"Introduction. In Romania legal medicine suppose professional activities as forensic pathologist and legal doctor are facing victims that request documenting traumatic lesions if any or sexual abuses, prejudices, working capacity, malpraxis, etc. Objective. The objective of our work is to determine the special kind of doctor-patient relationship in clinical legal medicine and to analyze the ethical influences. Material and methods. More than 1500 cases of medico-legal examinations are performed in the National Institute of Legal Medicine Mina Minovici in Bucharest each year. Most of them are domestic violence requests or car accidents. Discussions. In the late 30 years (Ezechiel JE, Linda LE, JAMA 1992) found out 4 different relationship models that may be applied in medical practice; the paternalistic model (the patient accepts his doctor as his legal and moral representative), informative model (the patient expresses his autonomy), interpretative model (the doctor is a good friend in need) and the deliberative model of self-construction of the knowledge that the patient must have in order to have a voluntary decision. Conclusions. In clinical legal medicine the patient is not only a patient but a victim also. Therefore, the relationship is double folded. With his patient the legal doctor develops initially an informative model, then an interpretative model. With his victim the doctor develops initially a paternalistic approach (passive character type) or an informative one (active character type), usually an interpretative model as most highly requested (What would you do if you were I?) or the deliberative model when evaluating prejudices, etc. "
The Patient Protection and Affordable Care Act of 2010 requires states to establish healthcare insurance exchanges by 2014 to facilitate the purchase of qualified health plans. States are required to establish exchanges for small businesses and individuals. A federally operated exchange will be established, and states failing to participate in any other exchanges will be mandated to join the federal exchange. Policymakers and health economists believe that exchanges will improve healthcare at lower cost by promoting competition among insurers and by reducing burdensome transaction costs. Consumers will no longer be isolated from monthly insurance premium costs. Exchanges will increase the number of patients insured with more cost-conscious managed care and high-deductible plans. These insurance plan models have historically undervalued emergency medical services, while also underinsuring patients and limiting their healthcare system access to the emergency department. This paradoxically increases demand for emergency services while decreasing supply. The continual devaluation of emergency medical services by insurance payers will result in inadequate distribution of resources to emergency care, resulting in further emergency department closures, increases in emergency department crowding, and the demise of acute care services provided to families and communities. The implementation of traditional medical practices is supported by several regulations including the Minister of Health Decree No. 1076 / MENKES / SK / VII / 2003 concerning Organizers of Traditional Medicine and Law No. 36 of 2009 concerning Health. This research was conducted using the normative-empirical method. The study population was taken by purposive sampling according to the research objectives. Data analysis in this legal analysis research uses qualitative methods. The rules and conditions set for the management of traditional medical practices in Bandar Lampung City are guided by PerMenKes No. 61 of 2016 concerning Empirical Traditional Health Services, PP of the Republic of Indonesia No. 103 of 2014 concerning Traditional Health Services and Decree of the Head of Lampung Provincial Health Office Number 442 regarding Guidelines for Developing Traditional Health Services in Lampung Province in 2009. The resulting legal analysis is evident from 60 respondents that there are 39 people or 65% already know that the traditional medicine where they seek treatment already have a permit, this will increase the confidence of patients to seek treatment to a legal license. The results of the study of 60 patient respondents were only 3 people who were given health insurance by traditional medical providers. While the remaining 57 patient respondents were not given health insurance by traditional medical providers. This proves that only 5% of traditional medicine dares to give health insurance to their patients. Patients or people who seek treatment are entitled to health insurance following the legal basis for health insurance. The Bandar Lampung City Health Office has not optimally conducted supervision and education on traditional medicine in the Bandar Lampung City. It is hoped that the mayor's regulations will effectively regulate traditional medical practices, preventive measures, and make patients more selective in choosing health healing facilities.

In vitro fertilization (IVF) is a hallmark of reproductive medicine. However, the inconclusive outcome leads to marital disharmonies; thus, the choices of gamete donation and surrogacy (GD/S) are often offered. In restricted countries, the child-rearing choice through foster/adoption care is promising, but the uptake remains low. We explore the current reproductive services and adoption scenarios in Asian countries to delineate this issue. The web and literature search using PubMed and Ichushi was conducted in Japanese and English using the keywords "adoption", "foster care", "reproductive medicine", including the interview with the respective Asian clinicians. We found that an established adoption system was seen in China, Malaysia, and the Philippines, mainly due to the restriction of GD/S. Although GD/S were allowed in Thailand, Singapore, and India, the different local affordability of IVF cost led to various adoption system scenarios. Nevertheless, the country's economic aspect does influence the establishment of adoption care, mainly due to financial support from local government. Otherwise, the significant barrier was the cultural/religious background leading to low adoption rates. We concluded that the adoption option should always be highlighted as an alternative strategy as it synergistically contributes to children's and infertile couples' welfare.

Вакцинация является наиболее влиятельным способом борьбы с инфекционными болезнями. Благодаря прививкам была ликвидирована натуральная оспа, резко снизилась заболеваемость полиомиелитом, столбняком. По данным ВОЗ, ежегодно в мире от инфекционных заболеваний погибает 12 млн детей. Из этого числа 7,5 млн смертей происходит за счет заболеваний, против которых мы пока не имеем вакцин, но более 4 млн человек умирает от болезней, которые можно предотвратить. Иммунизация считается одним из самых эффективных и экономически целесообразных среди мероприятий медицинского вмешательства в эпидемический процесс, которые существуют в настоящее время. Чем более экономически развито государство, тем от большего количества болезней оно защищает население путем проведения иммунопрофилактики. Для того чтобы создать коллективный иммунитет, ВОЗ рекомендует осуществлять прививки не менее 95 % лиц. Однако во Львовской области и Украине в целом сложилась негативная тенденция снижения охвата населения профилактическими прививками до 45–53%, что вызывает опасность возникновения и эпидемического распространения болезней, для профилактики которых есть вакцины.Цель исследования: анализ законодательной базы по вопросам иммунопрофилактики, особенности планирования профилактических прививок, требования к кабинетам прививок и современные противопоказания к вакцинации.Материалы и методы. Обработаны нормативно-правовые документы по вопросам иммунопрофилактики, проведено анкетирование 210 семейных врачей по вопросам осведомленности относительно вакцинации.Заключение. Сегодня существует необходимость в непрерывном профессиональном развитии врачей по вопросам иммунопрофилактики, в том числе путем проведения тренингов, курсов теоретического усовершенствования, стажировки в странах Европы и украинских центрах вакцинации. ; Вакцинація є найбільш дієвим заходом у боротьбі з інфекційними хворобами. Завдяки щепленням була ліквідована натуральна віспа, різко знизилась захворюваність на поліомієліт, правець. Згідно з даними ВООЗ, щорічно у світі від інфекційних захворювань гине 12 млн дітей. З цього числа 7,5 млн життів втрачається за рахунок захворювань, проти яких ми поки не маємо вакцин, але більше 4 млн людей помирають від хвороб, яким можна запобігти. Імунізація вважається одним із найбільш ефективних та економічно доцільних серед заходів медичного втручання в епідемічний процес, які сьогодні існують. Чим більш економічно розвинена держава, тим від більшої кількості хвороб захищає населення шляхом проведення імунопрофілактики. Для того щоб створити колективний імунітет, ВООЗ рекомендує здійснювати щеплення не менше 95% осіб. Of these, 7.5 million lives are lost to diseases against which we do not yet have vaccines, but more than 4 million people die from preventable diseases. Immunization is currently considered to be one of the most effective and cost-effective medical interventions in the epidemic process. The more economically developed a country is, the more diseases is its population protected from with the help of immunoprophylaxis. In order to create herd immunity, WHO recommends that at least 95 % of individuals should be vaccinated. However, in the Lviv region andUkrainethere is a negative tendency to reduce the coverage of preventive vaccinations up to 45–53%, which is a danger of the emergence and epidemic spread of preve ntable diseases.The objective: was to analyze the legislative framework on immunoprophylaxis, the peculiarities of planning preventive vaccinations, the requirements for vaccination offices and modern contraindications for vaccination.Materials and methods. Legal documents on immunoprophylaxis were analysed, 210 family doctors were surveyed on immunoprophylaxis awareness.Conclusions. There is a necessity of continuous professional development of physicians on immunoprophylaxis, including training, theoretical improvement courses, internships in European countries and Ukrainian vaccination centers.
