The aim of this thesis is to describe Political Science students' experiences of studies and work life. Students' reflections on their study situation as well as their envisaged and later experienced work life have been analysed through cross-sectional as well as longitudinal data. Central research questions are how students at the beginning and the end of their studies perceive their study program, how the communication patterns between teacher and students are negotiated, and what discourses of knowledge and competence operate in the program and in work life as well as how students of Political Science experience the transition from higher education to working life at two European universities, and how Political science students and students in two professional programmes experience the transition from higher education to work life. The empiri-cal studies are presented in four articles. Ethnography, phenomenography and discourse analysis have been applied. The results show that students embark on the program with dreams of being in the halls of power in Brussels or becoming famous by appearing in the media. They are, however, also driven by a Bildung incentive. Towards the end of their studies, more down-to-earth visions of the future as investigators have replaced their previous dreams. Another result is the ongoing negotiations about their role and the purpose of e.g. seminars, which convey double messages about what to say and when to contribute. They also report a rational and generic relationship between their studies and the work tasks, indicating that they have acquired a set of general skills that are helpful for them in their work. When moving from studies into work life, they report having a feeling of being squeezed between the politicians and the general public. Surprisingly, few of them regard themselves as having power in the decision-making arena. ; Syftet med avhandlingen är att beskriva studenter inom statsvetenskap och deras resa genom akademin ut i arbetslivet. Genom att undersöka studenternas reflektioner och tankar kring sin studiesituation och hur de ser på sitt kommande arbete genom att använda en undersökningsdesign som består av en tvärsnittsstudie och en longitudinell studie. Med utgångspunkt i syftet har ett antal forskningsfrågor formulerats; Hur uppfattas utbildningen av studenter i början och slutet av sina studier? Vilka diskurser kring kunskap och kompetens återfinns i studieprogrammen? Vilka diskurser kring kunskap och kompetens återfinns i arbetslivet? Hur upplever studenter i statsvetenskap vid två europeiska universitet övergången mellan akademi och arbetsliv? Hur upplever studenter i statsvetenskap och studenter från två professionsutbildningar övergången mellan akademi och arbetsliv? De empiriska studierna rapporteras i fyra uppsatser. Etnografi, fenomenografi och diskursanalys är de metoder som använts vid ana-lyserna av empiriska data. Många studenter väljer att läsa statsvetenskap på grund av ett allmänbildningsintresse, de uttrycker också drömmen om att få arbeta i maktens korridorer eller bli kända som experter i media. I slutet av sin utbildning infinner sig snarare en bild av en framtida roll som utredare. Eftersom studier i statsvetenskap har en rationell karaktär med fokus på generiska färdigheter är detta inte särskilt förvånande. En grupp studenter som följdes under sina första fem veckor i en seminarieserie fick dubbla signaler rörande sin roll vid seminarier. Relationen mellan utbildning och arbete beskrivs som i första hand rationell och generisk, dvs de har förvärvat allmänna färdigheter som är dem till stor hjälp i arbetet. När de som nya på arbetsmarknaden talar om sin identitet, framgår att de beskriver sig som klämda från två håll i sin roll som förmedlare mellan allmänheten och makthavarna. Förvånansvärt få anser att de har någon reell makt när det gäller beslutsfattande.

Legislation is important to recordkeeping strategies and requirements, since it determines which records are needed for legally legitimate actions. Yet the concepts of a document, a record, a case, a transaction, etc., are frequently defined differently in various laws even within national boundaries and are urgently in need of harmonization to minimize complications and barriers to e-business transactions. Every country has its own legislation system which has been adjusted over time in correlation to national context and its surrounding society (Duranti, 1989-1990, p. 5). Duranti and Goh (2012) provided sound arguments as to why it is important to ensure national archival legislation keeps pace with our digitally networked society with its increasingly blurred borders. In addition to their own national law, members of the European Union (EU) are obliged to follow EU-legislation requirements. There are three types of EU legislation: regulations, directives and decisions. A regulation is applicable in all EU countries. Directives are general rules. A decision is a stipulation of issues of concern for specifically mentioned persons or organizations (European Commission, 2013). Recommendations and statements are not binding, but may, for example, be used by the EU as support in interpretation of EU-legislation (Sveriges Riksdag, 2013). Hence, within public organizations, there is a close relation between records, recordkeeping processes, legislation and accountability. Yet research in Sweden has shown that 24/7 e-service web-interfaces serving e-government strategies and ambitions are sometimes developed without consulting recordkeeping professionals, in ignorance of legal requirements for capture of records and that preservation issues are rarely considered when acquiring new digital systems (Kallberg, Svärd and Sundberg, 2010). Several other European studies also identify a lack of recordkeeping awareness within public organizations (Barata, 2004; Valtonen, 2007; Shepherd, Stevenson, & Flinn, 2009; Riksarkivet, 2010). However, there is currently a strong European political ambition to implement eGovernment services, which aims to promote cross-border business development. On the other hand, this challenges archival thinking to meet business, technological and legal requirements stipulated for each country. Varying legal definitions used can cause major problems for transactions both within a country and across national borders, even in something as seemingly simple as exchanging e-invoices. This paper will discuss an approach to identifying a way forward for harmonizing legislation that contains recordkeeping requirements across the EU. Identifying the range of / types of laws within countries and EU that define a record Using the ICA multilingual terminology database, to check and add to definitions already found there Checking against definitions found in international standards. Research towards harmonizing these crucial definitions will be very challenging, given linguistic and cultural differences but the ICA multilingual terminology database is a potential vehicle for checking existing usage, adding to the definitions and quotations from legalislation and the literature already found there and adding new terms. The foundation version of the ICA-funded Terminology Database, developed as an ICA Section for Archival Educators' project led and hosted by the University of British Columbia, contains definitions with examples of usage for an initial 300 archival terms across 16 languages. Due to expected impacts such as flooding, landslides, and biodiversity loss, climate change adaptation has become recognized as an inevitable part of climate change policy and practice. However, our understanding of how to organize the management of adaptation is lacking, and few concrete measures have yet been implemented. Knowledge gaps exist relating to constraints on and opportunities and facilitating factors for adaptation. This study aims to fill such gaps by analyzing how Swedish climate change vulnerability and adaptation management is integrated across issues, sectors, and scales in public policy. The analysis is supported by two interconnected sub-studies. The first maps the national and local institutionalization of adaptation through document analyses at different policy levels. The second analyses practical approaches to and perceptions of vulnerability and adaptation management in two case municipalities. In the latter sub-study, qualitative interviews and stakeholder dialogues were held with officials from various local sector departments. The results indicate that climate change adaptation is poorly integrated into Swedish public policy. Constrains on local horizontal integration include a lack of cross-sectoral coordination and knowledge, weak local political interest, and varying opportunities for sector departments to influence policy. These constraints result in climate vulnerability being considered late in municipal and regional strategic planning processes. They also reduce the possibility of identifying overarching municipal goals. At the national level, horizontal integration is negatively affected by a lack of government guidelines and by unclear division of responsibility. Constraints on vertical integration include poor fit between the national and municipal levels, due to a perceived absence of national goals, guidelines, and funding, and the lack of a sufficient knowledge base for decision-making. This makes it difficult to now what measures to prioritize and how to evaluate progress. The analysis of adaptation policy integration also gives insights into some general factors found to either constrain or facilitate implementation of adaptation. In Sweden, both horizontal and vertical integration has been facilitated by the few national and regional guidelines established to date, indicating that national steering would offer a useful way forward. Policy integration could be increased by formulating national adaptation goals, creating a national adaptation fund, creating municipal adaptation coordinator posts, and paying greater attention to climate change vulnerability in proactive economic planning. ; Förväntade konsekvenser av klimatförändringar såsom översvämningar, jordskred och biodiversitetsförluster har utvecklats till viktiga klimatpolitiska frågor. Förståelsen av hur hantering av klimateffekter kan organiseras är dock begränsad. Kunskapsluckor existerar även kring hinder och möjligheter för implementering av klimatanpassning. Därför har få anpassningsåtgärder hittills realiserats. Denna studie syftar till att bidra med kunskap för att fylla ovanstående luckor, genom att analysera hur hantering av klimatsårbarhet och anpassning är integrerade i andra frågor, sektorer och nivåer i svensk offentlig policy. Analysen stöds av två sammanlänkande delstudier. Den första kartlägger genom dokumentanalyser, hur anpassning institutionaliserats på nationell och lokal nivå. Den andra analyserar praktiska tillvägagångssätt till, och åsikter om, hantering av anpassning i två svenska kommuner. I den senare delstudien har kvalitativa intervjuer och intressentdialoger genomförts med kommuntjänstemän från olika sektorsförvaltningar. Studien visar att anpassningsfrågan är undermåligt integrerad i svensk offentlig policy. Hinder för horisontell integrering på det lokala planet inkluderar en brist på tvärsektoriell koordinering och kunskapsuppbyggnad, ett svalt lokalpolitiskt intresse och olika möjligheter för sektorsförvaltningar att påverka beslutsfattande. Detta får till följd att klimatsårbarhet övervägs sent i kommunal och regional strategisk planering. Det försvårar också möjligheten att finna övergripande kommunala mål. På den nationella nivån påverkas den horisontella integreringen negativt av en avsaknad av statliga riktlinjer och fördelning av ansvar. Hinder för vertikal integrering inkluderar en brist på matchning mellan nationell och lokal policynivå på grund av en upplevd avsaknad av nationella mål, riktlinjer, finansiering och kunskapsunderlag. Detta gör det svårt att prioritera bland åtgärder, och att utvärdera framsteg. Analysen av policyintegrering ger även insyn i mer generella faktorer som antingen hindrar eller främjar implementering av klimatanpassning. De få statliga och regionala riktlinjer som hittills utvecklats i Sverige, har främjat horisontell och vertikal integrering av anpassning. Detta indikerar att nationell styrning är viktigt för att underlätta implementering av klimatanpassning. För att stärka policyintegreringen föreslås därför att nationella anpassningsmål utvecklas, att en nationell anpassningsfond instiftas, att en tjänst inom kommuner för att koordinera klimatanpassning etableras och att en större vikt ges till klimatsårbarhet och anpassning i proaktiv ekonomisk planering.

Society is facing significant challenges in transforming to a sustainable food system where healthy food is provided, while reducing the negative environmental impact. Yet, it is debatable whose responsibility it is to provide healthy, environmentally friendly food from sustainable food systems. As food retailers have huge power through their supply and placement, it has been argued that they could steer consumers towards sustainable choices. Shifting from nudging- and sustainability marketing initiatives towards retail led in-store choice restrictions have therefore been suggested to enable a sustainable food system but has not been executed to great extent due to the fear of losing consumers. The aim of the study was to explore consumer understandings of and attitudes towards retail led in-store choice restrictions aiming to reduce the negative environmental and health impacts from food consumption. This was assumed to provide insights regarding where the perceived burden of responsibility lies and in what ways food retailers could be a leverage point for shaping sustainable consumption. The study used a qualitative approach where four semi-structured individual interviews with Axfood, Coop, ICA and WWF as well as four semi-structured focus group interviews with consumers were conducted and continuously integrated with a literature review. A content analysis of the collected empirical data was conducted with the help of the theoretical framework following Kahneman's fast- and slow thinking systems, perceptions, the Functional Theory of Attitudes, nudging, choice editing and different types of paternalism. The results indicated that consumers have diverse attitudes towards paternalistic measures. Food retailers' choice editing strategies aiming to reduce the negative environmental impacts from food consumption were highly encouraged and accepted due to a perceived collectivistic responsibility for maintaining our common earth. On the other hand, food retailers' choice restrictions aiming to reduce the negative health impacts met great disapprovals, due to health limitations being perceived as an insult towards consumers' individual body, identity and liberty. However, eliminations contributing to a greater overall health- and well-being was encouraged only if executed by legitimate and trusted authorities with reasonable, non-profitable driving forces where a democratic society was at the foundation. Choice editing is not an easy strategy for food retailers to apply as it interferes with consumers freedom of choice but is necessary as nudging and sustainability marketing are too soft strategies to steer consumer's often irrational decision-making and will not be enough for a global, sustainable transition. However, food retailers alone cannot bear the responsibility- or be the only leverage point as governmental measures are needed. ; Samhället står inför betydande utmaningar när det gäller att omvandla till hållbara livsmedelssystem där hälsosam mat tillhandahålls, samtidigt som negativ miljöpåverkan minskar. Der är oklart vems ansvar det är att tillhandahålla hälsosam, miljövänlig mat från hållbara livsmedelssystem. Eftersom dagligvaruhandeln har enorm kraft genom sitt utbud och placering, har det hävdats att de skulle kunna styra konsumenterna mot hållbara val. Att skifta fokus från nudging- och hållbarhetsmarknadsföring till att dagligvaruhandeln införde val-begränsningar i butik har därför presenterats för att möjliggöra ett hållbart livsmedelssystem, men har inte genomförts i större utsträckning på grund av rädslan att förlora konsumenter. Syftet med denna studie var att förklara konsumenternas uppfattning om detaljhandelsledda konsumtionsval i butik i syfte att minska de negativa miljö- och hälsoeffekterna av livsmedelskonsumtion. Detta antogs ge insikter om var den upplevda ansvarsbördan låg och på vilka sätt dagligvaruhandeln kan vara en hävstång för att utforma hållbar konsumtion. Studien följde ett kvalitativt tillvägagångssätt där fyra semistrukturerade, individuella intervjuer med representanter från Axfood, Coop, ICA och WWF samt fyra semistrukturerade fokusgruppsintervjuer med konsumenter. De empiriska studierna genomfördes och integrerades kontinuerligt med en litteraturöversikt. En innehållsanalys av den insamlade empiriska datan genomfördes med hjälp av det teoretiska ramverket; Kahnemans snabba och långsamma tänkande, uppfattningar, funktionell teori om attityder, nudging, valredigering och olika typer av paternalism. Resultaten visade att konsumenterna har olika attityder till paternalistiska åtgärder. Dagligvaruhandelns valredigeringsstrategier som syftar till att minska negativ miljöpåverkan från livsmedelskonsumtion uppmuntrades och accepteras på grund av ett kollektivistiskt ansvar för att upprätthålla vår gemensamma jord. Å andra sidan mötte dagligvaruhandelns valredigeringsstrategier som syftar till att minska de negativa hälsoeffekterna stort motstånd, då begränsningar som rör den individuella hälsan anses vara en kränkning mot konsumentens kropp, identitet och frihet. Elimineringar som bidrog till förbättrad hälsa och ett ökat välbefinnande uppmuntrades endast om de utfördes av en legitim och betrodd stat med rimliga, icke-lönsamma drivkrafter där ett demokratiskt samhälle fortfarande bestod. Slutsatsen var att valredigering inte är en lätt strategi för dagligvaruhandeln att tillämpa eftersom det stör konsumenternas valfrihet, men är nödvändig eftersom nudging och hållbarhetsmarknadsföring är för mjuka strategier för att styra konsumentens ofta irrationella beslutsfattande och kommer inte att räcka för en global, hållbar övergång. Dagligvaruhandeln kan dock inte ensam bära ansvaret då statliga åtgärder också behövs.

Public policies affect all of us, regardless of who we are or where we live. The study of public policymaking necessarily entails the study of the entire political system and to this end, researchers employ a multitude of frameworks, theories, and models, which tend to be complementary rather than mutually exclusive. The focus of this dissertation is on political entrepreneurship as an actor-based framework to examine and understand policy change. The dissertation's main aim is to conceptually enhance entrepreneurship and the entrepreneur in the polis by leveraging them in the Swedish political context. In this research, political entrepreneurship and the political entrepreneur are examined in the background of the Swedish corporatist policymaking system with its consensual sensibilities. The five (two theoretical and three empirical) papers as well as the cover essay comprising this dissertation attempt to answer the following questions: first, how do contextual factors inform the realization of entrepreneurial agency? Second, how do contextual factors inform the strategies entrepreneurial actors use to affect change? Third, what is the role of political entrepreneurship and the political entrepreneur in macro-level theories such as critical junctures and policy transfer? Contextual factors here are understood to be the general political system; the level of governance; the substantive policy sector, and the stage of the policy process. Predominantly qualitative methods and a variety of analytical tools, ranging from formal social network analysis (SNA) to process tracing are used to investigate the research questions in the national, regional, and local levels of governance and in the fields of crisis management, risk governance, and economic development respectively. Findings suggest that overwhelmingly, political entrepreneurs come from the ranks of public officials and thus political entrepreneurship is a feature of the policy implementation stage rather than the agenda setting stage of policymaking. There is not a place for the outsider, single issue entrepreneur in the Swedish consensual system, which provides for extensive inclusion, but of actors organized in interest groups. Political entrepreneurs are action-oriented, problem solving doers, characterized by perseverance and resourcefulness and are key in consolidating policy change in the aftermath of a crisis. Though in broad terms the strategies political entrepreneurs use in the Swedish context are concomitant with the ones used in pluralistic contexts, specificities diverge. In the Swedish corporatist consensual system, political entrepreneurship becomes a conduit facilitating interconnections among a multitude of actors; opens up additional channels of communication, while the political entrepreneur is a network maker. Finally, political entrepreneurship is focused on forging a consensus rather than winning the competition: the art of quiet cooperation and collaboration. ; Den offentliga politiken berör alla medborgare, oavsett vem man är och var man bor. Forskning om policyskapande och politiskt beslutsfattande omfattar det hela politiska systemet. Därför använder sig forskare av ett stort antal komplementära ramverk, teorier och modeller. Denna avhandling använder politiskt entreprenörskap som ett aktörsbaserat ramverkför att undersöka och förstå policyförändring. Avhandlingens huvudsyfte är att utveckla de teoretiska begreppen "politiskt entreprenörskap"och "politiska entreprenörer" genom att undersöka dem inom det svenska politiska systemet. Avhandlingen består av en kappa och fem artiklar, varav två är uteslutande teoretiska och tre omfattar analys av empiriskt material. Alla fem söker att besvara följande forskningsfrågor: Först, hur påverkar kontextuella faktorer möjliggörandet av entreprenöriell agens? För det andra, hur präglar kontextuella faktorer de strategier som entreprenöriella aktörer använder sig av för att åstadkomma förändring? För det tredje, vilken roll har politiskt entreprenörskap och politiska entreprenörer i makroteorier, särskilt critical junctures och policy transfer? Med kontextuella faktorer avses här det politiska systemets karaktär; governance-nivå; policyområde, och stadie i policyprocessen. Forskningsfrågorna undersöks huvudsakligen genom kvalitativa metoder; data analyseras med formell social nätverksanalys, innehållsanalys och process-spårning. Studierna behandlar olika politiska områden på olika politiska nivåer: krishantering på den nationella nivån, risk-governance på den regionala nivån och tillväxt på den lokala nivån. Resultaten visar att politiska entreprenörer huvudsakligen återfinns bland byråkrater, vilket innebär att politiskt entreprenörskap sker i implementeringen av policyer snarare än i det politiska agendasättandet. Svensk korporatism kännetecknas av en omfattande inkludering av aktörer som organiserar sig inom intressegrupper. Däremot finns litet utrymme för enskilda politiska entreprenörer. Politiska entreprenörer beskrivs ofta som handlingsinriktade problemlösare, uthålliga och rådiga. Efter samhällskriser är de viktiga i förändringen av policyer. Även om de strategier som svenska politiska entreprenörer tillämpar i stort sett liknar de som används inom pluralistiska system, finns det också flera skillnader. Inom den svenska korporatismen, blir politiskt entreprenörskap en kanal som underlättar kopplingar mellan flera olika aktörer–entreprenören blir en nätverkskapare. Politiskt entreprenörskap i Sverige tycks handla om att skapa konsensus snarare än att vinna i konkurrens: det är det tysta samarbetets och samverkans konst. ; Vid tidpunkten för disputationen var följande delarbeten opublicerade: delarbete 3 inskickat, delarbete 4 accepterat, delarbete 5 inskickat. Extreme weather events and effects of climate change are threats to the transport sector's functionality and safety. Risk management in this context implies a necessity to focus on the connection between near-term experiences and coping strategies on one hand, and long-term adaptation analyses on the other. How learning from past events and subsequent knowledge sharing can be adopted is a question that needs to be explored, discussed and tested. A systematic approach to lessons learned calls for measures of investigation, reporting, planning, implementation and evaluation. A qualitative case study approach was used in this thesis. In the first paper the practices of accident investigation in operation and maintenance were inventoried within the Swedish Transport Administration (STA). Three accident investigation methods were applied and tested on a cloudburst event, causing flooding in a railway tunnel in Sweden. In the second paper, semi-structured interviews, documents, and archival records were used as means for penetrating deeper into the attitudes and understanding of lessons learned concerning extreme weather events within a procured public-private partnership. The results of the two studies showed weak signals of feedback on lessons learned. Partly, these weak signals could be traced back to weak steering signals. Various obstacles impeded learning curves from lessons learned. The obstacles were of both hard and soft values, e.g. resources in time and equipment, systematic investigation methods, incentives for lessons learned, education and knowledge, values, norms and attitudes towards how and why identified problems should be solved. Successful knowledge sharing requires that close attention is paid to such obstacles and that an adaptive approach is adopted. ; Den pågående och framtida klimatförändringen sätter press på aktörer att möta risker som associeras med klimatförändring. Syftet med denna avhandling är att bidra med kunskap om lärande och kunskapsöverföring inom offentlig förvaltning av landtransportinfrastruktur. Eftersom lärande och kunskapsöverföring är grundläggande för planering och beslutsfattande om strategier och åtgärder som främjar ett robust transportsystem. Målet för detta arbete är att belysa lärande och kunskapsöverföring inom och mellan olika organisationer i det svenska samhället. Det socio-tekniska systemperspektivet - som används som analysram i båda studierna ger en djupare förståelse för bakomliggande faktorer. En kvalitativ ansats, som omfattar intervjuer, deltagande observationer och dokumentanalys, har används i detta licentiatarbete. I den första studien belyses möjligheten att använda sig av industriella utredningsmetoder på naturolyckor i en svensk kontext av en översvämning i en järnvägstunnel. De industriella utredningsmetoderna visade sig vara användbara för utredning av konsekvenser efter skyfall, med olika metodologiska fördelar och nackdelar. Den andra studien utforskar hur Trafikverkets verksamhetsområde Underhåll arbetar med lärande kopplat till väderextremer; hur de fångar upp erfarenheter och kunskap som finns hos kontrakterade entreprenörer, samt hur entreprenörerna uppfattar att lärande och erfarenhetsåterföring sker. Resultatet från båda studierna visar på både svaga styrsignaler och svaga återkopplingssignaler, vilket medför svaga lärandekurvor. Olika hinder sågs ligga bakom med svaga styr- och återkopplingssignaler bl.a. resurser i tid och processer, systematik i utredning av naturolyckor, incitament att lära av varandra, utbildning och kunskap, värderingar normer och attityder till hur och varför identifierade problem ska lösas. Ett adaptivt förhållningssätt innebär att man behöver ta hänsyn till dessa hinder på ett systematiskt sätt. ; The agreement in Paris in 2015 was an historic manifestation that society has to work with both mitigation and adaptation to achieve a reduction of the adverse effects of climate change. One way to achieve adaptation is through the integration of present coping strategies. A first step is to study the existing processes and routines that support short-term coping. This licentiate thesis targets different aspects of learning as a strategy for coping and building adaptive capacity. Road infrastructure and maintenance in relation to extreme weather are used as the physical context and the Swedish Transport Administration as a case to study. Paper I shows the possibility to apply industrial accident investigation methods to an extreme weather event and get useful insights into underlying root causes. Paper II shows the intra- and interrelated patterns that exist in public-private partnerships (PPP) in Sweden. The paper describes a parallel of systems with infrequent overlaps regarding lessons learned. In both papers the socio-technical perspective approach was used to highlight aspects of learning from and investigating damage due to extreme weather at different tiers in society. The socio-technical perspective provides an understanding of how decisions and legislation that affect our actions and behavior today may have been taken in different time and space settings. Increasingly, eGovernment (the use of ICTs in order to achieve better government) is moving its focus from web presence and electronic service provision to striving for an interoperable public sector. Interoperability refers to the ability for information exchange across organizational borders, concerning technology as well as business aspects. Policy for such change has been formulated and implementation is currently taking place in many government sectors. In such programs there is a strong need for coordination with regard to the way in which interoperability is to be implemented. Interoperability work requires coordination, as it is a complex endeavour because of the interrelatedness of information systems, public services, departments and organizations, as well as policies, constraints and regulations. In order to achieve interoperability, architectural approaches are increasingly used in the public sector to try to coordinate interoperability work. One such approach, Enterprise Architecture (EA), is becoming increasingly influential. EA has been defined as an overview of the complete business processes and business systems, both in terms of how they overlap and their interrelatedness. However, previous research show that state-of-the-art EA is seldom fully applied in practice. Previous research has also proposed that information infrastructures and architectures should be seen as evolving dynamically during the implementation process through changing relationships between actors. The implementation of IS architecture for interoperability is thus seen as an evolving process of social production. As the research field is still immature further research on the evolution of public information infrastructures and architectures is needed, as well as how the strategic alignment of handling of goals, and ambiguities in implementation is done. This thesis hence addresses the challenges of implementing national public sector interoperability as an evolving process by addressing the research question: How is interoperability interpreted and enacted by different actors in public sector implementation? In order to approach the research question, an interpretive case study is performed. The case studied in this thesis is from the implementation of the Strategy for eHealth in Sweden, where healthcare is mainly publicly funded, and catered for by 20 county councils (who mainly focus on healthcare, and 290 municipalities (who also cater for a great deal of other public services). The case is an example of how interoperability is implemented, from the early stages of outlining a general picture of goals and requirements, to the formulation of a strategy and an architecture. This case is also an example of how EA influences an interoperability program through enterprise thinking. The research uses an interpretive case study approach influenced by Actor-Network Theory (ANT). ANT is used as a toolbox for telling stories about technology in practice, as emergent in socio-material relations. A number of complementary qualitative methods are used. These include semi-structured interviews, observations and document analysis, with the foremost part of the empirical material being first-hand. In order to understand interoperability implementation in the public sector I examine the background to eGovernment implementation, by contrasting conceptualizations of eGovernment evolution to contemporary theories of public policy implementation. It is shown that, during the last decade, stage models have been used as tools for describing, predicting and directing the evolution of eGovernment. The stage model approach has been criticized for presenting a linear development which has little empirical support and delimits the understanding of eGovernment development as a dynamic process. Newer stage models have started to take this criticism into consideration and alternative models on eGovernment have also been developed. Consequently, eGovernment implementation is in this thesis perceived as a process in which technologies, policies and organizations are in a process of mutual shaping, where policymaking and policy implementation are intermingled. Implementing interoperability is hence not a matter of disseminating a policy that is to be implemented in every setting exactly as stated on paper, but a process in which the goals and means of interoperability are constantly being negotiated. Also, EA has been proposed as an approach to treat technology and business in the public sector as interrelated. However, since previous research show that state-of-the-art EA is seldom fully applied in public sector practice, the concept of enterprise thinking is developed in this thesis. Enterprise thinking is intended to be a concept that describes EA as a contemporary zeitgeist which in practice is adopted in varying ways. The results of the thesis show how interoperability in eHealth was roughly outlined before implementation although still containing conflicts and ambiguities. Central to this thesis is the controversy of defining "the enterprise", as the health care sector was delimited as one enterprise, which became increasingly problematic during implementation. This to a large extent concerned municipalities, whose business area stretched much wider than just the health care sector, and hence the definition of the enterprise became problematic. Another central aspect was legal obstacles to cooperation, as there was a clash between the values of efficiency and patient privacy as a result of a new law that had been implemented in order to allow for information sharing. The legal grounds for sharing information proved to be problematic, which lead to that several involved actors perceived that a large portion of the patients in health care could not benefit from interoperability as their information could not be shared despite this law. The legal challenges also dampened the enthusiasm for the eHealth program as a whole. The program had also outlined a technology architecture before implementation. This architecture was however treated in conflicting ways, both as a blueprint (something to be implemented) and a tool for communication (as a way of discussing what was to be implemented). For instance, several municipalities perceived that the planned infrastructure was unsuitable to their business needs (as it did not meet the requirements of other actors in eGovernment), and thus questioned it, using it as a tool for communication rather than a blueprint. Meanwhile, other actors argued that the blueprint had already been decided, and thus needed to be implemented. The case also shed light on the use of informal networks, outside traditional bureaucracy, as a means to deal with interoperability. Such networks were used in order to align actions and perceptions of a large number of autonomous municipalities. This revealed issues concerning local decision as knowledge of, and resources for, ICT and architectural work was lacking in several municipalities. Also, as the networks lacked formal power no decisions could be taken jointly, but in the end had to be negotiated locally. This made coordinated decision making hard as the processes were lengthy and often lacked clear incentives. Furthermore, ambiguous feedback from national authorities, as well as an overall lack of understanding among local actors, concerning what was legal to do in terms of procurement and information sharing, complicated the situation further. These findings are summarized in four main conclusions; The process of defining which organizations are to be made interoperable, or what is to be considered as "the enterprise", is a political process which might be brought into question and require re-negotiations throughout implementation, as the drawing of boundaries of "the enterprise" can be filled with conflict. Different perspectives on an enterprise, from different architectural viewpoints, are often described as complementary, and it has previously been shown that different architectural metaphors can be used by different actors during implementation. However, in practice, different use of metaphors for architectures can open up for discussion and conflict. These may not only be different, but may also contradict other actors' use of metaphors, since different metaphors might clash. Interoperability work can be a novel task for some local governments. Therefore, there is a need for negotiation and to establish forms of formal decision making and informal dissemination because such structures might be lacking. It should be anticipated that implementation might be slow because of a lack of understanding about interoperability programs (particularly in terms of something other than ICT). In addition, there may be few forums for coordinated decision making, or there may be obstructions in the form of prior formal and legal arrangements. Enterprise thinking is interconnected with Enterprise Architecture as a zeitgeist for interoperability work. It draws upon EA as an ideal, whilst acknowledging that public organizations are influenced by this zeitgeist, although practical conditions might not allow for adoption of an EA approach. Enterprise thinking thus refers to the notion of EA as an ideal, not as a specific way of applying EA. Enterprise thinking has a process focus on interoperability. ICT, business goals, and work practices are perceived as interconnected, and hence need to be treated from a holistic perspective. How this is approached is, however, dependent upon the context in which it is implemented. Further research efforts could approach how enterprise thinking affects interoperability work in the long run with a longitudinal approach. Also, as this thesis has shown how the use of different architectural metaphors can clash, further research could focus on the positive and negative effects of negotiations being initiated by such conflict. From a project management perspective the risks and benefits of using smaller projects as "enrolment devices" for interoperability programs, where an architecture cannot be pushed but is voluntary, should be of interest. Furthermore the use of EA as an ideal which cannot be fully followed in several public settings, although might intentionally be used as a guiding light, is interesting for further research. For instance, it would be of interest to see how the rhetoric of EA may be applied in practice in order to legitimate programs. This is of interest as to examine to which extent the use of such concepts influence actual practice, or if they are only "empty words". The conceptualization of enterprise thinking proposed in the conclusions of this thesis can be used in further research. Indeed, they could be useful for investigating different approaches, influenced by EA, in different contexts. For instance, it may be of interest to countries that might not share the same institutional characteristics of Sweden, but are influenced by enterprise thinking in different ways. This would be of interest for outlining different practical approaches to enterprise thinking. Also, the further development in Sweden specifically could be of interest, as other sectors are at the time of writing preparing their own interoperability programs, and aim to benefit from the lessons learned in the healthcare sector. ; I ökande utsträckning har e-förvaltningens (användningen av IKT för att förbättra verksamheten i offentlig sektor) fokus förflyttats från webb-närvaro och elektroniska tjänster mot att sträva efter en interoperabel offentlig sektor. Interoperabilitet avser möjligheten för informationsutbyte över verksamheters olika gränser, vilket innefattar teknologiska såväl som organisatoriska aspekter. Policys för sådana förändring har utarbetats och implementeras för närvarande i flera offentliga organisationer. I sådana program finns det ett starkt behov av samordning gällande det sätt på vilket interoperabilitet skall genomföras. Interoperabilitetsarbete kräver således samordning, eftersom det är en komplex uppgift på grund sammanvävda informationssystem, offentliga tjänster, organisationer, policys, begränsningar och regler. För att uppnå interoperabilitet används arkitekturella tillvägagångssätt alltmer inom den offentliga sektorn, för att försöka samordna arbetet. Ett sådant tillvägagångssätt, Enterprise Architecture (EA), har fått ett ökande inflytande. EA har definierats som en översikt av hela affärsprocesser och affärssystem, både vad gäller hur de överlappar med varandra och hur de hänger samman. Dock visar tidigare forskning att EA i praktiken sällan tillämpas fullt ut. Tidigare forskning har också påvisat att informationsinfrastrukturer och arkitekturer bör ses som dynamiskt framväxande under implementeringsprocessen, genom förändrade relationer mellan aktörer. Implementering av informationssystemsarkitektur för interoperabilitet bör därmed ses som en framväxande och socialt producerad process. Då forskningsområdet fortfarande är omoget behövs ytterligare forskning om framväxten av offentliga informationsinfrastrukturer och arkitekturer, samt hur strategisk sammanjämkning av mål och oklarheter i implementeringen sker. Denna avhandling behandlar därför utmaningarna som finns i implementeringen av interoperabilitet i offentlig sektor, som en framväxande process, genom att behandla frågeställningen: Hur tolkas och sätts interoperabilitet i praktiken av olika aktörer under implementering i offentlig sektor? För att närma sig frågeställningen utförs en tolkande fallstudie. Fallet som studerats i denna avhandling är från implementeringen av strategin IT-strategin för vård och omsorg (eHälsostrategin) i Sverige, där vården i huvudsak är offentligt finansierad, och tillhandahålls av 20 landsting (som huvudsakligen fokuserar på sjukvård), och 290 kommuner (som också tillhandahåller en mängd andra offentliga tjänster). Fallet är ett exempel på hur interoperabilitet implementeras, från de tidiga skeden då en generell bild av mål och krav målas upp, till utformningen av en strategi och en arkitektur. Detta fall är också ett exempel på hur EA påverkar ett interoperabilitetsprogram via "enterprise thinking". En fallstudie genomförs med en tolkande ansats, influerad av Actor-Network Theory (ANT). ANT används som en verktygslåda för att berätta historier om teknik i praktiken, som framväxande genom sociomateriella relationer. Merparten av det empiriska materialet har samlats in i förstahand och ett antal kompletterande kvalitativa metoder används. Dessa metoder inkluderar semi-strukturerade intervjuer, observationer och dokumentanalys. För att förstå interoperabilitetsimplementering i offentlig sektor undersöker jag bakgrunden till implementation av e-förvaltning, genom att kontrastera begreppsbildningar av hur e-förvaltningen växer fram mot samtida teorier om implementation i offentlig sektor. Jag påvisar att under det senaste decenniet har stegmodeller använts som verktyg för att beskriva, förutsäga och styra utvecklingen av e-förvaltning. Denna typ av modeller har kritiserats då de framhåller en linjär utveckling som har bristande empiriskt stöd och begränsar förståelsen för e-förvaltningens framväxt som en dynamisk process. Nyare stegmodeller har börjat ta hänsyn till denna kritik och alternativa modeller på e-förvaltning har också utvecklats. Följaktligen ses i denna avhandling implementeringen av e-förvaltning som en process där teknik, policy och organisationer är i en ständig process av ömsesidig påverkan, där policyskapande och policyimplementering är sammanvävt. Att implementera interoperabilitet är därför inte en fråga om att sprida en policy som skall genomföras av varje aktör så som det står angivet på pappret, utan en process där mål och metoder för interoperabilitet ständigt omförhandlas. EA har föreslagits som ett tillvägagångssätt för att behandla teknik och verksamhet som integrerade. Dock, eftersom tidigare forskning visar att EA sällan tillämpas fullt ut i praktiken i offentlig sektor, så utvecklas begreppet enterprise thinking i denna avhandling. Enterprise thinking är avsett att vara ett koncept som beskriver EA som en samtida tidsandan som i praktiken närmas på olika sätt. Resultaten i avhandlingen visar hur interoperabilitet i e-hälsa skisserades ut grovt innan implementeringen, i en bild som innehöll konflikter och tvetydigheter. Centralt för denna avhandling är problematiken i att definiera verksamheten ("the enterprise"), då hälso- och sjukvårdssektorn var avgränsad som en verksamhet, vilket blev allt mer problematiskt under implementeringen. Detta berörde till stor del kommuner, vars verksamhetsområde sträcker sig mycket bredare än bara hälso- och sjukvården. Därmed blev definitionen av verksamheten problematisk. En annan central aspekt var juridiska hinder för samverkan, då det fanns en konflikt mellan värdena effektivitet och patientens integritet. Detta var delvis en följd av en ny lag som hade införts just i syfte att möjliggöra informationsutbyte. De rättsliga grunderna för att dela information visade sig vara problematiska, vilket ledde till att flera inblandade aktörer uppfattade att en stor del av patienterna i vården inte kunde dra nytta av interoperabilitet, eftersom deras information inte kunde delas trots denna lagändring. De rättsliga utmaningarna dämpade också entusiasmen för eHälsoprogrammet som helhet. Programmet hade också skisserat ut en teknikarkitektur innan implementeringen. Denna arkitektur behandlades dock på motstridiga sätt, både som en "ritning" (en klar bild av vad som skulle genomföras) och som ett verktyg för kommunikation (som ett sätt att diskutera vad som skulle genomförts). Till exempel uppfattade flera kommuner att den planerade infrastrukturen var olämpliga för deras verksamhetsbehov (då den inte uppfyllde kraven från andra aktörer inom e-förvaltning), och ifrågasatte därmed den genom att använda arkitekturen som ett verktyg för kommunikation snarare än en ritning. Samtidigt menade andra aktörer att man redan hade tagit beslut om denna ritning och att den därmed skulle följas. Fallstudien kastar också ljus på användningen av informella nätverk, utanför den traditionella byråkratin, som ett sätt för att arbeta med interoperabilitetsfrågor. Sådana nätverk användes i syfte att sammanjämka åtgärder och uppfattningar hos ett stort antal självstyrande aktörer. Detta visade på en problematik gällande lokalt beslutsfattande och resurser gällande IKT och arkitekturellt arbete, då kompetenser och erfarenheter för detta saknades hos flera kommuner. Relaterat till detta är att i de nätverk där diskussionerna fördes saknades även formell makt, och inga beslut kunde fattas gemensamt därigenom. I slutändan var man istället tvungna att diskutera de frågor som togs upp där lokalt. Detta gjorde samordnat beslutsfattande svårt eftersom processerna var långa och ofta saknade tydliga incitament. Dessutom fanns en problematik i att nationella aktörer ofta gav tvetydig respons på frågor, samt en allmän brist på förståelse bland lokala aktörer, gällande vad som var lagligt att göra beträffande upphandling och informationsutbyte. Detta komplicerade situationen ytterligare. Dessa resultat sammanfattas i fyra huvudsakliga slutsatser; 1. Processen med att definiera vilka organisationer som skall göras interoperabla, eller vad som ska betraktas som "verksamheten", är en politisk process som kan ifrågasättas och kräva omförhandlingar under implementeringen, eftersom hur man definierar gränserna kring "verksamheten" kan vara konfliktfyllt. 2. Olika perspektiv på en verksamhet, från olika arkitekturella perspektiv, beskrivs ofta som komplementära, och det har tidigare visats att olika arkitekturella metaforer kan användas av olika aktörer under implementeringen. I praktiken kan dock olika användning av arkitekturella metaforer öppna upp för diskussion och konflikt. Dessa är inte nödvändigtvis bara annorlunda och komplementära, utan kan också motsäga andra aktörers användning av metaforer, då olika metaforer kan kollidera. 3. Interoperabilitetsarbete kan vara en ny uppgift för lokala aktörer. Det finns därför ett behov av förhandlingar och att upprätta former för formellt beslutsfattande och informell "spridning" av information eftersom strukturer för detta kan saknas. Detta kan vara en långsam process på grund av bristande förståelse för interoperabilitetsprogram (särskilt i fråga om att de skulle handla om något annat än bara IKT). Dessutom finns ibland få (om ens några) forum för samordnat beslutsfattande, och det kan även finnas formella och legala hinder för detta. 4. Enterprise thinking ("verksamhetstänkande") är sammankopplat med Enterprise Architecture, som är en tidsanda för interoperabilitetsarbete. Det bygger på EA som ett ideal, då offentliga organisationerna påverkas av denna tidsanda, men att praktiska förutsättningar kanske inte möjliggör att man antar en EA-strategi. Enterprise thinking hänvisar således till EA-begreppet som ett ideal, och är alltså inte ett specifikt sätt att tillämpa EA. Enterprise thinking har ett processfokus på interoperabilitet. IKT, verksamhetens mål och arbetsrutiner ses som sammanlänkade och måste därför behandlas utifrån ett helhetsperspektiv. Hur man närmar sig detta i praktiken är dock beroende på i vilken kontext det implementeras. Vidare forskning skulle kunna behandla hur enterprise thinking påverkar interoperabilitetsarbete på lång sikt, med en longitudinell ansats. Då denna avhandling visar på hur användningen av olika arkitekturella metaforer kan kollidera föreslås ytterligare forskning som fokusera på de positiva och negativa effekterna av att förhandlingar initieras av sådana konflikter. Från ett projektledningsperspektiv kan riskerna och fördelarna med att använda mindre projekt som symboler för att få med flera aktörer i interoperabilitetsprogram fokuseras. Detta är i synnerhet intressant i arkitekturprogram där en arkitektur inte kan tvingas på aktörerna, men att implementation är frivillig. Användning av EA som ett ideal som ofta inte kan följas fullt ut i offentlig sektor, men avsiktligt användas som en ledstjärna, är intressant för vidare forskning. Till exempel skulle det vara intressant att se hur EA-retorik kan tillämpas i praktiken för att legitimera program. Det är av intresse att undersöka i vilken utsträckning användningen av dessa begrepp påverkar verksamheten, eller om de bara är tomma ord. Begreppet enterprise thinking föreslås som användbart för vidare forskning. Det kan vara användbart för att undersöka olika tillvägagångssätt, influerade av EA, i olika kontexter. Exempelvis kan det vara av intresse att se till länder som inte har samma institutionella egenskaper som Sverige, men påverkas av enterprise thinking på olika sätt. Detta skulle vara av intresse för att undersöka på vilka olika sätt enterprise thinking närmas i praktiken. Även den fortsatta utvecklingen i Sverige kan vara av särskilt intresse, då andra sektorer i skrivande stund förbereder egna interoperabilitetsprogram och ämnar dra nytta av lärdomar från arbetet inom vårdsektorn.

eParticipation is a new research domain focusing the development of ICT-supported participation in processes of government and governance. These processes may concern involvement of practitioners, citizens and politicians in electronic public administration, service delivery, policy-making and decision-making. The overall objective of this thesis is to discuss how eParticipation is enacted and shaped, in and by practice, and thus contribute to development of practice-based conceptualisation as well as development within the differing practices of eParticipation. The study is based on interpretive case studies as well as theoretical perspectives assisting the analysis of the research field as multiple and co-related processes and relations of change and learning. The empirical data has been gathered during participation in several research and development projects, conducted within a local municipality in Southeast Sweden. Several of the projects were also part of national and international collaboration. The methodological approach comprises ethnographic studies, including interviews, participatory observations and document analysis. The approach of ethnomethodology was also inspirational for the close examining of how various actors organised their participation or non-participation in the various settings of preparing for or conducting eParticipation. The theoretical basis is multi-disciplinary, drawing on perspectives from technological and social theories, such as political science, ANT and feminist theories along with IS (information systems) research. The concept of symbolic eParticipation is coined in order to explore how the preconceived ideas of managing participation seem to be constricting and limiting local and situated development. At the same time, symbolic eParticipation is inspiring development of local interpretations and participatory work. The mutual shaping of these activities leads to the formulation of the notion malleability of organisations and citizenship. The findings indicate that activities of for instance customisation of software or evaluation of consultation tools contribute in creating socio-technical mechanisms, of which they are themselves a part. Those mechanisms embed power relations, and thus become a delegated function of opening up or closing for participation. An example of such socio-technical mechanisms is the notion of "active citizenship", which is given higher legitimate status if it is conducted mainly as an electronically mediated activity. The term "symbolic active citizenship" is suggested as a concept which describes the legitimate active citizenship. The process of becoming active is thoroughly addressed in this thesis, including variations such as pro-activity and active passivity. These are also mediated by processes of learning in communities of practice. Active participants alternate between being active and actively passive in the processes which are supposed to constitute, form and sustain activities of eParticipation. This fluidity of citizenship has implications for future design of technology and for how to perceive participation in these activities. The interplay of symbolic eParticipation and organisational and civic malle¬ability described in this thesis, underscores the significance of providing space for negotiations of situating eParticipation. ; eDeltagande kan kortfattat beskrivas som deltagande med IT-stöd i processer som omfattar styrning av beslutsfattande och policyskapande (eGovernment och eGovernance)och fördjupning av e-demokrati. Det övergripande syftet med avhandlingen har varit att analysera hur eDeltagande konstrueras i och genom praxis, hur "e" respektive "deltagande" gestaltas och omformas i samspelet mellan praktiker och visioner. Studien baseras på fallstudier som analysera utifrån en tvärvetenskaplig ansats, som syftar till att lyfta fram flerfaldiga och ömsesidigt påverkande processer och relationer rörande förändring och lärande som eDeltagande består av. Empiriskt material har samlats in inom ramen för ett flertal forsknings- och utvecklingsprojekt, med både lokal, nationell och internationell anknytning. Forskningsstudierna omfattar etnografiska studier, det vill säga intervjuer, deltagande observationer och dokumentanalyser. Etnometodologi har också inspirerat denna närstudie av hur olika aktörer i lokala, situerade praktiker organiserar sitt deltagande respektive icke-deltagande i skilda kontexter. Dessa kontexter kan utgöra förberedelse frö eDeltagande lika väl som faktiskt utförande av eDeltagande. Den teoretiska basen är mångvetenskaplig, och omfattar bland annat perspektiv från socioteknisk forskning inom området informatik, demokratiteori, ANT och feministisk teori. Termen symboliskt eDeltagande föreslås som ett praktik- och teoribaserat koncept som kan användas för att diskutera hur de förutfattade föreställningarna och uppfattningarna om hur deltagande bör hanteras både begränsar och öppnar för lokal och situerad utveckling. eDeltagande inspirerar till lokalt tolkningsarbete och omförhandling av hur deltagande skall stimuleras och genomföras, lika väl som det normaliserar och skapar enhetlighet av aktiviteter. Den ömsesidiga jämkningen och anpassningen av ett formbart aktivt medborgarskap kräver också formbara organisationer och formbara politiska praktiker. Lokal anpassning av IT-applikationer eller utveckling av IT-verktyg för till exempel e-konsultationer bidrar i sin tur till att skapa sociotekniska mekanismer av vilka de själva är en del. Dessa mekanismer bäddar in maktrelationer i frusna strukturer som i sin tur tilldelas delegerad funktion att öppna och stänga för deltagande. Ett konkret exempel på en sådan socioteknisk mekanism är termen aktivt medborgarskap som får en högre legitim status om aktiviteterna utförs med hjälp av IT-stöd. Termen symboliskt aktivt medborgarskap föreslås illustrera det legitima aktiva medborgarskapet. Processen att bli aktiv adresseras utförligt i denna avhandlingsom också diskuterar variationer som proaktivitet och aktiv passivitet. Dessa processer medieras också i processer av lärande i och genom olika intressegemenskaper. Aktiva deltagare alternerar mellan att vara aktiva och aktivt passiva i dessa läroprocesser som både omformar och befäster vilka aktiviteter som skall benämnas eDeltagande. For a long time, patients were seen as weak and passive recipients of care, whose only role was to provide information and comply with doctors' orders. This is beginning to change, and patients are more seen as autonomous, active, and involved collaborators in care, co-creating value with service providers and others. In parallel, the healthcare sector is changing due to an aging population, advances in technology, medical knowhow, and the prevalence of chronic diseases, which all call for a more involved patient. During the last decade, patient involvement in healthcare has been recognized as important to provide more efficient, integrated, patient-focused healthcare. Despite this recent gain in attention, there is a gap between rhetoric's and practice, since the meaning and benefits of patient involvement are unclear both in theory and practice. This thesis takes an alternate perspective on patient involvement, departing from service theory on value creation and customer involvement. It aims to understand and explore patient involvement and how patients can be involved in both the use, and development, of healthcare services. This thesis is based on three different studies using both qualitative and quantitative research methods. The first study is a systematic literature review of healthcare research, addressing the topic of patient involvement and related concepts. Based on a total of 125 reviewed empirical articles, this study serves as an introduction and orientation to the diverse field. It aims to contribute to the knowledge base in the growing research field of patient involvement. The second study addresses and explores lead-user theory as a method to identify highly innovative patients who can be suitable for involvement in healthcare development. The third study explores how patients, depending on disease, care process and context, can take different roles in healthcare development. The results indicate that patient involvement is not an isolated activity but influences the whole healthcare system. This extends the view of patient involvement from just decision-making and isolated encounters to patients potentially being substantially involved in all aspects of healthcare. This is also important in involvement in use. The patient's individual experiences, context, and type of illness play an important role in development initiatives. Patients should be selected carefully, for involvement in healthcare development, depending on the goal of the initiative. The type of illness and the patient's context are key factors to the kind of contributions patients can make. Depending on the type of illness, and if it shows up mostly at home or at the care provider's, patients develop different contributions. This thesis contributes to understanding patient involvement by taking a service perspective on co-creation and customer involvement. This approach to patient involvement extends the traditional view by proposing that patients should be involved in all stages of healthcare. Understanding how individuals create value and manage their health is important for individuals, healthcare providers, and government. Much of a patient's value creation takes place outside the patientprovider sphere, and is therefore unknown to the healthcare provider. By actively involving patients in both use and development, healthcare providers can apply a whole-person perceptive. ; Förutsättningarna för vården har förändrats under de senaste decennierna. Anledningar till dessa förändringar utgörs av bland annat en åldrande befolkning, teknisk och medicinsk utveckling och en ökad förekomst av kroniska sjukdomar. Traditionellt har patienter setts som passiva mottagare av vård, vars roll endast varit att svara på frågor samt följa läkarens ordination av behandling. På senare tid har detta dock börjat förändras. Patienter börjar att i högre grad ses som självständiga, engagerade och deltagande i vården. Enligt detta nya betraktningssätt kan patienter bidra aktivt till värdeskapande, tillsammans med vårdpersonal och andra resurser. Under det senaste decenniet har patientinvolvering setts som en allt viktigare del för att kunna leverera en mer effektiv, integrerad och patientfokuserad vård. Trots detta ökande intresse, finns det en skillnad mellan retorik och praktik. Patientinvolvering och fördelarna med patientinvolvering är oklara både i teori och praktik – bland forskare, sjukvårdspersonal och patienter. Denna licentiatsavhandling utgår från ett tjänsteperspektiv på patientinvolvering och syftar till att förstå och undersöka hur patienter kan vara involverade i användandet och utvecklingen av vården. Avhandlingen bygger på tre olika studier med både kvalitativa och kvantitativa forskningsmetoder. Resultaten av studierna tyder på att patientinvolvering inte är en isolerad process utan istället kan ses som något som påverkar alla delar av sjukvården. Detta utökar synen på vad patientinvolvering kan vara. Istället för att se patientinvolvering som kopplat till att patienten ger information och är involverad i beslutsfattande, kan patienten vara involverad i alla aspekter av sjukvården, både i själva utförandet och utvecklingen. Men det är också viktigt att patientens individuella erfarenheter och preferenser, sammanhang och sjukdomsbild spelar en stor roll för hur mycket och vilken typ av involvering som är lämplig. Vid patientinvolvering i utvecklingen av vården, bör patienter väljas noggrant beroende på mål med utvecklingen och vilken typ av involvering det rör sig om. Även sammanhang och typ av sjukdom är viktiga faktorer för vilken typ av bidrag som kan förväntas av patienter. Beroende på typ av sjukdom och kontext, kan patienter förväntas bidra på olika sätt. Denna avhandling bidrar till en ökad förståelse för patienters involvering i vården genom att ta utgångspunkt ur ett tjänsteperspektiv men fokus på värdeskapande och patienters engagemang. Detta förhållningsätt till patientinvolvering utökar den traditionella synen på involvering genom att föreslå att patienter ska vara involverade i alla steg och aktiviteter i vården. Mycket av patientens värdeskapande sker utanför vården, i den privata sfären, och är därför dolt för vårdgivaren. Att förstå hur patienter skapar värde och sköter sin hälsa är grundläggande för att kunna förbättra vården och stödja patientens egna ansträngningar. Genom att aktivt involvera patienter både i den egna vården men även i utvecklingen av vården i stort är det möjligt att gemensamt skapa en bättre vård.
