In today's global world, people move more, move further, and move for increasingly varied reasons, such as work, study or the search for exoticism. Seeking health care away from one's home is a part of this trend and many patients these days do not hesitate to cross borders or to cover sometimes thousands of kilometers in order to consult a well-known doctor or to benefit from a fast, high-quality and cheap health-care service. Our study concerns the questions raised by patients' movements at a regional scale, more precisely in the Lao-Thai border area where new behaviors are appearing around health care mobility. Lao nationals are crossing the border to seek health care in Thai hospitals in ever increasing numbers. A sizeable part of the population is involved in this practice, which is connected to the recent political and cultural opening of Laos and at the same time emphasizes the development gap between the two countries. Border discontinuity between the two countries is reflected by a medical discontinuity which helps explain these movements, facilitated also by the proximity and historical links of the populations living along the Mekong. What does this mobility reveal regarding the socio-economic and political reality of the Lao-Thai border area and what light can it shed on the relationship between the two countries? How is the cross-border territory being reconstructed in the new era? We use health movements as a means to analyze the socio-territorial dynamics of the Lao-Thai border area. Notre étude s'intéresse à ces questions, les flux de patients, appréhendés à l'échelle régionale et plus précisément au sein de l'espace frontalier lao-thaïlandais où apparaissent des dynamiques thérapeutiques transfrontalières : le recours aux soins des Laotiens dans des hôpitaux thaïlandais limitrophes. Cette pratique engagée par une partie de la population vivant dans les bordures frontalières accompagne l'ouverture récente du Laos et souligne l'écart de développement existant entre les deux pays. La discontinuité géographique entre le Laos et la Thaïlande se double d'une discontinuité sanitaire, propice à la mobilité des patients et facilitée en outre par la proximité et les liens historiques entretenus par les populations riveraines du Mékong. Que révèlent ces mobilités de patients sur le fonctionnement de l'espace lao-thaïlandais et sur les relations entre les deux pays ? Comment participent-elles à une recomposition du territoire transfrontalier ? En utilisant les recours aux soins transfrontaliers comme révélateur, notre objectif, en tant que géographe est d'analyser les processus socio-territoriaux en présence actuellement au sein de l'espace lao-thaïlandais. L'entrée par la santé constitue un point de vue original pour renouveler la réflexion sur la notion de frontière.

Aim: To define how mobile health can enhance the current offering of care in France. Method: Review of the international literature, performed on the data bases Medline, SUDOC, PASCAL, NHS Economic Evaluation Database, Google until February 2016 with the keywords: "m-­health", "mHealth", "mobile phone". Results: The study of the French and European legislation has defined the regulatory and legal framework for m-­health in France, some fuzzy must be elucidate. The analysis of medical apps has defined their interest and in the course of care, but also some dangerous aspects. There are many obstacles to the development of m-­health: regulatory (certification, labeling, protection of user data), economic (lack of a model defined, few studies on the impact to enable management by authorities), structural (complexity of our health systems and poor integration of the m- ‐health to them, fear of health's ubérisation, risk of digital divide), technological (lack of interoperability, low reliability sensors, lack of security). The study of global m-­health programs has offered development path in France. Conclusions: The m-­health will allow greaterprevention and improved quality of life, more efficient and sustainable health systems, more responsible patients. Identification of the promises and obstacles to development allows us to propose the essential components for the apps of the future and the necessary changes in the health system and the French legislation. ; Objectif : Définir comment la santé mobile peut améliorer l'offre de soin actuelle en France. Méthode : Revue de la littérature internationale, réalisée sur les bases de donnée Medline, SUDOC, PASCAL, NHS Economic Evaluation Database, Google, jusqu'en février 2016 avec les mots clés : « m‐santé », « mHealth », « mobile phone ». Résultats : L'étude de la législation française et européenne a permis de définir le cadre réglementaire et juridique de la m-­santé en France, certains flous restent néanmoins à éclaircir. L'analyse d'applis a défini leur intérêt médical et dans le parcours de soins, mais aussi certains aspects dangereux. Il existe de nombreux freins au développement de la m-­santé : réglementaires (certification, labélisation, protection des données des utilisateurs), économiques (absence d'un modèle défini, peu d'études sur l'impact pour permettre une prise en charge par les autorités), structuraux (complexité de nos systèmes de santé et mauvaise intégration de la m‐santé à ceux-­ci, crainte d'une ubérisation de la santé, risque de fracture numérique), technologiques (absence d'interopérabilité, faible fiabilité des capteurs, manque de sécurisation). L'étude des programmes mondiaux de m-­santé a permis d'ouvrir des pistes de développement en France. Conclusions : La m‐santé peut permettre une prévention accrue et une meilleure qualité de vie, des systèmes de santé plus efficients et durables, des patients plus responsables. L'identification de ses promesses et des freins à son développement permet de proposer des composants essentiels pour les applis du futur, ainsi que des évolutions nécessaires du système de santé et de la législation Française.

Cette thèse de doctorat est composée de trois chapitres distincts et indépendants qui ont pour objectif d'analyser certaines politiques de santé au Québec notamment les incitatifs financiers, les délais d'attente et les soins à domicile sur la santé des patients et l'utilisation de services hospitaliers. Le premier chapitre porte sur l'impact des incitatifs financiers sur la santé des patients, spécialement le cas des soins spécialisés. Il faut noter de prime abord que la demande de soins de santé a beaucoup augmenté au Québec ces dernières années pour certains types de chirurgies. Cette augmentation, dont les causes sont multiples notamment les changements démographiques, a entraîné des temps d'attente plus élevés que les temps médicalement requis pour ces chirurgies. Pour résorber ce problème, le gouvernement québécois a mis en place en 2004 le Programme d'Accès à la Chirurgie (PAC), afin d'inciter financièrement les hôpitaux à pratiquer plus de chirurgies pour lesquelles les délais d'attente étaient plus importants. Ce programme a permis de diminuer la durée moyenne d'attente pour ces chirurgies, mais a aussi entraîné une augmentation des dépenses liées à ces chirurgies. Ces dépenses accrues étaient également dûes à certaines lacunes du programme. Une réforme majeure, qui consistait essentiellement à la modification des incitatifs financiers, a eu lieu en avril 2011, afin que le financement vienne appuyer la bonne pratique et le choix du plateau technique le plus adéquat. Notre étude, constituant une première évaluation de cette réforme, a pour but d'analyser l'impact de cette réforme sur la santé des patients, notamment les durées de séjour à l'hôpital après l'opération chirurgicale et les réadmissions après la sortie de l'hôpital. Notre application porte sur les chirurgies thoraciques et cardiovasculaires. Les résultats montrent une diminution significative des durées de séjour après l'introduction de la réforme et un impact non significatif sur les réadmissions urgentes. Dans le premier chapitre, les différentes politiques mises en places ont été initialement motivées par des délais d'attente excessifs. Pourtant, peu d'études ont analysé les impacts des délais d'attente sur la santé des patients au Québec. Tel est l'objectif du deuxième chapitre de la thèse, analyser si un délai d'attente élevé est associé à un risque de détérioration de la santé des patients. Nous utilisons des modèles empiriques pour analyser cette problématique. Nous introduisons le différentiel de distance du domicile du patient à l'hôpital le plus proche relativement à l'hôpital le plus proche parmi les hôpitaux avec de faibles délais d'attente comme variable instrumentale pour tenir compte de l'endogénéité potentielle des temps d'attente. Dans ce chapitre, nous analysons les variables de santé en termes de probabilité de réadmission urgente et de durée de séjour excédentaire (durée de séjour supplémentaire après la durée de séjour maximal recommandée). Les résultats montrent que les longs temps d'attente augmentent la probabilité d'être réadmis en urgence pour les patients qui ont eu une chirurgie de l'arthrose du genou, une chirurgie thoracique ou cardiovasculaire, une neurochirurgie, ou une chirurgie pour une tumeur de l'utérus. Il n'y a pas d'effet significatif des temps d'attente sur la probabilité de réadmission pour les chirurgies de l'arthrose de la hanche et des tumeurs de la prostate. Les longs temps d'attente augmentent également la durée de séjour à l'hôpital et les coûts d'hospitalisation après une chirurgie de l'arthrose du genou ou de la hanche. Le vieillissement de la population québécoise entraîne plusieurs enjeux cruciaux pour les services de soins de santé notamment les soins à domicile pour les personnes âgées (OIIQ, 2017). Les soins à domicile sont composés de tous les soins de santé que les établissements publics ou privés offrent aux individus à leur domicile. Ces services sont souhaitables pour toute personne qui a besoin de soins pour maladies chroniques, de soins palliatifs, de soins de réadaptation, de soins de fin de vie, ou des soins pour perte d'autonomie liée au vieillissement. Les soins à domicile pourraient constituer une alternative sécuritaire, à moindres coûts, aux soins de santé dans les hôpitaux. Les soins à domicile contribuent donc au maintien à domicile des personnes en offrant des services paramédicaux des infirmières, des aide-soignantes, coordonnés avec ceux des autres intervenants à domicile comme les kinésithérapeutes, les aides-ménagères, les auxiliaires de vie. Nous analysons dans le troisième iv chapitre, dans quelles mesures les services hospitaliers et les soins à domicile pour les personnes âgées sont substituables. Nous analysons également l'impact des soins à domicile sur l'admission dans les centres d'hébergement de soins de longue durée (CHSLD). Notre analyse montre que l'augmentation des soins à domicile pour les personnes âgées réduit la probabilité d'admission et la durée de séjour en urgence. L'effet des soins à domicile est plus prononcé chez les personnes âgées avec des pertes d'autonomie plus légères. Les résultats montrent également qu'une augmentation des soins à domicile réduit la probabilité d'être admis en CHSLD. ; This doctoral thesis is composed of three separate and independent chapters that aim to analyze certain health policies in Quebec, including financial incentives, waiting times and home care on patients' health and the use of hospital services. The first chapter focuses on the impact of financial incentives on the health of patients in Quebec, especially the case of specialized care. It should be noted, first of all, that the demand for health care has increased significantly in Quebec in recent years for certain types of surgery. This increase, which has many causes, including demographic changes, resulted in higher waiting times than the medically required times for these surgeries. To solve this problem, the Quebec government implemented in 2004 the Access to Surgery Program (ASP) to financially encourage hospitals to perform more surgeries for which waiting times were longer. This program allowed to reduce the average waiting time for these surgeries, but has also led to an increase in expenses related to these surgeries. These increased expenditures were also due to some weaknesses in the program. A major reform, which consisted mainly of modifying the financial incentives, took place in April 2011, so that the funding would support good practice and the choice of the most appropriate technical platform. Our study, which is a first evaluation of this reform, aims to analyze the impact of this reform on the health of patients, including the length of stay in hospital after surgery and readmissions post-discharge. The results show a significant decrease in length of stay after the introduction of the reform and a non-significant impact on urgent readmissions post-discharge within 30 days. In the first chapter, the various policies were initially motivated by excessive waiting times. Yet, few studies have analyzed the impact of wait times on patient health in Quebec. This vi is the purpose of the second chapter, to analyze whether a high waiting time is associated with a risk of deterioration of the patients' health. We use empirical models to analyze this problem. We introduce the differential distance from the patients' home to the nearest hospital relative to the nearest hospital among hospitals with low wait times as an instrumental variable to account for the potential endogeneity of the waiting time. In this chapter, we analyze health variables in terms of the probability of urgent readmission post-discharge within 30 days and excess length of stay (additional length of stay after the maximum recommended length of stay). The results show that long waiting times increase the likelihood of emergency readmissions for patients who had a knee surgery, a thoracic or cardiovascular surgery, a neurosurgery or a surgery for a tumor of the uterus. There is no significant effect of waiting times on the probability of readmission for hip and prostate surgeries. Long waiting times also increase the hospital length of stay and costs of hospitalization for knee and hip surgeries. The aging of the Quebec population is leading to many crucial issues for health care services, particularly home care for the elderly (OIIQ, 2017). Home care is composed of all health care that public or private institutions provide to individuals in their homes. These services are desirable for anyone who needs care for chronic illnesses, palliative care, rehabilitation care, end-of-life care, or care for loss of autonomy linked to aging. Home care could be a safe, low-cost alternative to health care in hospital. Home care, thus contributes to people to stay at home and receive paramedical services from nurses and care assistants, coordinated with those received from other home care providers such as physiotherapists, housekeepers and carers. In the third chapter, we analyze the extent to which hospital services and home care for the elderly are substitutes. We also analyze the impact of home care on admission to long-term care facilities (LTCFs). Our analysis shows that increased home care for the elderly reduces the probability of admission and the length of stay in emergency. The effect of home care is greater for seniors with less disabilities. The results also show that an increase in home care reduces the probability of being admitted to LTCFs.

Despite substantial legislation, access to healthcare remains a threatened right. The poorest patients face complex administrative procedures, stigmatization from the administration and caregivers. Social inequalities in health have profound consequences. PASS (Health Care Access Permanence) welcome people without social or partial coverage and/or in precarious situations. Their mission is to promote the return to common law. Many studies show insertion between 60 and 65% in the wake of PASS care. The objective was to evaluate the percentage of patients leaving the Henri MONDOR hospital's dedicated medical PASS, once acquired rights, continuing their follow up with a single-handed general practitioner. Secondarily, we investigate in the various factors that can influence this insertion. The inclusion criteria were the absence of open social security rights at the first consultation, at least one social consultation and one medical consultation. The included patients were contacted by phone at 3 months. The primary outcome was a consultation with a general practitioner within 3 months. 138 patients were included in our study between April 2017 and October 2017: 114 patients were analyzed and 24 lost to follow-up. Among the analyzed patients, 78% had consulted a general practitioner. The mastery of the French language was significantly more represented in this population. Consequently, to this result, we plan to map the general practitioners speaking a language other than French in Ile-de-France and to identify the places to learn French for this population. ; Malgré une législation importante, l'accès aux soins reste un droit menacé. Health services have long been insulated from the process of European integration. In this article, however, we show that we are witnessing their re-configuration in an emerging EU health-care system. The article uncovers the structuring lines of this system by focusing on three interrelated EU-wide processes influencing the integration of national health-care systems into a larger whole. First, the privatisation of health-care services following the constraints of Maastricht economic convergence and the EU accession criteria; second, health-care worker and patient mobility arising from the free movement of workers and services within the European Single Market; and third, new EU laws and country-specific prescriptions on economic governance that the EU has been issuing following the 2008 financial crisis. After 25 years of existence, the Brazilian health-care system shows good results, mainly because our population's social condition has improved, mostly through the advent of currency stability and State organization and, also, through the transfer of income to the poverty-stricken populations. Nevertheless, despite having been created from a people-driven movement (the so-called sanitary movement) and having citizen participation as one of its tenets, the Brazilian health-care system does not allow for a direct contribution by the users who, in view of the system's serious structural problems, have been demanding further participation, which can be seen through the increase in number of lawsuits against the public and the private systems and against professionals and hospitals. To face up to this problem and make the participation of the citizen an effective one, we take this opportunity to introduce the French system to the Brazilian system, to encourage further exchange between these two countries and draw on the former for inspiration on the evolution of the latter, specifically with respect to some aspects of Kouchner Act dated March 4, 2002. ; Le système de santé brésilien bénéficie d'atouts indéniables depuis sa création il y a 25 ans et aussi en particulier depuis l'évolution de la condition sociale de la population à la suite de la stabilité monétaire, de l'organisation de l'État et de politiques sociales en faveur des plus démunis. To face up to this problem and make the participation of the citizen an effective one, we take this opportunity to introduce the French system to the Brazilian system, to encourage further exchange between these two countries and draw on the former for inspiration on the evolution of the latter, specifically with respect to some aspects of Kouchner Act dated March 4, 2002. ; Le système de santé brésilien bénéficie d'atouts indéniables depuis sa création il y a 25 ans et aussi en particulier depuis l'évolution de la condition sociale de la population à la suite de la stabilité monétaire, de l'organisation de l'État et de politiques sociales en faveur des plus démunis. Cependant, la participation directe des usagers dans le système reste limitée, malgré l'influence du mouvement pour la santé (dit « mouvement sanitariste ») et le fait que la « participation de la collectivité » ait été consacrée à l'article 198 de la Constitution de 1988 comme un principe déterminant du système. Or, la population souhaite participer activement pour résoudre des problèmes importants concernant la structure du système ; cette volonté peut être illustrée par le nombre important d'actions judiciaires intentées contre le système public ou privé, les professionnels et les hôpitaux. Pour résoudre ce problème et favoriser la participation des usagers, la solution serait de présenter le système brésilien aux Français, de développer des échanges internationaux d'informations (théoriques ou pratiques) et de modifier le système en transplantant au Brésil certaines règles de droit français, notamment des dispositions de la loi française du 4 mars 2002 (loi dite « Kouchner »).

New urban and rural areas are emerging with limited access to primary health care, while existing ones grow even bigger. Since 2008, the liberal primary care team (PCT) are financially supported by the public authorities. They hope this structure will attract and retain general practitioner in the aforementioned areas.This study aims to explore how the space and the territory contribute to the localisation of PCT and how these structures affect spaces and territories. Localisation and impact of PCT are studied at different level, using quantitative and qualitative approaches. Spatial analysis of PCT is based on several tools such as SIG and typology in synergy with field investigation in two French regions. Effects of PCT on general practitioner density are studied on a national scale comparing the evolution of this density in space with PCT and similar space without PCT.
