Cost-effectiveness analysis (CEA) is potentially an important aid to public health decision-making but, with some notable exceptions, its use and impact at the level of individual countries is limited. A number of potential reasons may account for this, among them technical shortcomings associated with the generation of current economic evidence, political expediency, social preferences and systemic barriers to implementation. As a form of sectoral CEA, Generalized CEA sets out to overcome a number of these barriers to the appropriate use of cost-effectiveness information at the regional and country level. Its application via WHO-CHOICE provides a new economic evidence base, as well as underlying methodological developments, concerning the cost-effectiveness of a range of health interventions for leading causes of, and risk factors for, disease. The estimated sub-regional costs and effects of different interventions provided by WHO-CHOICE can readily be tailored to the specific context of individual countries, for example by adjustment to the quantity and unit prices of intervention inputs (costs) or the coverage, efficacy and adherence rates of interventions (effectiveness). The potential usefulness of this information for health policy and planning is in assessing if current intervention strategies represent an efficient use of scarce resources, and which of the potential additional interventions that are not yet implemented, or not implemented fully, should be given priority on the grounds of cost-effectiveness. Health policy-makers and programme managers can use results from WHO-CHOICE as a valuable input into the planning and prioritization of services at national level, as well as a starting point for additional analyses of the trade-off between the efficiency of interventions in producing health and their impact on other key outcomes such as reducing inequalities and improving the health of the poor.
We developed a mathematical model to compare various emergency responses in the event of an airborne anthrax attack. The system consists of an atmospheric dispersion model, an age-dependent dose–response model, a disease progression model, and a set of spatially distributed two-stage queueing systems consisting of antibiotic distribution and hospital care. Our results underscore the need for the extremely aggressive and timely use of oral antibiotics by all asymptomatics in the exposure region, distributed either preattack or by nonprofessionals postattack, and the creation of surge capacity for supportive hospital care via expanded training of nonemergency care workers at the local level and the use of federal and military resources and nationwide medical volunteers. The use of prioritization (based on disease stage and/or age) at both queues, and the development and deployment of modestly rapid and sensitive biosensors, while helpful, produce only second-order improvements.
New 14C dates from archaeological sites in Oaxaca, Mexico, support R. C. Kelly's observation that intervillage raiding may begin as soon as a region has segmentary societies. The oldest defensive palisade dates to 3260–3160 B.P. in conventional radiocarbon years, only a few centuries after village life was established. Over the next millennium raiding evolved into war, with residences and temples burned, captives killed, and populations moving to defensible hills. 14C dates are now available for the first use of hieroglyphic writing to record a captive's name, military victories leading to the consolidation of the Zapotec state, the first skull rack, and the building of a fortress in conquered territory.

Er fand seine allmähliche akademische Codierung durch Wissenschaftler, die dem 1873 gegründeten "Verein für Socialpolitik" angehörten. Seine institutionelle Karriere begann mit den Bismarckschen Sozialreformen, doch dauerte es lange ; bis er sich hier gegenüber konkurrierenden deutschen Begriffen und auch international durchsetzen konnte. .
This paper provides an empirical appraisal of the influence of politics on the evolution of unemployment rates in 13 industrialized democracies (12 European Union countries and the U.S.) from 1960 to 1999. We conduct new tests of opportunistic and partisan business cycle models, using richer data and more general specifications than previous studies. In contrast to most previous studies, we pay particular attention to the importance of labor market structure in conditioning the influence of politics on unemployment. We also investigate the relationship between political stability and economic stability. The results suggest the existence of partisan effects, with higher unemployment rates prevailing under "right" parties than "left" parties. There is more support for "rational" partisan models that embody transient partisan impacts than for models with permanent effects. We find evidence that union power is associated with higher average unemployment rates, but that centralized bargaining institutions tend to lower unemployment rates. Passés entre le ministère de l'Intérieur, les Régions et certaines villes et communes, ces contrats ont pour but, moyennant un soutien financier, le développement de projets locaux visant principalement à lutter contre le sentiment d'insécurité et " la petite délinquance ", à rétablir une qualité de vie et à restaurer la confiance ébranlée de la population dans les institutions, notamment la police.Participant à un " programme d'urgence sur les problèmes de société " adopté par le gouvernement au lendemain d'élections marquées par la montée de l'extrême droite, la mise en œuvre des contrats de sécurité s'est accompagnée de recherches évaluatives commanditées par le ministère de l'Intérieur à divers organes scientifiques indépendants dont les rapports représentent un ensemble de données qui, pour l'heure, n'ont pas toujours été publiées. This paper presents national estimates of the population likely to identify with more than one race in the 2000 census as a result of a new federal policy allowing multiple racial identification. A large number of race-based public policies—including affirmative action and the redistricting provisions of the Voting Rights Act—may be affected by the shift of some 8–18 million people out of traditional single-race statistical groups. The declines in single-race populations resulting from the new classification procedure are likely to be greater in magnitude than the net undercount in the U.S. census at the center of the controversy over using census sampling. Based on ancestry data in the 1990 census and experimental survey results from the 1995 Current Population Survey, we estimate that 3.1–6.6% of the U.S. population is likely to mark multiple races. Our results are substantially higher than those suggested by previous research and have implications for the coding, reporting, and use of multiple response racial data by government and researchers. The change in racial classification may pose new conundrums for the implementation of race-based public policies, which have faced increasing criticism in recent years.
The concept of "institutional radicalization" or "the changing of institutions from their roots" is put forward as critical for understanding the manner in which civil societies in most African countries have been grappling with the challenges of development. Given the well embedded precapitalist social formation in most African countries, various institutions have been radicalized around such critical identities as age, gender, land, occupation, credit, community, and rituals of solidarity to promote development. However, until the state intervenes to legitimize these initiatives of civil societies, their full impact on the development process is bound to be moot. The enormous tasks entailed in such legitimization through identifying, registering, realigning, providing incentives, directing, and monitoring these radicalized institutions remain critical and imperative for African countries if they are to accelerate and deepen the extent of their integration into the global free market economy and democratic governance.
Der Kampf gegen den Konsens: Von der Société du Mont-Pèlerin zum Institute of Economic Affairs: Kurz nach dem Zweiten Weltkrieg nahm der Veränderungswille, der seinen Ausdruck in dem völlig unerwartetn Sieg der Labour Party in den Parlamentswahlen vom Juni 1945 gefunden hatte, in einem von dem neugewählten Premierminister Clement Attlee vorgelegten Programm ehrgeiziger (aber schlecht vorbereiteter) Wirtschafts- und Sozialreformen Gestalt an. Diese Reformen - Verstaatlichung der Schlüsselsektoren der britischen Wirtschaft, groß angelegte Umgestaltung des sozialen Sicherungssystems, durch die ein echter Sozialstaat geschaffen wurde, Schaffung eines nationalen Gesundheitsdienstes, der die Ärzte zu Angestellten des britischen Staates machte und eine kostenlose medizinische Behandlung gewährleistete - waren von zwei Männern geprägt, nämlich von William Beveridge, dem Autor eines der britischen Regierung im Dezember 1942 ausgehändigten Berichts über das soziale Sicherungssystem, und vor allem von John Maynard Keynes. .
This paper reports the results of a randomized field experiment involving registered voters in the city of New Haven. Nonpartisan get-out-the-vote messages were delivered through personal canvassing shortly before the November 1998 election. We find that personal canvassing increased voter turnout by ≈6. The effect of personal contact seems to be slightly smaller for voters registered with a major political party and higher for unaffiliated voters, although the hypothesis that all voters are equally affected could not be rejected. Study of several alternative political messages provided equivocal evidence suggesting the superiority of a canvassing appeal that emphasizes the closeness of the election.
Vorwort: Als ich im Herbst 1994 erstmals vortragsweise versuchte, die gegenwärtigen Herausforderungen des deutschen Sozialstaates im Zusammenhang zu skizzieren, war das Thema erst Gegenstand publizistischer und noch nicht politischer Auseinandersetzungen. Nun stehen wir mitten im Kampf um neue Renten- und Steuergesetze; Kürzungen im Bereich der Sozialhilfe und der Arbeitslosenversicherung sind bereits erfolgt, und Debatten um eine weiterreichende Umstrukturierung der Krankenversichrung wollen nicht zur Ruhe kommen. .
Mehr noch als die soziologische Osteuropaforschung generell zählt die soziologische Forschung über Südosteuropa zu den lange Zeit weitgehend vernachlässigten Gebieten der deutschen Soziologie. Dies hat viele außerwissenschaftliche wie auch wissenschaftsimmanente Gründe, die an anderen Stellen näher dargelegt worden sind. Selbst wenn sich die äußeren Rahmenbedingungen heute wesentlich verbessert haben und auch das Interesse an der soziologischen Beschäftigung mit Ost- und Südosteuropa im Rahmen der deutschen Soziologie deutlich gestiegen ist, sind die Dinge doch noch weit davon entfernt, einen befriedigenden Entwicklungsstand erreicht zu haben. Im ersten Aufsatz dieses Heftes ("Zur soziologischen Südosteuropa- und Rumänienforschung") soll daher ein Überblick über die soziologische Südosteuropaforschung und insbesondere die Rumänienforschung gegeben werden, der einige Schwerpunkte der Forschung und der wissenschaftlichen Zusammenarbeit, aber auch auffällige Defizite anspricht sowie einige Desiderate im Hinblick auf die zukünftige Arbeit formuliert. Ein Kernproblem der soziologischen Forschung über Südosteuropa liegt in den unübersehbaren theoretischen Mängel und empirischen Lücken der Sozialstrukturanalyse. Im Vergleich zur Kultur und Geschichte, zur ethnischen Minderheitenproblematik und Bevölkerungsentwicklung, zum politischen Geschehen, zum Institutionenwandel oder den wirtschaftlichen Verhältnissen erscheinen die sozialstrukturellen Gegebenheiten südosteuropäischer Gesellschaften weitgehend unteranalysiert. Selbst die wesentlichen Grundzüge ihrer Sozialstrukturen sind wenig bekannt. Viele Schwierigkeiten liegen schon in den Ausgangspunkten der sozialstrukturellen Analysen und Betrachtungsweisen begründet, zumal gängige Klassen- und Schichtungskonzepte, mit denen die Sozialstrukturforschung häufig operiert, auf die Gegebenheiten südosteuropäischer Gesellschaften nicht ohne weiteres anwendbar erscheinen. Im zweiten Aufsatz des Heftes ("Überlegungen zur Klassentheorie und zur Sozialstruktur südosteuropäischer Gesellschaften") soll - vor dem Hintergrund entsprechender theoretischer Erörterungen - auf der Grundlage empirischer Befunde gezeigt werden, warum eine klassentheoretische Betrachtung der Sozialstrukturen südosteuropäischer Gesellschaften unangemessen wäre und weshalb eher Prozesse der sozialen Schließung im Mittelpunkt der Analyse stehen sollten. Mit Vorgängen der sozialen Schließung und der Rolle des "sozialen Kapitals" wird sich auch der dritte Aufsatz ("Soziales Kapital und soziale Schließung") näher beschäftigen, in dem einige grundlegende Überlegungen zum "sozialen Kapital" entwickelt und einige Hinweise zur Rolle des "sozialen Kapitals" in fortgeschrittenen westlichen und in ost- und südosteuropäischen Gesellschaften gegeben werden. Die drei Aufsätze, die hier zusammengeführt wurden, gehen auf Beiträge zu wissenschaftlichen Tagungen zurück, die im Herbst 1995 stattfanden. Der Aufsatz "Zur soziologischen Südosteuropa- und Rumänienforschung" stellt eine überarbeitete Fassung meines Vortrages "Die soziologische Ost- und Südosteuropaforschung in der Bundesrepublik Deutschland - unter besonderer Berücksichtigung der Rumänienforschung" dar. Den Vortrag habe ich auf dem Symposion "Stand der Südosteuropa-Forschung in Deutschland und Rumänien - Möglichkeiten der deutsch-rumänischen Zusammenarbeit" gehalten, das vom 5. bis 6. Oktober 1995 von der Südosteuropa-Gesellschaft und der Rumänischen Kulturstiftung anläßlich der Deutschen Kulturwochen in Rumänien in Bukarest veranstaltet wurde. Bei dem Aufsatz "Überlegungen zur Klassentheorie und zur Sozialstruktur südosteuropäischer Gesellschaften" handelt es sich um eine überarbeitete Fassung meines Beitrages "Die Grenzen klassentheoretischer Analysekategorien in der Sozialstrukturanalyse südosteuropäischer Gesellschaften" zu der Arbeitstagung: "Klassentheorie am Ende der Klassengesellschaft" der Sektion "Soziale Ungleichheit und Sozialstrukturanalyse" in der Deutschen Gesellschaft für Soziologie, die am 7. bis 8. Oktober 1995 in Hannover stattfand. Der Aufsatz "Soziales Kapital und soziale Schließung" schließlich geht auf den Beitrag "Anmerkungen zum Begriff und zur Rolle des sozialen Kapitals in der Leistungsgesellschaft", zum 14. Österreichischen Kongreß für Soziologie, der vom 28. bis 30. September 1995 in Innsbruck ausgerichtet wurde, zurück. Ein Band mit dem Titel "Kontinuitäten und Wandel in Rumänien und Südosteuropa" befindet sich in Vorbereitung und soll voraussichtlich in diesem Jahr erscheinen. Darin werden hier vorgestellte Gedanken wiederaufgenommen und in den Zusammenhang weiterführender und ergänzender Überlegungen gestellt werden.
