Zentrales Ziel der Pflegeversicherung war und ist, zu einer qualitativ guten pflegerischen Versorgung im Alter beizutragen – auch unabhängig von der individuellen familiären Situation und angesichts demografischer und gesellschaftlicher Veränderungen. Wenn es darum geht, 20 Jahre nach Einführung der Gesetzlichen Pflegeversicherung ihre Erfolge vorläufig zu bewerten, scheiden sich aber die Geister. Aus der Sicht der einen ist die Pflegeversicherung eine Erfolgsgeschichte für Pflegebedürftige, ihre Angehörigen und die Pflege als professionelles Handlungsfeld. Aus Sicht anderer sind die Leistungen der Pflegeversicherung zu gering und Qualität und Arbeitsbedingungen in der Pflege unzureichend. Ziel des Beitrags ist es, die bestehenden Mythen in der deutschen Pflegepolitik darzustellen, kritisch zu untersuchen und mit empirischen Fakten zu konfrontieren, um so eine sachliche Bewertung der Reformoptionen für die Gesetzliche Pflegeversicherung zu erlauben. ; The main purpose of the German long-term care insurance was — and still is — to contribute to an adequate supply with and a good quality of long-term care services for older people — regardless of their individual family situation and in the light of demographic and societal changes. When seeking to preliminarily assess the benefits 20 years after the introduction of the statutory long-term care insurance, opinions tend to differ. From some people's point of view the long-term care insurance is a success story for people in need of care, their families and nursing as a profession. Others consider the capped benefits of the long-term care insurance as insufficient and the quality and working conditions as inadequate. This article therefore aims to describe the current "myths" in German long-term care politics, critically assess them and contrast them with empirical facts that allow for a fact-based appraisal of the various options for statutory long-term care insurance reform.

Some explanation of the origin of this short paper may assist in understanding its content and point of view. It is the result of a study of certain official papers dealing with cases of federal stress. These papers deserve, and indeed demand, careful study by all who exert any influence on government policy in Canada. I refer to (a) A Submission on Dominion-Provincial Relations and the Fiscal Disabilities of Nova Scotia within the Canadian Federation, presented by Professor Rogers to the Nova Scotia Royal Commission Provincial Economic Inquiry (1934); (b) Province of Nova Scotia, Report of the Royal Commission Provincial Economic Inquiry (1934), especially the supplementary report by Dr. Innis, and the memorandum by Professor R. A. MacKay on the "Financial Relations between the Provinces and the Dominion" published in the volume of Appendices to the Report; (c) British Columbia's Claim for Readjustment of Terms of Union presented by the Honourable T. D. Pattullo, the Honourable G. McG. Sloan, and the Honourable John Hart (1934); (d) The Case of the People of Western Australia in Support of their Desire to Withdraw from the Commonwealth of Australia (1934). Though these official papers have led me to try to think through the economic problems, and the economic philosophy, of federalism, the position I take is not new to me. It is simply a development of an attitude towards problems of regional conflict with an attempt to determine what difference exists when the state is federal rather than unitary. My attitude towards problems of regional conflict is exemplified in a paper contributed last year to The Canadian Economy and its Problems, but no reference is there made to the conflict of provinces as such. In view of this approach it may be less surprising if I add to the list of official papers given above one from the United Kingdom; (e) Reports of Investigations into the Industrial Conditions in Certain Depressed Areas of I West Cumberland, II Durham and Tyneside, III South Wales and Monmouthshire, IV Scotland (1934, Cmd. 4728).

Der Artikel zeigt in den grundlegenden Zügen die Entwicklung in Ostdeutschland seit dem Beginn der Umstrukturierung auf und zieht eine Bilanz zum Stand des Anpassungsprozesses 20 Jahre nach dem Mauerfall. Wie sich an der Entwicklung der Wirtschaftsleistung zeigen lässt, ist der wirtschaftliche Neuaufbau gut vorangekommen. Allerdings ist das Resultat hinter den Erwartungen zur Wendezeit zurückgeblieben. Und weiterhin sind die neuen Bundesländer weit davon entfernt, wirtschaftlich auf eigenen Beinen stehen zu können. Sie werden noch massiv vom Westen unterstützt. Ausdruck der Rückständigkeit ist neben einem relativ großen Mangel an Beschäftigungsmöglichkeiten eine vergleichsweise geringe Produktivität. Deren Ursachen sind vielfältig. So mangelt es an Beschäftigung und Wertschöpfung in solchen Sektoren, die generell eine vergleichsweise hohe Produktivität vorweisen. Exemplarisch für das verarbeitende Gewerbe lässt sich zeigen, dass größere Betriebe unterrepräsentiert sind, und dass Arbeitsplätze, die eine höhere Qualifikation voraussetzen, relativ weniger als im Westen zu finden sind. Hinzu kommt eine bisher nur wenig beachtete Einflussgröße: die im Vergleich zu den alten Bundesländern viel dünnere Besiedlungsdichte Ostdeutschlands. Insbesondere für die alten Bundesländer lässt sich zeigen, dass eine überdurchschnittliche Wirtschaftsleistung in erheblichem Maße mit einer starken Besiedlungsdichte zusammenhängt. In Ostdeutschland ist das auch der Fall – wegen der enormen Ausgleichswirkungen der Sozialtransfers aber weniger deutlich als im Westen. Es wäre eine Illusion zu glauben, solche Strukturunterschiede mittelfristig ausgleichen zu können, zumal die Differenzen in der Raumstruktur zwischen Ost und West historischen Charakter haben. Überdies gibt es sowohl innerhalb der alten wie innerhalb der neuen Bundesländer große regionale Disparitäten. Fast 20 Jahre nach der Deutschen Einheit sollte man deshalb Abschied von den meist zu simplen Ost-West-Vergleichen nehmen. Viel mehr muss die Frage in den Vordergrund gerückt werden, welche Wachstumsmöglichkeiten die einzelnen Regionen Ostdeutschlands – auch angesichts des Bevölkerungsrückgangs – überhaupt haben. Zu einem neuen politischen Realismus gehört neben dem längst überfälligen Abbau von Subventionen eine Neuausrichtung der Regionalförderung – und zwar nicht speziell für Ostdeutschland, sondern im gesamtdeutschen Rahmen unter Einbeziehung der neuen Bundesländer.
