Drug shortages have been identified as a public health problem in an increasing number of countries. Our study found that formal definitions of drug shortages currently exist in only a few countries. The characteristics of drug shortages, including their assortment, duration, frequency, and dynamics, were found to be variable and sometimes difficult to assess. Numerous information hubs were identified. Providing public access to information on drug shortages to the maximum possible extent is a prerequisite for performing more advanced studies on the problem and identifying solutions. Imposing public service obligations, providing the formal possibility to prescribe unlicensed medicines, and temporary bans on parallel exports are widespread measures. A positive finding of our study was the identification of numerous bottom-up initiatives and organizational frameworks aimed at preventing or mitigating drug shortages. The experiences and lessons drawn from these initiatives should be carefully evaluated, monitored, and presented to a wider international audience for careful appraisal. To be able to find solutions to the problem of drug shortages, there is an urgent need to develop a set of agreed definitions for drug shortages, as well as methodologies for their evaluation and monitoring. This is being progressed.

Personalization services are developing in England as a social policy response to user demands for more tailored, effective and flexible forms of health and social care support. This process is being implemented under the personalization which is also seen as a vehicle for promoting service user rights through increasing participation, empowerment and control while also promoting self-surveillance by having users manage the costs of their health and social care., There has been an accelerating interest in the implementation of personalisation policies relying upon a relentless political campaign to legitimise an enforced obligation to care, ie, UK Prime Minister Cameron's notion of a "Big Society". The use of personalisation that focus on self-assessment and inspection, can, in this policy and austere climate, become a means of self-surveillance. It is argued that Michel Foucault offers a set of strategies (Foucault 1977: 205) for understanding how the discourses on personalisation construct service users experiences and their identities, as constructed subjects and objects of social policy and managerial knowledge.

Contemporary drug policy debates are riven by a protracted antagonism between discourses that emphasise addiction, habit and despair, and those that endorse recreation, pleasure and self-control. This article eschews this antagonism by examining drug use in relation to what Michel Foucault called an ethics of care. My goal is to begin to imagine how drug use may be governed otherwise, in ways that avoid the normalisation associated with existing policy responses to drug problems. I ground this discussion in qualitative research conducted in 2012 in Melbourne, Australia with 31 drug consumers, and 15 health service providers. I explore how consumers govern their own drug use, and how these practices fit within consumers' broader efforts to promote or maintain their health. I then use these findings to sketch a novel approach to drug policy formulation, less concerned with the amelioration of drug problems and more interested in promoting an ethics of care.

AbstractObjectivesTo translate and cross‐culturally adapt the oral health impact profile‐14 (OHIP‐14) questionnaire into Indian sign language (ISL) using a video format and to evaluate its psychometric properties.MethodsThe study was conducted among hearing‐impaired individuals attending the National Institute of Speech and Hearing, Thiruvananthapuram, Kerala. The OHIP‑14 was translated to ISL in a video format. The content validity and face validity were assessed. The final questionnaire was renamed as OHIP‐12 ISL and was administered on 120 hearing‐impaired individuals. Oral health indicators, OHIP‐12 ISL scores, and a single‑item global question (GQ) scores were used for construct validity assessment. Internal consistency and test‐retest reliability were assessed using Cronbach's alpha and intraclass correlation (ICC) coefficient. Discriminant validity was assessed by comparing OHIP‐12 ISL scores with oral health indicators by using Mann‐Whitney U test and Fischer's exact test. Convergent validity was assessed by correlating OHIP‐12 ISL scores with GQ scores and oral health indicators by using Spearman's rank correlation coefficient.ResultsThe OHIP‐12 ISL demonstrated high Cronbach's alpha (α = 0.80). Average ICC coefficient was 0.98 (0.97‐0.99) and demonstrated acceptable construct validity.ConclusionsTranslated and culturally adapted OHIP‐12 ISL is a valid and reliable instrument to measure the OHRQoL among hearing impaired in India.

Public health nursing (PHN) practice is population-focused and requires unique knowledge, competencies, and skills. Early public health nursing roles extended beyond sick care to encompass advocacy, community organizing, health education, and political and social reform. Likewise, contemporary public health nurses practice in collaboration with agencies and community members. The purpose of this article is to examine evolving PHN roles that address complex, multi-causal, community problems. A brief background and history of this role introduces an explanation of the community participation health promotion model. A community-based participatory research project, Youth Substance Use Prevention in a Rural County provides an exemplar for description of evolving PHN roles focused on community health promotion and prevention. Also included is discussion about specific competencies for PHNs in community participatory health promoting roles and the contemporary PHN role.

BACKGROUND AND OBJECTIVE: In Sweden, mortality from cardiovascular diseases (CVD) increased steadily during the 20th century and in the mid-1980s it was highest in the county of Västerbotten. Therefore, a community intervention programme was launched - the Västerbotten Intervention Programme (VIP) - with the aim of reducing morbidity and mortality from CVD and diabetes. DESIGN: The VIP was first developed in the small municipality of Norsjö in 1985. Subsequently, it was successively implemented across the county and is now integrated into ordinary primary care routines. A population-based strategy directed towards the public is combined with a strategy to reach all middle-aged persons individually at ages 40, 50 and 60 years, by inviting them to participate in systematic risk factor screening and individual counselling about healthy lifestyle habits. Blood samples for research purposes are stored at the Umeå University Medical Biobank. RESULTS: Overall, 113,203 health examinations have been conducted in the VIP and 6,500-7,000 examinations take place each year. Almost 27,000 subjects have participated twice. Participation rates have ranged between 48 and 67%. A dropout rate analysis in 1998 indicated only a small social selection bias. Cross-sectional, nested case-control studies and prospective studies have been based on the VIP data. Linkages between the VIP and local, regional and national databases provide opportunities for interdisciplinary research, as well as national and international collaborations on a wide range of disease outcomes. A large number of publications are based on data that are collected in the VIP, many of which also use results from analysed stored blood samples. More than 20 PhD theses have been based primarily on the VIP data. CONCLUSIONS: The concept of the VIP, established as a collaboration between politicians and health care providers on the one hand and primary care, functioning as the operating machinery, and the public on the other, forms the basis for effective implementation and endurance over time. After more than 20 years of the VIP, there is a large comprehensive population-based database, a stable organisation to conduct health surveys and collect data, and a solid structure to enable widespread multidisciplinary and scientific collaborations.

Background: The recent interest in recovery from mental health problems has not meaningfully addressed the perspectives of ethnic minorities. Aim: To contribute to the discussion of recovery-oriented service with a study on the experience of Chinese people using UK mental health services. Methods: In-depth life history interviews were carried out with the users. The qualitative data were analysed using thematic analysis. Results: Four themes emerged as hindrances to personal recovery: (1) language difficulty creates hurdles, (2) diagnostic label is experienced as a double-edged sword, (3) treatment-related stigma and (dis)empowerment are identified, and (4) grievances are found in hospitalisation. Discussion: Having mental illness and being an ethnic minority in the UK experienced double hazard in their recovery journey. While the deprivation of agency and the stigma process in the health care system hinders their recovery, they are further disadvantaged by their ethnic minority status. Four pointers for service improvement, that apply to Chinese users in the United Kingdom and have general implications for users beyond this group, are proposed.

Abstract Background With the date for achieving the targets of the Millennium Development Goals (MDGs) approaching fast, there is a heightened concern about equity, as inequities hamper progress towards the MDGs. Equity-focused approaches have the potential to accelerate the progress towards achieving the health-related MDGs faster than the current pace in a more cost-effective and sustainable manner. Ghana's rate of progress towards MDGs 4 and 5 related to reducing child and maternal mortality respectively is less than what is required to achieve the targets. The objective of this paper is to examine the equity dimension of child and maternal health outcomes and interventions using Ghana as a case study. Methods Data from Ghana Demographic and Health Survey 2008 report is analyzed for inequities in selected maternal and child health outcomes and interventions using population-weighted, regression-based measures: slope index of inequality and relative index of inequality. Results No statistically significant inequities are observed in infant and under-five mortality, perinatal mortality, wasting and acute respiratory infection in children. However, stunting, underweight in under-five children, anaemia in children and women, childhood diarrhoea and underweight in women (BMI < 18.5) show inequities that are to the disadvantage of the poorest. The rates significantly decrease among the wealthiest quintile as compared to the poorest. In contrast, overweight (BMI 25-29.9) and obesity (BMI ≥ 30) among women reveals a different trend - there are inequities in favour of the poorest. In other words, in Ghana overweight and obesity increase significantly among women in the wealthiest quintile compared to the poorest. With respect to interventions: treatment of diarrhoea in children, receiving all basic vaccines among children and sleeping under ITN (children and pregnant women) have no wealth-related gradient. Skilled care at birth, deliveries in a health facility (both public and private), caesarean section, use of modern contraceptives and intermittent preventive treatment for malaria during pregnancy all indicate gradients that are in favour of the wealthiest. The poorest use less of these interventions. Not unexpectedly, there is more use of home delivery among women of the poorest quintile. Conclusion Significant Inequities are observed in many of the selected child and maternal health outcomes and interventions. Failure to address these inequities vigorously is likely to lead to non-achievement of the MDG targets related to improving child and maternal health (MDGs 4 and 5). The government should therefore give due attention to tackling inequities in health outcomes and use of interventions by implementing equity-enhancing measure both within and outside the health sector in line with the principles of Primary Health Care and the recommendations of the WHO Commission on Social Determinants of Health.

While there is sufficient evidence of an association between oral health and overall health, little is discussed in terms of the cost savings that can be realized by exploiting this linkage. Evidence of costs and savings generated from providing oral care to several populations is garnered from the scholarly literature to demonstrate a few strategies that can help policy makers in realizing these savings.
