There have been substantial achievements in legislative and human rights for lesbian, gay, bisexual and transgender (LGBT) older people and their visibility in health and social care has equally increased. These appear to have surpassed the ability of care services to meet their needs given documented concerns about the accessibility, inclusiveness and safety of care services particularly institutionalised care. This requires systemic change not easy to operationalise. This paper describes an action research initiative where six care homes belonging to a national care provider, collaborated to assess and develop their services with the support of local LGBT "Community Advisors" and academic partners. Framed within Rogers' (2003) change management framework and combined with a participatory leadership approach, a programme of intervention was implemented comprising structured activities around seven key areas thought to promote LGBT inclusion. A formal evaluation was conducted involving 35 pre- and post-intervention qualitative interviews with 18 people (community advisors; care home managers and senior managers). The findings are presented across three key themes (1) starting points on the journey; (2) challenges encountered along the journey (organisational and interpersonal); and (3) making change happen; opportunities, initiatives and gains. We make recommendations on the value of a programme approach for achieving tangible outcomes that demonstrate increased inclusion for older LGBT people living in long-term care settings.

This paper centres on a neglected area of social work with older people—the social inclusion of older lesbian, gay and bisexual (LGB) adults in long-term care environments. The translation of equality law into the delivery of adult care services is a challenging endeavour for organisations, even more so in the morally-contested terrain of sexual wellbeing. In this paper we report findings from a mixed method study into the provision of long-term care for older adults who identify as LGB. Herein we present findings from a survey of care workers and managers (n=121) and from focus groups with equality and LGB stakeholder representatives (n=20) in Wales. Focussing on the current knowledge and understanding of staff, we suggest that affirmative beliefs and practices with sexual minorities are evident amongst care workers and managers, however the inclusion of LGB residents needs to be advanced systemically at structural, cultural and individual levels of provision. There is a need for enhancing awareness of the legacy of enduring discrimination for older LGB people, for cultural acceptance in care environments of older people's sexual desires and relationships, and for a more explicit implementation of equality legislation. Social workers in adult care can advance this agenda.

The adult social care sector in England has been encouraged to increase the role of volunteers in service delivery. To understand the volunteer role in care delivery and its impact upon paid care work, we undertook 94 qualitative interviews in seven care settings for older people in England. While the boundaries between care worker and volunteer were clearly established in some organisations, they were more indistinguishable in others. We discuss how both clear and murky boundary making, especially regarding 'emotional' and 'bodily' aspects of care, can contribute towards paid care work's invisibility, lack of recognition and poor remuneration.

Faced with unprecedented challenges, the adult social care sector in England has seen increasing attention given to the potential of volunteers to contribute to service provision. This article reports the findings of a qualitative study that explored the contribution made by volunteers to social care services for older people. The article draws attention to the difficulties associated with recruiting and training volunteers to work in the sector, particularly during a period of reduced public expenditure, which is putting the sector under strain. Given the challenges faced, the article considers whether it is appropriate to involve volunteers in care work.

Reflections are based on qualitative evaluation data gathered pre- and post-project, which includes 37 interviews with care home staff, managers and community advisors (two of which are co-authors). RESULTS AND CONCLUSION: We discuss how the co-production turn emerged during research and evaluate how the politics of this approach helped advance inclusion – itself crucial to well-being. We argue for the value of co-produced research in instigating organisational change in older people's care environments and of non-didactic storytelling in LGBT awareness-raising amongst staff.

The term 'social care' emerged in both official and academic publications in England in the 1990s but has not been defined in legislation. How the phrase has varied in usage over the last 20 plus years is outlined in this article. Whilst the element of 'personal care' has been present in the range of descriptions of 'social care', the policy context has changed dramatically, affecting the broader debate about priorities in public support for vulnerable adults. What 'social care' means as a policy rather than a practice has changed over time. The notion of indeterminacy provides a plausible explanation of changes in terminology and meaning in policies and practices.

Abstract
The current policy emphasis in adult social care in England is on promoting independence, preventing or delaying the need for more intensive support and the provision of personalised services. However, there is little evidence available on how social workers (SWs) identify and meet the complex needs of older service users in practice. In this article, we present findings from a study of innovative social work practice with older adults in England (2018–2019). We present five case studies of social care and integrated services in which SWs are integral team members. Twenty-one individuals participated in interviews; this included service managers and practitioners with social work backgrounds, and other professionals, including nurses and occupational therapists. Specific practices contributing to innovative service delivery included: the strong demonstration of social work values influencing the practice of multidisciplinary teams; positive risk management; importance of timing and ensuring continuity of relationships; and, the proactive application of legal knowledge to promote older people's rights. While some of these features can be seen as returning to the 'heart' of social work, we argue that they are promising in forging new paths for social work with older people that turn away from more managerialist- and procedurally driven approaches.
