Walter Benjamin, Carl Schmitt, then Giorgio Agamben questioned the idea of norm in the State faced with violence, together with the question of exception. Now the state of exception comes down to dealing juridically with political matters and ignoring what really or illusorily triggers off the state of exception, namely war. A proposition to grasp both the difficulty for law to fit to the sudden appearance of violence and the outburst of the various forms of war listed in history, consists in leaving aside the idea of juridical fiction and replacing it by that of "political fictions". Thus the notion of norm will be substituted with that -- more appropriate to describe violence -- of "reference" to standard war to assess real wars. Malgré de nombreuses études sur la référence/évacuation, elle demeure toujours confrontée à de nombreux défis d'ordre organisationnel. Il s'agissait d'une étude rétrospective descriptive allant du 1 er janvier 2012 au 31 Décembre 2013 dont l'objectif était d'évaluer le système de référence/évacuation dans le service de gynécologie obstétrique du centre de santé de référence de la commune II du district de Bamako. Il s'agissait d'un échantillonnage systématique exhaustif portant sur toutes les patientes évacuées ou référées ayant un problème obstétrical reçues et prises en charge dans notre service soient 503 patientes. Au cours de cette étude, sur 6574 patientes admises dans notre service, 503 soient 7,65% de nos patientes étaient référées ou évacuées. La tranche d'âge de 14 à 19 ans était la plus représentée soit 62,50%. Les femmes au foyer étaient les plus représentées avec 42,3% des cas. La dilatation stationnaire était le motif le plus fréquent de référence/évacuation. La qualification de l'agent référent/évacuant n'était pas connue dans 49,1% des cas. Le moyen de transport le plus utilisé était l'ambulance dans 71,0% des cas. Il y avait une discordance entre le motif de référence/évacuation et le diagnostic retenu au CSRéf de la commune II dans 55,5%. L'accouchement s'est effectué par voie basse dans 61,4% des cas. Nous avons enregistré 2 cas de décès maternel soit 0,4% dont les causes étaient l'allergie au produit anesthésique et l'hémorragie du postpartum immédiat et 9 cas de décès périnatal soit 1,9% des cas. La référence/évacuation, maillon essentiel de la réduction de la mortalité maternelle et néonatale connait encore de nombreux problèmes dans sa réalisation malgré l'existence d'une législation bien claire en la matière. La nécessité d'une prise de conscience de tous les acteurs s'impose afin d'améliorer le pronostic materno-foetal. ; Despite numerous studies on referral/evacuation, it still faces many organizational challenges. This was a retrospective descriptive study from January 1 st , 2012 to December 31, 2013, the purpose of which was to evaluate the referral/evacuation system in the Gynecology and Obstetrics Department of the Reference Health Center of commune II of Bamako district. This was a comprehensive systematic sampling of all patients with obstetric problems received and managed in our department. In this study, 7.65% of our patients were referred or discharged. The 14 to 19 age group was the most represented at 62.50%. Housewives were the most represented with 42.3% of cases. Stationary dilatation was the most common pattern of reference/evacuation. The qualification of the referring/ evacuating agent was not known in 49.1% of cases. The most common mode of transportation was ambulance in 71.0% of cases. There was a discrepancy between the reference / evacuation pattern and the diagnosis retained in the Reference Health Center of commune II in 55.5%. In 61.4% of the cases the delivery was carried out vaginally. We recorded 2 cases of maternal death (0.4%), the causes of which were allergy to the anesthetic product and bleeding, and 9 cases of perinatal death (1.9% of cases). The reference-evacuation, essential link of the reduction of the maternal and neonatal mortality still knows many problems in realization despite the existence of a very clear legislation in the matter. The need for an awareness of all actors is necessary to improve the maternal-fetal prognosis.
