IGA-BEM for lifting flows
Combining Iso-Geometric analysis (IGA) with Boundary Element Methods (BEM) for inviscid lifting flows imposes a number of difficulties. Firstly, an IGABEM collocation scheme has to take into account the tangent-plane discontinuity occurring along the trailing edge (TE). More important, the scheme has to handle the non-linear Kutta condition, securing continuity of the normal velocity and pressure through the a-priori unknown wake, a force-free boundary surface emanating from TE. In this presentation we shall review the status of our work towards developing a pair of IGABEM collocation schemes for computing steady lifting flows around 2D and 3D bodies , e.g., hydrofoils, marine propellers; [1,2]. In the latter case, the ansatz functions are inherited from T-spline representations which are free from singularities occurring, e.g., at the tip of a propeller blade, when NURBS are used. ZusammenfassungWir berichten über eine 17-jährige Patientin, die sich mit seit ca. 2 Jahren bestehenden Hautveränderungen vorstellte. Histologisch konnte die Diagnose einer linearen IgA-Dermatose gesichert werden, die Antikörper zeigten sich jedoch negativ. Die Erkrankung tritt typischerweise in der Kindheit auf, aber auch ältere Patienten können betroffen sein. Die meisten Formen treten idiopathisch auf, aber auch Medikamente wie Vancomycin können ursächlich sein.Dapson stellt die etablierte Therapieform dar. Auffällig war bei unserer Patientin die deutliche und auch bilddokumentierte Verschlechterung während der Menstruation.

Lineare IgA-Dermatose − interessanter Fall einer seltenen Erkrankung

ZusammenfassungBullöse Autoimmundermatosen sind seltene, aber wichtige Hauterkrankungen mit relativ hoher Morbidität und Mortalität. Bei der Unterteilung dieser Hauterkrankungen ist es von Bedeutung, ob eine subepidermale oder intraepidermale Blasenbildung besteht. Subepidermale, autoimmune, bullöse Erkrankungen der Haut und der Schleimhäute umfassen eine große Gruppe chronischer Erkrankungen. Diese beinhaltet u. a. bullöses Pemphigoid, Pemphigoid gestationis, Schleimhautpemphigoid, lineare IgA-Dermatose, Anti-p200-Pemphigoid und Epidermolysis bullosa acquisita. Bei diesen Erkrankungen werden Auto-Antikörper gegen strukturelle Bestandteile der Desmosomen, Hemidesmosomen und der Basalmembranzone gebildet, wodurch sich die Blasen bilden. Die lineare IgA-Dermatose ist eine sehr seltene, v. a. bei Kindern und jungen Erwachsenen auftretende Hauterkrankung. Sie ist durch lineare Ablagerung von IgA entlang der Basalmembran und dadurch bedingte Störung der dermo-epidermalen Verbindung gekennzeichnet. Die Erkrankung kann sowohl medikamentös als auch idiopathisch auftreten. Hier berichten wir über den Fall eines 56-jährigen Patienten mit seit ca. 2 Monaten bestehenden, stark juckenden, prallen Blasen am Rumpf und an den Extremitäten, die mit klarer Flüssigkeit gefüllt waren. Aufgrund mangelhafter Compliance erfolgte ambulant noch keine Behandlung. Histologisch wurde eine subepidermal autoimmun-blasenbildende Dermatose beschrieben. Die genaue Diagnose konnte nur dank der indirekten Immunfluoreszenz-Untersuchung gestellt werden.
Long-Term Morbidity Following IGA Vasculitis in Childhood

Background/ObjectivePatients with IgA vasculitis (IgAV aka Henoch Schonlein) may require aggressive initial treatment, are prone to disease relapses and may have sustained abnormal IgA responsiveness. We investigated whether the relatively rare diagnosis of childhood IgAV is associated with subsequent comorbidity, hospitalisation and interventions.
MethodsUsing linked administrative health data from the Western Australian Rheumatic Disease Registry we performed an observational cohort study comparing hospitalisation and ED visit rates(per 1000 person years), comorbidity accrual (Charlson comorbidity index; CCI) and procedures between 494 IgAV patients < 20 years and 1385 matched controls over a twenty-year period.
ResultsHospitalisation was proportionally (73.5 vs 51.5%) and by rate (21.7 vs 18.9; rate ratio 1.15, p<0.01) increased in IgAV patients. IgAV patients more often underwent diagnostic and medical procedures than controls, who had higher rates of surgical interventions. ED visit frequency (25 vs 16%) and rate (10.8 vs 8.43, rate ratio 1.29, (p<0.01) was also increased in IgAV patients, who more often accrued peptic ulcer and renal disease than controls and had an increased accrual of CCI ≥3.
Conclusions/ImplicationsA diagnosis of IgAV in childhood significantly increases the risk and rate of subsequent hospital admission, ED visits and accrual of severe comorbidity. Although often considered a self-limiting disease, the occurrence of childhood IgAV signifies the presence of a sustained predisposition to illness, likely related to abnormal IgA functioning.
