Palliative care is in crisis between care and cure. If their emergence appeared as a necessity in the face of the human scandal of abandoning the patient at the end of their life, considered in the same way as death as shameful in the 1970s, their philosophy could not always be sufficient. The discipline, whose volunteerism and usefulness are well established, faces internal and external psycho-social brakes because of their very purpose: the dying person. Could not their sometimes "too great assurance" in their merit, be a denied shame, which reverses in to its opposite? And the difficulty of being recognized as a worthy medicine could not be based on an epistemological conflict between medical science that does not know shame and human sciences that can analyze it. Knowing that this difficulty for palliative care to be recognized would also seem to be exacerbated by a social context where performance and profitability are the foundations of social organization. There is indeed in our neoliberal society a great difficulty to appreciate unconditionally the worth of the damaged body, of dependence, suffering and agony in becoming medical knowledge. Shame more often denotes scandal and opprobrium in every language when it is linked to dignity, modesty and guilt. If it is classically understood as a negative affect from which it will be legitimate to free oneself, it is no longer possible in the confrontation at the end of life. Shame is a phenomenon that is very present in the palliative clinic and yet it cannot be said, to name oneself and even less to accept oneself. Crossing all the layers of the human being (in other words, Dasein in Heideggerian terms), from the most superficial to the deepest, until reaching the unbearable being unveiled in its primary nudity, would it not be the driving force of both polarities of the world ? Si leur émergence est apparue comme une nécessité devant le scandale humain que constituait l'abandon du malade en fin de vie, considéré au même titre que la mort comme honteux dans les années 1970, leur philosophie n'a pas pu toujours s'imposer comme suffisante. La discipline, dont la bénévolence et l'utilité ne sont plus à démontrer, se heurte à des freins psycho-sociaux internes et externes du fait de leur objet même : le mourant. Leur parfois « trop grande assurance » dans leur bien-fondé ne pourrait-elle pas relever d'une honte déniée se renversant dans son contraire ? Et la difficulté à se faire reconnaître comme médecine digne ne pourrait-elle pas se fonder sur un conflit épistémologique entre science médicale qui ne connaît pas la honte et sciences humaines qui peuvent l'analyser. Sachant que cette difficulté pour les soins palliatifs à se faire reconnaître semblerait également s'exacerber du fait d'un contexte social où la performance et la rentabilité constituent les fondements de l'organisation sociale. Il y a en effet dans notre société néolibérale une grande difficulté à apprécier dignes inconditionnellement le corps abîmé, la dépendance, la souffrance et l'agonie devenue savoir médical. La honte désigne plus souvent dans le langage courant le scandale et l'opprobre alors qu'elle entretient un lien avec la dignité, la pudeur et la culpabilité. Si elle est classiquement entendue comme un affect négatif duquel il serait légitime de se libérer, ce n'est plus possible dans la confrontation à la fin de vie. La honte est un phénomène bien présent dans la clinique palliative et pourtant elle ne peut se dire, se nommer et encore moins s'accepter. Traversant toutes les couches de l'étant humain (autrement dit du Dasein en terme heideggérien) des plus superficielles aux plus profondes jusqu'à atteindre l'inassumable de l'être dévoilé dans sa nudité première, ne serait-elle pas motrice des deux polarités du monde ? Elle se love dans le pire de la dégradation pour peut-être permettre une ouverture à une possible expérience ontologique thérapeutique. La reconnaissance d'une honte ontologique au même titre qu'une dignité ontologique permettrait de désigner une dignité hontologique, terme oxymorique faisant sens pour un accompagnement authentiquement fraternel. Une dignité hontologique permettrait-elle à notre société de mieux élaborer le clivage toujours persistant entre un accompagnement solidaire et une mort donnée et peut-être érigerait-elle mieux les soins palliatifs comme discipline digne travaillant de concert avec la techno-science

General practitioners' evaluation of communication from hospital's palliative care: the example of Libourne's hospital designated palliative care beds ; Évaluation du lien ville-hôpital par les médecins généralistes dans la prise en charge palliative : exemple des Lits Identifiés de Soins Palliatifs...
Introduction: the community and hospital network (CHN) is a major health policy issue. It needs to be of good quality to optimize the management of patients in palliative care. Objectives: the main objective was to collect the opinion of general practitioners (GP) of the quality of the CHN considering the hospitalization of patients in Palliative Care Designated Beds (PCDB). The secondary goal was to know the GP improvements expectations. Methods: we conducted a quantitative observational survey by telephone and we interrogated GP whose patients were in the PCDB of the general hospital of Libourne in the period from march to december 2018. Results: the response rate was 35.5%. Half of the GP were dissatisfied with information initiatives on the presence of palliative care in the hospital. During the stay, 66.7% were very dissatisfied with the exchanges and 44.4% were very dissatisfied with their involvement in decision-making. At the end of hospitalization, 51.9% were satisfied with coordination of their patient returning home. 56% of the MG were satisfied with the summary letter but expressed discontent about the amount of time before receiving it, considered too long. Discussion: promoting information on the provision of healthcare, facilitating contact between concerned health care workers and improving communication means were several areas for improvement, based on the results of the study, and seem to be essential factors to develop and strengthen the CHN. Conclusion: the strengthening of the network is first and foremost a result of individual and institutional wishes, based on the political healthcare strategy which is in favour to the decompartmentalization of medical activity. ; Introduction : le lien ville-hôpital (LVH) est un enjeu majeur de la politique de santé. Il doit être de bonne qualité pour optimiser la prise en charge de patients en situation palliative. Objectifs : l'objectif principal était de recueillir l'avis des médecins généralistes (MG) sur la qualité du LVH dans le cadre de l'hospitalisation des patients sur les Lits Identifiés de Soins Palliatifs (LISP). L'objectif secondaire était de connaître les améliorations attendues de leur part. Méthodes : il s'agit d'une étude observationnelle quantitative réalisée par questionnaire téléphonique auprès des médecins généralistes des patients hospitalisés sur les LISP du Centre Hospitalier (CH) de Libourne de mars à décembre 2018. Résultats : le taux de réponse était de 35,5%. La moitié des MG était insatisfaite de l'information sur l'offre de soins palliatifs sur le CH. Pendant le séjour, 66,7% étaient très insatisfaits des échanges et 44,4% étaient très insatisfaits de leur implication dans la prise de décision. En fin d'hospitalisation, 51,9% étaient satisfaits de la coordination en cas de retour à domicile et 56% des MG étaient satisfaits du courrier mais regrettaient le délai de réception trop long. Discussion : promouvoir l'information sur l'offre de soins, faciliter le contact entre les différents interlocuteurs et perfectionner les moyens de communication sont les axes d'amélioration qui ont été dégagés de l'étude et qui semblent être des facteurs essentiels pour développer le LVH. Conclusion : le renforcement du LVH passe avant tout par des volontés individuelles et institutionnelles dans un contexte politique de santé favorable au décloisonnement des pratiques.

Following the French national strategy to develop palliative care promulgated in 2010, several regional pediatric palliative care teams have been implemented nationwide. Our work will present a fundamental ethical concept, which we call "second order care".The primary mission of these teams is to integrate a palliative approach in all areas of health care, and they are especially dedicated to assisting health care professionals. For "second order care teams", this entails advising and supporting "first order care teams" who directly provide for patients in palliative care. However, the daily practice of these second order care teams reveals underlying ethical tensions between the principle of justice which created these teams initially (palliative care must be available to all patients in need of such care) and the ethics of care (importance placed upon the singularity of each situation). In order to resolve these ethical tensions, we used a qualitative methodology known as "participant objectivation".Second order care implies an ethical practice combining one's disposition, outlook and activities, which are acquired by voluntary thought movements. The ethical aim of second order carers, with the help of inductive empathy, must be to assist first order carers in being autonomous in identifying and managing their own needs. Using clinical narratives, the second order team must adapt and harmonize the different temporalities in play: that of the patient, of their family and of the first order care team. Ultimately, second order care implies a major paradigm shift: to consider our inter-dependence in place of an illusionary autonomy. Mon travail présente un concept aux fondements éthiques appelé « care de second ordre ».Ces équipes, dont la mission est l'intégration de la démarche palliative dans la pratique de tout professionnel de santé, sont particulièrement dédiées aux professionnels de santé. En effet, les professionnels de ces équipes de care de second ordre, doivent assurer conseil et soutien aux professionnels de care de premier ordre ayant la charge d'un patient en situation palliative. Or la pratique quotidienne de ce care de second ordre révèle des tensions éthiques entre le principe de justice qui sous-tend la création de ces équipes (tous les patients qui le nécessitent doivent avoir accès à des soins palliatifs) et les éthiques du care (l'importance de la singularité des situations). Pour résoudre ces tensions éthiques, nous avons utilisé une méthodologie qualitative appelée objectivation participante.Le care de second ordre suppose une pratique éthique, faite d'attitudes et d'activités qui s'acquièrent par un mouvement volontaire de la pensée. Avec l'aide de l'empathie inductive, le care de second ordre doit avoir pour visée éthique l'autonomisation du professionnel de care de premier ordre dans l'identification et la prise en charge de ses propres besoins. Grâce à la narration des histoires cliniques, le care de second ordre doit ajuster les temporalités en jeu, celles du patient, celles de ses proches et celles des professionnels de care de premier ordre. In fine, ce care de second ordre implique un changement majeur de paradigme : la considération de notre interdépendance en lieu et place de notre semblant d'autonomie. Ce concept de care de second ordre, a priori extrapolable aux équipes ayant les mêmes missions, contribue à la réflexion nécessaire sur la modernisation de notre système de santé.

Survey of girondin general practioners concerning the use of palliative care professionals ; Enquête de pratique auprès des médecins généralistes girondins concernant le recours aux professionnels de soins palliatifs
Introduction: palliative cares are a recent medical discipline. Faced the increase of life expectancy and political will to enact palliative care at hospital or at home, general practitioners are increasingly confronted, but they find their formation insufficient and need of help from colleagues. Method: a quantitative survey has been done with the participation of 124 general practitioners in Gironde from October 2017 to February 2018 about resort to palliative care professionals. Results: 49 general practitioners accepted to participate so a participation rate of 39.5%. The majority is between 41 and 55 years old (n=36). 43% work in town and 41% in semi-rural areas. 45% told that palliative care are occupying increasingly position in their daily work. 61% treated 1 to 3 palliative care patients in 2016. 33% said they were not trained in palliative care. Patient refusal is the first reason why, GPs don't resort to palliative care professionals. 98% don't know Pallia 10. General practitioners would like more meetings with palliative care colleagues to contact them easily and quickly. Conclusion: regional disparities are pronounced in Gironde concerning the supply of care in palliative care structures. General practitioner can feel isolated to take care of their patients in difficult situations or when their patients refuse palliative care professionals help. Hotline, telemedicine, increase hospital and non-hospital structures are tracks proposed by general practitioners to reduce inequalities. ; Introduction : les soins palliatifs représentent une discipline récente dans le domaine de la médecine. Face à l'augmentation de l'espérance de vie et devant une volonté politique de promulguer les soins palliatifs en milieu hospitalier ou à domicile, les médecins traitants y sont de plus en plus confrontés dans la prise en charge de leurs patients. Or, ces derniers se trouvent insuffisamment formés et les situations peuvent les amener à faire appel à des confrères. Méthode : une enquête quantitative a été menée auprès de 124 médecins généralistes girondins entre Octobre 2017 et Janvier 2018 concernant le recours aux professionnels et structures de soins palliatifs. Résultats : 49 médecins ont accepté de participer soit un taux de participation de 39.5%. Une majorité a entre 41 et 55ans (n=36). 43% sont installés en milieu urbain et 41% en milieu semi-rural. 45% ont déclaré que les soins palliatifs avaient une place de plus en plus importante dans leur exercice médical. 61% ont suivi entre 1 et 3 patients en soins palliatifs durant l'année 2016. 33% ont déclaré n'avoir eu aucune formation en soins palliatifs. Le refus du patient apparait comme le frein principal au recours à une structure de soins palliatifs. 98% n'ont pas connaissance de l'outil Pallia 10. Les médecins sont demandeurs de rencontres avec leurs confrères, d'un moyen de communication rapide et efficace. Conclusion : il existe une disparité de l'offre de soins en soins palliatifs sur le département de la Gironde. En cela, les médecins peuvent se sentir isolés pour prendre en charge leur patients lors de situation difficile ou lors du refus du patient d'être accompagné par une épique spécialisé. Permanence téléphonique, télémédecine, augmentation des structures hospitalières et extrahospitalières sont des pistes avancées par les médecins afin de rétablir les inégalités.

Our research concerns the conditions of acceptance of different ways to treat pain in palliative care by health professionals and laypeople. Two ways are studied: the use of analgesic, including strong opioids ; implementation of sedation. Our research is based on the Functional Theory of Cognition by Norman Anderson (1981). On the study on the use of analgesics : 192 participants rated the degree of acceptability of each of the 56 proposed scenarios, resulting from the combination of four factors: « decision-making process », « request of the person to be relieved of his pain », « pain level, expressed through digital pain scale », « decision of the physician in terms of painkiller prescription ». For the study of sedation : 192 people judged the acceptability of the 48 proposed scenarios. They are the result of four factors combination: « request for sedation », « type of sedation », « life expectancy », « decision-making process ». In the study on the use of painkillers, especially strong opioids, it appears that only three of the manipulated factors played a role in the acceptability of the doctor's decision. The result by ascending order of the factors is: « pain level », « decision » and « decision-making process ». The less influential factor, since having no significant effect, is the factor « request ». The analysis in clusters also allowed us to discriminate 6 groups of participants, each with their own political judgment. Regarding the study on sedation, we also find that three of the four manipulated factors had influenced the judgment of acceptability. In ascending order, these factors are « request », « type of sedation » and « decision-making process ». The « life expectancy » factor had no significant effect. The analysis in clusters enabled us to distinguish 4 classes of participants, coming together according to their political judgment. Pour l'étude portant sur la sédation : 192 personnes ont jugé du degré d'acceptabilité des 48 scénarios proposés. Ils sont le résultat de la combinaison des quatre facteurs suivants : « demande de la personne », « type de sédation », « espérance de vie », « concertation du médecin avec l'équipe de soins ». Pour l'étude portant sur l'utilisation des antalgiques, notamment de palier III, il ressort que seuls trois des facteurs manipulés ont joué un rôle dans l'acceptabilité de la décision du médecin. Il s'agit par ordre croissant des facteurs « niveau de douleur », « décision du médecin » et « concertation du médecin ». Le facteur le moins influent puisque n'ayant pas eu d'effet significatif est le facteur « demande de la personne ». L'analyse en clusters nous a également permis de discriminer 6 groupes de participants, ayant chacun leur propre politique de jugement. Concernant l'étude sur la sédation, nous retrouvons également que trois des quatre facteurs manipulés ont eu une influence sur le jugement d'acceptabilité. Par ordre croissant, il s'agit des facteurs « demande de la personne », « type de sédation » et « concertation du médecin ». Le facteur « espérance de vie » n'a pas eu d'effet significatif. L'analyse en clusters nous a permis de différencier 4 classes de participants, se regroupant selon leur politique de jugement. L'acceptabilité des différents moyens utilisés pour soulager la douleur en fin de vie est largement influencée par les facteurs intervenant dans les scénarios.

La Fondation de France et les soins palliatifs : Une action publique under cover
International audience ; This research studies the role of the Fondation de France (FDF) in the development of palliative care in France. While the history of palliative care in France has been the subject of major studies over the last twenty years, the role of the FDF has hardly be noticed. However, the FDF developed many activities in this field, through the financing of projects that contributed to the formation of an organizational field. Based on interviews and a consultation of the archives of the FDF's palliative care program, this research shows how these interventions contributed to a public policy in favour of palliative care. Initially, FDF approached palliative care advocates, building affinity and legitimacy with them. Secondly, FDF facilitated the institutionalization of palliative care by acting as a catalyst for the actors favourable to its development, including within the State. Finally, the FDF establishes an inter-temporal mediation by maintaining attention on this issue and by brokering these initiatives from one period to another. However, some studies show that concomitant palliative management and often leading to oncological therapeutic abstention increases the survival time and quality of life of patients. The concept of end-of-life, which is widely used in the literature and is entitled to several laws, is not clearly defined at the legislative level. What is the end of life in Oncology? The main objective of our work was to define the notion of end of life through the study of the representations of physicians oncologists and practicing palliative care and its implementation in the exercise of the medical function. A qualitative study was conducted in the Pays de la Loire region using 14 semi-structured interviews from January 2019 to March 2019. The interviews were recorded and analyzed according to a so-called thematic content analysis. The end of life is a complex notion that could not be defined in a consensual and unique way. Two end-of-life concepts have been established: the end of life "objective" and the end of life "subjective". These two concepts made it possible to establish two decision models for stopping active oncology therapeutics. The "objective" end of life was not precisely defined but referred to the clinico-biological approach to death, the accumulation of arguments or evidence in favor of the inevitability of death and decreasing the prevailing medical uncertainty. The end of life "subjective", meant the understanding of the near death and by these multiple representations it shaped the sense of the medical actions. Cependant, certaines études démontrent qu'une prise en charge palliative concomitante et entrainant souvent une abstention thérapeutique oncologique augmentait la durée de survie et la qualité de vie des patients. La notion de fin de vie très utilisée dans la littérature et intitulée de plusieurs lois n'est pourtant pas clairement définie sur le plan législatif. Qu'est-ce que la fin de vie en Oncologie ? L'objectif principal de notre travail était de définir la notion de fin de vie grâce à l'étude des représentations chez les médecins oncologues et exerçant les soins palliatifs et de sa mise en pratique dans l'exercice de la fonction médicale. Une étude qualitative a été menée dans la région Pays de la Loire à l'aide de 14 entretiens semi-dirigés de Janvier 2019 à Mars 2019. Les entretiens ont été enregistrés puis analysés selon une analyse de contenu dite thématique. La fin de vie est une notion complexe qui n'a pas pu être définie de manière consensuelle et unique. Deux concepts de fin de vie ont pu être établis: la fin de vie « objective » et la fin de vie « subjective». Ces deux concepts ont permis d'établir deux modèles décisionnels d'arrêt des thérapeutiques actives oncologiques. La fin de vie « objective » n'était pas définie précisément mais désignait l'approche clinico-biologique de la mort, l'accumulation d'arguments ou de preuves en faveur de l'inéluctabilité de la mort en diminuant l'incertitude médicale régnante. La fin de vie « subjective », désignait la compréhension de la mort proche et par ces multiples représentations façonnait le sens des actions médicales. Ce travail apporte un éclairage supplémentaire sur la notion de fin de vie mettant en évidence l'intégration d'une composante personnelle subjective soumise à de nombreuses représentations à la pratique médicale rationnelle fondées sur des connaissances et des compétences validées.
