Increasing evidence links oral health to total physical and mental health and, underscores the need to integrate dental health programs with medical care. The newly emerging transition to Patient Centered Medical Homes (PCMH) and Accountable Care Organizations (ACOs) aligns payment and provides incentives for health systems to pursue this strategy. PCMH and ACOs are efforts to improve patient outcomes by creating partnerships between patients, physicians, hospitals, and other caregiving institutions to provide evidenced-based medical care, while slowing the growth of health-related costs. Although primary medical care is customary in ACOs and in PCMHs, dental care has been largely omitted from these new delivery systems. Many health systems are becoming ACOs and being certified as PCMHs. The adoption of electronic medical and dental records in many health systems creates an opportunity to bring together data from two traditionally separate systems. Information Technology (IT) integration of dental and medical records is more common as health care systems are purchasing integrated records. Therefore, the clinical integration for medical and dental interprofessional care coordination should yield financial and health outcome improvements. The goal of this article is to describe why and how dental professionals should and can participate in PCMH transitions to be a part of a Patient Centered Medical-Dental Home (PCM-DH) and ACO. This position paper provides valid reasons for developing a PCM-DH and ACO that includes dental care. This perspective is relevant to United States healthcare system delivery models that are part of the U.S. healthcare reform legislation of 2010.

Within the last decade in the United States, kinship care (placement with relatives or those non-related friends of family known as fictive kin) has evolved from an infrequently utilised option for temporary care and/or permanence, to a widely used and often preferred solution for children in need of care. Emily Jean McFadden discusses the background to this development and how it is related to the rising placement of children of colour, particularly African American children and adolescents who are over-represented in the American foster care system. Wide professional recognition of the importance of culture in identity formation and advocacy by professional groups has led to the acknowledgment of kinship care as a preferred placement option; it is now used extensively in many states, both in informal care which takes place outside of court intervention and in the formal foster care system.

As the United States devotes extensive resources to health care, evaluating how successfully the U.S. system delivers high-quality, high-value care in an equitable manner is essential. At the request of Congress, the Agency for Healthcare Research and Quality (AHRQ) annually produces the National Healthcare Quality Report (NHQR) and the National Healthcare Disparities Report (NHDR). The reports have revealed areas in which health care performance has improved over time, but they also have identified major shortcomings. After five years of producing the NHQR and NHDR, AHRQ asked the IOM for guidance on how to improve the next generation of reports. The IOM concludes that the NHQR and NHDR can be improved in ways that would make them more influential in promoting change in the health care system. In addition to being sources of data on past trends, the national healthcare reports can provide more detailed insights into current performance, establish the value of closing gaps in quality and equity, and project the time required to bridge those gaps at the current pace of improvement

Marijuana use and its distribution raise several complex health, social and legal issues in the United States. Marijuana is prohibited in only 23 states and pro-marijuana laws are likely to be introduced in these states in the future. Increased access to and legalization of medical marijuana may have an impact on recreational marijuana use and perception through increased availability and decreased restrictiveness around the drug. The authors undertook an analysis to characterize the policy features of medical marijuana legislation, including an emphasis on the types of medical conditions that are included in medical marijuana laws. A high degree of variability in terms of allowable medical conditions, limits on cultivation and possession, and restrictiveness of policies was discovered. Further research is needed to determine if this variability impacts recreational use in those states.
