The Colombian reform of health system, adopted by Law 100 of 1993, constitutes a public policy with great pretensions and significant institutional and financial changes. The main goal refers to universal insurance coverage, financed with public resources, with a state direction and an administration entrusted to health promotion companies, who are responsible for guaranteeing access to the health benefits plan through the institutions that provide services. From this baseline, a look at the main regulatory changes is developed to answer the question of how much the system adopted by Law 100 has changed. As far as insurance is concerned, the system remains intact and has rather been strengthened, seeking universal coverage and improvements in access. The main changes have been in financing. After a cycle with low government contributions, since 2010 there have been greater fiscal resources and a lower share of contributions. On the institutional side, some improvements are presented, such as the creation of the entity that administers the resources of the system and the Institute for Technological Evaluation in Health, as well as the strengthening of price regulation and pharmaceutical policy, but the disappearance of the national council and the regulatory commission is a loss, which means a weakening of governance. ; La reforma colombiana al sistema de salud, adoptada mediante la Ley 100 de 1993, constituye una política pública con grandes pretensiones y cambios institucionales y financieros significativos. La participación en Colombia es un tema que ha venido generando diversas opiniones entre los analistas expertos en el tema, definiéndola unos como un "proceso social que resulta de la acción intencionada de individuos y grupos en busca de metas específicas, en función de intereses diversos y en el contexto de tramas concretas de relaciones sociales y de poder. Es, en suma, un proceso en el que distintas fuerzas sociales, en función de sus respectivos intereses (de clase, de género, de generación), intervienen directamente o por medio de sus representantes en la marcha de la vida colectiva con el fin de mantener, reformar o transformar los sistemas vigentes de organización social y política" para otros la definen como un derecho "toda persona tiene derecho a participar en el gobierno de su país, directamente o por medio de representantes libremente escogidos. It is, in short, a process in which different social forces, depending on their respective interests (class, gender, generation), intervene directly or through their representatives in the march of collective life in order to maintain, reform or transform the current systems of social and political organization "for others define it as a right" everyone has the right to participate in the government of their country, directly or through freely chosen representatives. ABSTRACT: Unemployment has been one of the main factors that have obstructed the achievement of universal health insurance in Colombia, just as Law 100 of 1993 proposed to reach. Those workers that had a history of labour life and lost their posts lack health insurance in a large percentage as well as their close relatives. In this way, they remain exposed to lose the family economical resources if any of the members requires health services he/she should pay for. Objective: the primary objective of this research was to establish the socio-economic characteristics of this population, its level of health insurance and the duration of the employment-unemployment cycle in the country. The results are to be used in a second phase that pretends to design and validate an intervention strategy to guarantee the maintenance of the health insurance of this population. Methods: it was a descriptive-retrospective study that used quantitative analysis techniques. Data was taken from official records of Colombian government, as well as from previous studies related with the matter. Results: principal finds show a persistent growing of participation of workers who lost their posts as part of the total unemployed population in the country. A high proportion of them (56%) have a medium educational level and a high percentage (57.8%) belongs to the medium socio-economic stratum. It is remarkable too that the mean duration of unemployment grew from 27.5 weeks in 1992 to 58.2 weeks in 2002. ; RESUMEN: Uno de los obstáculos que ha tenido Colombia para lograr el objetivo de la Ley 100 de 1993 de universalizar el aseguramiento en salud ha sido el alto nivel de desempleo durante toda la década trascurrida desde entonces. Los trabajadores cesantes (aquellos que han tenido historia de vinculación laboral y que han perdido su empleo) constituyen con sus familias una población que en alto porcentaje carece de aseguramiento en salud, con lo cual queda expuesta al riesgo de pérdida del patrimonio familiar cuando alguno de sus integrantes se vea precisado a demandar atención que debe financiarse con los propios recursos. Objetivo: esta investigación se propuso precisar las características socioeconómicas y del aseguramiento en salud de los trabajadores cesantes, así como los ritmos temporales de empleo-desempleo en el país, buscando que sus resultados sirvan de base para el desarrollo de una segunda fase que diseñará y validará una estrategia de intervención para garantizar la sostenibilidad de su aseguramiento. Metodología: se trató de una investigación descriptiva-retrospectiva que utilizó técnicas de análisis cuantitativo. La información se obtuvo de bases de datos oficiales del gobierno colombiano y de estudios previos que tienen relación con el tema. Resultados: los resultados muestran una creciente participación de los trabajadores cesantes dentro del conjunto de la población desempleada del país, destacándose que una alta proporción de ellos (56,0%) han accedido a la educación media y que un alto porcentaje (58,7%) pertenecen al estrato socioeconómico medio. También resalta que la duración media del tiempo de cesantía pasó de 27,5 semanas en 1992 a 58,2 semanas en el año 2002.
