Order fulfillment errors and military aircraft readiness

Purpose
This paper aims to measure the effect of supply discrepancy reports (SDRs) on military aircraft readiness metrics, including aircraft availability, not mission capable supply (NMCS) hours, cannibalizations and mission-impaired capability awaiting parts (MICAP) hours.
Design/methodology/approach
Monthly SDR, NMCS, aircraft cannibalizations and MICAP data from 2009 to 2018 are analyzed using linear regression and independent samples t-tests to examine whether discrepant shipments negatively impact aircraft readiness.
Findings
Results of linear regression were significant in 4 of 12 analyses, suggesting that SDRs are a significant predictor of increased cannibalizations. Results of independent samples t-tests found MICAP hours were significantly higher on discrepant shipments compared to nondiscrepant shipments in all three analyses.
Practical implications
This research will increase awareness of the extent to which SDRs degrade aircraft readiness, and provide an opportunity for United States Department of Defense (DoD) supply chain leaders to take action to improve order fulfillment performance in their organizations.
Originality/value
Little research has been done investigating the impact of SDRs within the DoD, and to the best of the authors' knowledge, no previous study has examined the effect of SDRs on military aircraft readiness metrics.

The Impact of Managed Care on Access to Highly Active Antiretroviral Therapy and on Outcomes among Medicaid Beneficiaries with AIDS

Medicaid has promoted managed care plans (MCPs) to control health care costs for HIV-infected enrollees. This article examines the impact of MCP enrollment on use of highly active antiretroviral therapy (HAART), antiretroviral therapy (ARV), hospitalization, and death among Medicaid beneficiaries with AIDS. A retrospective longitudinal cohort study of 7,028 Medicaid beneficiaries with AIDS in California was conducted from 1999 to 2001. The impact of MCP enrollment was estimated using single-equation and bivariate probit models. Baseline HAART use was found to be significantly lower for beneficiaries in MCPs versus fee-for-service (FFS). The effect was attenuated and not significant after correcting for selection bias. MCP enrollment was not associated with overall ARV use, mortality, or hospitalization. MCP enrollment does not substantially affect medication use or clinical outcomes. Before making changes to care delivery, policy makers should address the potential costs and benefits of MCP over FFS Medicaid for chronically ill beneficiaries.
Contractual procurement alternatives of air force contingency pharmaceuticals: a cost-benefit analysis

Purpose
This paper aims to explore effective supply chain principles, through the theory of transaction cost economics, as measures to improve current contingency pharmaceutical item shortfalls in the Air Force Medical Service (AFMS) Contingency Pharmaceutical Programme.
Design/methodology/approach
In this research, AFMS contingency pharmaceutical data was collected from various databases, including the Joint Medical Asset Repository, Medical Contingency Requirements Workflow and the Medical Requirements List. Through the methodology of cost-benefit analysis, alternative sourcing and fulfilment practices are evaluated.
Findings
The findings of this research indicate that the application of centralized purchasing principles, in an effort to leverage prime vendor contract fill rates for shortage items, can lead to 12%–17% increases in pharmaceutical material availability across the programme.
Originality/value
This research clearly shows that consolidating demand for shortage items across Active Duty War Reserve Material assemblages, though applications of centralized purchasing principles that leverage prime vendor contract fill rates, can lead to substantial increases in material availability at costs that justify the calculated benefits.
