Fronek P, Cuthbert D. The future of inter‐country adoption: a paradigm shift for this centuryThe only existing international framework for inter‐country adoption (ICA) is a legal one. Current legal and welfare approaches have limitations in that ICA tends to be conceptualised in individualistic terms, while strategies that prevent ICA by strengthening communities and addressing structural inequalities in the first instance are neglected and divorced from ICA practice. A vision and an overarching framework based on a socioecological approach that informs other arenas, such as health promotion, are proposed. The article concludes that the development of collaborative, intersectoral approaches, the reorientation of public and private resources and international leadership from academics, legislators, policy makers, practitioners and communities are necessary for a paradigm shift for this century.
The world of animals, humans, and environment is interlinked, giving rise to a number of benefits as well as a spread in zoonosis and multifactorial chronic diseases. With the emergence of antimicrobial resistances and environmental pollution, addressing these diseases needs an interdisciplinary and intersectoral expertise. "One Health (OH)" refers to such collaboration between local, national, and global experts from public health, health care, forestry, veterinary, environmental, and other related disciplines to bring about optimal health for humans, animals, and environment. The concept of OH is still in embryonic stage in India and increasingly gaining importance. The Government of India has taken some initiatives to tackle burgeoning problems such as antimicrobial resistance, zoonotic diseases, and food safety using the OH approach, but there are several challenges at the level of implementation. The major bottlenecks in implementing OH include absence of a legal framework to implement OH, poor coordination among different governmental and private agencies, lack of proper surveillance of animal diseases, poor data-sharing mechanism across sectors, and limited budget. Implementing systematic zoonotic surveillance; regulated antibiotic use among humans and animals; development of a zoonotic registry in the country; constitution of a wide network of academic, research, pharmaceutical, and various implementation stakeholders from different sectors is the need of the hour to effectively use OH in order to combat increasing zoonotic diseases.
"The welfare effects of trade shocks depend crucially on the nature and magnitude of the costs workers face in moving between sectors. The existing trade literature does not directly address this, assuming perfect mobility or complete immobility, or adopting reduced-form approaches to estimation. We present a model of dynamic labor adjustment that does, and which is, moreover, consistent with a key empirical fact: that intersectoral gross flows greatly exceed net flows. Using an Euler-type equilibrium condition, we estimate the mean and the variance of workers' switching costs from the U.S. March Current Population Surveys. We estimate high values of both parameters, implying both slow adjustment of the economy, and sharp movements in wages, in response to a trade shock. Simulations of a trade liberalization indicate that despite the high estimated adjustment cost, in terms of lifetime welfare, the liberalization is Pareto-improving. As a follow-up to the national health programming process developed in 1975 in Sudan, a primary health care programme for the whole country was formulated with assistance from WHO. In this article the methods used in the programming and formulation are described and discussed. These methods ensured an intersectoral approach on which technical, cultural, socioeconomic, financial, and political considerations were based. Areas in the field of health and rural development requiring government and community action during the period 1977/78-1983/84 are identified. Details on the strategies for population coverage of rural and nomadic communities with primary health care are given. Fundamental to these strategies is community participation in the development of primary health care within community development as a whole.

Intersectoral output linkages have long been of much concern to development economists and policy makers in developing countries. One of the unequivocal conclusions of the developmental debates over the years, however, is that GNP growth per se does not alleviate unemployment. Rather it is necessary to take into account the employment potential in determining sectoral priorities in the formulation of development policy. In this context, the aim of the paper is to identify key sectors of the Sri Lankan economy taking into account both output and employment linkages. (DÜI-Sbt)
Purpose The reopening of schools during the SARS-CoV-2 pandemic is full of challenges for families, which are heightened for children and young people (CYP) who live with chronic illnesses. This paper aims to offer a framework to support the successful return of CYP with chronic illnesses to school using appropriate intersectoral strategies.
Design/methodology/approach This paper draws on research data on the impact of school closure on CYP with chronic illness and emerging findings of global research about their lives during the pandemic. It is also informed by the perspectives of practitioners in the field, who are working with these CYP and their families.
Findings A framework based on three different strategies for a successful return is established. A small but significant group of CYP living with conditions such as cancer will not yet return and will need ongoing home education provision. CYP with well-controlled symptoms of chronic illness will benefit from school routines and socialization with peers. CYP with poorly controlled illness will need close supervision and individual plans. All groups will benefit from better intersectoral working across education and health and from recent rapid developments in hybrid learning models and telemedicine.
Originality/value This viewpoint highlights the need for a strategic approach to the return to school of CYP with chronic illness that goes well beyond classifying them as vulnerable students. This group of CYP is already at risk of lower educational attainment, so widening inequalities must be halted. This paper provides a framework for anchoring local intersectoral approaches adapted to the different situations of CYP.
Background. Today, an incidence and a mortality of incurable chronic diseases is increasing in the world, growing the relevance of Palliative and Hospice Care (PHC) systems and services creating and developing. The unfavorable medical and demographic situation in Ukraine requires a substantiation and implementation of an affordable, high-quality and effective PHC System that requires of health care providers of all levels involving, interagency and intersectoral coordination and cooperation in the PHC delivery. The purpose was to substantiate the optimal functional and organizational model of specialized PHC providing to the population in Ukraine. Materials and methods. Medical statistics, national and international scientific literature, results of sociological research were used in the work; methods of systematic and structural-functional analysis, medical cartography, biblio-semantic, statistical and sociological methods of research were applied in the work. Results. The optimum functional and organizational model of specialized PHC providing to the population in Ukraine is substantiated, in particular the health care providers involved in specialized PHC providing and their tasks were defined. Conclusions. 1. A medical and social analysis of morbidity and mortality rates and medical and demographic data shows high mortality rate from malignant neoplasms and severe complications of chronic noncommunicable diseases, rapid aging of the population in Ukraine, which determine the high need of the population in PHC. This requires the substantiation and development of an accessible, high-quality and efficient PHC System based on innovative multidisciplinary, interagency and intersectoral approaches, which requires the coordination of the work of institutions that subordinated to different ministries and departments, involvement in PHC providing and social care for palliative patients of health care providers of all levels and social care institutions. 2. A systematic analysis of the PHC worldwide experience in many countries around the world, the recommendations of WHO, the European Association for Palliative Care (EAPC) and other reputable international professional organizations, allows us to propose a three-levels model of PHC System in Ukraine: 1) Palliative care approach or primary PHC; 2) General palliative care; 3) Specialized PHC (hospice or end-of-life care). This was the basis of the developed of the PHC System delivery to the population in Ukraine concept. 3. It is grounded, developed and tested at the level of the innovative components of the functional and organizational model of specialized PHC that based on a patient-family-oriented paradigm, a multidisciplinary, interagency and intersectoral approach at all levels of health care delivery that meets current international standards and norms, to integrate PHC into the Health Care System, the continuity of its delivery, and feedback through the involvement and cooperation of health care providers and social care institutions, public and charitable organizations, patients and their families. 4. The functional and organizational model of specialized PHC providing to the population in Ukraine is a high-tech, highly professional component of the Healthcare System, which requires modern high-tech equipment and effective medicines, special professional training and continuous professional development and motivation of the staff, appropriate legal framework and, of course, proper financial and political support of the Government of Ukraine, regional and local authorities and the interest and support of the whole society. ; Актуальность. Сегодня в мире растет распространенность и смертность населения от инкурабельные хронических заболеваний, что увеличивает актуальность создания и развития систем и служб оказания паллиативной и хосписной помощи (ПХП) населению. Неблагоприятная медико-демографическая ситуация в Украине требует обоснования и внедрения системы доступной, качественной и эффективной ПХП, что требует привлечения к оказанию ПХП медицинских работников учреждений здравоохранения (УЗ) всех уровней медицинской помощи, межведомственной и межсекторальной координации и сотрудничества. Цель работы: обосновать оптимальную функционально-организационная модель оказания специализированной ПХП населению в Украине. Материалы и методы. В работе были использованы данные медицинской статистики, национальная и международная научная литература, результаты социологического исследования; применены методы системного и структурно-функционального анализа, медицинской картографии, библиосемантический, статистический и социологический методы исследования. Результаты. Обоснована оптимальная функционально-организационная модель оказания специализированной ПХП населению в Украине, в частности определены учреждения, которые привлекаются к оказанию специализированной ПХП, и их задачи. Выводы. 1. Медико-социальный анализ показателей заболеваемости, смертности и медико-демографических данных свидетельствует о высоком уровне смертности от злокачественных новообразований (ЗН) и тяжелых осложнений хронических неинфекционных заболеваний (ХНИЗ), быстрое постарение населения в Украине, определяющих высокую потребность населения в ПХП. Указанное требует создания и развития системы доступной, качественной и эффективной ПХП, основанной на инновационных мультидисциплинарных, межведомственных и межсекторальных подходах, что требует координации работы учреждений, подчиненных различным министерствам и ведомствам, привлечения к оказанию ПХП и социальной услуги по паллиативному уходу (СУПУ) медицинских работников УЗ всех уровней медицинской помощи и работников учреждений социальной защиты населения. 2. Системный анализ мирового опыта внедрения ПХП во многих странах мира, рекомендаций ВОЗ, Европейской Ассоциации паллиативной помощи (ЕАРС) и других авторитетных международных профессиональных организаций позволяет предложить внедрить в Украине трехуровневую модель системы оказания ПХП: 1) Паллиативный подход или первичная ПХП; 2) Общая паллиативная помощь; 3) Специализированная ПХП (хосписная помощь или «помощь в конце жизни» - «end of life care». Отмеченное выше было положено в основу разработанной концепции системы оказания ПХП населению в Украине. 3. Обоснована, разработана и апробирована на уровне отдельных инновационных составляющих функционально-организационная модель специализированной ПХП, основой которой является пациент-семья-ориентированная парадигма, мультидисциплинарный, межведомственный и межсекторальный подход на всех уровнях оказания медицинской помощи, которая соответствует современным международным стандартам и нормам. Обґрунтовано, розроблено та апробовано на рівні окремих інноваційних складових функціонально-організаційну модель спеціалізованої ПХД, основою якої є пацієнт-сім'я-орієнтована парадигма, мультидисциплінарний, міжвідомчий і міжсекторальний підхід на усіх рівнях надання медичної допомоги, що відповідає сучасним міжнародним стандартам і нормам. Це забезпечить інтеграцію ПХД в систему охорони здоров'я, безперервність та наступність її надання і зворотний зв'язок шляхом залучення і співпраці закладів охорони здоров'я і установ соціального захисту населення, громадських і благодійних організацій, пацієнтів і їх родин. 4. Background. Myanmar is a premalaria elimination country with artemisinin-resistant malaria. A strategy for transmission control is focused on vulnerable groups such as mobile and migrant populations (MMPs), and includes improving access to insecticidetreated bed nets in the Myanmar artemisinin resistance containment (MARC) zones using multisectoral approaches (MSA). Methods. This narrative systematic review addressed MSAs targeted to MMPs in Myanmar for malaria prevention. We searched relevant studies in electronic databases and present the narrative findings in 4 domains: stakeholder groups, net coverage and utilization, social determinates, and facilitators/barriers. Results. Nine studies were included. The review identified stakeholders involved in intersectoral collaboration. Net ownership was higher than utilization rates in the MARC zones and rates remained below the WHO recommended target of 100%. There was inadequate description of roles and responsibilities for implementation and on channels of communication within the partnerships and with the Government. Conclusions. Findings show that interventions to distribute treated bed nets were supported by the multiple stakeholders. Due to the design of the primary studies, analysis of the added value of intersectoral collaboration was limited. More attention must be paid to designing studies to document and evaluate the contributions and outcomes of intersectoral collaboration. The crisis caused by virus SARS COV 2 brought to the forefront security in Europe, testing both the resilience once the infrastructure of critical complicated, and preparation for crisis situations and the means for managing them. Eastern Europe, plagued by various conflicts in recent decades, still faces many challenges today, such as uncontrolled population growth, declining living standards and climate change due to global warming. The latter will exacerbate the situation in the future. As environmental security is an emerging concern that cannot be addressed with traditional solutions, new ways of dealing with it need to be developed. Cooperation forms the core of such means, aiming an intersectoral approach and ensuring broad stakeholder participation in order to integrate the principle of sustainable development into national policies.

Climate change is already severely impacting people's health and threatening people's guaranteed fundamental rights, including those to life, health, self-determination, and education, among others. Across geographical regions, population groups and communities who are already marginalized due to age, gender, ethnicity, income, and other socioeconomic factors, are those who are disproportionately affected by climate impacts despite having contributed the least to global emissions. Although scholars have been calling for a human rights-based approach and a health perspective to climate action, the literature looking at this multidisciplinary intersection is still nascent, and governments have yet to implement such intersectoral policies. This commentary begins to reflect on the relationship between climate change, human rights, and public health from the perspective of young people engaged in climate action and discourse at the national and international levels. It presents a way forward on what we, as youth climate advocates and researchers, believe is a priority to bring intersectoral integration of human rights and public health approaches to climate change to fruition. First, scholars and practitioners should examine and support youth-led climate interventions that tackle human rights and public health violations incurred by the climate crisis. Second, participatory approaches to climate change must be designed by working synergistically with climate-vulnerable groups, including children and young people, practitioners and scholars in public health and human rights sectors to holistically address the social, health, and environmental impacts of the climate crisis and root causes of injustice. Finally, we recommend more holistic data collection to better inform evidence-based climate policies that operationalize human rights and public health co-benefits.

Baskground. The analysis of medical and statistical data сoncerning the development of a medical and demographic situation in the world and in Ukraine, scientific literature, normative legal documents of Ukraine and authoritative international organizations shows the rapid aging of population, the spread of morbidity and mortality from chronic non-infectious and infectious diseases, low availability of palliative and hospice care (PHC) for the population in Ukraine. Objective: to determine the best models for the PHC in Ukraine based on a medical and social analysis of current international definitions, approaches and standards of palliative care. Materials and methods. The legal documents of international organizations and Ukraine, informational and analytical materials of activities of palliative and hospice care institutions, statistic data and scientific literature were used. Common methods were used. Results. A comparative analysis of the normative legal documents of Ukraine and authoritative international organizations shows that the definition of palliative care in the Ukrainian legislation is somewhat inconsistent with modern international practice; in accordance with the legal and regulatory framework of Ukraine, the definition of palliative care is a kind of medical care and relates only to terminally ill patients with predicted (limited) longevity and their family members, while World Health Organization (WHO) defines palliative care as an approach that improves the quality of life of patients and their families facing the problem associated with life-threatening illness. Of course, such approach greatly expands the cohort of patients and increases the need for PHC. Medico-statistical data testify to the rapid aging of the population in Ukraine and in the world, the spread of mortality from life-threatening and incurable diseases, in particular from cancer and chronic non-infectious and infectious diseases, low availability of palliative care for the population. The article substantiates multidisciplinary and intersectoral model of the palliative care in in-patient and out-patient settings, including home care, the conditions for the creation and implementation of affordable, high-quality and efficient palliative care system in Ukraine. The authors demonstrate the importance and implications of a strategy of PHC development in Ukraine based on internationally recognized definitions, and discuss possible models and levels of PHC integration in health care practice. Conclusions. According to the WHO definition, palliative care is an approach that can improve the quality of life of patients (children and adults) and their families facing with life-threatening illnesses that significantly expands the cohort of patients and increases the need for PHC. Now, PHC is a modern multidisciplinary patient-family-oriented approach that most adequately provides the needs and quality of life of palliative patients and their families, promotes the protection of rights and the preservation of the human dignity of the severely ill persons, including incurable patients with limited life expectancy. A comparative analysis of the normative legal documents of Ukraine and authoritative international organizations shows that the definition of palliative care in Ukrainian legislation is not entirely consistent with current international practice, because in Ukraine, palliative care is a kind of medical care and applies only to incurable patients with a limited prognosis of a life length and members of their families. The government officials, politicians, PHC specialists, human rights defenders and public organizations representatives should take responsibility for the creation and implementation of an innovative approach — an affordable, high-quality, and efficient PHC system for the population and optimization of regulatory documents in accordance with international approaches and standards in the context of reforming the public health system in Ukraine. PHC service, based on international standards and approaches, should become an integral part of the health care system and social protection for Ukraine population. ; Актуальность. Анализ медико-статистических данных по развитию медико-демографической ситуации в мире и Украине, научной литературы, нормативно-правовых документов Украины и авторитетных международных организаций свидетельствует о быстром старении населения, распространении заболеваемости и смертности от хронических неинфекционных и инфекционных заболеваний, низкой доступности паллиативной и хосписной помощи (ПХП) для населения Украины. Цель. На основании медико-социального анализа современных международных определений, подходов и стандартов паллиативной помощи определить оптимальные модели оказания ПХП в Украине. Материалы и методы. При выполнении работы были использованы нормативно-правовые документы международных организаций и Украины, информационно-аналитические материалы деятельности учреждений паллиативной и хосписной медицины, данные отраслевой статистики, научная литература. При обработке использовались общепринятые методы. Результаты. Сравнительный медико-социальный анализ отечественного законодательства, нормативно-правовых документов Украины и авторитетных международных организаций свидетельствует, что определение «паллиативная помощь» в украинском законодательстве не совсем соответствует современной международной практике, поскольку в соответствии с нормативно-правовой базой Украины, понятие «паллиативная помощь» является видом медицинской помощи и применяется только в отношении инкурабельных пациентов с прогнозируемой (ограниченной) продолжительностью жизни и членов их семей, в то время как Всемирная организация здравоохранения (ВОЗ) определяет паллиативную помощь как подход, позволяющий улучшить качество жизни пациентов (детей, взрослых) и их семей, столкнувшихся с проблемами, связанными с угрожающими жизни заболеваниями. Безусловно, такой подход значительно расширяет контингент пациентов, увеличивает потребность в ПХП. Обосновывается мультидисциплинарная и межсекторальная модель оказания паллиативной помощи в стационарных и амбулаторных условиях, в том числе на дому, условия создания и внедрения доступной, качественной и эффективной системы паллиативной помощи в Украине. Авторы показывают важность и значение разработки стратегии создания и развития системы ПХП в Украине, основанной на международно признанных определениях, а также обсуждают возможные модели и уровни интеграции ПХП населению в практику здравоохранения. Выводы. Согласно определению ВОЗ, паллиативная помощь является подходом, позволяющим улучшить качество жизни пациентов (детей, взрослых) и их семей, столкнувшихся с проблемами, связанными с угрожающим (опасным) для жизни заболеванием, что значительно расширяет контингент пациентов, увеличивает потребность в ПХП. Сегодня ПХП является современным мультидисциплинарным пациент-семья-ориентированным подходом, который наиболее адекватно обеспечивает потребности и надлежащее качество жизни паллиативных пациентов и членов их семей, способствует обеспечению прав, сохранению человеческого достоинства тяжелобольных, в том числе инкурабельных пациентов с ограниченным прогнозом продолжительности жизни. В условиях реформирования системы общественного здравоохранения в Украине чиновники, политики, специалисты по ПХП, правозащитники, представители общественных организаций должны взять на себя ответственность за создание и внедрение инновационного подхода — доступной, качественной и эффективной системы ПХП населению, что требует усовершенствования и оптимизации нормативно-правовых документов в соответствии с международными подходами и стандартами. Служба ПХП, основанная на международных стандартах и подходах, должна стать неотъемлемой интегрированной составляющей системы здравоохранения и социальной защиты населения Украины, что будет свидетельством европейского выбора нашего государства. ; Актуальність. Аналіз медико-статистичних даних щодо розвитку медико-демографічної ситуації у світі і в Україні, наукової літератури, нормативно-правових документів України і авторитетних міжнародних організацій свідчить про швидке постаріння населення, поширення захворюваності та смертності від хронічних неінфекційних та інфекційних захворювань, низьку доступність паліативної та хоспісної допомоги (ПХД) для населення в Україні. Мета. На підставі медико-соціального аналізу сучасних міжнародних визначень, підходів і стандартів паліативної допомоги визначити оптимальні моделі надання ПХД в Україні. Матеріали та методи. Під час проведення роботи були використані нормативно-правові документи міжнародних організацій і України, інформаційно-аналітичні матеріали діяльності закладів паліативної та хоспісної медицини, дані галузевої статистики, наукова література. При обробці матеріалів використовувались загальноприйняті методи. Результати. Порівняльний медико-соціальний аналіз вітчизняного законодавства, нормативно-правових документів України та авторитетних міжнародних організацій свідчить, що визначення «паліативна допомога» в українському законодавстві не зовсім відповідає сучасній міжнародній практиці, оскільки відповідно до нормативно-правової бази України, поняття «паліативна допомога» є видом медичної допомоги та застосовується тільки стосовно інкурабельних пацієнтів із прогнозованою (обмеженою) тривалістю життя і членів їх сімей, у той час як Всесвітня організація охорони здоров'я (ВООЗ) визначає паліативну допомогу як підхід, що дозволяє поліпшити якість життя пацієнтів (дітей, дорослих) і їх сімей, які зіштовхнулися з проблемами, пов'язаними із загрозливими для життя захворюваннями. Безумовно, такий підхід значно розширює контингент пацієнтів, збільшує потребу у ПХД. Обґрунтовується мультидисциплінарна та міжсекторальна модель надання паліативної допомоги у стаціонарних і амбулаторних умовах, зокрема вдома, умови створення та впровадження доступної, якісної й ефективної системи паліативної допомоги в Україні. Автори показують важливість і значення розробки стратегії створення та розвитку системи ПХД в Україні, заснованої на міжнародно визнаних визначеннях, а також обговорюють можливі моделі й рівні інтеграції ПХД населенню в практику охорони здоров'я. Висновки. Відповідно до визначення ВООЗ, паліативна допомога є підходом, що надає можливість поліпшити якість життя пацієнтів (дітей, дорослих) і їх сімей, які зіштовхнулися з проблемами, пов'язаними із загрозливим (небезпечним) для життя захворюванням, що значно розширює контингент пацієнтів, збільшує потребу у ПХД. Сьогодні ПХД є сучасним мультидисциплінарним пацієнт-сім'я-орієнтованим підходом, який найбільш адекватно забезпечує потреби та належну якість життя паліативних пацієнтів і членів їх сімей, сприяє забезпеченню прав, збереженню людської гідності тяжкохворих, зокрема інкурабельних пацієнтів з обмеженим прогнозом тривалості життя. Порівняльний аналіз нормативно-правових документів України й авторитетних міжнародних організацій свідчить, що визначення «паліативна допомога» в українському законодавстві не зовсім відповідає сучасній міжнародній практиці, оскільки в Україні паліативна допомога є видом медичної допомоги і застосовується тільки стосовно інкурабельних пацієнтів з обмеженим прогнозом тривалості життя і членів їх сімей. В умовах реформування системи громадської охорони здоров'я в Україні урядовці, політики, фахівці з ПХД, правозахисники, представники громадських організацій повинні взяти на себе відповідальність за створення та впровадження інноваційного підходу — доступної, якісної та ефективної системи ПХД населенню, що вимагає удосконалення, оптимізації нормативно-правових документів відповідно до міжнародних підходів та стандартів. Служба ПХД, яка базується на міжнародних стандартах і підходах, повинна стати невід'ємною інтегрованою складовою системи охорони здоров'я та соціального захисту населення України, що буде свідченням європейського вибору нашої держави.
