Ouvrage sous la direction de Stephanie Latte Abdallah ; The article tackles the evolution of UNRWA's representation of the Palestine refugees as displayed in its official discourse. It shows that, since the early years of the refugees' exile in the fifties, UNRWA's narrative has constituted a political tool aimed at managing and responding to the multiple pressures that its various stakeholders (from the refugees and their various representatives to the donor and host countries) have exerted on it for political and socioeconomic purposes. In so doing the article looks more specifically into the dynamics that take place in the long run between assistance providers and assistance recipients. Indeed, UNRWA's history offers us a textbook example of the interactions that may occur between operational considerations as those articulated by humanitarian agencies and political interests as those promoted by refugees involved in a national liberation movement. ; L'article porte sur l'évolution de la représentation du réfugié de Palestine dans le discours official de l'UNRWA (l'Office de secours et de travaux des Nations Unies pour les réfugiés de Palestine au Proche-Orient). Il montre que, des premières années de l'exil dans les années 1950, cette fonction de représentation s'est apparentée à un véritable instrument politique destiné à répondre aux pressions qui se sont exercées sur lui de la part des acteurs avec lesquels il lui a fallu négocier les termes de sa mission humanitaire, soit les représentants des réfugiés, les bailleurs de fonds, ainsi quel les autorités des pays d'accueil. A travers cette étude, c'est plus précisément la dynamique des rapports « assistant-assisté » qui est cernée. L'exemple de l'UNRWA nous offre en effet un parfait exemple des interactions qui peuvent se produire sur le long terme entre des impératifs de type opérationnel, tels que formulés par les agences humanitaires, et des intérêts de nature politique, tels que défendus par les réfugiés engagés dans un mouvement de libération nationale.

Ziel dieser Modellbildung und Untersuchung ist es, der Frage nachzugehen, inwieweit die seit der zweiten Hälfte der 70er Jahre andauernden Reformbemühungen im bundesdeutschen Ge-sundheitswesen zu einer langfristigen und grundlegenden Modifikation, Umgewichtung oder gar Neudefinition gesundheitspolitischer Zielvorstellungen im stationären Sektor geführt ha-ben. Geprüft wird die Hypothese, ob als Folge der zahlreichen Kostendämpfungsgesetze und insbesondere mit der Verabschiedung des Gesundheitsstrukturgesetzes im Jahre 1993 ein ord-nungspolitischer Wandel eingeleitet wurde, der sich bis heute fortsetzt. Neben dieser politik-feldbezogenen Fragestellung, wird auf einer allgemeineren Ebene das grundsätzliche Problem der Messung von langfristigem sozialpolitischen Wandel angesprochen und einer Operationa-lisierung zugeführt. Im Mittelpunkt der Studie stehen eine systematische Analyse der bundesdeutschen Reformge-setzgebung im Gesundheitswesen bis zum Jahre 2003 und eine qualitative Befragung von Chefärzten im Bundesland Bremen ebenfalls aus dem Jahr 2003. Ausgehend von einer Unter-suchung der Makrobene (Bundesgesetze), wird anhand einer Studie auf der Mikroebene der Implementation (Chefärzte) gezeigt, wie erfolgreich die politisch angestrebte Veränderung des Akteurshandelns auf Seiten der Leistungserbringer im stationären Sektor bisher gewesen ist. ; Matters of long-term social policy shift analysis do not figure prominently in German health care research. It is still highly disputable to what extent 30 years of reform efforts in the German health care system have resulted in long-term, fundamental modifications or shifts in the hierarchical order of health care targets. The author tests the hypothesis whether the numerous laws aiming at cost containment as well as the German Health Structure Law (Gesundheitsstrukturgesetz/GSG) in 1993 have induced an ongoing process of structural change in regulatory policy (ordnungspoli-tischer Wandel). A theoretical model to measure this kind of change in German health care policy is presented and applied. This study analyses German health care legislation up to the year 2003 as well as qualitative interviews with head physicians in hospitals (Chefärzte). These interviews were conducted in the state (Bundesland) of Bremen in 2003. Moving from a macro-level investigation (federal legis-lation) to a micro-level investigation of implementation (head physicians) the author shows to what extent changes in legislation have invoked changes in attitude and behaviour on the part of service providers in the inpatient sector of health care services.

El objetivo de este trabajo es analizar el impacto del proceso de urbanización en el crecimiento económico en México utilizando los métodos econométricos modernos. Los resultados obtenidos indican que existe una relación estable de largo plazo y positiva entre el producto, la inversión y un índice de urbanización. Ello indica que la inversión y el proceso de urbanización han favorecido el ritmo de crecimiento económico de México. En este sentido, el acelerado proceso de urbanización del país durante el siglo XX tuvo efectos positivos sobre el ritmo de crecimiento económico. Debe sin embargo considerarse que las características del proceso de urbanización en México se han traducido en un alto nivel de concentración económica, en particular en la Zona Metropolitana de la Ciudad de México, con costos crecientes para el crecimiento económico de largo plazo. Ello se refleja en un proceso de desconcentración de las megalópolis a zonas conurbadas en la década de los ochenta, que se ha estabilizado relativamente en los últimos años. AbstractThis article seeks to analyze the impact of urbanization on economic growth in Mexico, using modern econometric methods. The results obtained suggest that there is a stable, long-term positive relationship between the product, investment and the urbanization index. This suggests that investment and urbanization have contributed to the rate of economic growth in Mexico. In this respect, the country's rapid urbanization in the 20th century had a positive effect on the economic growth rate. One should, however, recall that the characteristics of urbanization in Mexico have translated into a high level of economic concentration, particularly in the Mexico City Metropolitan Zone, with growing costs for long-term economic growth. This is reflected in the decentralization of megalopolises to conurbations during the 1980's which has stabilized somewhat in recent years.

By its own the global warming issue synthesises all the relevant dimensions and difficulties met by the project of sustainable development: long term effects and threats, real global dimensions, need of incentives for decentralizing action to billions of people, scientific uncertainty. More than fifteen years were necessary to build a new global regime of climate protection. In spite of that, the situation is still blocked in 2004 and seemingly condemned to lead to dead-ends, failing to grasp problems at their true level. The initial question was the following: how can countries that are opposed by any criterion succeed in finding an agreement to protect a public good that is felt essential by some of them, while no superior political authority exists that could impose a common law to all countries? An exploration of this question with the help of the three main theoretical approaches used to think the issue of international coordination shows that none can lead to success. An examination of a few alternatives strategies does not allow finding a convincing solution. The fact that language has been an important issue for all Mauritanian leaders does not mean they have always implemented their policies in a rigorous and well planned manner. According to the author of this article, a serious lack of long-term planning is a common fault of the Mauritanian government, and in cases of language planning this fault is aggravated by a very narrow approach. While language status seems to various authorities to be controllable by state decisions, Mauritanian language policy is often limited, for all intents and purposes, to legislating only at th school-instruction level. In the spring of 1999, a new (and umpteenth) reform was announced, once again without prior consultation of various stakeholders in the education system. The author describes this reform and analyzes its goals and possible limitations. She also chronicles the different stages through which th education system has passed over time, and identifies the linguisic habits that shape the current language market of the country. AbstractThe authors suggest a revision of the current strategy of reconstruction and peace‐building in Afghanistan with reference to four points. (i) The programme of reconstruction should have a clear strategic focus and be designed as a state‐building project. The main problem Afghanistan faces is the absence of a monopoly of power and of other basic state functions, without which no sustainable economic development is to be expected. Fostering 'civil society'—the standard pre‐occupation of many current development co‐operation programmes—may have a negative impact in the cases of countries suffering from state failure. (ii) A monopoly of power cannot be established only by building up a central army and disarming or integrating local armed forces. Rather, the loyalty of the country's citizens must be won through a long‐term process of providing them with equality before the law and protection from arbitrary violence in such a way as to make them independent of the 'protection' of local strong men or regional warlords. (iii) It is too early to achieve democracy in Afghanistan through elections. A democracy can only release the potential for political integration following successful political stabilization and institutional consolidation. And the institutional framework should be designed in such a way as to avoid an escalation of conflicts along ethno‐religious lines. An ethno‐religious quota system of the sort that many have in mind at the moment would tend rather to increase such conflict than to reduce it. (iv) A programme of reconstruction and peace‐making must necessarily take a long‐term perspective. Spending the resources that have now been approved over the next four years and hoping for a continuation of aid approvals in the future is too risky, given the dependence of these approvals on the future political climate. © 2003 John Wiley & Sons, Ltd.

Per capita and total beef consumption decreased in the EU over the past decade but evolutions differ significantly among the various Member States. However, EU beef consumption recovered from the second BSE crisis, except in Germany. Prospects suggest that world beef trade should increase in the medium and long term. The EU would not be able to take profit from this increase because of its low price competitiveness relative to other exporters. The EU beef market equilibrium is highly sensitive to various parameters related to assumptions on production, imports, per capita consumption and subsidised exports. <i>Objective:</i> To assess the effectiveness of a behavioural group intervention for smoking cessation, which included as its most original feature the recommendation to participate with a relative or close friend. <i>Methods:</i> A total of 1,060 subjects entered the programme, which consisted of 9 group sessions over a period of 5 weeks. The intervention consisted of a modified version of the Five-Day Plan, the main differences being the use of behavioural therapy techniques and small group work, and the addition of 4 weekly booster sessions. About two thirds of the participants came with a relative or close friend. Long-term abstinence from smoking was assessed with follow-up telephone interviews. <i>Results:</i> Very few subjects were lost to follow-ups (9.2% at 6 months, 9.7% at 1 year, 10.8% at 2 years). The observed quit rates were 42.6% at 6 months, 35.5% at 1 year and 32% at 2 years. When considering as smokers all subjects who were lost to follow-ups, quit rates were also satisfactory (38.7% at 6 months, 32.1% at 1 year and 28.6% at 2 years). The main predictors of a good outcome were being male, smoking less than 20 cigarettes per day, having started smoking after 18 years of age, having made previous quitting attempts, not having a history of unsuccessful participation to smoking cessation interventions and attending the sessions with a relative or close friend. <i>Discussion:</i> Although some limitations inherent in the design of our study suggest caution in interpreting the results and in making comparisons, the long-term effectiveness of the intervention was satisfactory. The inclusion of a relative or close friend appeared useful. This simple and inexpensive strategy may deserve recommendation, though in the future it should be tested in controlled trials.

La création de l'Union monétaire européenne confère une dimension expérimentale à l'acquisition de la crédibilité par une banque centrale et à sa mise à l'épreuve. Les modèles théoriques qui fondent la notion de crédibilité réduisent la configuration institutionnelle à un schéma très simple et l'évaluation collective de cette crédibilité est réduite à l'opinion moyenne des marchés financiers. Aussi, nous procédons à quelques variations sur un thème de Barro et Gordon (1983). Alors que dans ce modèle, les salariés fixent le salaire nominal avant que l'inflation soit effective, ici, les marchés financiers fixent le taux d'intérêt de long terme de telle façon qu'il égalise l'offre et la demande agrégée de biens. Pour être crédible, la banque centrale devra viser une inflation très faible pendant une longue période afin de faire converger les anticipations des marchés vers un niveau bas d'inflation. Si on projette ces raisonnements sur l'expérience européenne, on peut craindre qu'ils ne confortent l'idée d'un penchant restrictif de la BCE. Ce modèle est éclairant à un second titre : il peut engendrer des équilibres multiples, sources de problèmes de coordination entre agents pour définir un équilibre qui fasse l'objet d'un accord commun. Pour échapper à ces problèmes, la cohérence temporelle de l'action de la BCE, c'est-à-dire une articulation harmonieuse des objectifs et règles de long terme avec une action conjoncturelle adaptée aux circonstances, nécessite la coopération effective des diverses institutions en charge de la politique économique européenne. L'insertion plus franche de la BCE dans un réseau institutionnel qui active les obligations de motivation et de responsabilité auxquelles elle devrait satisfaire contribuerait à clarifier l'horizon qui gouverne ses décisions. <i>Objective:</i> As sequencing of the human genome is completed, there is a need for population-based research to assess frequencies of genetic variants and their associations with human diseases. The authors therefore assessed the current climate regarding the donation and storage of blood for genetic research. <i>Methods:</i> Data from the American Healthstyles Survey of health attitudes and behavior were examined. In the 1998 survey, four questions regarding blood donation and storage for genetic research were posed to the participants. <i>Results:</i> Of 3,130 participants, 2,621 (84%) completed these questions. Of the respondents, 42% were in favor of both blood donation and long-term storage for genetic research, 37% were in favor of either blood donation or storage but not both and 21% were not willing to donate blood or have it stored for genetic research under any circumstances. Loglinear analysis demonstrated that the characteristics of respondents who favored both blood donation and long-term storage for genetic research were attitudinal; specifically, those believing that genetic research will prevent disease [odds ratio (OD) 2.9; p < 0.001]; those believing in genetic determinism (OR 1.5; p = 0.004) and those agreeing they would participate in government research (OR 2.9; p < 0.001). The model also demonstrated that characteristics indirectly associated with attitudes towards blood donation/storage for genetic research were demographic and included higher education, white race, living in the Mountain/Pacific or mid-Atlantic regions of the United States and positive family history of a genetic disorder (p < 0.05). <i>Conclusion:</i> Understanding the various factors contributing to knowledge, attitudes and behavior regarding the donation and storage of blood specimens for genetic research will contribute to future actions in communicating genetic research goals to the public and recruitment for population-based genetic studies.

A letter report issued by the General Accounting Office with an abstract that begins "Before Social Security, being old often meant being poor. Today, dependency on public assistance has dropped to a fraction of its Depression-era levels, and poverty rates among the elderly are now lower than for the population as a whole. At the same time, Social Security has become the single largest source of retirement income for more than 90 percent of persons aged 65 and older. Automatic adjustments were introduced in 1972 to reflect increases in the cost of living. Other program changes gradually increased social security coverage to larger portions of the workforce and extended eligibility to family members and disabled workers. Other benefit programs, such as Supplemental Security Income (SSI), Medicare, and Medicaid, have also been added over the years. With regard to measuring income adequacy, various measures help examine different aspects of this concept, but no single measure can provide a complete picture. For various subgroups of beneficiaries that have lower lifetime earnings, poverty rates have also declined. Although the Social Security benefit formula favors lower lifetime earners, their lower earnings and work histories can leave them with incomes below the poverty level when they retire or become disabled. The outlook for future Social Security benefit levels and income adequacy depend on how the program's long-term financing imbalance is addressed, as well as on the measures used. GAO concludes that reductions in promised benefits and increases in program revenues will be needed to restore the program's long-term solvency and sustainability. Possible benefit changes might include adjustments to the benefit formula or reductions in cost-of-living increases. Possible revenue sources might include higher payroll taxes or transfers from the Treasury's general fund."

OBJECTIVE: To analyse the compliance with the primary health care quality criteria of the Committee for Quality Improvement of the Catalan Society of Family and Community Medicine. DESIGN: Cross-sectional descriptive study. SETTING: Reformed primary care centres (PCC) in Catalonia (186), of which 73 (39.2%) filled out the disk with the results. MEASUREMENTS AND MAIN RESULTS: A total of 202 criteria, divided into five sections (structure and material resources, human resources, work organisation, systems of recording and access, and continuity, confidentiality and participation), were self-evaluated in each PCC. These criteria were defined as basic or optimal and compliance with them was graded from 1 to 4. 72.8% of the criteria were fully complied with. In 7.4% there was partial compliance, and in 19.7% non-compliance. The basic criteria had 74% total compliance, whereas the optimal criteria had 51.9%. The groups with least compliance (sum of the total plus the partial) were records systems (68.6%) and human resources (73.5%). The organisation group had the best compliance (85.5%). By sub-groups, the greatest compliance was found in privacy (93.7%), direct care (89.2%) and recording actions (89.1%). Sub-groups with least compliance were long-term treatments (61.8%), vaccinations (46.4%) and storage (42%). Eight of the 18 criteria not complied with by 50% or more were considered responsibility of the team. There was 37.8% total compliance and 24% partial in the criteria referring to the specific training of primary care professionals. Up to 80% non-compliance was found in quality of long-term prescription records, vaccinations and storage. CONCLUSIONS: The high level of compliance in the sections which depend most on professionals should be noted (accessibility-continuity and organisation). This study is a first approach towards understanding the quality of the structure of PC in Catalonia, and enables proposals for improvement to be set up.
