and mothers who were in care are more likely to have their children placed in care.
Objectives and ApproachLinkable administrative data were used to determine whether adolescent mothers in care are at greater risk of having their child placed in care before age two. A population-based cohort of adolescent mothers whose first child was born in Manitoba, Canada between April 1, 1998, and March 31, 2013 (n = 5,942) was used. Adjusted odds ratios (AOR) of having that first child taken into care before their second birthday were compared between mothers who were in care (n = 576) and mothers who were not in care (n = 5, 366) at the birth of their child using logistic regression models.
ResultsAdolescent mothers who were in care had greater odds of having their child taken into care before the child's second birthday (AOR = 7.53; 95% CI = 6.19-9.14). Specifically, their children had higher odds of being taken into care in their first week of life (AOR = 11.64; 95% CI = 8.83-15.34), between one week and their first birthday (AOR = 3.63; 95% CI = 2.79-4.71)., and between their first and second birthday (AOR = 2.21; 95% CI = 1.53-3.19).
Conclusion/ImplicationsFindings support an intergenerational cycle of involvement with child protection services. More and better services are required for adolescent mothers who give birth while in care of child protection services.

Context: Information-rich environments, with access and funding provided by government, make it possible to organize longitudinal administrative data to support analyses of policy-relevant questions. This paper describes insights into children's well-being and social equity obtained from data available in Manitoba, Canada, and highlights findings that have engaged policymakers.

ObjectiveThe objective was to examine the relationship between maternal intimate partner violence (IPV) victimization and children's developmental health using linked population-wide administrative datasets. We examined developmental vulnerability (DV) at kindergarten of children exposed to maternal IPV victimization aged 0 to 5 using provincial prosecution records compared to unexposed counterparts.
ApproachThis retrospective cohort study linked administrative datasets (legal, health, education, social services) from the Population Research Data Repository at the Manitoba Centre for Health Policy. Exposed mother-child pairs with 1+ prosecution records of maternal IPV victimization during early childhood (child aged 0 to 5) between 2003-2018 in Manitoba (n = 5,728) were matched to unexposed pairs (1:3) based on sex/birthdate of child and neighbourhood income. DV at kindergarten was measured across 5 domains (physical, social, emotional, language/cognitive [LC], communication/general knowledge) using the Early Developmental Instrument (EDI). Children without eligible EDI scores were excluded. Multiple logistic regression models were conducted.
ResultsThe cohort included 5321 children (exposed n=1365, unexposed n=3956). 32.98% of the cohort was developmentally vulnerable in one or more domains (1/+) and 19.60% was developmentally vulnerable in two or more domains (2/+). Unadjusted relationships between maternal IPV victimization from age 0 to 5 and developmental vulnerability at kindergarten were statistically significant across all 5 domains (e.g., LC OR=2.76 [2.36, 3.23]) and in 1/+ (OR=2.72 [2.39, 3.09]) as well as 2/+ (OR=2.89 [2.51, 3.34]) domains. After adjusting for covariates, children who were exposed to maternal IPV victimization from ages 0 to 5 had increased odds of being developmentally vulnerable in social competence (aOR=1.33 [1.07, 1.66]) and emotional maturity (aOR=1.29 [1.03, 1.62]), also in 2/+ domains (aOR=1.42 [1.15, 1.73]) at kindergarten, compared to unexposed counterparts.
ConclusionThe study provided Canadian population-wide evidence of the association between maternal IPV victimization and early childhood development, specifically later socio-emotional vulnerability. Interventions and support systems for this population of families should be developed and implemented, with an emphasis on mitigating long-term socio-emotional developmental risks in children exposed to IPV.

ObjectivesResponding to the Truth and Reconciliation Commission of Canada's (TRC) Call to Action #19 to close the gap in maternal/child outcomes, the goal of this study was to provide a baseline for select outcomes and demonstrate how an Indigenous/decolonizing framework can be applied to population health research involving Indigenous people.
ApproachThis retrospective cohort study was embedded within a decolonizing and Indigenous framework. Data extracted from administrative data housed at the Manitoba Centre for Health Policy was utilized to create a cohort of low-risk women residing in FN communities delivering a baby between 2005-2015. Two groups of mother/child dyads were compared: those evacuated for birth and those who were not required to leave home. Data were analyzed to assess the association between the evacuation policy on health outcomes.
ResultsDecolonizing and Indigenous frameworks are feasible, essential, and necessary in population health research involving Indigenous people. This methodology does not detract from scientific rigor. In keeping with Indigenous methodology, Knowledge Keepers and a Grandmother Advisor informed the research from the onset, including insightful dialogue about the study findings. Using such an approach, this study generated evidence that the present-day OFC policy continues to harm Indigenous women, families, and communities. The OFC policy is associated with increased odds of inadequate PNC (OR 1.64 1.51, 1.79 CI) and small for gestational age births (OR 1.25 1.02, 1.50 CI) and decreased breastfeeding initiation (OR 0.55 0.50, 0.61 CI) and maternal psychological distress diagnoses (OR .43 0.36, 0.51), after adjusting for various confounders.
ConclusionThis study documented a journey of an Anishinaabekwe in the space where western and Indigenous methodologies met. In answering the TRC call to improve maternal and infant outcomes, epidemiological and population health research requires epistemological frameworks that adequately incorporate the voices and realities of Indigenous people's lives while remaining scientifically rigorous.

Children without eligible EDI scores were excluded. Multiple logistic regression models were conducted to address the objective.
ResultsThe eligible cohort included 927 children (exposed n=229, unexposed n=698); 31.07% of the cohort was developmentally vulnerable in one or more domains (1/+) and 19.53% was developmentally vulnerable in two or more domains (2/+). Children who were prenatally exposed to maternal IPV victimization had increased odds of vulnerability across all 5 developmental domains (e.g., physical health/wellness: OR=2.83[1.95,4.10]; LC development: OR=2.45[1.65,3.64]). Unadjusted ORs showed statistically significant associations between maternal exposure of prenatal IPV victimization and DV in 1/+ (OR=2.70[1.98,3.68]) and 2/+ (OR=2.48[1.75,3.50]). When adjusted for covariates (e.g., maternal income assistance, mental health, child abuse history), no statistically significant relationship was found for any of the domains (e.g., LC development: aOR=0.98[0.53,1.81]), 1/+ (aOR=1.17[0.72,1.88]), and 2/+ (aOR=1.14[0.67,1.95]).
ConclusionThe unadjusted, statistically significant associations suggest children exposed to maternal IPV victimization prenatally may face associated social/health risks. The finding highlights the need to consider potential factors that put children at risk of DV when developing and implementing support systems/interventions for children exposed to maternal IPV victimization.

Objectives and ApproachAnalysis was conducted of women with children born in Manitoba between April 1, 1984 and Mach 31, 2012, with follow up till 2013 using linkable administrative and novel clinic data. Study group included women whose child(ren) were diagnosed with FASD from 1999 to 2012 (n=702) at a centralised FASD diagnostic clinic. Comparison group included women from the general population whose children who did not have an FASD diagnosis (n=2097), matched on the index child's birthdate, postal code, and SES. Adequacy of PNC utilization was defined using the revised Graduated Index of Prenatal Care Utilization.
ResultsThis is the first population-based study to investigate rates of PNC usage of women who have given birth to children with FASD. Rates of inadequate PNC were higher among the study group (adjusted RR 2.47, 95% CI 2.08 to 2.94), as well as no PNC (adjusted RR 3.55, CI 2.42 to 5.22). Among the study group 63% accessed PNC, with 59% receiving intermediate, adequate, or intensive PNC.
Conclusion/ImplicationsResults represent opportunity for screening and brief intervention programs to be delivered in PNC health care settings, as well as outreach programs to facilitate the uptake of PNC among high risk women.

IntroductionThe quality of early education children receive influences their developmental trajectories, with long-term effects extending into adulthood. First Nations children face many structural barriers to academic success. Few studies have examined the impact of education programs on removing these barriers to support better outcomes amongst First Nations children.
Objectives and ApproachWe examined educational outcomes associated with full-day (FDK) versus half-day (HDK) kindergarten among First Nations children using data from the Manitoba Population Research Data Repository. We linked children's education records with the Manitoba Health Registry and the First Nations Registry to identify all First Nations children who attended kindergarten in a Winnipeg school division (1998-2011). Children enrolled FDK were age- and sex- matched to children enrolled in HDK. Propensity scores used to adjust for confounding. Outcomes included academic achievements in grades 3, 7, and 8 and high school graduation. We used generalized linear models to test for differences in education.
ResultsWe identified 324 First Nations children enrolled in FDK and 595 matches in HDK in the study period. Among these, 37% FDK and 31% HDK students met or approached numeracy expectations in grade 3; and 30% FDK and 33% HDK met or approached numeracy expectations in grade 7. For reading expectations, 37% FDK and 33% HDK met or approached grade 3 reading expectations; in grade 8, roughly half of each group met or approached expectations for reading and writing. High school graduation rate for First Nations children in both FDK and HDK children was 60%. We found no differences in education outcomes when we tested for differences between HDK and FDK children.
Conclusion/ImplicationsEducation outcomes did not differ between First Nations children enrolled in FDK vs. HDK programs. Kindergarten programs may be insufficient to overcome structural barriers to academic success that these children face. Culturally appropriate education strategies may be needed to support improved outcomes amongst this population.
