Anesthesia facilitates surgery in millions of young children every year. Structural brain abnormalities and functional impairment observed in animals have created substantial concerns among clinicians, parents, and government regulators. Clinical studies seemed ambivalent; it remains unclear whether differential species effects exist towards anesthetic exposure. The current literature search and analysis attempts to unify the available clinical and animal studies, which currently comprise of > 530 in vivo animal studies and > 30 clinical studies. The prevalence of abnormalities was lowest for exposures < 1 hour, in both animals and humans, while studies with injurious findings increased in frequency with exposure time. Importantly, no exposure time, anesthetic technique, or age during exposure was clearly identifiable to be entirely devoid of any adverse outcomes. Moreover, the age dependence of maximum injury clearly identified in animal studies, combined with the heterogeneity in age in most human studies, may impede the discovery of a specific human neurological phenotype. In summary, animal and human research studies identify a growing prevalence of injurious findings with increasing exposure times. However, the existing lack of definitive data regarding safe exposure durations, unaffected ages, and non-injurious anesthetic techniques precludes any evidence-based recommendations for drastically changing current clinical anesthesia management. Animal studies focusing on brain maturational states more applicable to clinical practice, as well as clinical studies focusing on prolonged exposures during distinct developmental windows of vulnerability, are urgently needed to improve the safety of perioperative care for thousands of young children requiring life-saving and quality of life-improving procedures daily. ; open

Objectives: The purpose of this study is to figure out the current state of patient safety research in Korea and to evaluate whether the research fund support project for patient safety research is proceeding in priority order. Methods: Through the literature search, a list of research projects in the field of patient safety in Korea were collected as of January 6, 2017. Four researchers independently evaluated the subject using the priority list of patient safety research derived from the previous study. Results: As of January 2017, a total of 21 research projects were conducted in the Korea Health Industry Development Institute and 42 research studies were conducted in the National Science & Technology Information. The results of the priority evaluation showed that there were no researches that ranked first and second priority. Many of the research topics were �쁀dverse drug events/drug errors,�� �쁇ealth information technology/information systems,�� and �쁋ess relevance.�� Conclusion: Patient safety research projects are not only diverse but also quantitative in Korea. Support for research topics that need to be prioritized is needed, such as developing safety indicators and measuring patient safelty levels, improving communication, and improving patient safety incident reporting systems. ; open

This study examined the influence of the separation of prescribing and dispensing roles(SPD) policy implemented in Korea in July 2000, especially on the change in the net profit of medical institutions. Using the data set from the Korea's National Health Insurance and the previous research, this study elicited the following main results. First, tertiary care institutions was estimated to lose about 631 billion won after the SPD policy. Second, general hospitals and hospitals gained about 557 billion and 564 billion won, respectively. Third, it is shown that clinics also gained 389-659 billion won. Finally, however, the change in net profit of medical institutions after the SPD policy largely depends on different estimation models. Moreover, it also varies from the assumptions on the price differential of a reimbursable drug which worked as cross-subsidy to insufficient physician's fee before the SPD policy. Despite such limitations as lack of data outside of the National Health Insurance's coverage, this study differs from others. This is the first research to explore the effect of the SPD policy on different types of medical institutions and to attempt to purely focus on the SPD policy. In this study, we can draw the policy implication that preparing for a policy change, the government should set up the policy evaluation system to collect the concerned data and develop the methodologies in advance to the policy implementation. ; open

학위논문 (석사)-- 서울대학교 대학원 : 행정학과(행정학전공), 2012. 2. 정광호. ; 사회가 민주화되고 가치가 분화됨에 따라 각종 국책사업을 둘러싼 공공갈등이 심화되고 있다. 단순한 이익갈등뿐만 아니라 가치갈등 또는 복합갈등이 빈번하게 발생하고 있으며, 갈등범위도 전국적으로 확대되고 있다. 본 연구는 국책사업 중에서 '세종시 수정안'을 사례로 선정하여 정책결정 과정을 둘러싼 갈등에 대해 분석하고자 하였다. 특히 세종시 수정안 정책결정을 둘러싼 갈등의 배경과 성격은 무엇이고, 그 갈등을 관리 또는 해결하기 위해 정부에서 선택한 의사결정 방안이 적정하였는지 등을 참여적 의사결정 하의 갈등관리시스템의 틀로 분석하였다. 이를 통해 향후 세종시 수정안 추진과 유사한 대형국책사업 추진에 있어서의 성공적인 갈등관리를 위한 시사점을 도출할 수 있을 것으로 기대하였다. 제1장에서는 연구의 목적과 필요성, 연구의 대상과 범위에 대해서 설명하고 연구에서 사용한 사례연구방법과 자료수집방법에 대해 밝혔다. 제2장에서는 공공갈등의 개념과 유형 등 이론적 논의를 한 후, 참여적 의사결정이론을 소개하였다. 이러한 이론적 배경과 선행연구 등을 바탕으로 연구의 분석틀을 설정하였다. 제3장에서는 세종시 수정안 갈등의 발생배경과 갈등의 성격, 갈등의 쟁점과 이해관계자 분석, 갈등시기별 전개과정 등을 살펴보았다. 제4장에서는 연구의 분석틀에 따라 참여적 의사결정 요인과 정치 환경 요인에 따라 세종시 수정안 사례를 분석하였다. 주요 분석결과를 요약하면 다음과 같다. 우선 참여적 의사결정 요인에 따른 분석결과, 세종시 수정안 사례는 전반적으로 참여적 의사결정 방법에 의한 갈등관리가 미흡하였다고 평가할 수 있다. 첫째, '여론수렴 및 정보제공'이 미흡한 것으로 나타났다. 초기부터 여론수렴 및 정보제공 보다는 정부주도의 일방향적인 의사결정 추진 및 설득에 치중함으로써 갈등관리에 부정적인 영향을 미쳤다고 볼 수 있다. 둘째, '참여적 의사결정 활용' 측면 또한 미흡한 것으로 나타났다. 관련분야의 학식과 경험이 풍부한 전문가들을 민간위원으로 포함시켜 '세종시 민관합동위원회'를 구성ㆍ운영하였으나, 인적구성에서 찬성의견과 반대의견에 대한 균형을 이루지 못하여 활동결과에 대한 신뢰도를 저하시키는 원인이 되었다. 셋째, '협력규칙의 적용 여부'를 살펴보면, 우선 참여규칙, 분해규칙 적용 측면에서 미흡하였다. 그리고 자원확대규칙 측면에서는 원안보다 큰 경제적 보상이라는 인센티브를 제공하였으나, 이해관계자들에게 정치적 신뢰를 얻지 못함으로써 큰 효과를 발휘하지 못하였다. 마지막으로 '결과의 공개 및 정책에의 반영 여부' 측면에서 볼 때, '세종시 발전방안'을 언론 브리핑 형태로 발표함으로써 결정결과의 공개 자체는 투명하게 이루어졌다고 할 수 있으나, 수정안 마련 과정이나 발표 이후 국민의견수렴 절차를 거치는 과정은 미흡했다고 볼 수 있다. 정치환경 요인에 따른 분석결과는 다음과 같다. 우선, 여당내 대통령의 리더십이 수정안 찬성집단과 반대집단으로 분절된 결과, 정책의제 설정 및 정책결정과정의 추진력이 약화되었다. 둘째, 야당이 타협의 의사 없이 세종시 수정 추진에 무조건 반대하는 방식으로 진행되어 갈등이 증폭되었다. 셋째, 핵심 어젠다 제기의 정치적 타이밍과 관련, 5년 단임제의 특성을 고려할 때 집권 2년이 다 되어 갈 무렵 세종시 수정이라는 어젠다가 공식 제기됨으로써 유리한 타이밍을 놓쳤다고 볼 수 있다. 이와 같이 불리한 환경적 요인들은 참여적 의사결정에 의한 갈등관리에 부정적인 제약요인으로 작용하였다고 볼 수 있다. 제5장에서는 이와 같은 연구결과를 요약하고, 다음과 같은 시사점을 도출하였다. 첫째, 갈등과 갈등 당사자의 특성, 갈등이슈 유형 등에 따라서 갈등관리방식의 선택 또한 달라져야 한다는 것이다. 특히, 극단적인 대립과 갈등이 예상되고 그 파급효과가 매우 큰 세종시 수정안과 같은 사례일수록, 사전에 갈등영향분석 등을 통해 갈등의 요인을 사전예측ㆍ분석하여 관리대책을 마련하고, 정책의제설정 및 정책결정과정에서 참여적 의사결정 수단의 적극적인 활용이 필요하다고 볼 수 있다. 둘째, 정책대상집단과 이해관계자에 대한 사회적 인식(social construction of target populations)에 따라 갈등관리의 구체적인 수단 또한 달라져야 한다는 점을 알 수 있다. 따라서 향후 유사한 갈등사례에 있어서 정부의 갈등관리 전략 마련시 대상집단에 대한 사회적 인식에 대한 관심을 바탕으로 유ㆍ무형의 인센티브를 적절히 활용하는 것이 필요하다고 할 수 있다. 셋째, 참여적 의사결정기법 활용을 위한 구체적인 법적ㆍ제도적 장치 마련이다. 세종시 수정안 사례에서 전반적으로 참여적 의사결정방법의 구체적인 수단 활용이 미흡했던 것은 이러한 방안을 준비하고 실행할 수 있는 제도적 뒷받침이 부족했던 점에서도 그 원인을 찾을 수 있다. ; As society is democratized and the value is divided, public conflict surrounding the various national projects is growing. Value conflicts or compound conflicts, as well as simple interest conflicts occur frequently, and conflict is expanding nationwide scope. This study selected the case of amendment of Sejong city among various national projects as a case study, and tried to analyze conflict over the decision-making process of the case. Especially, this analyzed over the background and characteristic of conflict on the amendment of Sejong city, and appropriateness of the system of conflict management selected by the government in order to manage or solve the conflict through the frame of conflict management system under participative decision-making. This promotion is similar to the amendment during the next three kinds of large national projects in promoting the success of the implications for conflict management was expected to be drawn. Through this study, it is expected to reach to an implication for the successful conflict management on the similar case of the amendment of Sejong city. Chapter 1 explains the purpose, necessity, range and target of the research, and it defines a method of case study as well as the method of collecting data that are used for the case study. Chapter 2 explains the theory of participative decision making after introducing a theoretical review such as concept and kinds of public conflict, and establishes a framework of the study by referencing previous studies. Chapter 3 reviews the background and nature of conflict on the amendment of Sejong city, conflict issues and stakeholders, and development process of conflict. Chapter 4 analyzes the case by the factors of participative decision making and the political-environmental factors through the framework. The summary of main analysis is as follows: First of all, the case of amendment of Sejong city can be evaluated that the conflict management through participative decision making is insufficient, as a result of analysis by the factors of participative decision making. First, the 'collection of public opinion and provision of information' was insufficient. From the beginning, the government focused on a one-way decision-making and persuasion rather than collection of public opinion and provision of information, and it negatively influenced on conflict management. Second, the 'use of participative decision-making' was also insufficient. Even though various experts participated in the Sejong city committee, the composition was biased. Therefore it was one of the main cause of declining the reliability of the results. Third, 'participating rule' and 'factoring rule' were not sufficiently utilized. In terms of 'resource expanding rule', the government suggested more incentive of amendment plan than one of original plan, but it was not effective because of distrust of the government. Finally, in terms of announcement of the result and feedback of policy', amendment of Sejong city was openly publicized through giving a briefing. However the process of collecting extensive opinions was insufficient. The result of analysis by means of political-environment factor is as follows: First, the leadership of President was divided into supporting groups and opposition groups. Therefore the momentum of agenda-setting and policy-making was weakened. Second, the opposition party opposed to the amendment of Sejoing city without any condition. Third, the political timing of agenda-setting was not timely. The government missed the advantageous timing because the agenda was proposed during second year of a 5-year term of presidency. These adverse environmental factors negatively effected on conflict management of participative decision making. Chapter 5 summarizes the results of these studies, and the following implications were drawn. First, the method of conflict management should be selected according to the nature of conflict and the conflict parties, conflict type of issue. In particular, when the extreme conflict is anticipated and the result of the policy is wide, the government should pre-establish measures of conflict management through conflict effect analysis, and participative decision making should be actively utilized. Second, the specific means of conflict management should be selected by social construction of target populations. Third, the government should prepare the specific legal and institutional means for the use of participative decision making. ; Master

Purpose: This research was, by investigating the relations between the cultural types of a nursing organization in military hospital and performance, to ascertain the cultural types to be able to improve performance. Method: The date were gathered from 255 nurse officers who were in 19 military hospitals by using the self-report type of questionnaire. The period of data collection was from April 20, 2003 to July 15, 2003. For this research, the following tools were used: the tools for measuring the organizational characteristics, organizational culture, and job satisfaction, the tool for measuring organizational commitment. For data analysis the SPSS Win 12.0 program was used. Result: 1) Most of the cultural types of a nursing organization in military hospital is Relation-oriented. 2) In the relation between general the characteristics of subject and the organizational cultural type, there was a difference in the innovation-oriented, relation-oriented, hierarchy-oriented, and task-oriented culture according to nurse officers careers, hospital types, year in hospital, marital status, and unit(p<0.05). 3) In the relation between general characteristics of subject and organizational performance, there was a difference in the job satisfaction, affective commitment, transactional commitment, and normative commitment according to nurse officers careers, education, madrigal status and unit(p<0.05). 4) In the relationship between the of a military hospital were correlated with the type of each culture(p=0.00), 5) In the relation between the organizational culture type of military hospital and its performance, there was a positive correlation among job satisfaction and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with job satisfaction(p<0.05). There was a positive correlation among affective commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture(p<0.05). And hierarchy-oriented culture showed that they had a weak negative correlation with affective commitment(p<0.05). There was no culture type significantly related to continuance commitment and there was a weak positive correlation among normative commitment and innovation-oriented culture, relation-oriented culture, task-oriented culture. The types to have an influence upon nurse officers' job satisfaction were relation-oriented culture and innovation-oriented culture(p=0.00). And relation-oriented culture and innovation-oriented culture were major variances for affective commitment and only relation-oriented culture was influential variance for normative commitment(p=0.00). Conclusion: The organizational culture type was found which had an influence upon nurse officers' job satisfaction and organizational commitment. These result are very significant in having showed the persons in charge of nursing administration a basic data for creation of an effective organizational culture. ; open

Background X-linked spondyloepiphyseal dysplasia tarda (SEDT-XL) is a skeletal disorder characterized by defective structures of vertebral bodies and/or of epiphyses of the long bones, resulting in moderately short stature and early joint degeneration. TRAPPC2 gene, which is important for collagen secretion, has been reported as causative for SEDT-XL. Case presentation Here, we report two variants of TRAPPC2 gene of SEDT-XL patients, a missense variant of start codon, c.1A > T, and a deletion variant, c.40delG. To understand molecular consequence of the variants, we establish an in vitro gene expression assay system and demonstrate that both mutated genes are transcribed, but are not properly translated, indicative of the pathogenic nature of those TRAPPC2 variants. Conclusions In the current study, we provide additional experimental data showing that loss-of-function TRAPPC2 variants are probably causative for SEDT-XL phenotype. Korean economic development has proceeded at the rapid rate of in the past two decades with neglect to some areas of human development, especially in the case of mental illness. But the government has belatedly recognized the importance and urgency of welfare and health programmes to cope with increasing problems. Mental health problems are more serious among the economically underprivileged than others. Because of large portion of mental patients from the economically underprivileged segment are at the without proper treatment or care. Thus it is imperative to develop easily assessible, cost-effective alternatives that would offer effective mental health care as well as useful social rehabilitation programmes. The government enacted the National Mental Health Law in December 1995. This project will assist to clarify the appropriate roles of the most effective care system. 20 nurse practitioners began a study on voluntary basis to assess the patients and their family with assessment tool which developed by research members. After that managed the 46 patients and their families through telephone or home-visiting, day care center programmes(such as social skill training, group therapy, movement therapy, art therapy) and counselling, refering hospital and family support group. The goals of research is to strengthen the capacity of psychiatric mental health nurse practitioners to undertake home and community-based mental health care services through various training programmes and expected outcome is this programme will be developed continuously and implemented to another community mental health center. Lastly, the cost-effective study of community-based mental health care will have been undertaken with realistic data provided by the pilot programme policy recommendations prepared for the Ministry of Health and Welfare. ; open

학위논문 (석사) -- 서울대학교 대학원 : 사회과학대학 정치외교학부(정치학전공), 2020. 8. 안도경. ; 이 연구는 엘리너 오스트롬(Elinor Ostrom)의 사회생태체계(Social-Ecological System, SES)를 분석틀로 채택하여 완도군 노화와 보길 지역의 물 분쟁을 경험적으로 풀어냈다. 사회생태체계는 사회·경제·정치적 환경, 생태계, 사용자, 자원, 거버넌스, 제도의 총체적인 상호작용을 설명하려는 시도이다. 2017년 최악의 가뭄이 닥치면서 두 지역이 공유하는 유일한 상수원이 말라 주민들은 식수난에 시달렸다. 완도군의 사회생태체계에는 다중심성(polycentricity)이 부재하여 외부 환경 변화에 유연하게 대처하지 못했다. 완도군의 물관리 제도는 명시적인 규칙만으로는 다원적이며 다층적이다. 그러나 실제로 작동하는 규칙에는 군 의회, 사용자, 군청 어디에도 중심이 존재하지 않았다. 즉, 자원을 지속 가능하게 만들지 못했던 거버넌스와 사용자, 그리고 그들이 만들어 낸 제도가 모두 물 분쟁의 원인이었다. 물관리에서 정부 부처와 지방 자치 단체의 수직적인 관계는 상호의존적이면서도 위기에 대응할 만한 신속한 의사 결정 구조를 구축하지 못했다. 군 사업은 주민들과의 합의보다 정부 부처의 승인이 먼저 이루어져 주민들의 반발에 부딪힐 수밖에 없었다. 특히 완도군 의회는 물관리 제도를 변화시킬 수 있는 주요 행위자였지만 군정을 일회성으로만 감시하는 데 그쳤다. 노화와 보길의 물 분쟁은 기후 변화가 도화선이 되었을 뿐 궁극적으로는 완도군의 거버넌스와 제도에 대한 주민들의 불신이 표출된 결과였다. 2018년 제정된 「물관리기본법」은 물 관련 법률의 '헌법적 법률'로서 그동안 일관성 없었던 물 정책을 환경부를 중심으로 유지할 것으로 보인다. 법률의 제정으로 명시적 규칙은 만들어졌지만 제도의 발전은 오랜 시간을 두고 이루어지는바, 30년간 쌓인 완도군 물 분쟁의 불신을 해소할 수 있는 상위 규칙이 될지는 귀추를 지켜봐야 한다. ; This study shows empirical evidence of the water conflict between Nohwa and Bogil islands in Wando county, focusing on the social-ecological system (SES) framework proposed by Elinor Ostrom. A SES implies interactions among its social, economic, and political settings, ecosystem, users, resources, government, and institutions. Since people in Wando had suffered from extreme drought in 2017, their single water resource had dried up. Thus, a water conflict ensued. The lack of polycentricity has prevented the SES in Wando from dealing with external variables in an adaptable manner. There has been no centricity among the county council, users, and local government at the rules-in-use level. The governance and the users, along with the institutions that they formed, has made the water resource unsustainable. The strict, hierarchical, and interdependent relationship between the central and the local governments has not built up an immediate and accurate decision-making process that could contribute to overcoming the crisis. Since the local government should take permission regarding local businesses from the central government before reaching an agreement with the residents, it encounters opposition from the people. While the county council could be the main actor of a change among the institutions related to water management, it only monitors local businesses in a one-time manner. Since climate change effects, like a drought event, has triggered the water conflict between the two islands, this ultimately has showed people's skepticism toward the governance and institutions of the local government. After the Framework Act on Water Management of 2018, the Ministry of Environment has been able to maintain its consistency in water policy. While rules-in-form have been made, it will take a long time to evolve them into rules-in-use. 학위논문 (석사) -- 서울대학교 대학원 : 사회과학대학 정치외교학부(정치학전공), 2020. 8. 안도경. ; 이 연구는 엘리너 오스트롬(Elinor Ostrom)의 사회생태체계(Social-Ecological System, SES)를 분석틀로 채택하여 완도군 노화와 보길 지역의 물 분쟁을 경험적으로 풀어냈다. 사회생태체계는 사회·경제·정치적 환경, 생태계, 사용자, 자원, 거버넌스, 제도의 총체적인 상호작용을 설명하려는 시도이다. 2017년 최악의 가뭄이 닥치면서 두 지역이 공유하는 유일한 상수원이 말라 주민들은 식수난에 시달렸다. 완도군의 사회생태체계에는 다중심성(polycentricity)이 부재하여 외부 환경 변화에 유연하게 대처하지 못했다. 완도군의 물관리 제도는 명시적인 규칙만으로는 다원적이며 다층적이다. 그러나 실제로 작동하는 규칙에는 군 의회, 사용자, 군청 어디에도 중심이 존재하지 않았다. 즉, 자원을 지속 가능하게 만들지 못했던 거버넌스와 사용자, 그리고 그들이 만들어 낸 제도가 모두 물 분쟁의 원인이었다. 물관리에서 정부 부처와 지방 자치 단체의 수직적인 관계는 상호의존적이면서도 위기에 대응할 만한 신속한 의사 결정 구조를 구축하지 못했다. 군 사업은 주민들과의 합의보다 정부 부처의 승인이 먼저 이루어져 주민들의 반발에 부딪힐 수밖에 없었다. 특히 완도군 의회는 물관리 제도를 변화시킬 수 있는 주요 행위자였지만 군정을 일회성으로만 감시하는 데 그쳤다. 노화와 보길의 물 분쟁은 기후 변화가 도화선이 되었을 뿐 궁극적으로는 완도군의 거버넌스와 제도에 대한 주민들의 불신이 표출된 결과였다. 2018년 제정된 「물관리기본법」은 물 관련 법률의 '헌법적 법률'로서 그동안 일관성 없었던 물 정책을 환경부를 중심으로 유지할 것으로 보인다. 법률의 제정으로 명시적 규칙은 만들어졌지만 제도의 발전은 오랜 시간을 두고 이루어지는바, 30년간 쌓인 완도군 물 분쟁의 불신을 해소할 수 있는 상위 규칙이 될지는 귀추를 지켜봐야 한다. ; This study shows empirical evidence of the water conflict between Nohwa and Bogil islands in Wando county, focusing on the social-ecological system (SES) framework proposed by Elinor Ostrom. A SES implies interactions among its social, economic, and political settings, ecosystem, users, resources, government, and institutions. Since people in Wando had suffered from extreme drought in 2017, their single water resource had dried up. Thus, a water conflict ensued. The lack of polycentricity has prevented the SES in Wando from dealing with external variables in an adaptable manner. There has been no centricity among the county council, users, and local government at the rules-in-use level. The governance and the users, along with the institutions that they formed, has made the water resource unsustainable. The strict, hierarchical, and interdependent relationship between the central and the local governments has not built up an immediate and accurate decision-making process that could contribute to overcoming the crisis. Since the local government should take permission regarding local businesses from the central government before reaching an agreement with the residents, it encounters opposition from the people. While the county council could be the main actor of a change among the institutions related to water management, it only monitors local businesses in a one-time manner. Since climate change effects, like a drought event, has triggered the water conflict between the two islands, this ultimately has showed people's skepticism toward the governance and institutions of the local government. After the Framework Act on Water Management of 2018, the Ministry of Environment has been able to maintain its consistency in water policy. While rules-in-form have been made, it will take a long time to evolve them into rules-in-use. Between 1995 and 2011, Peruvian GDP grew by 4.9% each year, driven by exports and private investment's increase. However, it only grew by 3.3% in 2015 and by 4% in 2016 due to a reduction in the international price of metals that the country exports. In this scenario, the government elected in 2011 faced the challenge of replacing private investment with public investment as a growth engine. However, this decision revealed two structural problems of the Peruvian economy: 1) the limited spending capacity of central and regional governments; and 2) the difficulty of increasing tax collection due to a very significant informal sector in the country. However, trade liberalization, stability in domestic prices and reforms in public management are strengths of the Peruvian economy, which would enable it to return in the next few years to growth rates of around 4% or more.

Background Low-quality services of emergency medical institutions, unclear function of institution classification, and overcrowding of emergency departments (ED) of regional emergency medical centers (regional centers) have been major problems in the Korean emergency medical services system (EMSS). In 2015, the Middle East Respiratory Syndrome (MERS) outbreak spread from patients waiting for hospitalization in the overcrowded ED. The MERS outbreak was an event that revealed the vulnerability of EMSS and opened the policy window to solve the problem of EMSS in Korea. Several policies for EMSS improvement were implemented after the 2015 MERS outbreak focusing the improvement of emergency medical services (EMS) delivery system and ED infection control. This study investigates the policy processes for improving emergency EMSS and infection control program after the MERS outbreak. The purpose of this study was to examine the policy processes regarding EMSS from the view of policy actors and draw lessons for improving the EMSS in Korea. 2. Methods A mixed method was utilized to identify and explain the complex subject of the policy processes of Korean EMSS during and after the MERS outbreak in 2015. As a quantitative study, changes in EMSS after MERS were investigated according to the WHO's model of a national health system infrastructure. Literature review and data analysis of Korean Health Insurance Service (NHIS) database were conducted. In Korea, the NHIS covers all citizens. A quantitative analysis was performed on 16,965,755 cases using ED during 2014-2016. In-depth interviews were conducted on 30 experts, who were involved in the emergency medical policy process, including clinicians, policy experts, and hospital management experts. The total interview time was 55 hours and 4 minutes. Changes in EMSS and the policy process after the MERS outbreak were evaluated from the view of policy actors. In this study, quantitative data was interpreted qualitatively, and independent qualitative evaluation was conducted, as well. Therefore, this research is a mixed-methods study focusing on qualitative research. SAS version 9.4 was used for the quantitative analysis and NVivo 12.0 was used for the qualitative analysis. 3. Results The quantitative analysis results showed that in the area of EMS delivery system, the human resource of ED and number of regional centers have been increased and facility, equipment, and manpower standards have been revised in order to expand emergency medical resources. As a result, the number of medical personnel and emergency intensive care units in regional centers increased and profitability of ED improved as well. The number of monthly ED visits temporarily decreased during the MERS outbreak but then recovered afterward. As ED overcrowding indicators were emphasized in the evaluation of EMS institutions, monthly mean ED length of stay of regional centers was decreased. However, despite the emphasis on providing critical care, transfer rate of severe emergency patients was not decreased. In the policy processes of EMS, experts evaluated that EMS delivery system was set up as a pseudo-agenda and exploration and comparison of policy alternatives were not sufficiently carried out. As a result, the physicians did not comply with the policy implementation and policy goals were not achieved. In the area of ED infection control program, the human resources for infection control and the number of isolation unit of ED were increased. Budgets for emerging infectious disease management were increased and profitability of infection control was also improved. The EMS Act was revised to respond to emerging infectious diseases and indicators related to infection control were strengthened in evaluation of EMS institutions. In the qualitative research, experts said that since the ED infection control program was such an intuitive and obvious problem, it was easy to become a policy-agenda. Despite the resistance of small hospitals, ED infection control program was implemented by powerful policy measures such as the revision of the Law. As a result, policy goals of expanding the infection control resources and improving the infection management environment were able to be achieved. From results of observing EMS policy process, the following characteristics of policy actors were found. First, there was difference of opinions within the physician subgroup as well as between government and medical professionals. ED staff, other department staff, and hospital executives, who have different views on policy alternatives could have all served as veto points in the policy process. Second, while the EMSS expert organization, which was an unofficial participant in the emergency medical policy process, could have played both roles of cause group and sectional group in the policy process, their sectional position has been emphasized, thereby weakening their insider status during policy process. Third, policy actors understood the publicness of EMS differently in terms of its ownership and function. The ED staff insisted that government support is needed because emergency medical care is based on public interest, however, some staffs were unwilling to provide less profitable cares, leading to conflict. Main characteristics of the emergency medical policy process that made it difficult to successfully implement are as follows: First, there was lack of consideration regarding variation of EMSS in Korea, such as distribution of medical resources among different regions, incentives to hospitals, and understanding of functional variations in ED. Second, the problem of the EMS delivery system was a problem of the low quality of emergency medical care. Third, the lack of professionalism in some physicians has lowered the capacity of emergency medical care, as well as trust between patient and doctor, therefore made the problem more difficult to solve. 4. Discussion Based on the analysis of the policy processes of the EMSS after the MERS outbreak in Korea, following lessons are suggested. First, setting the goals of EMS from the perspectives of citizen is warranted. The final performance goal should focus on improving health status of citizen and satisfaction with emergency medical care. Second, to close the gaps among policy actors in understanding policy content and policy process, actors should work together to explore the root causes of the problems and establish a policy agenda together. Third, a policy alternative should be causal in solving the problems in EMSS considering variation of EMS. Fourth, compliance with policies could be secured by internal persuasion among policy actors with proper incentives and reasonable punishment, especially EMSS expert organization. Fifth, policy outcomes should be evaluated based on both policy output and side effects together to identify policy impact. Sixth, policy governance regarding EMSS should be established and the roles of EMSS expert organization should be strengthened as the cause group rather than sectional group. This study examined the emergency medical policy processes from the view of policy actors through the 2015 MERS outbreak in Korea. The improvement of EMSS should be based on understanding of the emergency medical policy process and policy actors. ; 1. 서론 응급의료기관의 응급환자 진료 역량부족과 불명확한 종별 기능 구분, 일부 응급실의 과밀화는 한국 응급의료체계(emergency medical services system, EMSS)의 고질적인 문제였다. 2015년 과밀화된 상급종합병원 응급실에서 입원대기 중이던 환자로부터 메르스(Middle East Respiratory Syndrome, MERS) 유행이 확산되었다. 메르스 유행은 우리나라 응급의료체계의 취약성이 드러난 사건이자 응급의료 문제를 해결할 수 있는 정책의 창이 열린 사건이었다. 정부는 메르스 유행 이후 여러 가지 응급의료 개선정책을 추진하였다. 이 연구는 취약한 응급의료 전달체계 하에서 응급환자 감염관리의 실패로 인해 메르스 유행이 발생했다는 점에 문제의식을 두었다. 그래서 응급의료 전달체계와 응급환자 감염관리 영역에서 메르스 유행 이후 이루어진 정책과정과 그로 인한 변화를 분석하였다. 응급의료 정책행위자의 관점에서 정책과정과 정책결과를 분석함으로써, 향후 응급의료 개선을 위한 정책과정에서 고려해야 할 점을 도출하는 것이 이 연구의 목적이다. 2. 연구방법 메르스 유행 이후 응급의료 정책과정 분석이라는 복잡한 주제를 파악하고 설명하기 위하여 혼합연구방법(Mixed methods research)을 사용하였다. 양적연구로 문헌고찰과 국민건강보험공단 자료 분석을 통해 메르스 유행 이후 응급의료체계의 변화를 보건의료체계의 구성요소별로 조사하였다. 한국은 1989년 전국민 의료보험제도를 도입하여 전 국민이 국민건강보험에 가입되어 있는 국가이다. 메르스 유행 전후 응급실 이용 변화를 조사하기 위하여 국민건강보험공단 DB의 2014-2016년 응급실 이용 진료건 16,965,755건을 분석하였다. 질적연구로 응급실 의료진, 정책 전문가, 병원 경영진 등 응급의료 정책과정의 관련 전문가 30명을 대상으로 심층면접을 수행하였다 전체 심층면접 분량은 55시간 4분이었다. 심층면접을 통해 정책 행위자의 관점에서 메르스 이후 이루어진 응급의료 정책과정을 고찰하고, 응급의료체계의 변화를 조사하였다. 양적연구 결과를 질적연구로 해석하고 질적연구로 독자적인 분석을 수행하여, 이 연구는 질적연구가 중심이 된 혼합연구로 볼 수 있다. 양적연구의 분석에는 SAS version 9.4를 사용하였고, 질적연구 분석은 NVivo 12.0을 사용하였다. 3. 연구결과 응급의료 전달체계 영역에서 응급의료 자원을 확대하기 위해 권역응급의료센터(권역센터)가 확대되고 시설·장비·인력기준과 관련 수가가 개정되었다. 그 결과 권역센터의 의료인력과 응급중환자실 병상 수가 증가하고 수익성이 개선되었다. 달빛어린이병원 사업을 확장한 결과 달빛어린이병원 수도 증가하였다. 응급실 과밀화를 완화하기 위해 응급의료기관 평가에 재실시간 지표를 수가연동지표로 도입하고 응급의료에 관한 법률에 응급의료센터 24시간 이상 재실환자 관리 규정을 넣었다. 그 결과 응급실 월별 방문환자 수는 2015년 메르스 유행 당시 일시적으로 감소한 후 지속 증가 추세이나, 권역센터 응급환자의 월별 재실시간 평균은 2014-2016년 기간 동안 감소 추세를 보였다. 응급의료기관의 응급환자 진료역량을 강화하기 위해 응급의료기관 평가에 중증해당환자 최종치료 제공 관련 지표를 강화하였으나 2017-2018년 기간 중증응급환자의 전원율은 개선되지 않았다. 전문가들은 응급의료 전달체계 개선은 위장의제(pseudo agenda)로 설정되어 정책대안 탐색과 비교평가가 충분히 이루어지지 않았으며 점중주의적으로 쓰레기통 모형의 정책결정이 이루어졌다고 평가하였다. 그 결과 정책대상집단인 의사들은 정책집행에 순응하지 않았고, 정책은 집행 후에도 정책목표를 달성하지 못했다. 응급환자의 감염관리 영역에서 권역센터 시설·장비·인력기준과 감염관리실 설치기준 및 인력기준이 개정되어 응급실 감염관리 자원이 확대되었다. 정부는 응급실 방문객 관리를 도입하고, 감염예방관리료 등 감염관리 관련 건강보험 수가를 신설하였으며, 신종감염병 대응 관련 법률 등 감염관리 관련 법률을 개정하여 응급실 감염관리 여건을 개선하였다. 전문가들은 응급환자 감염관리는 직관적이고 명확한 문제라는 특성이 있어 병원 내 감염관리 강화로 의제화되기 쉬웠다고 평가하였다. 감염관리 개선정책은 보호적 성격을 가진 규제정책으로 집행 과정에서 중소병원을 중심으로 일부 병원의 반발이 있었다. 그러나 응급의료에 관한 법률 개정과 같은 강력한 정책수단을 사용하여 집행된 결과, 응급실 감염관리 자원확대와 감염관리 환경개선이라는 정책목표를 달성하였다. 응급의료 정책행위자들의 관점에서 응급의료 정책과정을 관찰한 결과 다음과 같은 정책행위자의 특징을 발견하였다. 첫번째, 정부 전문가와 의료진 전문가 사이뿐 아니라 의사 하위집단 내에서도 견해 차이가 존재하였다. 정책대안에 대해 다른 견해를 가지고 있는 응급실 의료진, 배후진료과 의료진, 병원 경영진은 정책과정에서 모두 거부점(veto point)으로 작용할 수 있었다. 두번째, 응급의료 정책과정의 비공식적 참여자인 응급의료 관련 전문학회는 정책과정에서 대의집단이자 파벌집단의 역할을 동시에 수행할 수 있으나, 정책과정에서 파벌집단의 역할이 강조됨으로써 내부자의 지위가 약화되었다. 세번째, 응급의료의 공공성에 대하여 소유주체 기준에 따른 공공성과 기능에 따른 공공성 개념이 혼재되어 사용되며 서로 다른 공공성 개념에 따른 정책행위자간 갈등이 발생하였다. 응급실 의료진은 응급의료가 공공의 이익을 실현하는 의료영역이므로 정부 지원이 필요하다고 주장했으나 일부 의료진은 수익성이 낮은 진료를 기피하는 모습을 보여 정부 전문가와 의견 차이를 보였다. 응급의료 정책과정의 성공적 추진을 어렵게 한 정책과정의 특징은 다음과 같다. 첫번째, 응급의료 정책과정에서 지역간 의료자원 분포, 병원에 작용하는 유인, 병원 내 응급실의 역할과 같은 응급의료에 존재하는 변이에 대한 고려가 부족했다. 두번째, 응급의료 전달체계 문제는 곧 응급의료기관의 취약한 응급환자 진료 역량의 문제였다. 세번째, 일부 의사들의 전문직업성(professionalism) 부족이 응급의료기관의 응급환자 진료 역량을 취약하게 하고, 환자-의사 사이 신뢰를 저하시켜 대형병원 쏠림현상을 악화시켰다. 4. 고찰 메르스 유행 이후 이루어진 응급의료 개선정책의 정책결과 분석과 정책과정에 대한 연구 결과를 근거로 하여 향후 응급의료 정책과정에 대하여 다음과 같이 제언한다. 첫번째, 국민의 관점에서 응급의료체계의 개선목표를 설정할 필요가 있다. 건강수준 향상과 응급의료 서비스에 대한 만족도 향상이라는 최종 성과목표가 개선정책의 목표로 설정되어야 한다. 두번째, 정책행위자의 인식 차이를 좁히기 위하여 여러 행위자가 함께 문제의 원인을 탐구하고 정책의제를 설정하는 과정이 필요하다. 세번째, 문제 해결에 인과성 있는 정책대안이 결정되어야 한다. 인과성 없는 정책대안은 정책이 집행되어도 정책문제를 해결하지 못한다. 네번째, 정책대상자의 순응을 확보할 필요가 있다. 유인과 처벌 이전에 전문가이자 정책대상자인 의사들에 대한 내면적 설득 과정이 필요하다. 다섯번째, 정책산물과 부수작용을 포괄하는 정책결과 평가가 필요하다. 여섯번째, 응급의료 정책 거버넌스를 확립하고 응급의료 관련 전문학회를 정책과정에 참여시켜 대의집단의 성격을 강화할 필요가 있다. 이 연구는 2015년 메르스 유행이라는 사건을 통해 응급의료 정책행위자들의 관점에서 우리나라 응급의료 정책과정을 고찰하였다. 응급의료 정책과정과 정책행위자에 대한 이해를 바탕으로 응급의료체계 개선을 추진해나가야 한다. ; I. 서 론 1 1. 연구 배경과 필요성 1 1) 한국 응급의료체계의 문제 1 2) 2015년 메르스 유행과 응급의료체계 4 3) 연구의 필요성 6 2. 연구 목적 10 II. 이론적 배경 12 1. 보건의료체계의 평가 12 1) WHO 보건의료체계의 구성요소 12 2) 보건의료체계 개혁을 위한 조종손잡이 14 2. 정책론 18 1) 정책과정의 이해 18 2) 응급의료 정책과정의 이해 25 III. 연구방법 35 1. 혼합방법 연구설계 35 2. 양적연구 방법 43 1) 양적 연구의 자료 수집 43 2) 양적 연구의 자료 분석 46 3. 질적연구 방법 54 1) 연구팀 구성 54 2) 심층면접 설계 55 3) 분석 방법 67 4. 혼합방법 연구의 통합 70 IV. 연구 결과 72 1. 양적연구 결과 72 1) 응급의료 전달체계 72 2) 응급환자의 감염관리 121 2. 질적연구 결과 134 1) 심층면접 참여자의 특성 134 2) 응급의료 전달체계 137 3) 응급환자의 감염관리 249 3. 연구결과의 종합 297 1) 2015 메르스 유행의 응급의료체계에 대한 의의 297 2) 메르스 유행 이후 응급의료 정책과정 분석 298 3) 응급의료 정책과정의 특징 340 V. 논의 363 1. 응급의료 정책과정 고찰 363 2. 연구의 한계 380 3. 연구의 의의 383 1) 방법론적 의의 383 2) 정책적 의의 384 4. 연구의 결론 385 1) 메르스 유행 이후 응급의료 전달체계와 응급환자 감염관리의 변화 385 2) 응급의료 정책과정 고찰 386 3) 응급의료체계 개선을 위한 정책과정에의 시사점 388 VI. 참고문헌 390 ; Doctor

학위논문 (석사)-- 서울대학교 대학원 : 행정학과(행정학전공), 2012. 2. 정광호. ; 국문 초록 본 연구에서는 고구려연구재단과 동북아역사재단의 설립과정을 역사적 사회 환경적 관점, Allison의 정부정치 모형적 관점, 그리고 Kingdon의 정책흐름 모형적 관점에서 비교 분석하여 두 기관의 설립, 두 기관의 제도적 성취 및 역할 범위의 차이를 가져온 요인이 무엇인가를 밝혀보고자 하였다. 일반적으로 조직설립을 다루는 이론으로서 역사적 사회 환경적 관점에서는 조직 환경론, 특히 구조적 상황이론과 조직생태이론이 유용한 분석도구로 활용되어져 왔다. 이러한 이론들은 조직설립이 그것을 둘러싼 맥락 또는 상황에 의존한다는 결정주의적 입장을 취하고 있기 때문에 특정 상황적 요인의 존재 또는 발생으로 조직이 설립되었다는 것을 설명하는 데는 유용한 관점인 것으로 인식되어져 왔다. 하지만, 본 연구에서는 조직 환경론적 관점만으로는 왜 특정 공공조직은 특정 상황 또는 맥락의 존재에도 불구하고 설립 또는 사멸되지 않으며, 왜 유사한 공공조직간 제도적 성취의 차이가 발생하는 가를 설명하는 데는 한계가 있다고 보고 조직설립과 관련된 조직 환경론적 관점의 한계를 적시함과 동시에 준 정부조직 설립과정의 특수성을 감안하여 정부정치모형과 정책흐름 모형을 추가로 적용함으로써 고구려연구재단 및 동북아역사재단 설립과정에 대한 비교분석을 시도하였다. 먼저, 역사적 사회 환경적 관점에서는 구조적 상황이론과 조직 생태이론을 적용하여 두 기관의 설립을 가져온 역사적 배경이 무엇이며, 동북아역사재단 설립과 더불어 이에 흡수 통합된 고구려연구재단의 사멸 요인이 무엇인지를 밝혀보고자 하였다. 동 연구 결과에 의하면, 고구려연구재단의 사멸은 낮은 신뢰성 및 예측가능성, 신생조직의 불리, 작은 조직 규모 및 예산, 핵심적 조직 변화의 시도 및 동북아역사재단의 설립에 기인한 것으로 나타났다. 두 번째로, Allison의 정부정치 모형을 통하여 두 기관 설립을 둘러싸고 벌어진 참여자들의 흥정과 타협을 분석하여 이들의 입장·선호·인식·기대이익 등의 차이가 두 기관의 설립 및 제도적 성취에 어떠한 영향을 주었는가를 설명하였다. 그 결과 두 기관의 설립 및 제도적 성취의 차이 발생은 핵심참여자로서의 대통령과 행정기관의 인식 및 선호의 차이에 의해 크게 영향을 받은 것으로 나타났다. 다시 말해, 고구려연구재단 설립과 관련된 대통령 및 행정기관의 인식 및 입장은 한·중 양국의 외교관계 등을 의식해서 상당히 유화적이었던 반면, 일본의 역사교과서 왜곡 및 독도 영유권 도발로 시작된 동북아역사재단 설립과 관련하여서는 대통령과 행정기관의 인식이 매우 단호하였던 것으로 밝혀졌다. 세 번째로, Kingdon의 정책 흐름 모형을 적용하여, 두 기관 설립을 둘러싼 문제 및 정책의 흐름의 차이가 어떠하였는지를 비교 분석하였다. 먼저, 문제 흐름의 측면에서 고구려 역사 왜곡 문제에 대한 정책 당국자 또는 학계의 인식은 약했으며, 정책의 흐름도 시민단체 주도로 이루어져 일관성 및 강도가 약했다는 것을 확인하였다. 반면, 일본의 역사 교과서 왜곡 및 독도 영유권 도발로 촉발된 동북아역사재단 설립을 둘러싼 문제의 흐름은 강하고 일관성이 있었으며, 정책 흐름 역시 대통령과 행정기관의 주도하에 하향식으로 이루어져 일관되고 강하게 진행되었다. 이러한 두 가지 흐름상의 차이가 두 조직간 제도적 성취의 차이를 가져온 또 다른 설명 요인이었다고 할 수 있다. 본 연구의 결과, 발견된 시사점은 다음과 같다. 먼저, 역사적 사회 환경적 관점만으로는 공공조직 설립과정을 명확히 이해하는 데는 한계가 있다는 것을 확인하였다. 다시 말해, 역사적 사회 환경적 관점은 조직의 구조나 기능을 결정하는 요인으로 특정 환경 혹은 맥락을 강조해왔지만, 동 관점만으로는 왜 특정 환경의 존재에도 불구하고 왜 특정시점에는 조직 설립 또는 사멸이 이루어지지 않다가 다른 시점에 와서야 이루어지는가에 대해서는 명쾌한 해답을 제공하지 못하는 한계가 있다는 것이다. 두 번째, 준 정부조직은 민간조직과는 달리 법적·행정적·제도적 제약 조건하에서 설립되는 경우가 많기 때문에 준 정부 조직의 설립과정을 보다 명확히 이해하기 위해서는 보다 다양한 관점 즉, 정부 정치적 관점 및 정책 흐름적 관점 등을 통한 기존 이론의 보완 또는 대체가 필요하다는 것이다. 특히 우리사회가 다원화된 사회로 진행되어감에 따라 공공조직 설립과정을 이해하기 위해서는 참여자의 정치적 흥정과 타협을 다루는 정부정치적 관점에 대한 고려가 필요할 것으로 본다. 주요어: 역사적 사회 환경적 관점, 구조적 상황이론, 조직 생태이론, Allison의 정부정치 모형, Kingdon의 정책흐름 모형, 준 정부조직 ; ABSTRACT A Comparative Analysis of the Establishment Processes of Two Quasi-Governmental Organizations: The Cases of the Koguryo Research Foundation and the Northeast Asian History Foundation Si Yung Jung Department of Public Administration Graduate School of Public Administration Seoul National University The purpose of this study is to analyze through a comparative approach the establishment processes of the Koguryo Research Foundation and the Northeast Asian History Foundation from the perspectives of historical-social environment, Graham Allison's Governmental Politics Model, and John W. Kingdon's Policy Streams Model in order to clarify the establishment processes of these two organizations and to identify which factors determined their institutional achievements. Generally, both contingency theory and organizational ecology theory have been widely used as tools for analysis of organizational establishment. These theories have strengths for explaining the contingencies and contexts which determine the establishment of organizations. However, these theories have weaknesses in explaining the reasons why an organization fails to be established in the presence of a certain contingency or why there are differences of institutional achievements between certain similar organizations. To further the understanding of organizational establishment and the incidence of differences of institutional achievement between the two organizations, this study adopted the Governmental Politics Model and the Policy Streams Model. In the perspective of the historical-social environment, both contingency theory and organizational ecology theory were adopted as tools for analysis. Contingency theory assumes that the structure or management style of an organization depends on the environment which surrounds the organization. In the perspective of contingency theory, the creation of the Koguryo Research Foundation was initiated by the Chinese distortion of Koguryo history, while the Northeast Asian History Foundation was created by the Japanese distortion of history and sovereignty provocation over Dokdo. The perspective of organizational ecology theory was adopted to explain the reasons why the Koguryo Research Foundation was closed and then merged into the Northeast Asian History Foundation after two and one-half years of activity. Through this perspective, this study found that the closure of the Koguryo Research Foundation resulted from such factors as lower reliability and accountability, the liability of newness, the small size of its organization and budget, the attempt of essential organizational change, and the creation of a new organization, the Northeast Asian History Foundation. In the perspective of the Governmental Politics Model, this study examined how the political resultants, the Koguryo Research Foundation and the Northeast Asian History Foundation, were drawn through bargaining and negotiation by the players involved in the game. This study revealed that the differences in the institutional achievements between the two quasi-governmental organizations resulted from the differences of the players' perceptions, preferences, and stands on the issue. In terms of perception, the perception of key players such as the president of the Republic of Korea and his administration on the issue of the Chinese distortion of Koguryo history was pacificatory, while the perception of the president and the administration on the issue of the Japanese distortion of history and the issue of Dokdo was quite determined. Through this lens, this study also found that the players perceived the issue not only on the basis of national interests, but also on the basis of their organizational interests and personal concerns. Finally, through the viewpoint of the Policy Streams Model, this study examined how the streams of problems and policy alternatives proceeded along the processes of the founding of the two quasi-governmental organizations. According to the analysis, the streams of problems surrounding the founding of the Koguryo Research Foundation were weaker than the streams of problems concerning the Northeast Asian History Foundation. In the perspective of policy streams, those concerning the founding of the Northeast Asian History Foundation were consistent and strong due to the determined stand of the president on the issue, while the policy streams regarding the Koguryo Research Foundation were incremental, inconsistent, and mutated due to the lack of presidential support and the passive attitude of the administration toward the issue. The first implication of the study is that a variety of aspects of organizational establishment should be reviewed to understand the process of organizational founding more clearly. Though organizational environment theories have assumed that environment or context determines the structure or function of the organization, the theories can not clearly explain why the birth or closure of an organization occurred in a specific time in spite of the presence of previous contingencies. The second implication is that policy makers or researchers should consider such factors as politics, administration, and the legality of the issues regarding the establishment of a quasi-governmental organization, because quasi-governmental organizations are governed by legal, administrative, and other institutional constraints unlike private organizations. The third implication is that, as Korea society is growing more pluralistic, the relevance of the Governmental Politics Model is likely to become higher in explaining the establishment processes of quasi-governmental institutions in the Republic of Korea. 학위논문 (석사) -- 서울대학교 대학원 : 사회과학대학 지리학과, 2020. 8. 신혜란. ; 본 연구는 정책이동(policy mobility)의 관점에서 서울로 7017의 조성 과정을 분석하였다. 특히 본 연구는 정책이 이동하는 과정에 있어서 도시 정치가 어떠한 방식으로 개입하게 되는지를 집중적으로 살펴보았다. 정책이동은 도시 연구 분야에서 주요 연구 주제로 떠오르고 있다. 전 세계 도시 간 협력과 교류가 활발해짐에 따라 정책의 이동 - 선진 정책의 벤치마킹 및 수입과 모방, 정책 관련 국제 컨퍼런스 발표 및 수상 등 - 은 활발하게 진행되고 있고, 이러한 현상은 도시정책의 추진 및 의사결정에 있어서도 직접적인 영향을 미치고 있다. 한편 기존 정책이동 관련 연구들은 정책이 이동되는 과정자체에 초점을 맞추어 도시들 간의 지역적 맥락이나 정치구조의 차이가 정책의 적용과 변용 등에 어떠한 영향을 미치는지를 주로 살펴보았다. 이에 반해 정책 이동 현상이 넓은 범위의 도시정치와 거버넌스에 어떠한 영향을 미치는지를 활발하게 연구되지는 못하였다. 본 연구는 이러한 부분에 있어서 기여하고자 서울로 7017이 조성되는 과정에서 나타났던 벤치마킹이나 정책 홍보 등의 정책이동 과정이 서울시 도시정책에 어떠한 영향을 미쳤는지를 분석하였다.연구의 결과는 다음과 같다. 첫째, 서울로 7017 조성과정에서 나타난 정책의 이동 – 뉴욕 하이라인의 벤치마킹 또한 서울로 7017 정책의 홍보와 선전 - 은 서울시가 도시정책을 정당화하는 주요한 방법으로서 활용되었다. 서울로 7017은 별다른 공론화 과정 없이 박원순 시장의 정치적 추진력을 통해 급하게 추진되었기 때문에 서울시는 사업에 대한 정당성을 보여주는 것이 필요했다. 이에 서울시는 이미 진행되었던 뉴욕 하이라인의 성공을 통해서 사업의 필요성과 당위성을 보여주었다. 또한 서울시는 사업의 가치와 성공을 국내와 해외로 홍보하는 과정을 통해서, 서울로 7017 대한 성공적인 이미지를 스스로 다듬어갔다. 이처럼 서울로 7017이 조성되는 과정에서 나타났던 벤치마킹과 정책 홍보는 서울시가 도시 정책을 정당화하기 위한 목적에서 진행되었다. 둘째, 서울로 7017의 조성에 있어서 서울시는 하이라인을 벤치마킹 하였지만, 이는 서울시의 정책적 목표와 필요에 따라 선별적으로 이루어졌다. 또한 이처럼 정책적 필요에 맞추어 벤치마킹과 모방이 진행되는 과정에서, 처음 모방하고자 하였던 하이라인의 운영방식과 내용은 큰 폭으로 변용되었다. 일례로 하이라인의 디자인은 유사한 형태로 모방되었지만, 시민참여를 대표하였던 하이라인의 운영방식은 보수적인 서울시의 행정 구조에 맞추는 과정에서 큰 폭으로 변용되었다. 또한 이와 같은 모방과 변용의 과정에서 서울시의 정책, 행정구조, 공간은 빠르게 변화되었다. 본 연구는 전세계적으로 활발하게 진행되고 있는 정책이동 현상이 도시를 어떻게 변화시키고 있는지를 서울로 7017의 실증사레를 통해서 확인하였다. 한편 본 연구는 정책이동을 통해서 이루어지고 있는 다양한 변화들을, 다소 개괄적인 범위에서 살펴보았다는 점에서 한계를 갖기도 한다. 발전된 후속 연구를 통해서 정책이동이 도시의 변화에 갖는 의미를 더욱 면밀하게 검토하고 구체적인 정책적 시사점이 제시할 수 있기를 기대한다. ; This study analyzes the development process of Seoullo 7017 through the theoretical lens of policy mobility. In the past few decades, transfers, diffusion, and movement of urban policies across national borders have increased rapidly. Accordingly, researches on policy mobility have developed in the fields of geography and urban studies. These studies have critically investigated the processes of adaptation, assemblage, and mutation of a certain policy, but have little focused on how the ordinary urban policymaking and the urban area has changed in the process of policy movement. In this regard, this study explores how the mobilization of certain policy has changed and reconstructed the urban policy and planning system. Especially, this research has focused on the interaction of local decision-making and policy mutation processes. The results of the study are as follows. First, the mobilization of policy that appeared in Seoullo 7017 including benchmarking and policy promotion was strategically used by the Seoul city government. The main purpose of these mobilizations was to legitimize the city's urban policy. Seoullo 7017, which was a mayor's landmark project, has proceeded rapidly without the civic consensus process. In this regard, the New York High Line's success was used as a tool to represent the legitimacy of the Seoullo 7017 project. Also, the branding and promotion of Seoullo 7017, which received great attention from the city government, was used as a self-curation tool to make the success story of Seoul's urban policy. Second, the mobilization and adaptation of the High Line were proceeded selectively according to specific political goals of the Seoul city government. Seoullo 7017 benchmarked both the design and governance of the High Line. Lots of critics pointed out that the design should be differentiated, as the regional context of Seoul and New York was different, However, the design was similarly imitated into Seoullo 7017 because of the symbolic image of the High Line's design itself. Also, the management style which emphasized citizen participation and governance were also imitated, but it ends with instrumental form due to bureaucratic political structure. As such, adaptation, mobilization, and mutation has been carried out in accordance with the process of legitimizing local policy and political authority. The importance of this research is that it investigates the consequences and implications of mobile urban policies through an empirical case at various points. However, it also has its limitations as this study looks into various aspects of mobile policies on a superficial level. 학위논문 (석사)-- 서울대학교 대학원 : 정책학과, 2012. 2. 박정훈. ; 오늘날과 같은 지식정보화 사회에서의 전자정부 구현은 정부경쟁력, 더 나아가 국가경쟁력을 제고할 수 있는 핵심수단으로 여겨지기에 세계 각국과 국제기구에서는 전자정부를 도입하고 발전시키기 위해 재정적, 정책적으로 많은 노력을 기울이고 있다. 그러나 전자정부를 발전시키려는 이러한 노력들에도 불구하고 정부의 효과성, 민주성, 투명성 등 정부경쟁력의 개선은 전자정부 도입 시 기대했던 바에 크게 미치지 못하고 있는 것이 현실이다. 본 연구는 이러한 문제의식에서 출발하여 각 국의 정부경쟁력을 한층 강화시켜 줄 것이라 생각했던 전자정부가 과연 실제 그 효과를 나타내고 있는지에 대한 문제를 제기하고 전자정부가 정부경쟁력에 어떻게 영향을 미치고 있는 실증 분석하고자 하였다. 이를 통해 전자정부 도입 시 본래 달성하고자 하는 바들이 얼마나 잘 달성하고 있는지, 또 서로 다른 부문들로 구성된 전자정부 요소들은 정부경쟁력에 각기 어떠한 영향을 미치고 있는지 살펴보고 향후 전자정부의 연구, 정부정책과 관련한 몇 가지 시사점을 도출하고자 하였다. 본 연구는 151개 국가들을 대상으로 실시하였으며 종속변수로는 UN에서 발표하는 'Global E-government Readiness Report'의 전자정부 준비지수(Global E-government Readiness), 독립변수로는 국제투명성위원회의 부패인식지수(CPI), World Bank의 정부효과성지수(GE), Freedom House의 정치적 권리(PR), 시민의 자유(CL)지수를 시차를 고려하여 다중회귀분석을 실시하였다. 분석결과 각 국의 웹과 정보통신인프라의 발전은 정부운영의 효과성과 시민의 정치적 권리와 자유를 증진시키고 정부부문의 부패를 감소시켜 정부경쟁력 강화에 유의미한 영향을 미치고 있는 것으로 나타났다. 이러한 웹과 정보통신인프라의 효과적인 정부운영 및 부패감소 효과는 OECD국에 비하여 비OECD 국에서 상대적으로 더 큰 것으로 나타났다. 반면 정부의 시민참여 노력정도를 반영하는 온라인참여(E-Participation)변수는 기대와 달리 시민의 정치적 권리와 자유를 증진시키는데 기여하고 있지 못하는 것으로 나타났다. 본 연구는 전자정부 운영 및 연구에 있어서 몇 가지 함의를 시사한다. 첫째 전자정부가 정부경쟁력 강화라는 본래 목표를 달성하기 위해서는 무엇보다 정보통신인프라의 확충과 웹사이트의 발전이 동시에 이루어져야 할 것이다. 둘째, 각 국이 처한 경제, 사회 등 여러 부문의 발전수준과 전자정부 각 요소들을 각 국에 도입했을 때 그 영향들을 고려한 전략적 접근이 전자정부를 성공적으로 이끄는데 하나의 중요한 요소로 작용할 것이라는 점이다. 셋째, 현 시점의 전자정부 뿐만 아니라 이전의 전자정부 수준역시 정부경쟁력에 지속적인 영향을 미치고 있는 것으로 나타났다. 이는 전자정부사업에 대한 성과가 나타나는 시간이 길기 때문에 시차를 고려한 전자정부 사업계획 및 성과평가가 이루어져야 함을 보이는 것이다. 마지막으로 정부의 온라인 국민 참여노력(E-Participation)이 실제 민주주의강화에 이어질 수 있도록 정부의 보다 강한 책임성과 대응성, 법‧제도의 구축이 이루어져야할 것이라는 점이다. ; In the information society, All countries of the world and international organizations have tried to develop E-government, considering it as core means to improve government competitiveness. However, It is true that the results of it fall short of our expectations regardless of our effort in E-government. The results of the analysis are as follows: With awareness of issues above, this study tried to solve research questions: Has E-government which plays a role in improving government competitiveness accomplished its aims? and which components of it have effect on the government competitiveness? In addition, this study aimed to offer proposals related to policies and studies of E-government. In research method, multi-regression analysis was used based on the data : Global E-government Readiness Index reported by UN, Corruption Perceptions Index issued by TI, Government Effectiveness Index reported by World Bank, Political Right and Civil Liberty issued by Freedom House with 151 countries in the world. As a result of analysis, It is verified that Development of Web and Telecommunication Infrastructure have influence on improving Government effectiveness and democracy in society. And Their influence is stronger in OECD countries than non-OECD. On the other hand It is revealed that an effort of government to encourage political participation of nation doesn't contribute to improving level of democracy. The lessons from this study are as follows. First, it is important to expand telecommunication infrastructure for more citizen to get information which is a little bit developing infrastructure highly and improve web site for citizen to get high quality information. Secondly, it is necessary to approach strategically considering a level of economy and society and influence of e-government predicted before planning policies about e-government in order to make e-government business successful. thirdly, It is revealed that not only a level of current e-government but also a level of past e-government have influence on current government competitiveness. It means that time lag has to be considered planning e-government business and evaluating performance of it. Last but not least, More responsibility and responsiveness of government and laws and institutions related to e-government are needed in order that an effort of e-participation each government carries out is connected to reinforcing democracy. ; Master

학위논문 (석사)-- 서울대학교 대학원 : 교육학과(교육학전공), 2012. 2. 박현정. ; 이 연구의 목적은 공군에 복무하는 병사들이 지각하는 사회적 지지, 자기효능감, 자아탄력성이 군 생활 적응에 어떠한 영향을 미치는지 살펴보고, 변인들 간의 구조적 관계 및 상대적인 영향력의 크기를 확인하는 것이다. 그리고 이 때 개인의 정의적 특성인 자기효능감, 자아탄력성이 사회적 지지와 군 생활 적응의 관계에서 매개변인으로 작용하는지 확인하는 것이다. 한 개인이 타인과의 의미 있는 상호작용의 결과로부터 얻을 수 있는 긍정적 자원인 사회적 지지는 스트레스에 대한 예방과 적응에 도움을 줄 수 있는 효과적인 완충제 역할을 함으로써 좌절을 극복하고 문제해결의 도전을 받아들이는 능력을 강화한다(Sarson, 1991; Sarson et al., 1983). 그러므로 병사들이 군대 환경의 맥락에서 지각하는 사회적 지지는 이들의 군 생활 적응에 유의미한 영향을 미치는 주요한 변인이 될 수 있다. Bandura(1995)는 긍정적 자기효능감은 어려운 과제나 장애에 직면했을 때 자신을 동기화하고 좌절과 고통을 극복하게 함으로써 장애로부터의 성공적인 회복과 치료에 중요한 영향을 미친다고 주장하였는데, 이는 부여된 임무를 수행하는 병사들의 과업 수행과 직무 만족도에 긍정적 영향을 미쳐 군 생활 적응을 매개하는 변인으로 작용할 수 있음을 나타낸다. 또한 환경의 역경과 스트레스 상황에서 융통성 있는 행동과 문제해결 전략을 동원하여 심리적 적응을 중재하는 변인으로 밝혀진 자아탄력성은 개인에게 낯선 삶의 경험과 수행을 요구하는 군대 환경의 맥락에서 병사들의 적응을 예측하고 매개하는 변인으로 작용할 수 있다. 이 연구에서는 이러한 이론과 선행연구 결과를 근거로 다음과 같은 연구문제를 설정하고 이를 검증하고자 하였다. 연구문제 1. 공군 병사들이 지각하는 사회적 지지가 자기효능감, 자아탄력성, 군 생활 적응에 영향을 미치는가? 연구문제 1-1. 공군 병사들이 지각하는 사회적 지지가 군 생활 적응에 직접적 영향을 미치는가? 연구문제 1-2. 자기효능감은 군 생활 적응에 직접적 영향을 미치는가? 연구문제 1-3. 자아탄력성은 군 생활 적응에 직접적 영향을 미치는가? 연구문제 2. 공군 병사들이 지각하는 사회적 지지는 자기효능감과 자아탄력성을 매개로 군 생활 적응에 영향을 미치는가? 연구문제 2-1. 공군 병사들이 지각하는 사회적 지지는 자기효능감을 매개로 군 생활 적응에 영향을 미치는가? 연구문제 2-2. 공군 병사들이 지각하는 사회적 지지는 자아탄력성을 매개로 군 생활 적응에 영향을 미치는가? 연구문제 2-3. 공군 병사들이 지각하는 사회적 지지는 자기효능감, 자아탄력성을 매개로 군 생활 적응에 영향을 미치는가? 연구문제 3. 공군 병사들의 자기효능감은 자아탄력성을 매개로 군 생활 적응에 영향을 미치는가? 위의 연구문제를 확인하기 위하여 공군에 근무하는 병사 520명을 대상으로 예비 및 본 조사 설문을 실시하였고, 연구모형에서 설정한 잠재변인의 구조적 관계를 확인하기 위해 구조방정식을 이용하여 모형을 분석하고 자기효능감, 자아탄력성의 매개효과를 검증하였다. 본 연구의 주요 결과를 요약하면 다음과 같다. 첫째, 선행연구 결과와 이론에 근거하여 설정한 사회적 지지, 자기효능감, 자아탄력성, 군 생활 적응의 구조적 관계에 대한 구조모형이 군대 환경의 맥락에서도 잘 지지되는 것으로 확인되었다. 모형 평가는 적합도 평가 지수의 기준이 확립된 TLI, CFI, RMSEA를 통해 평가하였고, 적합도 지수가 수용 가능한 것으로 확인되었다. 따라서 이 연구에서 설정한 연구모형은 비교적 연구 자료를 적절하게 잘 설명하는 것으로 볼 수 있다. 둘째, 병사들이 지각하는 사회적 지지와 개인의 정의적 특성인 자기효능감, 자아탄력성은 군 생활 적응에 모두 직접적인 효과를 나타내는 것으로 확인되었다. 직접효과 크기는 자기효능감이 가장 큰 효과크기를 나타냈고, 이어서 사회적 지지, 자아탄력성 순으로 나타났다. 셋째, 사회적 지지는 자기효능감, 자아탄력성 각각을 매개로 군 생활 적응에 간접적인 영향을 미치고 또한 자기효능감과 자아탄력성 두 개의 매개변인을 통해 군 생활 적응에 간접적인 영향을 미치는 것으로 확인되었다. 간접효과의 크기는 자기효능감을 매개로 하는 효과가 가장 크게 나타났고, 이어 자기효능감과 자아탄력성을 매개로 하는 효과, 자아탄력성을 매개로 하는 효과 순으로 나타났다. 특히 자기효능감은 군 생활 적응에 미치는 직접효과 및 간접효과에 있어 가장 큰 주요 변인으로 확인되었다. 이 연구 결과를 통해 사회적 지지, 자기효능감, 자아탄력성, 군 생활 적응 간의 인과관계를 확인함으로써 병사들의 군 생활 적응을 예측하고 중재할 수 있는 유의미한 변인을 확인할 수 있었다. 이러한 결과는 병사들의 효과적인 군 생활 적응 중재방안을 수립하는 데 있어 여러 변인들 간의 구조적 관계 및 효과에 대한 보다 체계적이고 통합적인 관점으로 접근할 필요가 있음을 시사 하였다. ; The primary purpose of this study was to investigate what effects the social support, self-efficacy, and ego-resilience perceived by airmen in the military service have on the adjustment to military life, and to identify comparative effect size as well as structural relationship among variables. The study also aimed to explore whether self-efficacy and ego-resilience which are affective characteristics operated as mediating variables. The social support, as a positive resource, obtained by an individual through meaningful interactions with other people plays an effective buffering role in preventing stress and helping adjustment to an environment, Thus it reinforces the ability to overcome frustration and embrace any challenges for solving problems (Sarson, 1991; Sarson et al., 1983). Furthermore, the social support perceived by airmen in the military context can serve as a principal variable that has a significant effect on their adjustment to military life. Bandura (1995) asserted that when people face obstacles and demanding assignments, positive self-efficacy plays a crucial role in enabling people to motivate themselves and to overcome frustration and pain. This implies that the self-efficacy can have a positive effect on task performance and job satisfaction by airmen, and act as a mediating variable of the adjustment to military life. Ego-resilience, a variable that is assumed to arbitrate psychological adjustment by enabling people to employ flexible behaviors and problem solving strategies, faced with circumstantial adversities and stressful situations, can be a factor that predicts and mediates the adjustment of airmen to the military context in which the airmen are forced to face an unfamiliar environment and deal with unaccustomed work. The present study, based on these theories, presents the following research questions. 1. Does the social support perceived by airmen have an effect on self-efficacy, ego-resilience, and the adjustment to military life? 1-1. Does the social support perceived by airmen have an direct effect on the adjustment to military life? 1-2. Does the self-efficacy have an direct effect on the adjustment to military life? 1-3. Does the ego-resilience have an direct effect on the adjustment to military life? 2. Does the social support perceived by airmen have an effect on the adjustment to military life through the mediation of self-efficacy, ego-resilience? 2-1. Does the social support perceived by airmen have an effect on the adjustment to military life through the mediation of self-efficacy? 2-2. Does the social support perceived by airmen have an effect on the adjustment to military life through the mediation of ego-resilience? 2-3. Does the social support perceived by airmen have an effect on the adjustment to military life through the mediation of self-efficacy, ego-resilience? 3. Does the self-efficacy have an effect on the adjustment to military life through the mediation of ego-resilience? In order to investigate the questions above, both preliminary and main questionnaires were conducted on 520 airmen served in ROK Air Force. The validation of the tool was made through the process of both exploratory factor and confirmatory factor analyses on the scale of the adjustment to military life. The model was also analysed by using structural equation model to identify the structural relations among latent variables established in the research model, with the verification of the mediating effect of self-efficacy and ego-resilience. The key findings of the research are summarized as follows: First, it was confirmed that structural model on the structural relationship between the three variables, social support, self-efficacy and ego-resilience and the adjustment to military life, which were based on previous studies and theories, was well supported in the military context. Model test was conducted through TLI, CFI, RMSEA, in which the criteria of model test fit were established, and it turned out that model fit could be acceptable. Thus, it may be seen that research data could be explained relatively appropriately by the research model established in this thesis. Second, it turned out that both the social support and self-efficacy as well as ego-resilience affected the adjustment to military life directly. With regard to the effect size of the variables, they were arranged in the order of self-efficacy, social support, and ego-resilience. Third, the social support had indirect effects on the adjustment to military life through the mediation of self-efficacy and ego-resilience respectively. In addition, the social support affected the adjustment to military life indirectly through the mediation of both self-efficacy and ego-resilience. The effect size was arranged in the order of the mediation of self-efficacy, the mediation of both self-efficacy and ego-resilience, and the mediation of ego-resilience. In particular, self-efficacy turned out to be the primary variable that affected the adjustment to military life both directly and indirectly. The result of the study showed the structural relationship between the three variables, social support, self-efficacy and ego-resilience and the adjustment to military life, those variables turned out to be the ones that can predict and mediate the adjustment to military life. This implies that in establishing effective intervention methods on the adjustment to military life, a systematic and unified approach, based on the structural relationship among various variables and their effectiveness, should be made. ; Master
