The rapid development of science has significantly expanded the boundaries of liberal discourse, by including bioethics to it. Bioethics has special status due to the fact that unlike other sectors of ethics within it specifically organized structures that affect the implementation of the relevant norms are formed. It is about bioethical committees as basic structural components of bioethics, becoming the subject exactly in the political and institutional dimensions. Specificity of the institutionalization of bioethics committees became the subject of analysis in this article, in particular, a comparison of European and American experience of their formation. The article explores the modern condition of the legal regulation of post-trial access to investigational treatment. The author has determined main arguments concerning the necessity of foreseeing such a right and the problematic issues that arise in case of its implementation. On the one hand, a subject can legitimately expect that s/he will continue to have access to beneficial treatment after the end of trial, for example, if it alleviates the symptoms of a chronic illness, depriving a trial subject from the beneficial post-trial access would defeat the basic principle of medical ethics. At the same time, this provision may have a large financial burden on the sponsor of the trial; it is difficult to control the occurrence of serious adverse reactions; providing post-trial access only to the subjects of the trial and denying others create disparity among patients.The particular attention is paid to international standards and experience of foreign countries in this field. The legal and regulatory acts of Ukraine do not have any provisions on post-trial access to investigational treatment. The main international ethical guidelines, such as Declaration of Helsinki, CIOMS/WHO Guidelines, contain such a right, but they have different formulations and volumes that only intensify professional discussions. The most common norm is as follows: «a responsibility of sponsor to supply details of the plan for treatment or care of subjects once their participation in the trial has ended», which, in contradistinction to the Ukrainian legislation, is not connected with the causing damage to the subject. The author also has researched a unique case of Brazil, where the post-trial drug access program must make the medication available and free of charge to the subjects of the research in the cases of concluding the trial or after the patients' participation in the study.On this base, the author offered to provide in the legislation of Ukraine requirements to inform in advance subjects about the presence or absence of post-trial access and to evaluate these provisions by ethics committees before and at the end of all trials in order to determine its real need in each individual case. The scope of the right on post-trial access to investigational treatment must be reasonably weighed in order to avoid, on the one hand, becoming an excessive stimulus for the subjects, and, on the other hand, not leading to a situation where on such regulation conducting of clinical trials in the country will no longer be appropriate. ; Исследовано современное состояние правового регулирования доступа к исследуемому лечению по завершении клинического исследования. Приведены основные аргументы относительно необходимости предусмотреть такое право и проблемные вопросы, возникающие при его введении. The main economic function of the university is associated with providing the organizations-economic operators with new highly qualified personnel. The formal grounds of professional ethos of high school teacher should first be: an adequate level of professional competence in a particular field of scientific knowledge, adequate pedagogical skills, high level of reflectivity and especially moral consciousness of the teacher, educational services market presence, a sufficient level of economic development, formation of basic communities involved in the use of professional competence of university lecturer (most professors, students, customers qualified university graduates, university administration etc.). Market society gave commercialism problem in education providing the industrial scale, that even post-industrial technologies are unable to overcome. The main economic benefit of university activity should evaluate in the terms of capital turnover - but, above all, symbolic capital, then social capital, then economic (including financial), and then all other forms of capital that one can imagine – political or any more. University produces and reproduces meanings – these meanings drive development of society in general, including economic. The situations of social interaction then becomes the foundation that creates institutions not formally, but informally and continuously: this generation is comparable to the democratic way of doing business every day as opposed to participation in democratic elections just once every five years. The model of conversation sets the thematic certainty, not just grammatical correctness of communication. So, for our study it refers to the specific educational ethical values that may limit some and stimulate other economic actions with specific educational ethics. These rules are in its most specific so that the better they work in education - the worse they can be applied in other fields such as politics, law, religion or any other. Therefore the use of this approach to education focuses on general research not of social norms, but of the specific educational standards, and within the education system is not to create a specific "institutional educational sub-world" as a variation of the overall institutional world that "rutinize" educational practice, but to study existing forms of educational communication to the (in Luhmann's terms) contingence in the course of educational interactions, which combines binding behaviors and casualty select from these options, ie a combination of situational over-situationality. This over-situationality appears not as typization of available, but as the identification of its selective character. The fact that certain educational practices used in various educational circumstances does not mean that it is a typical or even successful, or some certified algorithms for solving educational problems. The same educational practice may be the answer to a completely different educational situations that have only a few identical components, which revealed sensitive for such practice. Accordingly, it can be more or less successful depending on how these "sensitive" components are crucial for any givensituation. This applies to the comparison of educational ethics norms with no-educational, and a variety of ethical standards. However, on educational standards - there should still be own, especially "sensitive"components in each case, ie in particular also has its own economic behavior ethics in education. The main economic criterion in the selection of education applicants is estimation of the prospects of the applicant as a future professional. Hardly anyone who participated in some role (as applicant or a member of the selection committee) in the admission campaign in university might argue that this particular economic criterion could be only partly formalized and contains a significant proportion of ethical (or unethical) ratio selection committee to the applicant . And this ethic is directly related to economic calculation - even when it is not aware of the selection committee members or the university administration. International databases and reports are used to demonstrate the low level of women's involvement into the "big politics". Ukraine has failed to fulfil international obligations of UN Millennium Development Goals on achieving gender equality in the country by 2015.In addition, the author has reviewed the Ukrainian legislation on gender equality in general and in political sphere in particular. Formally, Ukraine has introduced a policy of equal rights and opportunities for women and men in society, which meets international standards. However, according to the results of the gender analysis of the Verkhovna Rada of Ukraine (VRU), the situation with women in the Parliament continues to remain unsatisfactory and far behind the ideas of gender equality.The author discusses the involvement of women in the VRU at the level of party lists, committees, the Apparatus, and permanent delegations. In particular, such problems have been identified: the low level of representation of women in the Verkhovna Rada (12 %), the gender imbalance in the composition of the main political parties of Ukraine, vertical and horizontal gender segregation at the level of the VRU, the under-representation of women among the leadership of permanent delegations, sexist comments among the deputies of the VRU, etc.Finally, the international experience of introducing a gender-sensitive parliament and its application in Ukraine has been studied. It includes such components as the gender legal expertise of legislation, the introduction of a Code of Ethics for deputies for the purposes of countering sexism, the introduction of a gender indicator of the activities of the VRU, etc.After conclusions, three major recommendations have been proposed. First, according to international and national experience, in order to achieve gender balance in the composition of the VRU, including its leadership positions, temporary (supporting) actions for women (for example, gender quotas) should be applied. Secondly, gender equality and non-discrimination, counteraction to sexist statements should be among the fundamental principles of the Code Ethics for deputies of the VRU. Thirdly, it is appropriate to introduce a mandatory and regular gender legal and anti-discrimination review of draft laws in the VRU. ; У статті проаналізовано представництво жінок у парламенті України в міжнародній перспективі. The article analyzes the notion 'public security and order' which is the main task of such law enforcement agency as the National Police of Ukraine. There are attempts to identify the place and legal nature of such category as 'public order' in the sphere of civil law and private international law. It has been noted that public order in private international law is a mean to limit application of foreign law in private relations complicated by a foreign element. The frequency and context of the use of notions 'public security and order' in the Law of Ukraine 'On the National Police' of July 2, 2015 have been presented. Normative enshrining of this and related notions such as 'civil security and order' and 'national security' in the Constitution of Ukraine, the Code of Ukraine on Administrative Offences, the Criminal Code of Ukraine and other legislative acts has been analyzed. Approaches of scholars to the understanding of the essence and differences between mentioned notions have been studied. Problems of the Draft Law 'On Public Order' have been shown. Foreign experience regarding the use of notions 'public security' and 'public order' has been investigated. The content of the Recommendation Rec (2001) 10 of the Committee of Ministers to Member States on the European Code of Police Ethics and the Law of Bavaria 'On Tasks and Powers of Bavarian Police,' the Law of Portugal 'On the Organization of Police and Public Security,' the Law of Poland 'On Police,' the Law of Slovenia 'On Police,' the Law of the Netherlands 'On Police,' the Law of Belgium 'On Police Activities.' It has been concluded that notions 'public security' and 'public order' are close to the notions 'civil security' and 'civil order.' Necessity to solve the problems which arose after introducing the Law of Ukraine 'On the National Police' regarding public security and order has been proved. Those notions are not defined in legislation and science. It has been proposed to use old definitions in national law terminology.
Current military and political situation around Ukraine and immediately in it has considerably intensified the problem of providing its military security. Therefore, the decision of the national command authorities of our country to join the EU, to intensify cooperation with NATO and accession to it in the nearest future has become very significant. Much attention is given to reforming and adaptation of the special services and agencies to the NATO standards. However, the issue of intensifying control over their activity is missing. Considering experience of many countries, it poses a danger. Here, the experience of Canada as the country, which is one of the most successful and supportive to Ukraine, can be useful.Canada focuses much on providing and supporting national and regional security and interests in a current, very hard military and political situation. Trying to counter possible threats and challenges by preventive measures, this country has got a series of special services and agencies putting them together as the Intelligence Community. The Intelligence Community functions within the existing legal framework. First of all, it is based on the Laws on the Canadian National Defence, on the Information Security and an on Privacy and a number of others.At first, it became necessary to intensify control over the activity of national special services and agencies of Canada at the end of the 70s of the 20th century when they abused their power against Que- bec National Liberation Movement. Indignation and protests of the public considering their authorities excessive, led the parliament's taking adequate measures: to adopt several special laws and decrees, to form a number of committees, commissions and special bodies designed to control the activity of special services and agencies, their observation of national laws and human rights.Besides national control, the Canadian special services and agencies have their own (departmen- tal) codes of honour authorizing their officers to be wotrthy a given right to serve to the nativeland in the respective special departments and observe current national laws. The bright example of such a code is the Canadian Communications Security Establishment Ethics Charter.Thus, the Canadian special services and agencies (Intelligence Community) function within the legal framework under the control of respective supervising and overseeing agencies, institutions and commissioners which can be a model for our state. ; Проаналізовано досвід організації й здійснення контролю за діяльністю спеціальних органів та служб (розвідувального співтовариства) Канади в умовах викликів і загроз її національній безпеці і інтересам наприкінці ХХ, початку ХХІ століття.
With hypothyroidism (HТ) there is a change in homeostasis in tissues sensitive to thyroidhormones, which in its turn leads to a violation of fat metabolism. HT induces a significantincrease in lipid peroxidation and reduces the activity of antioxidant enzymes, oxidativestress, which is formed in HT, can play a key role in the progression of pancreaticdysfunction (PD).Purpose – to study the indicators of the lipid profile and the level of thyroid hormones inpatients with CP and CP, combined with HT.Material and methods 107 people were examined, including 29 patients with CP (group1), 30 patients with HT (group 2), 28 patients with CP on the associated with HT (group3), 20 almost healthy individuals (AHI) (group 4). The age of the examined patientsranged from 44 to 72 years, the main percentage (53%) was dominated by people ofworking age from 49 to 58 years (mean age - 54.26 ± 1.87). The gender distribution wascomparable across all groups. According to the local ethics committees for biomedicalresearch, guided by Ukrainian health legislation and the 2000 a. declaration of Helsinki,all patients and AHI surveyed gave written informed consent to participate in the study.All patients were hospitalized at the Chernivtsi Regional Clinical Hospital and theChernivtsi Regional Endocrinology Center.The results. According to the results of the study of the lipid profile in all groups of patientsthere were manifestations of dyslipidemia, but they were more pronounced in patientswith combined pathology: total cholesterol increased 36.04% (p <0.05) compared withAHI; 21.37% compared with the group of patients with CP and by 15.06% compared withthe group of patients with CP. The level of triglycerides increased in the group of patientswith CP, associated with HT in 2.2 times compared with AHI group, in comparison ofthe groups of the combined pathology with CP patients and with HT patients it is higher 34.51% and 9.4% (p <0, 05). Patients in all 3 groups had an increase in very low-densitylipoprotein cholesterol compared with AHI, with a tendency to increase in patients inwhom CP was combined with HT.Conclusion. The most pronounced signs of dyslipidemia were found in patients with CPcombined with HT, which confirms the close pathogenetic link between these nosologies- a marker of thyroid insufficiency (increased TSH levels, decreased T4), manifestationsof dyslipidemia, which is a predictor of pancreatic lesions. The study of comorbidity ofthese nosologies is a promising area of clinical gastroenterology and endocrinology. Theresults prove the need for more detailed and in-depth study of the clinical course andinteraction of these diseases, in order to optimize the therapeutic treatment regimens, thatwill allow to avoid the development of complications and disability. ; При гипотиреозе (ГЗ) происходит изменение гомеостаза в чувствительных ктиреоидных гормонов тканях, что в дальнейшем ведет к нарушению жировогообмена. ГЗ индуцирует значительное повышение ПОЛ и снижает активностьантиоксидантных ферментов; окислительный стресс, который образуется приГЗ, может играть ключевую роль в прогрессировании дисфункции поджелудочнойжелезы (ПЖ).Цель работы – исследовать показатели липидного профиля и уровня тиреоидныхгормонов у пациентов с ХП и ХП, совмещенным с ГЗ.Материалы и методы. Обследовано 107 человек, из них 29 больных ХП (группа 1), 30 больных ГЗ (группа 2), 28 больных ХП на фоне ГЗ (группа 3), 20 практическиздоровых лиц (ПЗЛ) (группа 4). Возраст обследованных больных колебался от 44до 72 лет, основной процент (53%) составили лица трудоспособного возрастаот 49 до 58 лет (средний возраст – 54,26 ± 1,87). Гендерный распределение былосопоставимым во всех группах. Согласно установок местных этических комитетовпо вопросам биомедицинских исследований, руководствуясь законодательствомУкраины об охране здоровья и Хельсинкской декларации 2000 г., обследованныепациенты и ПЗЛ дали письменное информированное согласие на участие висследовании. Все пациенты находились на стационарном лечении в Черновицкойобластной клинической больнице и в Черновицком областном эндокринологическомцентре.Результаты. По результатам исследования липидного профиля во всех группахпациентов наблюдались проявления дислипидемии, однако более выраженнымиони были у пациентов при сочетанной патологии: показатели общего холестеринаувеличены на 36,04% (р <0,05) по сравнению с ПЗЛ; на 21,37% по сравнению сгруппой больных ХП и на 15,06% по сравнению с группой больных ГЗ. Показательуровня триглицеридов увеличен в группе больных ХП, сочетанным с ГЗ в 2,2 раза посравнению с группой ПЗЛ, при сравнении группы сочетанной патологии с больнымиХП и с больными ГЗ выше на 34,51% и 9,4% (р < 0,05). Больным всех 3 групп былоприсуще увеличение холестерина липопротеидов очень низкой плотности посравнению с ПЗЛ, с тенденцией к большему росту у пациентов, у которых ХПсочетался с ГЗ.Вывод. Наиболее выраженные признаки дислипидемии обнаружены у больных с ХП,совмещенным с ГЗ, что подтверждает тесную патогенетическая связь междуданными нозологиям – маркером тиреоидной недостаточности (повышениеуровня ТТГ, снижение показателей свободного Т4), проявлениями дислипидемии,что является предиктором поражения ПЖ. Исследование коморбидности этихнозологий является перспективным направлением клинической гастроэнтерологиии эндокринологии. Результаты доказывают необходимость более детально иуглубленно исследовать особенности течения и взаимовлияния этих болезнейс целью оптимизации терапевтических схем лечения, что позволит избежатьосложнений и инвалидизации. ; При гіпотиреозі (ГЗ) відбувається зміна гомеостазу в чутливих до тиреоїднихгормонів тканинах, що у наслідку призводить до порушення жирового обміну.ГЗ індукує значне підвищення пероксидації ліпідів та знижує активністьантиоксидантних ферментів; окиснювальний стрес, який утворюється при ГЗ,може відігравати ключову роль у прогресуванні дисфункції підшлункової залози(ПЗ).Мета роботи – дослідити показники ліпідного профілю та рівня тиреоїднихгормонів у пацієнтів із ронічним панкреатитом (ХП) та ХП, поєднаним із ГЗ.Матеріали та методи. Обстежено 107 осіб, із них 29 хворих на ХП (група 1),30 хворих на ГЗ (група 2), 28 хворих на ХП на тлі ГЗ (група 3), 20 практичноздорових осіб (ПЗО) (група 4). Вік обстежених хворих коливався від 44 до 72 років,основний відсоток (53%) становили особи працездатного віку від 49 до 58 років(середній вік – 54,26±1,87). Гендерний розподіл був порівняний у всіх групах. Згідноз настановами місцевих етичних комітетів із питань біомедичних досліджень,керуючись законодавством України про охорону здоров'я та Гельсінської декларації2000 р., обстежені пацієнти та ПЗО дали письмову інформовану згоду на участьу дослідженні. Усі пацієнти перебували на стаціонарному лікуванні в Чернівецькійобласній клінічній лікарні та в Чернівецькому обласному ендокринологічномуцентрі.Результати. За результатами дослідження ліпідного профілю у всіх групахпацієнтів спостерігались прояви дисліпідемії, однак більш вираженими вони булиу пацієнтів при поєднаній патології: показники загального холестерину збільшеніна 36,04 % (р<0,05) порівняно з ПЗО; на 21,37 % порівняно з групою хворих наХП та на 15,06 % порівняно з групою хворих на ГЗ. Показник рівня тригліцеридівзбільшений у групі хворих на ХП, поєднаний із ГЗ у 2,2 раза порівняно з групоюПЗО, при порівнянні групи поєднаної патології із хворими на ХП та з хворими наГЗ вищий на 34,51 % (р<0,05) та 9,4%(р<0,05). Для хворих усіх 3 груп властивезбільшення холестерину ліпопротеїдів дуже низької щільності, порівняно з ПЗО,з тенденцією до більшого зростання у пацієнтів, у яких ХП поєднувався з ГЗ.Висновок. Найбільш виражені ознаки дисліпідемії виявлено у хворих із ХП,поєднаним із ГЗ, що підтверджує тісний патогенетичний зв'язок міжцими нозологіями – маркером тиреоїдної недостатності (підвищення рівнятиреотропного гормону, зниження показників вільного тироксину), проявамидисліпідемії, що є предиктором ураженням ПЗ. Дослідження коморбідностіцих нозологій є досить перспективним напрямком клінічної гастроентерологіїта ендокринології. Результати доводять необхідність більш детально тапоглиблено дослідити особливості перебігу та взаємовпливу цих хвороб із метоюоптимізації терапевтичних схем лікування, що дасть змогу уникнути ускладненьта інвалідизації.
The article deals with the most important factors which shape challenges for educational policy and directions of its reformation in transnational educational space. In context of global society formation educational policies of developed countries demonstrates experiences of development and implementation of transversal (transferable, transcultural) competencies as key competencies of the 21st century in order to generate collective nous, peace, social justice and sustainable economic development. As one of the main goals of key competencies development considered promotion lifelong learning; It emphasizes the need for situational, contextualize, projective teaching and constructivist approaches. Particular attention is paid to analysis of key competencies in the trans-European educational space, of transferable competencies as US educational strategy and transversal competencies as the strategies proposed by UNESCO. ; В статті розглядаються найбільш значимі фактори, що формують виклики для освітньої політики, та напрями її реформування у транснаціональному освітньому просторі. В контексті становлення глобального суспільства з метою генерування колективного розуму, досягнення миру, соціальної справедливості і стійкого економічного розвитку освітня політика розвинених країн демонструє досвід розвитку та імплементації трансверсальних (трансферабельних, транскультурних) компетенцій як ключових для 21 століття.
