Zahlreiche Studien konnten in den vergangenen Jahren die defizitäre chirurgische Versorgung in low- and middle-income countries belegen und den offenen Versorgungsbedarf weltweit und länderspezifisch quantifizieren. Dennoch fehlen theoretische Erklärungsmodelle für die Diskrepanz zwischen der chirurgischen Versorgungslage und ihrer gesundheitspolitischer Priorisierung. Gleichzeitig erfordern Epidemien immer häufiger weltweite gesundheitspolitische Zusammenarbeit und verstärken den Sicherheitsfokus globaler Gesundheit. Vor dem Hintergrund limitierter Ressourcen ergibt sich damit zwangsläufig eine Priorisierung sicherheitsrelevanter Gesundheitsthemen vor anderen. Die hier vorliegende Arbeit diskutiert das politikwissenschaftliche Modell der Versicherheitlichung als theoretischen Erklärungsansatz für die untergeordnete Rolle der globalen Chirurgie. Die zentrale Fragestellung der Analyse lautet: Dient die Versicherheitlichung gemäß der Copenhagen School als Erklärungsmodell für die chirurgische Unterversorgung und ihre marginale politische Bedeutung auf der internationalen Gesundheitsagenda? Methodisch erfolgte eine Fallstudie der Ebola-Epidemie in Sierra Leone im Jahr 2014 zur Analyse der sprachlichen und politisch-kontextuellen Strukturen. Der epidemische Ausbruch von Ebola und der damit zwangsläufig verbundene Einsatz krankheitsspezifischer vertikaler Gesundheitsmaßnahmen schwächte die chirurgische Versorgung und deren zukünftige Entwicklung. Als wesentlicher Mechanismus sind dabei Sicherheitsbedenken wirksam geworden, die das Angebot, die Inanspruchnahme, aber auch die langfristige Weiterbildung chirurgischer Gesundheitsdienstleistungen beeinträchtigten. Die hohe Kontagiosität von Infektionskrankheiten verbunden mit dem potenziellen Auftreten von Prävalenzgipfeln kennzeichnet das große Versicherheitlichungspotential im Vergleich zu chirurgischen Erkrankungen. Darüber hinaus stellen public health emergencies of international concern die einzige im Bereich der Gesundheit existierende Sicherheitsinstitution dar und verschaffen Infektionskrankheiten damit eine privilegierte und institutionalisierte Sicherheitsposition. Vor diesem Hintergrund fungiert das Konzept der Versicherheitlichung als theoretisches Erklärungsmodell sowohl für die marginale politische Position der globalen Chirurgie als auch für die tatsächliche chirurgische Unterversorgung. ; While the global undersupply of surgical care is well recorded, contextual explanations are absent. Simultaneously, outbreaks of infectious diseases force the international community more and more frequently to urgent health action and lead to an increasing security focus on global health. Considering the limiting character of resource allocation, all other medical conditions must compete with the top spot of health security matters. This research introduces the constructivist concept of securitization according to the Copenhagen School to explain the structural handicap of global surgery. Therefore, a case study of the epidemic of Ebola virus disease in Sierra Leone in 2014 was conducted to study the contextual and linguistic structure. Epidemic spread of Ebola in Sierra Leone and the necessarily enacted vertical health measures weakened surgical care and its future development. Security concerns led to a reduced supply, utilization, and training programs of surgical health services. Surgical conditions are non-contagious without the risk for disease outbreaks, hardly preventable and their treatment is often infrastructurally demanding. These key features mark their low securitizing potential. Additionally, as public health emergency of international concern is the only securitizing institution in the realm of health, infectious diseases have a privileged role in health security. Securitization serves as an explanatory model for the inferior political position of global surgery as well as its actual undersupply.
