Medical artificial intelligence (AI) systems have been remarkably successful, even outperforming human performance at certain tasks. There is no doubt that AI is important to improve human health in many ways and will disrupt various medical workflows in the future. Using AI to solve problems in medicine beyond the lab, in routine environments, we need to do more than to just improve the performance of existing AI methods. Robust AI solutions must be able to cope with imprecision, missing and incorrect information, and explain both the result and the process of how it was obtained to a medical expert. Using conceptual knowledge as a guiding model of reality can help to develop more robust, explainable, and less biased machine learning models that can ideally learn from less data. Achieving these goals will require an orchestrated effort that combines three complementary Frontier Research Areas: (1) Complex Networks and their Inference, (2) Graph causal models and counterfactuals, and (3) Verification and Explainability methods. The goal of this paper is to describe these three areas from a unified view and to motivate how information fusion in a comprehensive and integrative manner can not only help bring these three areas together, but also have a transformative role by bridging the gap between research and practical applications in the context of future trustworthy medical AI. In September 2015, the Volkswagen Group (VW) admitted the use of 'defeat devices' designed to lower emissions measured during VW vehicle testing for regulatory purposes. Globally, 11 million cars sold between 2008 and 2015 are affected, including about 2.6 million in Germany. On-road emissions tests have yielded mean on-road NO[subscript x] emissions for these cars of 0.85 g km[superscript −1], over four times the applicable European limit of 0.18 g km[superscript −1]. This study estimates the human health impacts and costs associated with excess emissions from VW cars driven in Germany. A distribution of on-road emissions factors is derived from existing measurements and combined with sales data and a vehicle fleet model to estimate total excess NO[subscript x] emissions. These emissions are distributed on a 25 by 28 km grid covering Europe, using the German Federal Environmental Protection Agency's (UBA) estimate of the spatial distribution of NO[subscript x] emissions from passenger cars in Germany. We use the GEOS-Chem chemistry-transport model to predict the corresponding increase in population exposure to fine particulate matter and ozone in the European Union, Switzerland, and Norway, and a set of concentration-response functions to estimate mortality outcomes in terms of early deaths and of life-years lost. Integrated over the sales period (2008–2015), we estimate median mortality impacts from VW excess emissions in Germany to be 1200 premature deaths in Europe, corresponding to 13 000 life-years lost and 1.9 billion EUR in costs associated with life-years lost. Approximately 60% of mortality costs occur outside Germany. For the current fleet, we estimate that if on-road emissions for all affected VW vehicles in Germany are reduced to the applicable European emission standard by the end of 2017, this would avert 29 000 life-years lost and 4.1 billion 2015 EUR in health costs (median estimates) relative to a counterfactual case with no recall. ; Germany. Citrinin (CIT) is a mycotoxin responsible for the contamination of many agricultural products, like wheat, barley, corn, rice and their products, as also other foodstuffs and feedstuffs used in human and animal nutrition. It is essentially produced by Penicillium citrinum, although it can also be biosynthesised from Penicillium expansum and Penicillium verrucosum and some species of Aspergillus and Monascus. However, several studies have shown that CIT is known for its genotoxic, hepatotoxic, fetotoxic and teratogenic properties. The aim of this study is to investigate the occurrence of CIT in wheat grain cultivated in Kosovo and Albania. Given the fact that wheat flour is the most consumed product in Kosovo and Albania, it is necessary to analyse the CIT in wheat in these two countries. In total, 60 wheat samples were tested from Fusha e Kosovës (Kosovo), Myzeqeja (Albania) and Fusha e Maliqit (Albania), as places with the highest wheat production. The enzyme- linked immunosorbent assay (ELISA) method was used to determine CIT concentrations. To identify moulds representing potential producers of CIT, traditional macroscopic and microscopic methods and the molecular PCR method of identification were implemented. CIT was detected in 96.6% and 86.6% of wheat grain samples collected in Kosovo and Albania, respectively. The maximum amount of CIT detected in wheat grain was 53.12 μg/ kg in Kosovo, and 45.74 μg/kg in Albania. The amount of CIT found in wheat grain is not comparable with the maximal limits (MLs), as the European legislation does not provide limits for this mycotoxin. Fifty-five fish samples belonging to three highly commercial species, European sardine (Sardina pilchardus), European anchovy (Engraulis encrasicolus), and European hake (Merluccius merluccius), were taken from the Western Mediterranean Sea. OPEs were detected in all individuals, except for two hake samples, with concentrations between 0.38 and 73.4 ng/g wet weight (ww). Sardines presented the highest mean value with 20.5 ± 20.1 ng/g ww, followed by anchovies with 14.1 ± 8.91 ng/g ww and hake with 2.48 ± 1.76 ng/g ww. The lowest OPE concentrations found in hake, which is a partial predator of anchovy and sardine, and the higher δ15N values (as a proxy of trophic position), may indicate the absence of OPEs biomagnification. Eleven out of thirteen tested OPEs compounds were detected, being diphenyl cresyl phosphate (DCP) one of the most frequently detected in all the species. The highest concentration values were obtained for tris(1,3-dichloro-2-propyl) phosphate (TDClPP), trihexyl phosphate (THP), and tris(2-butoxyethyl) phosphate (TBOEP), for sardines, anchovies, and hakes, respectively. Insufficiency of water resources in the Middle East Region represents vital factors that influence the stability of the region and its progress. Expectations indicate that the condition will be dimmer and more complicated, especially in Iraqi territory. Iraq, which is situated in the Middle East, it covers an area of 433,970 square kilometers and populated by about 32 million inhabitants. Iraq greatly relies in its water resources on the Tigris and Euphrates Rivers as a surface water resources, and several productive groundwater aquifers in which from the hydrogeological point of view divided into several major aquifer units including Foothill, Al-Jazira, Aquifer System, Mandali-Badra-Teeb, Mesopotamian and Desert Aquifer system. Recently, Iraq is suffering from water shortage problems. This is due to external and internal factors affecting the water quality of water resources; they are controlled and uncontrolled factors. The uncontrolled factors are climate change and its consequences, such as reduction of precipitation and temperature increasing. The controlled factors have a significantly negative influence on water resources, but their effects involve more specific regions. The controlled factors are mainly represented by building dams and irrigation projects within the upper parts of the Tigris and Euphrates catchments, Al-Tharthar Scheme, waste water, solid wastes and wastes from wars, which has a significant effect on surface water in Iraq because about 80% of the water supply to Euphrates and Tigris Rivers come from Turkey. In addition, the pressures resulting from the high demand for water resources, and the continued decline in their quantity rates have led to major changes in the hydrological condition in Iraq during the past 30 years. The decrease in surface water levels and precipitation during these three decades reflects the drop in the levels of water reservoirs, lakes, and rivers to the unexpected levels. The level of main country's water source, Tigris, and Euphrates Rivers has fallen to less than a third of its natural levels. As storage capacity depreciates, the government estimates that its water reserves have been reduced precariously. According to the survey from the Ministry of Water Resources, millions of Iraqi people have faced a severe shortage of drinking water. Since of the importance of water for human life and the need to monitor temporal and spatial changes in quality and quantity, there is a need to develop a general Iraqi Water Quality Index (Iraq WQI) to monitor surface water and groundwater and classify it into five categories, very good, good, acceptable, bad and very bad, in terms of suitability for domestics, irrigation and agriculture depending on the Iraqi and WHO standards for drinking water. In addition, strict establishment for the regular quantitative monitoring surface water and groundwater setting and processes. Prospects are more negative for all riparian countries. This implies that solving these problems requires actual and serious international, regional, and national cooperation to set a prudent plan for water resources management of the two basins. The present research aims to analyse the phenomenon of unaccompanied minors arriving from Morocco to the European Union from 2010 to 2021. This recent migratory phenomenon supposes an emerging challenge for governments, international organisations, and societies, in order to guarantee the protection of children according the UN Convention on the Rights of the Child (UN, 1989). The research focuses on the children «on the move», travelling alone from Morocco and arriving in Spain through the Strait of Gibraltar. The so-called Western Mediterranean Migratory Route has become one of the gateways to Europe. Spain hosts nowadays a big community of Moroccan unaccompanied minors and Catalonia has become the second Spanish Autonomous Community with a major number of unaccompanied minors, only surpassed by the enclave of Melilla located on the very border of Morocco. The present research gives special attention to the study of the Amazigh Nationalism question, as something to look at among the reasons why Moroccan unaccompanied minors migrate to Europe, choosing, particularly, Catalonia as a model to pursuit an Amazigh nation. ; A presente pesquisa tem como objetivo analisar o fenómeno da chegada de menores não acompanhados provenientes de Marrocos à União Europeia entre 2010 a 2021. Research indicates that sources of work-related stress (WRS) impact on the physical, social, and psychological health of pilots. Furthermore, specific features of the job can increase a pilot?s risk in relation to developing a mental health (MH) issue. It is impossible to remove all stress from the work life of pilots. A high stress situation may not necessarily be det-rimental to the person, once they have learned to cope with it in a healthy manner. Nonetheless, risk pertaining to WRS need to be effectively managed by a pilot?s employer. Therefore, it is important to identify solutions at an airline and pilot self-management level. This paper reports on the findings of human factors research undertaken with commercial pilots pertaining to work-related stress (WRS) and its impact on wellbeing, performance, and safety. The findings of a series of co-design workshops and a follow-up anonymous survey were analysed to identify potential solutions at (1) an airline and (2) pilot self-management level. Potential solutions are framed in relation to six impact scenarios. Furthermore, they are located within the existing regulatory framework, including the latest implementation rules (IR), acceptable means of compliance (ACM), and guidance material (GM) as outlined by the European Union Aviation Safety Agency (EASA 2019). Proposed interventions should promote wellbeing and positive mental health while also addressing suffering and mental ill health. Airline interventions might focus on enhancing existing Safety Management System (SMS) approaches to better manage risks pertaining to WRS, advancing new tools to enable wellbeing briefing, risk assessment, and reporting, and training pilots in relation to MH awareness, risk identifying behaviour, and coping strategies. Furthermore, new role/functions might be introduced to support the implementation and management of WRS/wellbeing/MH safety/risk processes at an airline level. Requirements for new digital tools to support pilot awareness of WRS/wellbeing/MH, self-management of WRS/wellbeing/MH and risk identification both inside and outside the cockpit are also proposed. Some of recommendations arising in this research require changes to the existing rule-making and/or modification to existing AMC and GM.
The Gnassingbé clan has ruled the country since 1967. The demand for political alternance, initiated by institutional and electoral reforms, constituted the major contentious issue between the government and the challengers of the Gnassingbé regime throughout the survey period. The legislative elections of 20 December 2018, boycotted by the major opposition parties, resulted in an easy victory of the ruling party. The first local elections since more than 30 years took finally place on 30 June 2019 and resulted in the victory of the ruling party. Shortly afterwards, in February 2020, the President won also the disputed presidential elections and even consolidated his power, assisted by the loyal army and security services. The outbreak of the Corona epidemic in Togo in April and the subsequent economic recession may have contributed to limit popular protest against the Gnassingbé regime. The human rights record of the government has improved but remains poor. Despite undeniable improvements to the framework and appearance of the regime's key institutions during the review period, democracy remains far from complete. However, the international community, notably Togo's African peers, the AU and ECOWAS, followed a 'laissez-faire' approach in the interests of regional stability and their national interests in dealing with Togo. Economic growth remained stable at about 5% per annum (before Corona). Public investment in infrastructure and increases in agricultural productivity, notably of export crops, had been the key drivers of economic growth. However, growth remains vulnerable to external shocks and the climate and has not been inclusive. Positive growth was overshadowed by increasing inter-personal and regional inequality as well as an increase in extreme poverty. Moreover, money-laundering, illegal money transfers and trafficking grew alarmingly. Cependant, la communauté internationale, notamment les pairs africains du Togo, l'UA et la CEDEAO, ont suivi une approche de «laissez-faire» dans l'intérêt de la stabilité régionale et de leurs intérêts nationaux face au Togo. La croissance économique est restée stable à environ 5 % par an (avant Corona). Les investissements publics dans les infrastructures et les augmentations de la productivité agricole, notamment des cultures d'exportation, ont été les principaux moteurs de la croissance économique. Cependant, la croissance reste vulnérable aux chocs externes et au climat, et le développement n'a pas été inclusive. La croissance positive a été éclipsée par l'augmentation des inégalités interpersonnelles et régionales ainsi que par une augmentation de l'extrême pauvreté. En outre, le blanchiment d'argent, les transferts d'argent illégaux et le trafic ont augmenté de façon alarmante. Öffentliche Investitionen in die Infrastruktur und die Steigerung der landwirtschaftlichen Produktivität, insbesondere in Exportkulturen, waren die Haupttreiber des Wirtschaftswachstums. Das Wachstum bleibt jedoch anfällig für externe Schocks und das Klima und war nicht inklusiv. Das positive Wachstum wurde durch die zunehmende zwischenmenschliche und regionale Ungleichheit sowie die Zunahme der extremen Armut überschattet. Darüber hinaus nahmen Geldwäsche, illegale Geldtransfers und Menschenhandel alarmierend zu. Das Geschäftsklima hat sich dennoch erheblich verbessert.
Palestiinalaispakolaiset ovat eläneet pakolaisleireillä jo seitsemän vuosikymmentä. Heidän pakolaisuutensa juontuu vuodesta 1948, jolloin Israelin valtion perustamisen yhteydessä enemmistö alueen palestiinalaisesta väestöstä joutui pakenemaan kotiseuduiltaan. Vuosikymmenten aikana pakolaisleirit ovat tulleet osaksi Lähi-idän maisemaa, ja niillä on kasvanut ja elänyt jo useita palestiinalaissukupolvia. Leirien pitkästä historiasta huolimatta niitä on kuitenkin pidetty väliaikaisina; määritelmällisesti leiri on aina olemassa vain rajatun ajanjakson ja palestiinalaisten kansallisissa narratiiveissa, jotka painottavat paluuta Palestiinaan, niitä ei ole ajateltu pysyvinä asuinpaikkoina. Käytännössä leirit kuitenkin muodostavat olosuhteet, joissa palestiinalaispakolaiset elävät arkeaan ja rakentavat tulevaisuuttaan. Pitkän historiansa, ainutlaatuisen institutionaalisen viitekehyksen sekä palestiinalaisten poliittiseen kamppailuun linkittyvien merkitysten takia leirit luovat omanlaisensa puitteet niillä asuvien pakolaisten elämälle. Tässä tutkimuksessa en silti tarkastele vain leirejä itsessään vaan lähestyn niiden kautta sitä, miten niillä asuvat palestiinalaiset neuvottelevat tulevaisuudentoiveitaan. Tämä etnografiaan pohjaava tutkimus on toteutettu Libanonissa, Jordaniassa ja Länsirannalla usealla eri pakolaisleirillä. Valitsin monikenttäisen lähestymistavan sillä lähtöoletuksella, että pakolaisuuden konteksti vaikuttaa merkittävästi siihen, miten tulevaisuuksia kuvitellaan. Palestiinalaispakolaisten asemamassa onkin merkittäviä eroja näillä kolmella alueella: Libanonissa he elävät ilman kansalaisuutta ja sen tuomia oikeuksia, Jordaniassa enemmistö pakolaisista on samanaikaisesti sekä maan kansalaisia että pakolaisia ja Länsirannalla palestiinalaispakolaiset joutuvat muiden länsirantalaisten tavoin kohtaamaan Israelin väkivaltaisen miehityspolitiikan. Näiden kontekstuaalisten erojen ja niiden vaikutusten ymmärtäminen on ollut tutkimuksen keskiössä. Tutkimuksen lähtöoletuksena on lisäksi ontologisen hermeneutiikan mukaisesti se, että tulevaisuuden tarkastelu ei ole mahdollista ilman, että ymmärtää menneen, nykyisyyden ja näiden välisen suhteen. Tästä syystä käyn läpi eri kentillä asuvien palestiinalaisten historiaa ja etenen leirien nykyolosuhteiden tarkasteluun. Nämä ajallisuudet luovat pohjan pakolaisten tulevaisuudentoiveiden ymmärtämiselle. Palestiinalaispakolaisten historia on täynnä väkivaltaa, pakenemista ja jatkuvaa epävarmuutta, ja pakolaisleirit ovat tilana tiivistäneet pakolaisuuteen liittyvät vaikeudet niin historiallisesti kuin nykyäänkin. Materiaaliset olosuhteet, ahtaus, suhde ympäröivään paikkaan ja yhteisöön sekä leirin identiteetilliset ja poliittiset konnotaatiot ovat kaikki vaikuttaneet siihen, miten tähän tutkimukseen osallistuneet palestiinalaispakolaiset kokivat elinolosuhteensa. Leirien ei koettu pystyvän tarjoamaan toivottua elintasoa, mutta samanaikaisesti ne muodostivat merkityksellisen yhteisön, johon liitettiin useita positiivisia ominaisuuksia. Leirit muodostivat myös osan pakolaisten poliittista identiteettiä: ne muistuttivat niillä asuvien pakolaisuudesta ja muodostivat linkin Palestiinaan ja siten ylläpitivät vaatimusta paluuoikeudesta. Kuitenkin, näistä positiivisista puolista huolimatta, elettyinä tiloina leirit eivät pystyneet luomaan toivottuja puitteita arjelle. Tässä tutkimuksessa painottuu myös se, että Libanonissa, Jordaniassa ja Länsirannalla tarjolla olleet mahdollisuudet eivät useinkaan riittäneet toivotun tulevaisuuden saavuttamiseen. Maastamuutto olikin monelle tapa kuvitella parempaa tulevaisuutta niin itselle kuin perheelle, erityisesti Libanonissa ja Länsirannalla. Parempaa tulevaisuutta neuvoteltiin useita eri reittejä, ja tässä tutkimuksessa keskityn erityisesti koulutuksen, työn ja naimisiinmenon tarjoamiin mahdollisuuksiin. Vaikka myös kansalliset tulevaisuudet, Palestiina ja paluuoikeuden toteutuminen nousivat esiin näissä neuvotteluissa, ilmenivät ne kuitenkin abstraktimmalla tasolla ja eri aikajanalla, kuin tulevaisuudet, joilla haettiin parempia mahdollisuuksia saavuttaa henkilökohtaiset elämäntavoitteet ja helpotusta arjessa kohdattuihin vaikeuksiin. Palestiinalaispakolaisten tulevaisuuskäsitysten erityisyys on juuri henkilökohtaisen ja kansallisen tulevaisuuden välille muodostuvassa ristivedossa. Kansallisen narratiivin mukaan pakolaisten (erityisesti leireillä asuvien) oletetaan sisukkaasti odottavan paluuta, kestävän pakolaisuuden haasteet ja taistelevan kansallisten tavoitteiden puolesta. Samanaikaisesti pakolaiset kuitenkin pyrkivät parantamaan arkeaan, elinolosuhteitaan ja mahdollisuuksiaan tarjolla olevin keinoin. Etnografiassani painottui erityisesti tämä henkilökohtainen puoli ja se, että kansallisten tavoitteiden saavuttamisen ei koettu pystyvän tarjoamaan parannusta pakolaisuuden historian muovaamiin nykyolosuhteisiin riittävän nopealla aikataululla. Väitänkin, että henkilökohtaisten tulevaisuuksien ensisijaisuus heijastaa leireillä asuvien palestiinalaisten kohtaamia vaikeuksia: heidän asemansa ja tulevaisuutensa ei ole tarpeeksi turvattu, jotta he voisivat odottaa ja keskittyä kansallisten tavoitteiden ajamiseen. Sen sijaan palestiinalaispakolaiset ovat pakotettuja tarttumaan tällä hetkellä tarjolla oleviin vaihtoehtoihin ja pyrkimään niihin suuntiin, joissa mahdollisuudet parempaan elämään ovat tarjolla. ; Palestinian refugee camps have been housing Palestinians for seven decades. They were formed after 1948, when in the process of establishing Israel as a Jewish state the majority of Palestinians living in Mandatory Palestine were dispossessed. Over these decades, the camps have become a durable part of the Middle Eastern landscape, and several generations of refugees have already been born, and are living, in them. Despite the fact that Palestinian camps have also been framed as temporary, not only by definition but also in the national narratives stressing the right to return to Palestine, in practice the camps have become sites where lives are lived and futures built. Due to their long history, unique institutional framework, and strong connotation as centers of Palestinian political struggle, Palestinian refugee camps have their own specificities that affect how they frame the life of their dwellers. Yet, this dissertation is not an ethnography of the camps per se; the aim is rather to look through them in order to explore the manifold futures that are negotiated by the Palestinian refugees dwelling in them, and especially by those just reaching adulthood. This ethnographic study has been conducted in several refugee camps in Jordan, Lebanon, and the West Bank. A multi-sited approach was adopted with the presumption that the context of refugeeness affects the ways in which futures are imagined. Palestinian refugees have been positioned differently in these places: as stateless and excluded in Lebanon, as refugee-citizens in Jordan, and as being among their own people yet enduring the difficulties of the oppressive Israeli occupation in the West Bank. Understanding these contextual specificities in the different host sovereigns has hence been central to the aims of this work. In this dissertation I argue, in line with ontological hermeneutics, that it is not possible to explore the future without first addressing the differing pasts and present conditions of the refugees. Therefore, to understand these differing experiences of Palestinian refugees living under the three host sovereigns, I start by looking at the histories that form the present realities through which Palestinians look to, have hopes for, and plan for their futures. The histories of Palestinian refugeeness have witnessed violence, multiple displacements, and enduring uncertainty. In all the fields that were included in this research it was the camp that embodied the difficulties of being a refugee. The material living conditions, crowdedness, camp community, and relations with the surrounding area and society, as well as the identificational and political connotations, have all affected how the camps have been experienced as places of dwelling by my interlocutors. In many ways they were considered inadequate in terms of providing the hoped-for standards of living, yet simultaneously they formed a significant community that was viewed from a positive perspective, though more so in the West Bank and Jordan than in Lebanon. Nevertheless, while the camps functioned as a marker of Palestinian refugeeness, and were even equated with Palestine and the right of return, as everyday spatialities they left much to be desired. Similarly, this research shows that the possibilities available to the refugees under the host sovereigns were not usually enough for my interlocutors to achieve their aspirations. Hence many – especially in Lebanon and the West Bank, though for different reasons – have come to consider that emigration could fulfil their hopes and provide better conditions for them and their families. Better futures were negotiated via different routes – education, employment, and marriage being the ones explored here. While both return and the national future of Palestine also emerged in these negotiations, they were situated in a different, more abstract timeframe, not one that could provide better possibilities or enhancements in the deprived conditions faced in everyday life. The specificity of Palestinian refugees' negotiations on the future nevertheless emerges from this discrepancy between the national and the personal. On the level of national discourse, Palestinian refugees (especially those living in camps) are people steadfastly waiting for the return, enduring life in the camp and fighting for the national cause. However, on the level of the everyday, they are – as is anyone – aiming to improve their situation with the means available and, as became evident in my ethnography, in the present situation and in light of past experiences those means direct their attention somewhere other than to the political objectives they hold as Palestinian refugees. This, I claim, reflects the difficulties that Palestinian refugees living in the camps face: they are not in a secure enough position to have the luxury of "waiting out" or to concentrate on the political in its national manifestation. Rather, they are forced to negotiate the options at hand, to reach out in those directions where possibilities are available.
Background Humans are regularly exposed to metals and metalloids present in air, water, food, soil and domestic materials. Most of them can cross the placental barrier and cause adverse impacts on the developing foetus. Objectives To describe the prenatal concentrations of metals and metalloids and to study the associated sociodemographic, environmental and dietary factors in pregnant Spanish women. Methods Subjects were 1346 pregnant women of the INMA Project, for whom the following metals arsenic (As), cadmium (Cd), cobalt (Co), copper (Cu), molybdenum (Mo), nickel (Ni), lead (Pb), antimony (Sb), selenium (Se), thallium (Tl) and zinc (Zn) were determined in urine, at both the first and the third trimesters of gestation. Sociodemographic, dietary and environmental information was collected through questionnaires during pregnancy. Multiple linear mixed models were built in order to study the association between each metal and metalloid concentrations and the sociodemographic, environmental and dietary factors. Results The most detected compounds were As, Co, Mo, Sb, Se and Zn at both trimesters. Zn was the element found in the highest concentrations at both trimesters and Tl was detected in the lowest concentrations. We observed significant associations between As, Cd, Cu, Sb, Tl and Zn concentrations and working situation, social class and age. Seafood, meat, fruits, nuts, vegetables and alcohol intake affected the levels of all the metals but Cd and Cu. Proximity to industrial areas, fields and air pollution were related to all metals except Cd, Sb and Se. Conclusions This is the first large prospective longitudinal study on the exposure to metals and metalloids during pregnancy and associated factors to include several cohorts in Spain.
