Med care in the US is organized around a large number of agencies both gov'al & non-gov'al, with insufficient co-ordination among them & no effective machinery for over-all planning. There are shortages & maldistribution of both personnel & facilities. The rise in specialization has resulted in continued decline in the number & status of the fam physician. Org'ed group practice has increased, but not to the extent of making a signif impact on the prevailing solo, fee-for-service pattern of Med practice. Hosp's face esp acute problems in the form of rapidly increasing costs & shortages of staff. Facilities for the care of the chronically ill & aged are insufficient in number & the care they offer is often far from adequate. Some trends that may presage a more rational org of Med services are the concentration of community Med resources in & around voluntary hosp's: the differentiation of care within hosp's to fit patient needs more closely; the development of hosp-based services for ambulatory & homebound patients; & increasing consideration to the establishment of closer relationships among hosp's, nursing homes & other facilities serving a given region.
