Розглянуто питання управління бюджетними коштами в умовах децентралізації у контексті нових підходів до методики його здійснення. На основі ґрунтовного та всебічного аналізу управління бюджетними коштами визначено низку удосконалень, які можуть бути застосовані з огляду на сучасний стан розвитку міжбюджетних відносин та законодавче забезпечення функціонування об'єднаних територіальних громад місцевого самоврядування загалом. Означено характерні особливості кожного з удосконалень та окреслено порядок їх застосування для управління бюджетними коштами. ; Problem setting. The process of decentralization, which was initiated in order to strengthen the material and financial basis of local self-government, requires the latest approaches to its implementation, given the complexity of the process, the large number of issues of the functioning of the territorial communities, which it covers and the ambiguity of the consequences it leads to. Since it, on the one hand, can contribute to the development of separate territorial communities at the expense of additional financial resources and, on the other hand, create barriers for the development of other communities with less economic potential and can not compete with more developed regions, provided that the transition to a self-financing model of local self-government is preferable .This is particularly evident in the management of budgetary funds that are available to territorial communities and should be used most rationally and efficiently on the one hand and have enough to finance their aspirated needs from another. And in view of this, there is a need for an analysis of the methods of managing budget funds in a decentralized environment that would correspond to the current trends in the development of territorial communities and were of a new character.Recent research and publications analysis. A number of scholars in their writings touched upon issues of budget activity in the conditions of decentralization, including in its fiscal aspect, among them: V. Bodrov, T. Buturlakin, A. Vasiliev, G. Voznyak, O. Kyrylenko, A. Lelechenko, Z. Lobodina, G. Lopushniak, M. Vlivdar, V. Kuybida, I. Lunin, S. Romaniuk, A. Tkachuk, G. Shakhov and others. Among foreign scholars should mention Sh. Blankarta, G. Bivaleets, M. J. Byukenen, R. A. Masgraveva, Y. Kulyavika and others.The scholars and the questions of the concept of fiscal decentralization were considered, which were reflected in the works of O. Borislavskaya, I. Volokhova, V. Hamana, M. Goncharenko, Y. Grinchenko, O. Kyrilenko, N. Kryshtof, O. Lisenko, O. Onishchenko, S. Romanyuk, M. Ruban, A. Yakymchuk, V. Yaroshchuk and others like that.However, the question of the methodology of managing budget funds in conditions of decentralization and its improvement was not the subject of a separate scientific analysis.Highlighting previously unsettled parts of the general problem. In general, the issue of budget activities in the context of decentralization was considered mainly from the point of view of the functioning of individual budgets and the needs of their adaptation to this process. That is, the scholars considered separate issues of budget activity in a decentralized environment without analyzing the methodology of managing budget funds in the course of decentralization processes.At the same time, it is the methods of management of budget funds that would correspond to the current state of decentralization processes taking place in local self-government were not the subject of separate scientific research, and therefore there is a need for scientific research in this direction.Paper main body. From the point of view of public administration of budgetary funds in the conditions of decentralization, it is necessary to determine the method of management of budget funds in the conditions of decentralization as a set of consistent actions aimed at achieving the result in the form of improved management of such budgetary funds. In view of the complex nature of budget relations and the issues of legal and practical regulation of fiscal decentralization, it is advisable to choose an approach based on development, with subsequent practical application, not one and a number of new methods that should improve the management of budgetary funds in conditions of decentralization. Having grasped the most important issues of managing budget funds in the territorial communities.At the state level, the relevant financial, property and budgetary rights of local self-government are guaranteed, with the allocation of local budgets as independent components in the budget system of Ukraine, and the opportunities for transferring resources to the bodies of self-organization of the population are guaranteed, which must be taken into account during the management of budget funds in conditions of decentralization.Until the start of a new budget period, territorial communities are not able to take advantage of all the financial opportunities provided to united territorial communities. Including receive corresponding revenues to the budget of the united territorial community, as provided for in Article 64 of the Budget Code of Ukraine. And this reduces the budget opportunities of the united territorial community, especially when the union took place at the beginning of the fiscal year. Therefore, it is expedient to allow such associations to be held and in the future, without limiting them only in the framework of 2018, by making appropriate amendments to the current legislation and adding in this part of the Methodology for the formation of capable territorial communities.The territorial community is estimated by the population criterion and the area without taking into account the particulars of the area (the mountainous location, the severity of the availability of particular settlements or parts of the territories), the existing infrastructure on the ground (its degree of development, wear, suitability for use), natural and climatic conditions, etc. Therefore, it is advisable to supplement the Methodology, as well as the Law of Ukraine "On Voluntary Association of Territorial Communities", with norms that would take into account a greater number of factors for the financing needs of territorial communities.The resources available to the territorial communities should be the basis of their activities and the effectiveness of their use will be directly dependent on the possibility of functioning of such communities, on the principle – the lack of resources dictates the need for joining another territorial community. Also, not all territorial communities are fully capable of self-financing, and therefore need support from the State Budget.An important issue is the transfer of powers without their proper funding, which leads to the creation of a significant amount of deficit within the local budgets to cover the use of funds that could be used to develop the territorial communities. Therefore, it is important in the Methodology for the formation of capable territorial communities to consider the mechanism of management of budget funds, which should be based on the appropriate level of financing of delegated powers, without which the execution of such powers or other functions of local self-government may be endangered. Also, during the management of budget funds, it should be possible to borrow all the budgets of local self-government bodies, especially the budgets of the joint territorial communities, which at the expense of the received funds will be able to implement projects that are important for the communities and develop the economies of such communities. At the same time, the restrictions contained in the Budget Code of Ukraine, regarding the financial possibilities of borrowing funds of territorial communities and their use within the budgets of development should be kept.There is a legal framework for involving civil society organizations in the consideration of budget issues of local self-government and their control by the budget process at the level of territorial communities. However, the Methodology for the formation of capable territorial communities does not provide for the possibility of conducting public financial control over the formation of the budget, primarily through its public discussion, and further control over its implementation by the public and public organizations of such a community. This shortcoming must be corrected by making appropriate additions to this Methodology.Conclusions of the research and prospects for further studies. The main new approaches to improving the methodology for managing budget funds in the territorial communities are as follows:– transition to the budget of the united territorial community from the budgets of the territorial communities that united without setting the time frame for such a transition;– management of budget funds in terms of allocating financial support between the budgets of the joint territorial communities, which needs to be improved and taking into account additional factors influencing the need for features of location (in particular landscape), natural and climatic conditions;– management of the budget funds of a territorial community is based on the following key provisions: 1) territorial communities should receive at least 50% (according to the model of European states) from taxes, fees, payments paid on the territory of the community; 2) territorial communities should be self-financing, which, on the one hand, will put them in an effective use of resources, and on the other, create additional incentives for the formation of united territorial communities that will be financially self-sufficient; 3) support from the state budget should be provided on the basis of functions, including social, carried out by the territorial communities, with full financing of these functions;– management of local budgets, which consists of borrowing opportunities for all budgets of local self-government bodies, especially budgets of united territorial communities;– introduction of public control in the field of local budgets, the formation and implementation of the budget of territorial communities.The prospect of further research is the detailed elaboration of the identified ways of improving the methodology of budget funds management and the formation of legislative acts for its implementation.

In the context of restoration of government framework and local authorities in the regions prone to external aggression, special attention should be paid to the study of the problem of organisation of communication between public authorities and local population. It is the dialogue and the effective exchange of information between the population and the local authorities that constitute one of the major pillars of reintegration of Donbass territories, covered by the antiterrorist operation, into Ukraine; the regional specifics of the given territories is the anarchy resulting from withdrawal of the local government bodies that were legally elected yet in 2010. In fact, escape of heads and deputies of local councils in the most challenging period in life of the region catalysed the local people, reinforcing the sense of insecurity and understanding the betrayal of representatives of local government bodies. Thus, the issues of enhancement of communication channels effectiveness, which were still burning before the beginning of the external aggression and the partial occupation of Donbass, have been compounded today by the problem of establishment of communication channels between the population and the new Ukrainian local authorities nearly from scratch. This needs to take into account that the armed conflict conditions remain and, obviously, leave their imprint on social interaction within local communities. At the same time, it is the development of communication in the ATO zone, especially in the areas that underwent occupation or were released, which may form the basis for the harmonious entry of the citizens into the Ukrainian socium. Establishment of civil­military administrations (CMAs) with the purpose to temporarily substitute the non­functioning local authorities in Donbass is the unique experience for Ukraine. Despite legal recognition of rights and duties of local government bodies, it should be understood that CMAs act in terms of the «undeclared war», increasingly choosing the model of restricted interaction with the population in this context. The analysis conducted in the article shows that among the traditional information channels, CMAs give preference to reception of citizens by executive authorities and placement of announcements. At the same time, such forms as media, Internet resources, reports and assemblies of citizens are hardly used by CMAs which altogether results in discontent among the population. But the main thing is that in the ATO zone such important components of communication as information exchange and involvement of population in the decision making by local authorities are totally missing. This trend also defines the level of interconnection not only between the government and the population in the ATO zone; on the contrary, the studies, conducted by the All­Ukrainian Association of local government bodies of «Association of Ukrainian Cities» in cooperation with the Norwegian Institute for Urban and Regional Research, show that population in peaceful cities of Ukraine also experiences difficulties in communication with the government. This increases mistrust of the local government both in the ATO zone and in the country in the whole. The author of the article arrives at the conclusion about the need to reconsider the communication component in the work of local government bodies. It is in the ATO zone where the unique environment was established for creation of an open dialogue format, involvement of citizens into rebuilding the infrastructure thus nourishing the sense of participation in the governance. Given that CMAs report to state institutions, the situation can be reversed and the communication can be established only with the state support and intention. As the most efficient mechanism, the state may attract representatives of the third sector to this region, who could serve as volunteers for establishment of information services under CMAs, promptly reflecting and distributing information among the population and providing feedback. ; У контексті відновлення основ організації керування й місцевої влади в регіонах підданих зовнішній агресії особливої актуальності заслуговує дослідження проблематики комунікації публічної влади. Саме діалог й ефективний обмін інформацією між населенням і місцевою владою важлива складова реінтеграції територій Донбасу охоплених антитерористичною операцією. Специфікою більшості територій Донбасу стало безвладдя, у результаті відхилення законно обраних ще в 2010 році органів місцевого самоврядування. По суті, втеча мерів, депутатів місцевих рад у найважчий період життя регіону активізував у місцевих жителів почуття незахищеності й зрадництва з боку місцевої влади. Тим самим можна констатувати, що до проблем підвищення ефективності каналів комунікації зберігає свою актуальність до початку зовнішньої агресії й часткової окупації Донбасу, сьогодні додалася проблема налагодження комунікаційних зв'язків між населенням і новою місцевою українською владою фактично із чистого аркуша. При цьому необхідно враховувати, що умови військового конфлікту зберігаються й, безумовно, відкладають свій відбиток на взаємодію. У той же час саме розвиток комунікації в зоні АТО, особливо в населених пунктах, які пережили окупацію й звільнені може стати основою для гармонійного входження жителів в український соціум. Створення військово­цивільних адміністрацій з метою тимчасово замінити нефункціонуючі органи місцевого самоврядування на Донбасі – унікальний досвід для України. Незважаючи на законодавче закріплення за ВЦА прав і обов'язків органів місцевого самоврядування, необхідно розуміти, що діють вони в умовах «неоголошеної війни» і тим самим у більшій мірі обирають модель обмеженої взаємодії з населенням. Проведений у статті аналіз показує, що серед традиційних каналів подачі інформації ВЦА віддають перевагу прийому громадян посадовими особами; вивішуванню інформаційних оголошень. При цьому такі форми, як ЗМІ, Інтернет­ресурс, звіти, збори громадян практично не використовуються в роботі ВЦА, що в цілому викликає невдоволення в населення. Але головне, що в зоні АТО абсолютно відсутні такі важливі компоненти комунікації, як обмін інформацією й включення населення в процес прийняття рішень місцевої влади. Хоча дана тенденція характеризує й рівень взаємин не тільки влади з населенням у зоні АТО, навпаки, дослідження, проведені Всеукраїнською асоціацією органів місцевого самоврядування «Асоціація міст України» разом з Норвезьким інститутом міських і регіональних досліджень, показують, що й у мирних містах України населення також зазнає труднощів у комунікації із владою. Це підсилює рівень недовіри до місцевої влади як у зоні АТО, так і в цілому по країні. Автор статті приходить до висновків про необхідність перегляду комунікаційної складової в роботі органів місцевої влади. Саме в зоні АТО склалися унікальні умови створити формат відкритого діалогу, залучення громадян у відродження інфраструктури і тим самим формувати почуття причетності до керування. Виходячи з підпорядкованості ВЦА державним структурам переломити ситуацію й налагодити комунікацію можливо тільки при сприянні й бажанні держави. Як найбільш діючий механізм може стати залучення державою в даний регіон представників третього сектора як волонтерів по створенню інформаційних служб при ВЦА, які б оперативно відбивали й поширювали інформацію серед населення й здійснювали зворотний діалог. ; У контексті відновлення основ організації керування й місцевої влади в регіонах підданих зовнішній агресії особливої актуальності заслуговує дослідження проблематики комунікації публічної влади. Виходячи з підпорядкованості ВЦА державним структурам переломити ситуацію й налагодити комунікацію можливо тільки при сприянні й бажанні держави. Як найбільш діючий механізм може стати залучення державою в даний регіон представників третього сектора як волонтерів по створенню інформаційних служб при ВЦА, які б оперативно відбивали й поширювали інформацію серед населення й здійснювали зворотний діалог.

The article analyzes the theoretical basis of Chinese playwrights of the twentieth century highlighted features of the genre in classical and colloquial drama based on theories dramas in Europe and highlights the innovation playwrights in the image enamored feelings and emotions of the characters. The exceptions were the works of modern and contemporary drama, which, according to plan, the artists had to bring their modern foreign samples. Also, the researchers ignored the problem remained a combination of Chinese drama of the twentieth century, visual and verbal, that is the key to understanding the text, presentation, creating characters, exposure to society, speech-syntax. The above work covering only part of the study of classical literature, experimental avant-garde drama. Taking into account the scientific concept works on the formation, development and current state of drama in China, we believe that singled out a significant, significant and undeniable influence on literature, and, hence, to the drama took place due to political and social events that were in China during past times; artists striving to update and go beyond the established boundaries of the genre of classical drama was caused by the passage of time, Western European trends. Вважаємо, що вагомий та беззаперечний вплив на художню літературу, а відтак і на драматургію, здійснювався через соціально-політичний устрій тогочасного Китаю. Background. Today, allergic respiratory diseases in children, including asthma and allergic rhinitis, are a relevant medical and social problem associated with a high risk of disability, a significant reduction in the quality of life of patients and their families, and a significant financial burden on patients and the government. Bronchial asthma and allergic rhinitis often coexist, complicating the course of each other. Children with respiratory allergic diseases are particularly vulnerable to acute respiratory infections. Respiratory viruses, in turn, play an important role in the pathogenesis of bronchial asthma and allergic rhinitis, contributing to the development of diseases and acting as triggers for their exa­cerbations. One of the possible causes of predisposition to frequent acute respiratory infections in children with allergic pathology of the respiratory system may be a lack of nonspecific factors of mucosal immunity of the respiratory tract — endogenous cationic amphiphilic antimicrobial peptides, in particular, cathelicidin and human defensin. According to many studies, these antimicrobial peptides have antiviral, antimicrobial and immunomodulatory effects. However, nowadays there is insufficient data on the state of production of antimicrobial peptides and their impact on respiratory morbidity in children with allergic diseases. At the same time, some literature sources describe the positive effect of individual probiotic strains, in particular Lactobacillus rhamnosus GG, on the overall resistance to respiratory viral infection and the prevention of atopic diseases. The purpose of our research was to study the concentrations of antimicrobial peptides (HbD-2 and LL-37) in the secretion of the mucous membranes of the upper respiratory tract in children with bronchial asthma and allergic rhinitis, to clarify their role in protection against respiratory viral infection and to estimate the effectiveness of a probiotic preparation containing Lactobacillus rhamnosus GG in the prevention of acute respiratory infections and reduction of the frequency of virus-induced exacerbations of allergic pathology. Materials and methods. We conducted a clinical and laboratory examination of 76 children aged 7 to 17 years, 24 of whom were diagnosed with allergic rhinitis, 28 children — bronchial asthma, and 24 — bronchial asthma and allergic rhinitis. The control group consisted of 20 clinically healthy children of matched age and sex. Research methods also included the analysis of respiratory disease patterns, determination of concentrations of antimicrobial peptides: cathelicidin (LL-37), human β-defensin 2 (HbD-2) in the secretion of the upper respiratory tract by ELISA, and statistical analysis. Results. It was found that children with allergic diseases of the respiratory tract are characterized by a higher frequency of acute respiratory infections (30 %) with more frequent involvement of the lower respiratory tract in the inflammatory process (72 %), which causes a longer duration of disease (95 %) compared to healthy children. The children with allergic rhinitis and bronchial asthma had a significant decrease in the concentrations of antimicrobial peptides in the secretion of the upper respiratory tract compared with the control group. It was found that the level of antimicrobial peptide concentrations in the secretion of the upper respiratory tract is most negatively affected by the allergic inflammation severity, accompanied by a lack of symptom control, and the use of basic therapy and no symptoms have a positive effect. However, even in the presence of complete control of the symptoms of allergic diseases, the levels of antimicrobial peptides were significantly lower than in children of the control group. It was found that there is an inverse relationship between the levels of antimicrobial peptides and the frequency and duration of acute respiratory infections in children with allergic diseases, most pronounced in bronchial asthma. Conclusions. Taking a probiotic drug containing a strain of Lactobacillus rhamnosus GG for a month in addition to the basic therapy of allergic rhinitis and bronchial asthma contributed to a significant increase in levels of HbD-2 (average 27 %) and ­LL-37 (average 60 %) in secretions of the upper respiratory tract, which was accompanied by a decrease in the frequency of acute respiratory infections by an average of 26 % (including episodes involving the lower respiratory tract), a decrease in the average duration of acute respiratory infections by 22 % over the next year and a decrease in virus-induced exacerbations by 25.7 %. ; Актуальность. На сегодняшний день аллергические заболевания респираторного тракта, в частности бронхиальная астма и аллергический ринит, у детей являются актуальной медико-социальной проблемой, ассоциированной с высоким риском инвалидизации, существенным снижением качества жизни пациентов и их семей, а также значительной финансовой нагрузкой на пациентов и государство. Бронхиальная астма и аллергический ринит часто сосуществуют, обременяя течение друг друга. Дети, больные респираторными аллергическими заболеваниями, являются особенно уязвимыми к развитию острой респираторной вирусной инфекции (ОРВИ), а респираторные вирусы, в свою очередь, играют важную роль в патогенезе бронхиальной астмы и аллергического ринита, способствуя развитию заболеваний и выступая триггерами их обострений. Одной из возможных причин склонности к частым ОРВИ у детей с аллергической патологией дыхательной системы может быть недостаточность неспецифических факторов мукозального иммунитета респираторного тракта — эндогенных катионоактивных амфифильных антимикробных пептидов, в частности кателицидина (LL-37) и человеческих дефензинов. По данным ряда исследований, эти антимикробные пептиды проявляют противовирусное, антимикробное и иммуномодулирующее действие. При этом данных о состоянии продукции антимикробных пептидов и их влиянии на респираторную заболеваемость у детей с аллергическими заболеваниями в современной литературе недостаточно. В то же время в ряде литературных источников описано положительное влияние отдельных пробиотических штаммов, в частности Lactobacillus rhamnosus GG, на общую резистентность к респираторной вирусной инфекции и профилактику атопических заболеваний. Целью нашей работы стало изучение уровней антимикробных пептидов (человеческого β-дефензина 2 (HbD-2) и LL-37) в секрете слизистых оболочек верхних дыхательных путей у детей с бронхиальной астмой и аллергическим ринитом, выяснение их роли в защите от респираторной вирусной инфекции у данного контингента больных и оценка эффективности применения пробиотического препарата, содержащего Lactobacillus rhamnosus GG, с целью профилактики ОРВИ и снижения частоты вирус-индуцированных обострений аллергической патологии. Материалы и методы. Нами было проведено клинико-лабораторное обследование 76 детей в возрасте от 7 до 17 лет, из которых у 24 был диагностирован аллергический ринит, у 28 детей — бронхиальная астма, а у 24 — бронхиальная астма и аллергический ринит. Группу контроля составили 20 клинически здоровых детей соответствующего возраста и пола. Кроме общепринятых клинических, методы исследования включали анализ паттернов респираторной заболеваемости, определение уровней антимикробных пептидов LL-37, HbD-2 в секрете верхних дыхательных путей методами иммуноферментного и статистического анализа. Результаты. Установлено, что дети с аллергическими заболеваниями респираторного тракта характеризуются большей частотой ОРВИ (на 30 %) с более частым вовлечением в воспалительный процесс нижних дыхательных путей (на 72 %), что обусловливает более длительную продолжительность заболеваний (на 95 %) по сравнению со здоровыми детьми. У детей с аллергическим ринитом и бронхиальной астмой обнаружено существенное снижение уровней антимикробных пептидов в секрете верхних дыхательных путей по сравнению с группой контроля. На уровни антимикробных пептидов в секрете верхних дыхательных путей в наибольшей степени негативно влияет степень активности аллергического воспаления, сопровождающегося отсутствием контроля симптомов, а положительно — прием препаратов базисной терапии и отсутствие симптомов. Вместе с тем даже при наличии полного контроля симптомов аллергических заболеваний, то есть при минимальном уровне аллергического воспаления в дыхательных путях, уровни антимикробных пептидов были достоверно ниже, чем у детей контрольной группы. Установлено, что существует обратная связь между уровнями антимикробных пептидов и частотой и продолжительностью ОРВИ у детей с аллергическими заболеваниями, которая в наибольшей степени выражена при бронхиальной астме. Выводы. Прием пробиотического препарата, содержащего штамм Lactobacillus rhamnosus GG, в течение 1 месяца дополнительно к базисной терапии аллергического ринита и бронхиальной астмы способствовал достоверному повышению уровней HbD-2 (в среднем на 27 %) и LL-37 (в среднем на 60 %) в секрете верхних дыхательных путей, что сопровождалось снижением частоты ОРВИ в среднем на 26 % (в том числе и эпизодов с привлечением нижних дыхательных путей), уменьшением средней продолжительности ОРВИ на 22 % в течение следующего года и снижением вирус-индуцированных обострений на 25,7 %. ; Актуальність. На сьогодні алергічні захворювання респіраторного тракту, зокрема бронхіальна астма й алергічний риніт, у дітей є актуальною медико-соціальною проблемою, асоційованою з високим ризиком інвалідизації, суттєвим зниженням якості життя пацієнтів та їх сімей, а також значним фінансовим навантаженням на пацієнтів і державу. Бронхіальна астма й алергічний риніт часто співіснують, обтяжуючи перебіг одне одного. Діти, хворі на респіраторні алергічні захворювання, особливо вразливі до розвитку гострої респіраторної вірусної інфекції (ГРВІ), а респіраторні віруси, в свою чергу, відіграють важливу роль у патогенезі бронхіальної астми й алергічного риніту, сприяючи розвитку захворювань і виступаючи тригерами їх загострень. Однією з можливих причин схильності до частих ГРВІ у дітей з алергічною патологією респіраторної системи може бути недостатність неспецифічних факторів мукозального імунітету респіраторного тракту — ендогенних катіоноактивних амфіфільних антимікробних пептидів, зокрема кателіцидину (LL-37) та людських дефензинів. За даними низки досліджень, дані антимікробні пептиди проявляють противірусну, антимікробну та імуномодулюючу дію. При цьому даних щодо стану продукції антимікробних пептидів та їх впливу на респіраторну захворюваність у дітей з алергічними захворюваннями в сучасній літературі недостатньо. В той же час в низці літературних джерел описано позитивний вплив окремих пробіотичних штамів, зокрема Lactobacillus rhamnosus GG, на загальну резистентність до респіраторної вірусної інфекції та профілактику атопічних захворювань. Метою нашої роботи стало вивчення рівнів антимікробних пептидів (людського β-дефензину 2 (HbD-2) та LL-37) у секреті слизових оболонок верхніх дихальних шляхів у дітей з бронхіальною астмою й алергічним ринітом, з'ясування їх ролі у захисті від респіраторної вірусної інфекції у даного контингенту хворих та оцінка ефективності застосування пробіотичного препарату, що містить Lactobacillus rhamnosus GG, з метою профілактики ГРВІ та зниження частоти вірус-індукованих загострень алергічної патології. Матеріали та методи. Нами було проведено клініко-лабораторне обстеження 76 дітей віком від 7 до 17 років, з яких у 24 був діагностований алергічний риніт, у 28 дітей — бронхіальна астма, а у 24 — бронхіальна астма й алергічний риніт. Групу контролю становили 20 клінічно здорових дітей відповідного віку та статі. Встановлено, що діти з алергічними захворюваннями респіраторного тракту характеризуються більшою частотою ГРВІ (на 30 %) з більш частим залученням у запальний процес нижніх дихальних шляхів (на 72 %), що обумовлює більшу тривалість захворювань (на 95 %) порівняно із здоровими дітьми. У дітей з алергічним ринітом і бронхіальною астмою виявлено суттєве зниження рівнів антимікробних пептидів у секреті верхніх дихальних шляхів порівняно з групою контролю. На рівні антимікробних пептидів у секреті верхніх дихальних шляхів найбільшою мірою негативно впливає ступінь активності алергічного запалення, що супроводжується відсутністю контролю симптомів, а позитивно — прийом препаратів базисної терапії та відсутність симптомів. Разом з тим навіть за наявності повного контролю симптомів алергічних захворювань, тобто при мінімальному рівні алергічного запалення в дихальних шляхах, рівні антимікробних пептидів були вірогідно нижчими, ніж у дітей контрольної групи. Встановлено, що існує зворотний зв'язок між рівнями антимікробних пептидів і частотою та тривалістю ГРВІ у дітей з алергічними захворюваннями, найбільш виражений при бронхіальній астмі. Висновки. Прийом пробіотичного препарату, що містить штам Lactobacillus rhamnosus GG, протягом 1 місяця додатково до базисної терапії алергічного риніту та бронхіальної астми сприяв вірогідному підвищенню рівнів HbD-2 (в середньому на 27 %) та LL-37 (в середньому на 60 %) у секреті верхніх дихальних шляхів, що супроводжувалось зниженням частоти ГРВІ в середньому на 26 % (в тому числі й епізодів із залученням нижніх дихальних шляхів), зменшенням середньої тривалості ГРВІ на 22 % протягом наступного року та зниженням вірус-індукованих загострень на 25,7 %.

Background. Today, allergic respiratory diseases in children, including asthma and allergic rhinitis, are a relevant medical and social problem associated with a high risk of disability, a significant reduction in the quality of life of patients and their families, and a significant financial burden on patients and the government. Bronchial asthma and allergic rhinitis often coexist, complicating the course of each other. Children with respiratory allergic diseases are particularly vulnerable to acute respiratory infections. Respiratory viruses, in turn, play an important role in the pathogenesis of bronchial asthma and allergic rhinitis, contributing to the development of diseases and acting as triggers for their exa­cerbations. One of the possible causes of predisposition to frequent acute respiratory infections in children with allergic pathology of the respiratory system may be a lack of nonspecific factors of mucosal immunity of the respiratory tract — endogenous cationic amphiphilic antimicrobial peptides, in particular, cathelicidin and human defensin. According to many studies, these antimicrobial peptides have antiviral, antimicrobial and immunomodulatory effects. However, nowadays there is insufficient data on the state of production of antimicrobial peptides and their impact on respiratory morbidity in children with allergic diseases. At the same time, some literature sources describe the positive effect of individual probiotic strains, in particular Lactobacillus rhamnosus GG, on the overall resistance to respiratory viral infection and the prevention of atopic diseases. The purpose of our research was to study the concentrations of antimicrobial peptides (HbD-2 and LL-37) in the secretion of the mucous membranes of the upper respiratory tract in children with bronchial asthma and allergic rhinitis, to clarify their role in protection against respiratory viral infection and to estimate the effectiveness of a probiotic preparation containing Lactobacillus rhamnosus GG in the prevention of acute respiratory infections and reduction of the frequency of virus-induced exacerbations of allergic pathology. Materials and methods. We conducted a clinical and laboratory examination of 76 children aged 7 to 17 years, 24 of whom were diagnosed with allergic rhinitis, 28 children — bronchial asthma, and 24 — bronchial asthma and allergic rhinitis. The control group consisted of 20 clinically healthy children of matched age and sex. Research methods also included the analysis of respiratory disease patterns, determination of concentrations of antimicrobial peptides: cathelicidin (LL-37), human β-defensin 2 (HbD-2) in the secretion of the upper respiratory tract by ELISA, and statistical analysis. Results. It was found that children with allergic diseases of the respiratory tract are characterized by a higher frequency of acute respiratory infections (30 %) with more frequent involvement of the lower respiratory tract in the inflammatory process (72 %), which causes a longer duration of disease (95 %) compared to healthy children. The children with allergic rhinitis and bronchial asthma had a significant decrease in the concentrations of antimicrobial peptides in the secretion of the upper respiratory tract compared with the control group. It was found that the level of antimicrobial peptide concentrations in the secretion of the upper respiratory tract is most negatively affected by the allergic inflammation severity, accompanied by a lack of symptom control, and the use of basic therapy and no symptoms have a positive effect. However, even in the presence of complete control of the symptoms of allergic diseases, the levels of antimicrobial peptides were significantly lower than in children of the control group. It was found that there is an inverse relationship between the levels of antimicrobial peptides and the frequency and duration of acute respiratory infections in children with allergic diseases, most pronounced in bronchial asthma. Conclusions. Taking a probiotic drug containing a strain of Lactobacillus rhamnosus GG for a month in addition to the basic therapy of allergic rhinitis and bronchial asthma contributed to a significant increase in levels of HbD-2 (average 27 %) and ­LL-37 (average 60 %) in secretions of the upper respiratory tract, which was accompanied by a decrease in the frequency of acute respiratory infections by an average of 26 % (including episodes involving the lower respiratory tract), a decrease in the average duration of acute respiratory infections by 22 % over the next year and a decrease in virus-induced exacerbations by 25.7 %. ; Актуальность. На сегодняшний день аллергические заболевания респираторного тракта, в частности бронхиальная астма и аллергический ринит, у детей являются актуальной медико-социальной проблемой, ассоциированной с высоким риском инвалидизации, существенным снижением качества жизни пациентов и их семей, а также значительной финансовой нагрузкой на пациентов и государство. Бронхиальная астма и аллергический ринит часто сосуществуют, обременяя течение друг друга. Дети, больные респираторными аллергическими заболеваниями, являются особенно уязвимыми к развитию острой респираторной вирусной инфекции (ОРВИ), а респираторные вирусы, в свою очередь, играют важную роль в патогенезе бронхиальной астмы и аллергического ринита, способствуя развитию заболеваний и выступая триггерами их обострений. Одной из возможных причин склонности к частым ОРВИ у детей с аллергической патологией дыхательной системы может быть недостаточность неспецифических факторов мукозального иммунитета респираторного тракта — эндогенных катионоактивных амфифильных антимикробных пептидов, в частности кателицидина (LL-37) и человеческих дефензинов. По данным ряда исследований, эти антимикробные пептиды проявляют противовирусное, антимикробное и иммуномодулирующее действие. При этом данных о состоянии продукции антимикробных пептидов и их влиянии на респираторную заболеваемость у детей с аллергическими заболеваниями в современной литературе недостаточно. В то же время в ряде литературных источников описано положительное влияние отдельных пробиотических штаммов, в частности Lactobacillus rhamnosus GG, на общую резистентность к респираторной вирусной инфекции и профилактику атопических заболеваний. Целью нашей работы стало изучение уровней антимикробных пептидов (человеческого β-дефензина 2 (HbD-2) и LL-37) в секрете слизистых оболочек верхних дыхательных путей у детей с бронхиальной астмой и аллергическим ринитом, выяснение их роли в защите от респираторной вирусной инфекции у данного контингента больных и оценка эффективности применения пробиотического препарата, содержащего Lactobacillus rhamnosus GG, с целью профилактики ОРВИ и снижения частоты вирус-индуцированных обострений аллергической патологии. Материалы и методы. Нами было проведено клинико-лабораторное обследование 76 детей в возрасте от 7 до 17 лет, из которых у 24 был диагностирован аллергический ринит, у 28 детей — бронхиальная астма, а у 24 — бронхиальная астма и аллергический ринит. Группу контроля составили 20 клинически здоровых детей соответствующего возраста и пола. Кроме общепринятых клинических, методы исследования включали анализ паттернов респираторной заболеваемости, определение уровней антимикробных пептидов LL-37, HbD-2 в секрете верхних дыхательных путей методами иммуноферментного и статистического анализа. Результаты. Установлено, что дети с аллергическими заболеваниями респираторного тракта характеризуются большей частотой ОРВИ (на 30 %) с более частым вовлечением в воспалительный процесс нижних дыхательных путей (на 72 %), что обусловливает более длительную продолжительность заболеваний (на 95 %) по сравнению со здоровыми детьми. У детей с аллергическим ринитом и бронхиальной астмой обнаружено существенное снижение уровней антимикробных пептидов в секрете верхних дыхательных путей по сравнению с группой контроля. На уровни антимикробных пептидов в секрете верхних дыхательных путей в наибольшей степени негативно влияет степень активности аллергического воспаления, сопровождающегося отсутствием контроля симптомов, а положительно — прием препаратов базисной терапии и отсутствие симптомов. Вместе с тем даже при наличии полного контроля симптомов аллергических заболеваний, то есть при минимальном уровне аллергического воспаления в дыхательных путях, уровни антимикробных пептидов были достоверно ниже, чем у детей контрольной группы. Установлено, что существует обратная связь между уровнями антимикробных пептидов и частотой и продолжительностью ОРВИ у детей с аллергическими заболеваниями, которая в наибольшей степени выражена при бронхиальной астме. Выводы. Прием пробиотического препарата, содержащего штамм Lactobacillus rhamnosus GG, в течение 1 месяца дополнительно к базисной терапии аллергического ринита и бронхиальной астмы способствовал достоверному повышению уровней HbD-2 (в среднем на 27 %) и LL-37 (в среднем на 60 %) в секрете верхних дыхательных путей, что сопровождалось снижением частоты ОРВИ в среднем на 26 % (в том числе и эпизодов с привлечением нижних дыхательных путей), уменьшением средней продолжительности ОРВИ на 22 % в течение следующего года и снижением вирус-индуцированных обострений на 25,7 %. ; Актуальність. На сьогодні алергічні захворювання респіраторного тракту, зокрема бронхіальна астма й алергічний риніт, у дітей є актуальною медико-соціальною проблемою, асоційованою з високим ризиком інвалідизації, суттєвим зниженням якості життя пацієнтів та їх сімей, а також значним фінансовим навантаженням на пацієнтів і державу. Бронхіальна астма й алергічний риніт часто співіснують, обтяжуючи перебіг одне одного. Діти, хворі на респіраторні алергічні захворювання, особливо вразливі до розвитку гострої респіраторної вірусної інфекції (ГРВІ), а респіраторні віруси, в свою чергу, відіграють важливу роль у патогенезі бронхіальної астми й алергічного риніту, сприяючи розвитку захворювань і виступаючи тригерами їх загострень. Однією з можливих причин схильності до частих ГРВІ у дітей з алергічною патологією респіраторної системи може бути недостатність неспецифічних факторів мукозального імунітету респіраторного тракту — ендогенних катіоноактивних амфіфільних антимікробних пептидів, зокрема кателіцидину (LL-37) та людських дефензинів. За даними низки досліджень, дані антимікробні пептиди проявляють противірусну, антимікробну та імуномодулюючу дію. При цьому даних щодо стану продукції антимікробних пептидів та їх впливу на респіраторну захворюваність у дітей з алергічними захворюваннями в сучасній літературі недостатньо. В той же час в низці літературних джерел описано позитивний вплив окремих пробіотичних штамів, зокрема Lactobacillus rhamnosus GG, на загальну резистентність до респіраторної вірусної інфекції та профілактику атопічних захворювань. Метою нашої роботи стало вивчення рівнів антимікробних пептидів (людського β-дефензину 2 (HbD-2) та LL-37) у секреті слизових оболонок верхніх дихальних шляхів у дітей з бронхіальною астмою й алергічним ринітом, з'ясування їх ролі у захисті від респіраторної вірусної інфекції у даного контингенту хворих та оцінка ефективності застосування пробіотичного препарату, що містить Lactobacillus rhamnosus GG, з метою профілактики ГРВІ та зниження частоти вірус-індукованих загострень алергічної патології. Матеріали та методи. Нами було проведено клініко-лабораторне обстеження 76 дітей віком від 7 до 17 років, з яких у 24 був діагностований алергічний риніт, у 28 дітей — бронхіальна астма, а у 24 — бронхіальна астма й алергічний риніт. Групу контролю становили 20 клінічно здорових дітей відповідного віку та статі. Окрім загальноклінічних, методи дослідження включали аналіз патернів респіраторної захворюваності, визначення рівнів антимікробних пептидів LL-37, HbD-2 у секреті верхніх дихальних шляхів методами імуноферментного та статистичного аналізу. Результати. Встановлено, що діти з алергічними захворюваннями респіраторного тракту характеризуються більшою частотою ГРВІ (на 30 %) з більш частим залученням у запальний процес нижніх дихальних шляхів (на 72 %), що обумовлює більшу тривалість захворювань (на 95 %) порівняно із здоровими дітьми. У дітей з алергічним ринітом і бронхіальною астмою виявлено суттєве зниження рівнів антимікробних пептидів у секреті верхніх дихальних шляхів порівняно з групою контролю. На рівні антимікробних пептидів у секреті верхніх дихальних шляхів найбільшою мірою негативно впливає ступінь активності алергічного запалення, що супроводжується відсутністю контролю симптомів, а позитивно — прийом препаратів базисної терапії та відсутність симптомів. Разом з тим навіть за наявності повного контролю симптомів алергічних захворювань, тобто при мінімальному рівні алергічного запалення в дихальних шляхах, рівні антимікробних пептидів були вірогідно нижчими, ніж у дітей контрольної групи. Встановлено, що існує зворотний зв'язок між рівнями антимікробних пептидів і частотою та тривалістю ГРВІ у дітей з алергічними захворюваннями, найбільш виражений при бронхіальній астмі. Висновки. Concept of semiotics by Theodor Adorno is comprehended in two main areas: the discovery of philosophical correlations and significance for the assessment of educational processes in modern Ukraine. The essential link of criticism of Adorno "half-education" with an understanding of goals and means of proper education is pointed out. Proper education is analyzed as effective education, which is countered by imitations of education, which appears as half-life. If secondary education is a substitute for education by its ersatzes in the form of numerous its reifications in a mass society, then half-life also appears as a substitute for effective action by accentuation on its means, which is imperceptibly, but invariably, substituting its goals. In the field of education, such half-dwellers are provided with educational services that are supposedly intended to ensure the unbreakable functioning of educational institutions, but in practice displaced from education its orientation towards the education of a person. Setting up education, constantly overcoming the existing and directing to positive as counter-faculty in the semester is replaced by the systematic implementation of the norms of consumption of educational services. In the field of legislative support of the educational sphere of Ukraine, there is a return to the definition of its tasks in terms of "educational services" and the reduction of educational tasks of the individual and the orientation of educational institutions to meet the educational needs of the learner. These negative changes became noticeable in comparison with the progressive Law of Ukraine "On Higher Education" in 2014 compared to the ambiguous Law of Ukraine "On Education" in 2017. The necessity of corresponding corrections of educational strategies, which should orient the person to effective education as the education of higher achievements rather than the fulfillment of norms, is grounded on the return of the elitist essence of education in opposition to its egalitarian character, which is taking place during the last decade. ; Концепт напівосвіти Теодора Адорно осмисле­но у двох основних площинах: виявлення філософ­ських кореляцій та значущості для оцінки освітніх процесів у сучасній Україні. Зазначено сутнісний зв'язок критики Адорно напівосвіти з осмисленням цілей і засобів здійснення належної освіти. Належна освіта проаналізована як освіта дієва, яка протистоїть імітаціям освіти, які постають як напівдії. Якщо на­півосвіта є заміною освіти її ерзацами у вигляді численних її реїфікацій у масо­вому суспільстві, то напівдія також постає як заміщення ефективної дії ак­центуацією на її засобах, які непомітно, але стійко заміщують її цілі. У сфері освіти такими напівдіями постають освітні послуги, які нібито покликані за­безпечувати незбитковість функціонування освітніх закладів, однак на прак­тиці витісняють з освіти її націленість на виховання (Bildung) особистості. Налаштування на здобуття освіти, на постійне подолання наявного і спряму­вання до позитивного як контрфактичного у напівосвіти заміщується систе­матичним виконанням нормативів споживання освітніх послуг. У сфері зако­нодавчого забезпечення освітньої сфери України спостерігається повернення до визначення її завдань у термінах «освітніх послуг» та редукції освітніх завдань особистості та орієнтації освітніх закладів на задоволення освітніх потреб того, хто навчається. Ці негативні зміни стали відчутними у порівнянні пере­важно прогресивного Закону України «Про вищу освіту» 2014 року з неодноз­начним Законом України «Про освіту» 2017 року. Обґрунтовано необхідність відповідних коректив освітніх стратегій, які мають орієнтувати особистість на дієву освіту як освіту вищих досягнень, а не виконання нормативів, на по­вернення елітарної сутності освіти на противагу її егалітаризації, яка трива­ла останнє десятиліття. 1. Purpose: studying of life quality indicators at elderly patients with osteoarthritis using international EuroQol questionnaire for evaluate of efficiency of sanatorium treatment.Materials and methods. 72 patients have been included in study at the age from 60 till 78 years, mean age was 67,6±8,7 years. Patients have been divided by method of randomization on two groups. Patients of one of groups in addition passed rehabilitation in sanatorium conditions for 18–21 days. Assessment of life quality related to health was conducted with use of international questionnaire EuroQol-5D-European Quality of Life instrument (EQ-5D).Results. At all patients with osteoarthritis health problems of varying degree have been revealed. In 12 months according to the questionnaire, reliable positive dynamics of life quality was observed at patients passing sanatorium treatment at the expense of improvement of ability to movement, reduce pain and discomfort and increase daily activity.Conclusions. Having of sanatorium stage of rehabilitation in complex treatment of osteoarthritis allows affect quality of life and keep remission for a longer period in comparison with outpatient treatment.KEY WORDS: quality of life, sanatorium treatments, osteoarthritis.Osteoarthritis (OA) is one of the most common disorders of the muscle-skeletal system, especially in the elderly age group ( in every third elderly person, reaching 70% among those who have over 65 years). During the natural aging occurs involutional changes in the connective tissue, especially in the tendons, ligaments, cartilage, bone tissue, in the walls of blood vessels, muscles [2]. Thus aging of body contributes to the accumulation of diseases. During the examination in elderly and senile patients were diagnosed from three to five different diseases. Modern elderly person is - a unique clinical phenomenon from the point of view on availability and the combination of its diverse in character and course of diseases that are competing for their prognostic significance and impact on quality of life. OA attributed to diseases with high comorbidity, and founded that patients with OA have a significantly higher risk of comorbid conditions than patients who do not suffering from OA [4].Clinical experience and numerous publications data suggest that OA is often associated with subsequent somatic disorders: hypertension, coronary heart disease, obesity, diabetes, lung diseases (chronic obstructive pulmonary disease) and gastro- intestinal tract diseases. The greatest burden on society observed in cases of combination OA and osteodeficiency (osteopenia, osteoporosis (OP)), which significantly decreses the quality of life [5].Quality of life - integrated description of physical, psychological, emotional and social functioning of a person based on its subjective perception. Assessment of quality of life at modern stage have more increasing strong position in medicine, reflecting on the one hand, the presence of new medical technologies that do not affect the life expectancy, but significantly improve its quality, and from the other hand - expanding activity of the patient, increasing of its role in choice of methods of diagnostic and treatment [9].When selecting the questionnaires it is important to consider that to be used in a clinical studying suitable only those that give results of the evaluation quality of life in a form of a single summary score from 0 to 1.0. These includes a generic questionnaire EQ-5D (EuroQol) [8]. This general questionnaire is easy to fill , widely used in different countries and gives during the processing of collected data single score to measure the quality of life, represented by values between 0 and 1, which also provides the possibility of its using in clinical trials. This questionnaire is widely used in various clinical situations , including in assessing the quality of life patients with rheumatic diseases [7].The aging process is controversial, because on the background of regression processes - atrophy, degradation, etc., develops progressive trends of creating the new compensatory-adaptive mechanisms to maintain homeostasis in an aging body, which, however, does not fully offset the growing phenomena of degradation [2]. It should be noted that the adaptive capacity of the aging body is reduced, the possibility of development various diseases increases. In this context, particularly important in cases of illness in the elderly patient is the role of sanogenetic mechanisms, their stimulation and support. A special interest belongs to the sanatorium stage of rehabilitation, whose mission is the prevention of disease progression, stimulation of compensatory capacity of the muscle-skeletal system and the possible restoration of joint function.Spa treatment has a special place in the treatment and preventive care of elderly patients , as a stage in the system of rehabilitation of many chronic diseases. Multi-disciplinary nature of medical rehabilitation in spa conditions, a wide range of rehabilitation methods can embrace patients of all age groups with the most common diseases [1].Compared with medication treatment, natural and artificial physical factors, when they are properly used, characterized by the absence of allergies, lower incidence and severity of side effects, the ability of positively influencing on the number of pathological processes and the whole body, thus helping to improve the quality of life and are important in the prevention of premature aging [2]. In the resort conditions further rehabilitation is indicated for patients with initial stages of OA, disabled (groups I and II), patients with resistant synovitis and comorbidity with the possibility of self-servicing (including general contraindications for a spa treatment) [3].The aim of the study. To investigate the quality of life in elderly patients with osteoarthritis using international EuroQol questionnaire to evaluate the effectiveness of spa treatment.Materials and methods. In study were included 72 patients aged from 60 to 78 years, middle age was 67,6 ± 8,7 years. Among the patients predominates women - 88.4 %. I radiographic stage of osteoarthritis by J.H. Kellgren-Lawrence [ 6] was diagnosed in 23,3 % of patients , II stage - in 76,7 %. In the view of modern geriatric approaches applying of the physical factors in patients with OA who were in the spa rehabilitation stage in the appointment of balneotherapeutic procedures we gave preference to ultrasonic inhalation of mineral waters and baths , from physiotherapy usually prescribed magnetic-lazer therapy, interferential therapy, patients also performed massage and physical rehabilitation . In order to prevent climate-adaptational and reaclimate-adaptational reactions and optimization process of climate-adaptation were included into the treatment complex ( based on established risk factors) adaptogens and treatment procedures that have adaptogenic action ( phytoaeroionisation , singlet-oxygen therapy). In the process of rehabilitation treatment in sanatorium conditions , we have selected the most effective combination of different methods of rehabilitation , which caused the most significant treatment effectiveness : a combination of balneotherapy , physiotherapy and exercise therapy. To improve continuity during medical rehabilitation, at discharging from the sanatorium patients were given written (in the form of special attractions ) recommendatitons about further treatment, lifestyle, physical activity, diet etc.By the method of randomization, patients were divided into two groups. A marked difference for the main source of clinical and functional parameters between the groups were not observed. The control group consisted of 14 patients of the same age without joint pathology. Treatment regimens differed between the studying groups the presence in one of the groups sanatorium stage of rehabilitation. In the first group (n=34) patients received Structum 500 mg 2 times a day ( within 6 months of the year ) and courses of NSAID, including mainly patients treated with meloxicam ( at a dose of 7,5 mg/d) or nimesulide (in dose of 100-200 mg/d) for 7-10 days during worcening. The patients of the second group (n=38) during the 18-21 day were on spa treatment ( once a year ), in the scheme of rehabilitation were: sitting hydrogen sulfide baths, a concentration of 80 mg/L for 10 minutes at a temperature of 360C , the course of 8 procedures every other day or ultrasonic inhalation by hydrogen sulfide water, period of 5 minutes; blue clay applications every other day at 260C temperature; interferential therapy and magnet-lazer therapy on the affected joints to 8 treatments alternately every other day ; pneumomassage ; classes of physical rehabilitation therapy, aromatherapy , singlet -oxygen therapy . After the sanatorium stage of rehabilitation patients are taking drugs containing chondroitin sulfate (within 6 months of the year), at worcening of NSAIDs.Assessment quality of life related to health , was conducted using an international EuroQol-5D-European Quality of Life instrument (EQ-5D), which consists of two parts. At first part the patient self- assessed his condition by 5 parameters: mobility, self-care , usual activities, pain and discomfort , anxiety and depression. In each variant patients could give three possible answers : No problem - 1 , there is some problem - 2, much of the problem - 3. In the second part of the questionnaire , patients assessed their health on a scale from 0 to 100 on the VAS, the so-called thermometer , where 0 means the worst condition , 100 - the best state of health of the patient. This part of the questionnaire is a quantitative assessment of general health.Statistical analysis of the results was done in the department of statistical system research at SHEE "Ternopil State Medical University by I. Y. Gorbachevsky Ministry of Health of Ukraine" in the software package Statsoft STATISTIC. To determine the reliability of the differences in the change of certain indicators were used parametric and non-parametric methods: criterion Wilkoksona, two-sided Fisher's criterion. A significant differences were considered when the degree of probability of error-free prognosis is (p) 95% (p <0.05). Results and discussion. General description of the health status of patients studied with OA according to EQ-5D questionnaire is given in the table.In all patients with osteoarthritis were found health problems of different severity degrees. Mostly suffers ability to travel and daily activity . This data confirms that the OA in the elderly age reduces quality of life. After 12 months, according to questionnaire, health status was different in two groups . Reliable positive trend was in patients from the second group 57,9 % (p < 0,05), as a result of improving the ability to travel in space , reducing pain and discomfort and increasing of daily activity.According to the second part of the questionnaire EQ-5D, at baseline, most patients had reduced general health assessment : the first group to 51,91 ± 1,07 ( median for «thermometer» EQ 52 points ) in the second group to 50,65 ± 1,17 ( median 48,5 points) in comparison with the best state of possible health. 12 months after the sanatorium stage of rehabilitation of patients with repeated testing were produced the following results : The average index quality of life on the " thermometer " EQ-5D questionnaire in the first group of patients was 54,76 ± 1,23 ( median - 56 points), the second group was 69,28 ± 0,89 ( median - 70 points), reliability differences is high : p < 0,001.In the category quality of life most of all patients reacted on the presence of sanatorium stage of rehabilitation in the category " movement in space " ( GPA retesting 1,60 ± 0,08, p < 0,001), " daily activity " ( GPA retesting 1 68 ± 0,07, p < 0,001), " pain / discomfort " ( GPA retesting 1,92 ± 0,08, p < 0,001), " anxiety / depression ( GPA retesting 1 52 ± 0,08, p < 0,05).Thus, the data suggested that in elderly patients with OA who were once a year on a spa treatment and took over 6 months structure-modification drugs significantly upgraded quality of life during 12 months.Conclusions1. Based on the analysis were founded that elderly patients with OA who underwent rehabilitation treatment in sanatorium- health resorts have continued improving quality of life in comparison with patients who were treated only in outpatient conditions.2. Availability sanatorium stage of rehabilitation in the treatment of elderly patients with OA contributes more complex influence on the human organism.3. Multi-disciplinary nature of medical rehabilitation in spa conditions , a wide range of used restorative treatment methods can affect the quality of life and maintain remission for more longer period in comparison with outpatient treatment.4. Monitoring the quality of life can not only control the functional state of the organism in the elderly aged people at various stages of treatment , but also to properly evaluate the effectiveness of treatment , and, if necessary, to carry out the correction of rehabilitation activities .Perspectives for future research. Aimed at studying the impact of rehabilitative and preventive measures on the quality of life in elderly patients with osteoarthritis at other stages of rehabilitation. У всіх хворих на остеоартроз виявлено проблеми зі здоров'ям різного ступеня. Через 12 місяців за даними запитальника достовірна позитивна динаміка якості життя спостерігалася у пацієнтів, що проходили санаторно-куротне лікування, за рахунок покращення здатності до пересування, зменшення болю і дискомфорту та збільшення повсякденної активності.Висновки. General description of the health status of patients studied with OA according to EQ-5D questionnaire is given in the table.In all patients with osteoarthritis were found health problems of different severity degrees. Mostly suffers ability to travel and daily activity . This data confirms that the OA in the elderly age reduces quality of life. After 12 months, according to questionnaire, health status was different in two groups . Reliable positive trend was in patients from the second group 57,9 % (p < 0,05), as a result of improving the ability to travel in space , reducing pain and discomfort and increasing of daily activity.According to the second part of the questionnaire EQ-5D, at baseline, most patients had reduced general health assessment : the first group to 51,91 ± 1,07 ( median for «thermometer» EQ 52 points ) in the second group to 50,65 ± 1,17 ( median 48,5 points) in comparison with the best state of possible health. 12 months after the sanatorium stage of rehabilitation of patients with repeated testing were produced the following results : The average index quality of life on the " thermometer " EQ-5D questionnaire in the first group of patients was 54,76 ± 1,23 ( median - 56 points), the second group was 69,28 ± 0,89 ( median - 70 points), reliability differences is high : p < 0,001.In the category quality of life most of all patients reacted on the presence of sanatorium stage of rehabilitation in the category " movement in space " ( GPA retesting 1,60 ± 0,08, p < 0,001), " daily activity " ( GPA retesting 1 68 ± 0,07, p < 0,001), " pain / discomfort " ( GPA retesting 1,92 ± 0,08, p < 0,001), " anxiety / depression ( GPA retesting 1 52 ± 0,08, p < 0,05).Thus, the data suggested that in elderly patients with OA who were once a year on a spa treatment and took over 6 months structure-modification drugs significantly upgraded quality of life during 12 months.Conclusions1. Based on the analysis were founded that elderly patients with OA who underwent rehabilitation treatment in sanatorium- health resorts have continued improving quality of life in comparison with patients who were treated only in outpatient conditions.2. Availability sanatorium stage of rehabilitation in the treatment of elderly patients with OA contributes more complex influence on the human organism.3. Multi-disciplinary nature of medical rehabilitation in spa conditions , a wide range of used restorative treatment methods can affect the quality of life and maintain remission for more longer period in comparison with outpatient treatment.4. Monitoring the quality of life can not only control the functional state of the organism in the elderly aged people at various stages of treatment , but also to properly evaluate the effectiveness of treatment , and, if necessary, to carry out the correction of rehabilitation activities .Perspectives for future research. Aimed at studying the impact of rehabilitative and preventive measures on the quality of life in elderly patients with osteoarthritis at other stages of rehabilitation. У дослідження було включено 72 пацієнти віком від 60 до 78 років, середній вік склав 67,6±8,7 року. Методом рандомізації пацієнтів було розподілено на дві групи. Пацієнти однієї з груп додатково проходили реабілітацію в умовах санаторію протягом 18-21 дня. Оцінка якості життя, пов'язаного зі здоров'ям, проводилась з використанням міжнародного запитальника EuroQol-5D-European Quality of Life instrument (EQ-5D).Результати. У всіх хворих на остеоартроз виявлено проблеми зі здоров'ям різного ступеня. Через 12 місяців за даними запитальника достовірна позитивна динаміка якості життя спостерігалася у пацієнтів, що проходили санаторно-куротне лікування, за рахунок покращення здатності до пересування, зменшення болю і дискомфорту та збільшення повсякденної активності.Висновки. Without a doubt agriculture sector is vital part of modern Ukrainian economy. Agriculture is ranked 3rd largest contributor to the national GDP with approx. 17% in 2018 (compared to 14.4% in 2016) of total GDP, followed with industrial sector (26.3%) and services sector (59.3%). It should be noted that it is very promising field in terms of potential economic growth and both regional and international investment opportunities. Food products being a necessity in everyday life of every person. Food and processing industries are the large industries that are interconnected to agricultural cultivation. These industries have great potential to increase the value of agricultural products due to the added value, as not raw materials are sold but finished products that can be consumed in the domestic market and exported. Meanwhile the global process of urbanization and changes in national politics, have had their impact on Ukrainian economy. The number of urban populations is continuing to increase, while great number of work force is migrating to neighboring countries. Several studies state that number of Ukrainian workers living aboard is around 2.5 million people. In this context we plan to investigate and build general forecast model on how urbanization is impacting agricultural land use and certain factors of its impact on economic development of region. On the other hand, we have examples of modern issues, such as climate change caused by rapid urbanization and the irrational use of natural resources. The study showed that urban development requires substantial land rescue for its expansion. By examining closely spatial data of urban and suburban areas, for example that of Kyiv city, agricultural land in suburban areas is used for new construction sites and infrastructural development. Another indirect effect of urban development is ecological changes to the surrounding areas. The affected areas become much less suitable for agricultural uses, require large financial investment and high technologies to renew the soil fertility.Research work that have been conducted in this area focused mostly on financial resources, international politics, and large agricultural holdings of agricultural land use. A lot of work has been done to highlight importance of advocating open land market in Ukraine. While other researchers in their research analyzed specific crop or certain agricultural industry markets. In general, the focus of previous publications was either on the current agricultural export or land use data in Ukraine. The goal of this research paper is to find, establish connections between urbanization and agricultural land use, while focusing attention on possibilities for economic growth and development of effective land use policies. This paper will consider open land market as future possibility; however, it is relations and impact with urbanization falls outside of current research work scope.With this research we plan to describe the connection between rapid urbanization process and changes in agricultural land use from economical perspective. The author focuses his research work and economical modeling on case Ukraine. We plan to showcase changes of agricultural enterprise income, in case of population migration to large urban centers. Additionally, we will study the cases of effective land resource management, where we will study whether it is more profitable for companies to establish the new enterprise closer or inside urban areas as opposed to farmland regions. In this research we plan to highlight the major economical differences of company's startup and operations costs based on available transportation network, land resources and local agricultural specialization. The research work should introduce possible outcomes for small, medium companies, also separating those that are part of international corporate structure and local business. The research will cover whether local company can benefit from urbanization and what are its effects on individual famers earnings. One of the expected research outcomes is to describe how to increase added value of agricultural products with focus on city markets.For this study we used data mining and analytics approaches. During research work we placed great emphasis on determining, locating, and analyzing statistical data from multiple sources. Important art of the research work is identifying right data in context of our research, the one that has clear connection to urbanization process. Spatial data visualization was used to determine key urban centers in Ukraine, as well as locations of agricultural, food and processing companies. Other types of material used are official notional statistics data, European Union public reports, U.S. Department of Commerce datasheets, Kyiv city council, Municipal Enterprise "Kyivgenplan", private companies report, economists' publications, and Public cadastral map of Ukraine. Mathematical framework was used to build robust forecast model. The methodology of the study employed various mathematical tools, such as statistical analytics, polynomial and linear approximations, mathematical correlation, ratio analysis. Besides profit and utility curves were used for growth modeling and global forecast. Spatial data have been outlined as highly effective for predictions modeling and establishing connections with the existing urbanization growth models. Urbanization processes serve as catalyst for land use change and shift in economic activities . As more population migrate to urban centers, remote villages and towns shrink in size, lose work force and often local enterprise become bankrupt or shut down. There is a consequent indirect effect of urbanization on agriculture land use that is when local community become poor or bankrupt, the farmlands are rented by large agricultural holdings. Many researchers noted that agricultural holdings have no interest in development of local communities or investment in local business. Even though they employ local people, the average income of holdings farm employees is much lesser, then those farmers whose lands are in private use or those working in agricultural sector in neighboring European Union countries. The average salary is important factor when looking at land use by large companies since agricultural holdings business goals centered about exporting raw resources and they are registered as business entities in offshore countries.The scope of current research works is limited to the analyses of medium to large size food and processing companies, that operate in local Ukrainian market, with brief overview of food products export business. To build economical model the existing transport infrastructure in Ukraine and its neighboring European countries was studied and used in system analyses. Author analyzed available human resources, amount of investments, industry type and available transportation connections. As a part of research work, physical geographical placement (distance by road) of food and processing production companies relative to closest and regional urban center positions have been outlined. A large array of economical and spatial data has been processed. It was determined that parts of this data have low correlation levels, which makes it difficult to include for modeling purposes. Author made decision to make the necessary edits, generalization to simplify the initial economical model.To build a research model we will make the following assumptions and data sets. Let us use the city of Kyiv as the main market for food products, i.e. it is the largest city in Ukraine, with a huge number of food products consumers and significant financial resources. We shall label it as an urban center for this research work. It is important to note that new companies and international corporations choose to establish their business in the Kyiv or in the 50-kilometer zone from it. For modeling we will choose 12 enterprises, which are located both in the capital and in different regions of Ukraine - west, south, east and center. We rank the selected enterprises by the number of employees.In the context of our economic modeling, consider the land market, namely: the average cost of rent of 1 hectare per year, the location of the enterprise in an industrial region or in a separate settlement. Note that the production capacity of the investigated enterprises is concentrated in one production site (complex). However, the exception is the corporation "Milk Alliance" which has 5 separate production facilities, we will focus on the largest of them, located in the city of Yagotyn (others are located in Peratyn, Zolotonosha, Bashtanka).It is reported that the total area of land plots in Ukraine is 60.4 million hectares, of which 42.4 million hectares are agricultural land (32 million hectares are cultivated per year), the area of built-up land plots is 2550.4 thousand hectares. Land for industrial purposes, which includes the food industry is 224.1 thousand hectares. The cost of annual lease of land in Ukraine for 2019 population 3518 UAH/hectare per year.At the same time, the statistics by region are as follows: Kyiv - 2694 UAH per year; Kyiv region - 3474 UAH per year; Zhytomyr and region – 4000 UAH per year; Mykolaiv Oblast – 4103 UAH per year; Kharkiv and oblast – 3152 UAH per year; Odessa region – 4065 UAH per year; Lviv region - 4172 UAH per year; Donetsk oblast - 1832 UAH per year. It is planned that in 2020 the total revenues to local budgets from land rent will amount to 185,714,285 UAH per year.To understand the general picture of the relationship and economic performance of enterprises located in Kyiv and different regions of Ukraine, we shall consider Kyiv as a self-sufficient object, both in terms of products sales and production. The largest food retailers have opened more than 575 stores in Kyiv as of 2018, and more than 60 new stores are opened each year. The production of food, beverages and tobacco by Kyiv enterprises is 46.6% of the total industrial production, which employs 1,730,000 people (7.4% of total in Kyiv), and the average wage in industry is from 16,511 UAH/month in 2019. In total, there are 9069 industrial enterprises in the capital, which employ 278,863 people, the volume of products sold by industrial enterprises of Kyiv in 2019 amounted to 892,1765,46.2 thousand UAH. At the same time, there are 7351 natural persons-entrepreneurs operating in the capital (11,877 people are employed, the volume of sold products is 5,622,011.9 thousand UAH) in industry, including the volume of products sold by food industry enterprises – 125,140,998.9 thousand UAH according to 2019 data.It should be noted that in Kyiv the main industrial capacities of food and processing enterprises are concentrated in the following areas: Industrial zone " Degtyarivska Street" covers an area of 17.7 hectares; Podilsko-Kurenivskyi industrial district - 539.5 hectares, Pirogovo industrial zone - 121.9 hectares; Voskresensky industrial district - 107.2 hectares.For economic analysis we focused on comparison of three large enterprises of Ukraine, typical for the agricultural sector and food industry, one of which is in Kyiv – "Astrata", "Sandora" and "Kyivhlib". For calculations of transportation costs let's use the following data, 20 to 22 tons van will charge 27 UAH per km outside of Kyiv, while 10 tons van charges 20 UAH per km outside of Kyiv and 250 UAH per km within Kyiv.The "Astrata" corpopration, a large agricultural enterprise, has 5,470 employees, 230,000 hectares of land (aprox. 1514090000 UAH per yar for land rent), with Net profit of 12,631,155,000, Gross profit of 2,432,488,000 UAH, and a total salary expenditure of UAH 427,597,000 (average per employee 6514 UAH per month). For our research we are using one of the sugar processing factories that is part of Astrata holding, it is in Hlobyne, Poltavska oblast. The distance from Hlobyne to Kyiv center is 287 km, so it will cost 7749 UAH per large van to transport products to Kyiv.One of the largest food manufacturers in Ukraine is "Sandora". It consists of 3230 employees, total salary expenses - 630 128 thousand UAH (average per employee 16,257 UAH per month), have a land area of 1.3646 hectares (aprox. 5600 UAH per yar for land rent) and a unit cost of production - 22.66 UAH per liter of apple. Single 10-ton van will charge 10020 UAH to transport products to Kyiv (501km distance to the city center).For Kyiv study we picked "Kyivhlib", which has 1911 employees, total salary expenses – 168,227 thousand UAH (average per employee 14,019 UAH per month), have a land area of 8 hectares (aprox. 21552 UAH per yar for land rent) and the unit cost of production - 14.9 UAH per kg of rye-wheat bread. It will cost 7500 UAH to transport products within 30 km of Kyiv center by 10-ton van. Distance from the production location to the market is not always proportional to net profit. The research results have proved that it is worth examining the data from the same sub-industries companies in several regions, with focus on large cities as main market for those companies' products. ; The paper is devoted to studying the economic and social connections between the urbanization process and agricultural land use in Ukraine. It is worth noting that both urbanization and agriculture require new lands for their future development. The important part of this connection is ecology and effective use of the available resources, as well as land use in the context of urbanization. After all, the process of urbanization can have a significant negative impact on the state of land resources. The research work determines which economic factors of urbanization are the most relevant to the land use in city suburbs and agricultural regions. The article introduces the notion of economic feasibility of land use by purpose and studies cases of food and processing companies. Food and processing industries were chosen as the main research subjects since they are the most promising fields for the future economic development of each individual region and country as a whole. These industries are centered around both import and export, besides producing added value products. Cities without a doubt are the main consumers of food products and at the same time, they drain labor resources from the countryside. In this research work, we studied the global situation in Ukraine, how 21-century urbanization has affected the agricultural sector in the country. This sector has experienced rapid growth in past decades, as opposed to industrial and manufacturing sectors, and increased the national wide level of land use. While the national wide level of urbanization might not be the highest in modern history (20th to 21st century), it continues to have a wide impact on the national economy. The scope of this impact falls outside of this research work as it consists of multidimensional data and a wide range of interdependencies, including policies and regulations. Research models require a large amount of data and cases, that's why we focused on the food and processing sector in this paper. They proved to be a good test ground to study the urbanization impact patterns as well as make economical modeling more convenient. In this context, the peculiarities of land use models were studied, as food and processing industries use land resources, can be located both inside the city, suburban area, or in remote farmland region. It is evident that cities and businesses form large supply and demand of natural resources, labor, and financial investments.
