Maģistra darbā "Noziedzīgu nodarījumu recidīvs" apskatīta viena no nozīmīgākajām problēmām mūsdienās - objektīva sociāla parādība, kura apdraud sabiedrības intereses un drošību. Darba galvenais mērķis ir sniegt padziļinātu izpratni par noziedzīgu nodarījumu recidīva jēdzienu, analizēt pastāvošās problēmas recidīva noteikšanā un izmantošanā, raksturot recidīva rādītājus, izstrādāt priekšlikumus par nepieciešamajām izmaiņām Latvijas normatīvajos aktos. Darbs sastāv no trīs nodaļām: pirmajā nodaļā galvenā uzmanība tiek pievērsta recidīva jēdziena vēsturei, veidiem un recidīva pazīmju attīstībai, lai radītu vispārīgu izpratni par to, kā dažādu laika periodu tiesību aktos valsts ir centusies radīt instrumentus, ar kuru palīdzību izdotos veikt noziedzīgu nodarījumu recidīva prevenciju. Otrajā nodaļā apskatīti valsts radīti noziedzīgu nodarījumu recidīva novēršanas instrumentu noteiktie mērķi, kas sekmēs kriminālsodu mērķa sasniegšanu un virzienu, kurā valsts attīsta krimināltiesības kopumā. Šajā nodaļā tiek pievērsta uzmanībā arī dzimumnoziedznieku recidīva problemātikai, kuras kaitējums ietekmē ne tikai cietušo, bet sabiedrību kopumā. Trešajā nodaļā tiek analizēta recidīva reglamentācija un piemērošana arī citu valstu krimināltiesībās. The main goal of the work is to provide the deepen conception of the recidivism, to analyse the existed problems of recidivism definition and use, to describe the recidivism rates, to develop proposals for the necessaries changes in Latvian legislation. The work consists of three chapters: the first chapter focuses on the concept of relapse history, types and characteristics of recurrence development in order to create a general understanding of how the different periods of time the state law has been trying to create tools with which to succeed in crime prevention. The second chapter reviews the state created crime relapse prevention tool set goals that will enhance criminal objective and the direction in which the state develops the criminal law. This chapter also draws attention to the problem of sex offender recidivism damage affects not only to the victim, but society as a whole. The third chapter analyzed relapse regulation and enforcement of the criminal laws of other countries. BACKGROUND: A number of treatments can help smokers make a successful quit attempt, but many initially successful quitters relapse over time. Several interventions have been proposed to help prevent relapse. OBJECTIVES: To assess whether specific interventions for relapse prevention reduce the proportion of recent quitters who return to smoking. SEARCH METHODS: We searched the Cochrane Tobacco Addiction Group trials register, clinicaltrials.gov, and the ICTRP in February 2018 for studies mentioning relapse prevention or maintenance in their title, abstracts, or keywords. SELECTION CRITERIA: Randomised or quasi‐randomised controlled trials of relapse prevention interventions with a minimum follow‐up of six months. We included smokers who quit on their own, were undergoing enforced abstinence, or were participating in treatment programmes. We included studies that compared relapse prevention interventions with a no intervention control, or that compared a cessation programme with additional relapse prevention components with a cessation programme alone. DATA COLLECTION AND ANALYSIS: We used standard methodological procedures expected by Cochrane. MAIN RESULTS: We included 77 studies (67,285 participants), 15 of which are new to this update. We judged 21 studies to be at high risk of bias, 51 to be at unclear risk of bias, and five studies to be at low risk of bias. Forty‐eight studies included abstainers, and 29 studies helped people to quit and then tested treatments to prevent relapse. Twenty‐six studies focused on special populations who were abstinent because of pregnancy (18 studies), hospital admission (five studies), or military service (three studies). Most studies used behavioural interventions that tried to teach people skills to cope with the urge to smoke, or followed up with additional support. Some studies tested extended pharmacotherapy. The present report concerns a case of pulmonary nocardiosis in an immunocompetent host. This patient was diagnosed as having smear positive pulmonary tuberculosis and received supervised antitubercular treatment for 6 months from a government run tuberculosis centre (Directly Observed Therapy, Short-Course (DOTS) centre). At 3 months after completion of treatment, she presented with fever and cough with posterior–anterior (PA) view chest x ray showing a cavitary lesion on left upper zone. She was subsequently diagnosed as having a case of pulmonary nocardiosis and responded to oral cotrimoxazole.

In the statistical calculation of the National Narcotics Board of the Republic of Indonesia or so called BNN, there were 70% of drug offenders with the status of drug relapse in 2019 reached (BNN: 2019). Such condition illustrates that the social rehabilitation mechanism for drug offenders in Indonesia still has the opportunity to present shameless people. Therefore, this study is intended to observe the significance of Lifestyle Related Shaming as an effort to reduce the number of drug relapses in Indonesia. Lifestyle Related Shaming is supported by 3 (three) macro theories, namely reintegrative shaming theory proposed by John Braithwaite, desistance theory proposed by Farral and routine activity theory proposed by Hiddlenlang The literature review method was applied here through the utilization and effort to develop an existing theory (grounded theory) by presenting certain concepts (variables) in accordance with the affordability of study data. In addition to qualitative approach, this research also performed a quantitative approach through survey method among respondents who experienced drugs relapse in Indonesia as well as in-depth interviews with drug relapse residents with various backgrounds and educational levels at the Center for Drug Rehabilitation of the Republic of Indonesia at Lido Bogor. It is expected that this study may provide a broad understanding to the government to prepare the best program for solving the drug relapse phenomenon, especially drug abuse in Indonesia. For the science, this study is expected to be a basis for the implementation of shaming in Indonesia. Furthermore, this study is also expected to be able to provide input in the process of drug relapse management in Indonesia, especially in an effort to prevent residents from falling into a shameless condition.

This study aims to identify a relationship between the personal factor of coping; the interpersonal factors of familial, friend, and societal support and the inclination to relapse. This study involves the participation of 169 former addicts that completed their treatment and rehabilitation period, by utilising four instruments: the Inventory of Drug-Taking Situations, (IDTS) by Annis and Martin (1985); Coping Strategy Inventory (COPE) by Carver and the Social Provisions Scale (SPS) by Russell and Cutrona (1984). The findings shows the inclination to relapse among former addicts are at a high level for the eight dimensions of the inclination to relapse. This means the participants of this study are at a high risk of relapsing into drug use. The descriptive analytical results towards the problem-focused coping, the emotional coping and evasive coping variables show it at a low level. This also applies to the analysis for the family, friend, and societal support variables. In addition, the correlation analysis for the variables of problem-focused, emotional, and evasive coping, selfefficacy, and family, friend, and societal support, shows a negative significant relationship with the inclination to relapse, with the relationship strength between r=-.60 and r=-.80. Therefore this study shows the importance of the personal and interpersonal factors in reducing the problem of inclination to relapse among former addicts that completed their treatment and rehabilitation period. In conclusion, results show the importance of the personal and interpersonal factor in the problem of inclination to relapse.

This article belongs to the Special Issue Dietary Patterns, Dietary Indices for Colorectal Cancer, Mortality and the Related Disease. ; Cancer mortality rates are undergoing a global downward trend; however, metastasis and relapse after surgery and adjuvant treatments still correlate with poor prognosis and represent the most significant challenges in the treatment of this disease. Advances in genomics, metabolomics, and proteomics are improving our understanding regarding cancer metabolic diversity, resulting in detailed classifications of tumors and raising the effectiveness of precision medicine. Likewise, the growing knowledge of interactions between nutrients and the expression of certain genes could lead to cancer therapies based on precision nutrition strategies. This review aims to identify the recent advances in the knowledge of the mechanistic role of bioactive phytochemicals in foodstuffs in tumor progression, metastasis, and chemo-resistance in order to assess their potential use in precision nutrition therapies targeting relapse in lung, breast, colon, and prostate cancer, and leukemia. A considerable number of bioactive phytochemicals in foodstuffs were identified in the literature with proven effects modulating tumor growth, progression, and metastasis. In addition, the use of foodstuffs in cancer, and specifically in relapse therapies, is being reinforced by the development of different formulations that significantly increase the therapeutic efficiency of these products. Abstract Two Plasmodium vivax recurrences in a Peruvian sailor with weight above the 60 kg (cap for primaquine dosage) highlight the importance of adequate radical cure weight dosage for patient treatment and control efforts, particularly within the military.

Countries that have experienced civil war suffer a greater risk for new conflict than countries with no prior history of civil war. This empirical finding has been called a conflict trap where the legacy of previous war - unsolved issues, indecisive outcomes, and destruction – leads to renewed fighting. Yet, countries like Cambodia, El Salvador, Indonesia, and Mozambique have managed to overcome decade-long conflicts without relapse. This dissertation addresses this empirical puzzle by seeking to dismantle the conflict trap and look at microlevel explanations for civil war resolution and relapse. It adds to existing scholarship in three ways: first, by using disaggregated empirics on war termination and how fighting resumes; second, by exploring government agency in conflict processes; and third, by disaggregating rebel organizations. Essay I present original data on the start and end dates and means of termination for all armed conflicts, 1946-2005. Contrary to previous work, this data reveal that wars does not always end through victory or peace agreement, but commonly end under unclear circumstances. Essay II addresses how developments exogenous to the conflict influence governments' decision to engage in a peace process. The results show that after natural disasters when state resources need to be allocated towards disaster relief, governments are more willing to negotiate and conclude ceasefires with insurgents. Essay III focuses on the post-conflict society, and posits that security concerns among former war participants will push them towards remobilizing into rebellion. The findings indicate that if ex-belligerent elite's security is compromised, the parties of the previous war will resume fighting, while insecurity among former rank-and-file leads to the formation of violent splinter rebel groups. Finally, Essay IV seeks to explain why governments sometimes launch offensives on former rebels in post-conflict countries. The results show that internal power struggles provide leaders with incentives to use force against domestic third parties to strengthen their position against intra-government rivals. Taken together, this dissertation demonstrates that there is analytical leverage to be had by disaggregating the processes of violence in civil war and post-conflict societies, as well as the actors involved – both the government and rebel sides.

Background UK NHS Stop Smoking Services provide cost effective smoking cessation interventions but, as yet, there has been no assessment of their provision of relapse prevention interventions. Methods Electronic questionnaire survey of 185 UK Stop Smoking Services Managers. Results Ninety six Stop Smoking Service managers returned completed questionnaires (52% response rate). Of these, 58.3% (n = 56) ran NHS Stop Smoking Services which provided relapse prevention interventions for clients with the most commonly provided interventions being behavioural support: telephone (77%), group (73%), and individual (54%). Just under half (48%, n = 27) offered nicotine replacement therapy (NRT), 21.4% (n = 12) bupropion; 19.6% (n = 11) varenicline. Over 80% of those providing relapse prevention interventions do so for over six months. Nearly two thirds of all respondents thought it was likely that they would either continue to provide or commence provision of relapse prevention interventions in their services. Of the remaining respondents, 66.7% (n = 22) believed that the government focus on four-week quit rates, and 42.9% (14 services) believed that inadequate funding for provision of relapse prevention interventions, were major barriers to introducing these interventions into routine care. Conclusions Just over half of UK managers of NHS Stop Smoking Services who responded to the questionnaire reported that, in their services, relapse prevention interventions were currently provided for clients, despite, at that time, there being a weak evidence base for their effectiveness. The most commonly provided relapse prevention interventions were those for which there was least evidence. If these interventions are found to be effective, barriers would need to be removed before they would become part of routine care.

Recently, a high proportion of patients with late-stage Trypanosoma brucei gambiense trypanosomiasis, who had been treated with melarsoprol in some disease-endemic areas, subsequently relapsed. To determine whether the frequency of postmelarsoprol relapses increased over time, we reviewed data from 2,221 trypanosomiasis patients treated with melarsoprol during this period in Nioki, Democratic Republic of Congo, from 1982 to 2001. The frequency of relapses was 5.6%(31/553), 6.8%(35/512), 4.5%(18/398), 11.4%(34/299), and 5.0%(17/343) for those treated from 1982 to 1985, 1986 to 1989, 1990 to 1993, 1994 to 1997, and 1998 to 2001, respectively. The higher frequency of relapses in 1994 to 1997 was associated with an incremental dosage regimen of melarsoprol. In multivariate analysis, after adjustment for treatment regimen, sex, residence, and trypanosomes in cerebrospinal fluid, postmelarsoprol relapses did not increase in Nioki, perhaps because 1) little drug pressure exists; 2) subtherapeutic doses have rarely been administered; 3) little potential exists for the preferential transmission of melarsoprol-resistant strains.

Abstract Background The purpose of this study was to determine the direct medical US cost of managing multiple sclerosis relapses. Methods Direct data analysis and cost modeling were employed to derive typical resource use profiles and costs in 2002 US dollars, from the perspective of a third-party payer responsible for comprehensive health-care. The location and scope of health care services provided over a 90-day period were used to define three levels of relapse management. Hospitalization and resulting subsequent care was defined as high intensity management. A medium level of intervention was defined as either use of the emergency room, an observational unit, or administration of acute treatments, such as intravenous methylprednisolone in an outpatient or home setting. The lowest intensity of care comprised physician office visits and symptom-related medications. Data were obtained from many sources including all payer inpatient, ambulatory and emergency room databases from several states, fee schedules, government reports, and literature. All charges were adjusted using cost-to-charge ratios. Results Average cost per person for high management level was $12,870, based on analysis of 4,634 hospital cases (mean age 48 years, 73% female). Hospital care comprised 71% of that cost. At discharge, 36% required inpatient sub-acute care, rehabilitation or home care. The typical cost per moderate episode was $1,847 and mild episode $243. Conclusions Management strategies leading to a reduction in the frequency and severity of a relapse, less reliance on inpatient care, or increased access to steroid infusions in the home, would have a substantial impact on the economic consequences of managing relapses.

Up to 70% of women who quit smoking relapse after birth, usually within 3 months postpartum. The wide adoption of mobile technologies, especially smartphones, in recent years in low- and middle-income countries (LMICs) offers the possibility of low-cost, novel, and innovative mobile phone-based interventions for smoking relapse prevention. This study presents the protocol of the RESPREMO clinical trial for postnatal smoking relapse prevention for enrolled women, who recently gave birth and quit tobacco smoking before or during pregnancy, and their life partners. This work relies on data collected in two of the largest government-owned obstetrics and gynecology clinics in Cluj-Napoca, Romania. Seventy-five couples were randomized into one of three groups: a) 24 couples were allocated to the first intervention group and asked to install and use the xSmoker app; b) 26 couples were randomized to the second intervention group, who, in addition to the use of the xSmoker app, received text messages with content focused on motivation, problem solving, and dyadic efficacy; and c) 25 couples were randomized into a control group. Several measures of engagement with the xSmoker app were assessed, including duration of app use, the frequency of utilizing the tool to calculate savings from quitting, number of app-delivered challenges accepted by users, and number of app-delivered cessation and abstinence tips. If effective, RESPREMO is expected to have a sustainable impact on the prevention of postnatal relapse tobacco smoking with positive effects for both the mother and the newborn. The implications are beyond tobacco control, and relevant to the design and implementation of other mHealth behavioral interventions focused on the pregnancy and reproductive years in general.

Plasmodium vivax parasites have a unique dormant stage that can cause relapses weeks or months after the initial infection. These dormant parasites are among the main challenges of vivax malaria control as they constitute a reservoir that is difficult to eliminate. Since field studies are confounded by reinfections and possible recrudescence of drug-resistant parasites, most analyses of P. vivax relapses have focused on travelers returning from regions of malaria endemicity. However, it is not clear whether these individuals accurately recapitulate the relapse patterns of repeatedly infected individuals residing in areas of endemicity. Here, we present analyses of vivax malaria patients enrolled in a tightly controlled field study in Cambodia. After antimalarial drug treatment was administered, we relocated 20 individuals to a nontransmission area and followed them for 60 days, with blood collection performed every second day. Our analyses reveal that 60% of the patients relapsed during the monitoring period. Using whole-genome sequencing and high-throughput genotyping, we showed that relapses in Cambodia are often polyclonal and that the relapsing parasites harbor various degrees of relatedness to the parasites present in the initial infection. Our analyses also showed that clone populations differed dynamically, with new clones emerging during the course of the relapsing infections. Overall, our study data show that it is possible to investigate the patterns, dynamics, and diversity of P. vivax relapses of individuals living in a region of malaria endemicity and reveal that P. vivax relapses are much more pervasive and complex than previously considered.
