This article uses 1960 census data to describe patterns of spouse selection among the native-born children of European immigrants. The analysis builds on previous studies of ethnic intermarriage, but is new in that it focuses specifically on the second generation. In addition, it considers intermarriage as a multidimensional phenomenon and evaluates how the relative importance of national and educational boundaries in marriage choice has changed. Comparisons of synthetic marriage cohorts suggest that second generation European Americans marry increasingly into the native stock, they marry increasingly out of their national origin group, and the national boundaries that separate them have become weaker over time. At the same time, it is found that educational homogamy has increased across cohorts. More generally, changes in the marital assimilation of the second generation can be characterized as a shift from national origins to education. Methodologically, the study is novel in that multidimensional logmultiplicative models of association are used as a new way of measuring marriage distances between groups.

Data are available for the first time for the year under report 2008 on the biological number of children of women from the comprehensive Microcensus sample. Previously, only the ongoing birth statistics provided information on cohort fertility as a total fertility rate of the synthetic female cohorts. Since both benchmarks bear different types of insecurity, a comparison is to provide important information on the quality of these indicators. This article shows that the average number of children per woman in the dataset of the Microcensus 2008 and the cohort fertility rate in the birth statistics show a highly similar course of cohort fertility. Whilst the agreement for the female cohorts in the new Länder (former GDR without Berlin East) and in Germany as a whole is almost complete, the values in the old Länder (Western Germany without Berlin West) are further apart. A major cause of this evidently lies in the high proportion of female immigrants in the old Länder. Some of the women who immigrated into the former territory of Federal Republic already gave birth to their children before entering Germany. These births could not be taken into account in the ongoing birth statistics, whilst the Microcensus provided information on all children born, regardless of where they were born. These survey-related deviations can be largely reduced if the comparison with the cohort fertility rate of the birth statistics is based not on the average number of children of all women, but only on the number of children of women who were born in Germany or who immigrated below the age of 26. The share of female migrants was low in the new Länder and did not impact cohort fertility. All in all, the comparison showed that both sets of statistics provide data with a good degree of concurrence on the fertility of the birth years from 1933 to 1992 for Germany as a whole, as well as for the old and the new Länder. This makes it possible to carry out detailed analyses of fertility in relation to comprehensive socioeconomic characteristics of the Microcensus.

Birth cohort effects in suicide rates are well established, but to date there is no methodological approach or framework to test the temporal stability of these effects. We use the APC- Detrended (APCD) model to robustly estimate intensity of cohort effects identifying non-linear trends (or 'detrended' fluctuations) in suicide rates. The new APC-Hysteresis (APCH) model tests temporal stability of cohort effects. Analysing suicide rates in 25 WHO countries (periods 1970–74 to 2005–09; ages 20–24 to 70–79) with the APCD method, we find that country-specific birth cohort membership plays an important role in suicide rates. Among 25 countries, we detect 12 nations that show deep contrasts among cohort-specific suicide rates including Italy, Australia and the United States. The APCH method shows that cohort fluctuations are not stable across the life course but decline in Spain, France and Australia, whereas they remain stable in Italy, the United Kingdom and the Netherlands. We discuss the Spanish case with elevated suicide mortality of cohorts born 1965–1975 which declines with age, and the opposite case of the United States, where the identified cohort effects of those born around 1960 increase smoothly, but statistically significant across the life course.

Several investigators have reported an increased frequency of attention/hyperactivity symptoms in international adoptees, though population-based studies are lacking. In this national cohort study, we aimed to determine the prevalence of ADHD medication in international adoptees in Sweden, in comparison to the general population. A further purpose was to study gender, age at adoption and region of origin as predictors of ADHD medication in international adoptees. The study population consisted of all Swedish residents born in 1985–2000 with Swedish-born parents, divided into 16,134 adoptees, and a comparison population of 1,326,090. ADHD medications were identified in the Swedish Prescribed Drug Register during 2006. Logistic regression was used to calculate the odds ratios. The rates of ADHD medication were higher in international adoptees than in the comparison population for both boys (5.3 vs. 1.5% for 10–15-year olds) and girls (2.1 vs. 0.3% for 10–15-year olds). International adoptees from all regions of birth more often consumed ADHD medication compared with the majority population, but the age and sex adjusted odds ratios were particularly high for adoptees from Eastern Europe, Middle East/Africa and Latin America. Adjusting for maternal education and single parenthood increased the odds ratios even further. The risk also increased with higher age at adoption. Adoptees from Eastern Europe have a very high risk for ADHD medication. A structured identification and support programme should be tailored for this group. Adoptees from other regions have a more moderately increased risk, which should be communicated to adoptive parents and to professionals who care for adoptees in their clinical practice.

Abstract Background To compare the results of a new-user cohort study design and the self-controlled case series (SCCS) design using the risk of hospitalisation for pneumonia in those dispensed proton pump inhibitors compared to those unexposed as a case study. Methods The Australian Government Department of Veterans' Affairs administrative claims database was used. Exposure to proton pump inhibitors and hospitalisations for pneumonia were identified over a 4 year study period 01 Jul 2007 -30 Jun 2011. The same inclusion and exclusion criteria were applied to both studies, however, the SCCS study included subjects with a least one hospitalisation for pneumonia. Results There were 105,467 subjects included in the cohort study and 6775 in the SCCS. Both studies showed an increased risk of hospitalisations for pneumonia in the three defined risk periods following initiation of proton pump inhibitors compared to baseline. With the highest risk in the first 1 to 7 days (Cohort RR, 3.24; 95% CI (2.50, 4.19): SCCS: RR, 3.07; 95% CI (2.69, 3.50)). Conclusions This study has shown that the self-controlled case series method produces similar risk estimates to a new-users cohort study design when applied to the association of proton pump inhibitors and pneumonia. Exposure to a proton pump inhibitor increases the likelihood of being admitted to hospital for pneumonia, with the risk highest in the first week of treatment.

IntroductionThe Dementias Platform UK (DPUK) Cohort Explorer is an interactive, online visualisation tool that allows users to explore data for a number of DPUK cohorts. Over 30 variables across cohorts have been harmonised, including information on demographics, lifestyle, cognition, health, and genetic biomarkers.
Objectives and ApproachThe tool has been developed to complement existing DPUK cohort metadata to provide a visual representation of participant numbers and field-level information for a selection of cohorts. This enables users to determine a cohort's eligibility before applying for access to a cohort's data, and aid in shaping potential hypotheses. Developed using Microsoft PowerBI, the Explorer hosts a subset of the cohort's baseline, harmonised data, allowing a user to interrogate the visualisations of the uploaded data in a secure manner on the DPUK Data Portal website. Visualisations are linked so that participant numbers and distributions can be explored interactively.
ResultsThis approach allows the user to explore the harmonised data across a number of cohorts simultaneously whilst setting and adjusting filters that are of interest to the user's search criteria. This provides a better understanding of the real-world data and enables the user to determine the feasibility of each cohort for potential studies, whilst facilitating meaningful comparisons across cohorts. The tool currently visualises five DPUK cohorts with a total of 82,391 participants, however it is being incrementally developed with more cohorts being added continually.
Conclusion / ImplicationsBy combing an easy-to-use, interactive dashboard with harmonised sets of real-world data, the tool allows the user to explore, interrogate and better understand field-level information in a secure manner with zero data transfer. This provides more insight for the user when applying for access to a cohort dataset using the DPUK Data Portal and may help the user to make more informed decisions and/or hypotheses.

Objective: To investigate the self-rated health of the UK military and explore factors associated with poor self-rated health. Compare self-rated health of the military to the general population. Design: A cohort study. Participants: A total of 7626 serving and ex-serving UK military personnel, aged between 25 and 49; 19,452,300 civilians from England and Wales. Setting: United Kingdom (military), England and Wales (civilians). Main outcome measures: Self rated health for both populations. Additional data for the military sample included measures of symptoms of common mental disorder (General Health Questionnaire-12), probable post-traumatic stress disorder (post-traumatic stress disorder checklist Civilian Version), alcohol use (Alcohol Use Disorders Identification Test), smoking behaviour, history of self-harm and body mass index. Results: In the military sample, poor self-rated health was significantly associated with: common mental disorders and post-traumatic stress disorder symptomology, a history of self-harm, being obese, older age (ages 35–49) and current smoking status. However, the majority of military personnel report good health, with levels of poor self-rated health (13%) not significantly different to those reported by the general population (12.1%). Conclusions: Self-rated health appears to relate to aspects of both physical and psychological health. The link between poor self-rated health and psychological ill-health emphasises the need for military support services to continue addressing mental health problems.

Justice between generations is still not as prominent on any agenda as justice between rich and poor or men and women. For the first time, this three-part book explores the situation of young people of today in comparison to their direct predecessors. The first part, The Financial Situation of the Young Generation in a Generational Comparison, deals with this generation's financial standing; the second part, The Rush Hour of Life, examines their time restrictions. Both are considered from a life-course perspective. The third part, On the Path to Gerontocracy?, addresses the demographic shift in favor of the elderly in aging Western democracies.

PurposeThe purpose of this paper is to investigate the association of social networks with being in work, contrasting those under age 50 with those over 50 years.Design/methodology/approachA case study is undertaken of a local labour market in Scotland. Data were collected by interview using a semi‐structured questionnaire from 194 people divided into four groups. Data include information on individuals' socio‐economic characteristics and on their networks. A four‐way comparison is made by age and employment status.FindingsThose in work have denser social networks populated with members with higher social and human capital. For those over 50 years, the more contacts one has with higher prestige employment positions (a proxy for social capital), and the stronger the ties with these contacts, the more likely that one is to be in employment. For those under 50 years, their own qualifications and the number of contacts are important.Research limitations/implicationsThis work adds to both research on employability and social networks.Social implicationsThe over 50s tend to be the age group that is most likely to be not in employment and as populations age there is a need to ensure that barriers to employment against those over 50 are reduced. Finding routes to reduce unemployment will also help combat social exclusion.Originality/valueThis is in the combination of a social network approach with age cohort analysis to give insight into how social capital is associated with being in employment.

OBJECTIVES: This study investigated the risk associated with interhospital transfer of patients with acute myocardial infarction (AMI) and clinical outcomes according to the location of the patient' residence. DESIGN: A nationwide longitudinal cohort. SETTING: National Health Insurance Service database of South Korea. PARTICIPANTS: This study included 69 899 patients with AMI who visited an emergency centre from 2013 to 2015, as per the Korea National Health Insurance Service database. PRIMARY OUTCOME MEASURE: The clinical outcome of a patient with AMI was defined as mortality within 7 days, 30 days and 1 year. RESULTS: Clinical outcomes were analysed and compared with respect to the location of the patient's residence and occurrence of interhospital transfer. We concluded that the HR of mortality within 7 days was 1.49 times higher (95% CI 1.18 to 1.87) in rural patients than in urban patients not subjected to interhospital transfer and 1.90 times higher (95% CI 1.13 to 3.19) in transferred rural patients than in non-transferred urban patients. CONCLUSIONS: To reduce health inequality in rural areas, a healthcare policy considering regional characteristics, rather than a central government-led, catch-all approach to healthcare policy, must be formulated. Additionally, a local medical emergency delivery system, based on allocation of roles between different medical facilities in the region, must be established.

This study was designed to inquire into the effects of aging versus age cohort by examining how men and women representing four age cohorts across the life span endorse masculinity ideology. Ascertaining whether different groups (such as age cohorts) understand a scale in the same way by assessing measurement invariance is a fundamental but oft-ignored prerequisite to comparing their scores on the scale. The Male Role Norms Inventory-Short Form (MRNI-SF) is a multidimensional measure used to assess beliefs in specific norms of masculinity, as well as general beliefs in traditional masculinity ideology (TMI). A five-item unidimensional MRNI -Very Brief (MRNI-VB) has also been developed measuring TMI only. This study administered the MRNI-SF (which includes the five items for the MRNI-VB) to 1,352 men and women in four age cohorts: young, established, middle-aged, and older. Multi-group confirmatory factor analyses found support for partial strong invariance for the MRNI-SF across age cohorts. Support for partial strict invariance was found for the MRNI-VB across age cohorts. For both scales, the effect sizes of non-invariant parameters were small in magnitude, thus non-invariance may have little practical significance. These results suggest that the MRNI-SF and the MRNI-VB measure similar masculinity ideology constructs across men and women in four age cohorts. Mean scores on each instrument were therefore compared across age cohorts with confidence, finding that the age cohorts commonly did not endorse "traditional" masculinity ideology, but differed in the level of disagreement with the tenets of TMI in six out of nine comparisons. In five of those six, older adults hewed more strongly to TMI than younger adults, suggesting that age cohort was more determinative of masculinity beliefs than age.
