Des Autorités de santé dans chacune de ces deux régions sont chargées de délivrer aux laboratoires pharmaceutiques cette autorisation après s'être assurées de la qualité, de la sécurité et de l'efficacité du nouveau médicament proposé.Elles conservent cette obligation de surveillance du produit après l'obtention par le laboratoire de l'Autorisation de Mise sur le Marché et durant toute la vie du médicament.Mais qui sont ces Autorités de santé en Europe et aux Etats-Unis ? Comment un médicament est-il enregistré dans ces deux zones ? Comment la maintenance de l'Autorisation de Mise sur le Marché du médicament y-est-elle réalisée ? Y-a-t-il des différences et des similitudes entre l'Europe et les Etats-Unis ? Ce travail a pour but de répondre à ces différentes questions.La première partie de cette thèse présente les Autorités de santé américaines et européennes, en évoquant l'historique de leur formation liée à l'évolution de la réglementation des médicaments. La deuxième partie est consacrée à l'étude de l'enregistrement d'un nouveau médicament. Enfin, la troisième partie est axée sur lamaintenance de l'AMM dans les deux zones.

La médecine humanitaire intéresse tout soignant mais demeure en général un concept flou, de par l'absence de définition réelle, et l'hétérogénéité de sa pratique qui n'en fait pas une discipline au sens propre. En effectuant une synthèse de la littérature écrite par des humanitaires, nous pouvons mieux comprendre les nombreux enjeux propres à cette pratique, et les difficultés générales qui entravent la réalisation des actions humanitaires. Ainsi nousprendrons conscience de l'influence historique de l'Occident et l'impunité du Bien qui auréolent l'humanitaire, et rendent plus ardue la tâche d'effectuer une critique objective des missions. Aussi, des enjeux financiers, politiques, idéologiques, de santé publique interfèrent avec les objectifs humanitaires d'une Organisation Non Gouvernementale, et accroissent les risques de détournement de l'aide.Les réflexions et dilemmes qui ont accompagné les missions antérieures dans l'histoire de l'humanitaire participent à nourrir l'élaboration d'outils pour mieux penser dans ces situations toujours complexes.De plus, la médecine humanitaire met en lumière certains aspects de la médecine générale. Il est intéressant de confronter la médecine humanitaire et la médecine générale à travers leurs définitions, leur histoire, puis leurs pratiques. Nous constaterons que les situations de précarité en France s'apparentent à des terrains humanitaires ; puis nous verrons en quoi certains enseignements tirés de l'histoire de l'humanitaire sont applicables à la pratique de la médecine générale, notamment à propos de la relation médecin-patient, de la formed'engagement qu'implique toute médecine pour son patient, de l'influence hygiéniste sur la gestion de la santé publique, et des difficultés d'indépendance du médecin vis-à-vis des champs politiques et économiques.L'humanitaire remet au centre le pouvoir dangereux de la médecine à gouverner les vies, et place chaque médecin devant sa responsabilité de citoyen du monde.

This PhD intends to explore the encounter between an exceptional moment – the years 1914-1918 – and a social space – that of medicine. The thesis also aims to show how the particular way the medical field went into war made a reform of the care system possible for wounded and sick soldiers by suspending the ordinary logic of its social functioning. This inquiry sheds light on the role of mobilized civilians, specifically health professionals – young university hospital professors, hospital physicians, laboratory researchers – whose descent into war was an unhappy experience of idleness and maladjustment. As a part of these medical elites were treated as subalterns in a medical community where the bureaucratic logic came first, from the autumn of 1914 onwards they mobilised their political networks and called for a "global reform" of the sanitary organization. This reform aimed to reorder wartime medicine by putting everyone back where they belonged according to their skills and by restoring the norms and hierarchies of the medical field. The dynamics of this thesis resides in the search for the breach between the historical positioning (in relation to scientific fact itself) of homeopathic medication in the history of medical sciences, and the representation made of it in the past and present. By distinguishing what may be at stake, we are led to identify the positioning and impact of the mode of transmitting scientific fact as lacking a historical link to the history of medical sciences, notably medication. Our research hypothesis thus postulates that the transmission mode is at the source of discrepancies in representation. The links between context breakthroughs, such as the role of influences, are just parameters that, when transmitted, both color and help define a subject. For our topic we have been able to demonstrate this lack of bridging as a lack of transmission of the history of medication over the course of time. The only type of transmission was an operational one. The hindsight and analysis of the evolution were barely transmitted, if at all.Given these observations and in order to approach our topic, we were led to retrace the history of medication in the history of civilizations, in relation to the cultural, political and scientific context. Then, we examined the origins of homeopathy in order to demonstrate its positioning in the history of medication, its influences on the history of medication, and all ensuing confrontations. Because of the lack of pluri-disciplinary transmission, aimed at binding the subject to its temporal context, we designed an on-line museum in order to better demonstrate this positioning, thanks to open and dynamic interpretation made possible by digital media. The aim of this work is to illustrate our hypothesis on the positioning of transmission mode via a concrete application which intends to reduce this breach. ; Résumé : La dynamique de cette thèse est dans la recherche de l'écart entre la place historique - en rapport au fait scientifique même - du médicament homéopathique dans l'histoire des sciences médicales et la représentation qui en est faite hier comme aujourd'hui. En dégageant ce qui peut être en jeu, nous sommes conduit à repérer la place et l'impact du mode de transmission du fait scientifique comme le manque de lien historique en histoire des sciences médicales, autour de la question du médicament. Notre hypothèse de recherche est donc que le mode de transmission est à la source des écarts de représentation. Le lien entre les avancées, comme le jeu des influences, des contextes sont autant de paramètres lorsqu'ils sont transmis qui colorent et contribuent à définir un sujet. Pour notre question nous avons pu montrer le manque de lien comme le manque de transmission de l'histoire du médicament au cours du temps. La seule transmission a été opérationnelle. Le recul comme l'analyse de l'évolution n'a pas été transmis ou très peu.De ces observations, nous avons été conduit pour aborder notre étude de retracer l'histoire du médicament dans l'histoire des civilisations en rapport au contexte culturel, politique, scientifique. De là nous avons travaillé l'origine de l'homéopathie afin de montrer sa place dans l'histoire du médicament, ses influences pour l'histoire du médicament comme les confrontations qui en sont nées. De ce manque de transmission pluridisciplinaire, visant à relier le sujet au contexte de son temps nous avons réalisé un musée en ligne afin de montrer la place comme l'apport d'une lecture ouverte, dynamique que nous permet le support numérique. L'objectif de cette réalisation est d'illustrer notre hypothèse sur la place du mode de transmission par une application concrète qui vise à réduire cet écart.

Since sixty years, Ladakh's political space in north-west Hashalayan India has been driven by a series of official requests aimed at regional autonomy. These autonomous movements have sometimes led to very violent demonstrations, but they have never been directed against the Indian Union. On the other hand, they express religious radicalism refusing the administration of Ladakh by Muslim Kashmir. "Why should we be subjected to the cultural aggression of the Kashiris?", recently asked for a ladakhi leader in an Indian national daily. He commented on "culture" and thus aims to disguise the religious nature of local demands. Medicine from Tibet, which is one of the most representative aspects of culture (Buddhist), offers a privileged reading of these regional tensions, while at the same time peculiar to them. ; Depuis soixante ans, l'espace politique du Ladakh, au nord-ouest de l'Inde himalayenne, est animé par une série de demandes officielles ayant pour objectif l'autonomie régionale. Ces mouvements autonomistes ont parfois conduit à des manifestations très violentes mais ils n'ont cependant jamais été dirigés contre l'Union Indienne. Ils expriment en revanche un radicalisme religieux refusant l'administration du Ladakh par le Cachemire musulman. «Pourquoi devrions nous être soumis à l'agression culturelle des Cachemiris ?», demandait récemment un dirigeant ladakhi dans un quotidien national indien. Sa remarque interroge la « culture » et entend camoufler ainsi la nature religieuse des revendications locales. La médecine issue du Tibet, qui est l'un des aspects les plus représentatifs de la culture (bouddhiste) ladakhie, offre une grille de lecture privilégiée de ces tensions régionales, tout en les particularisant.
