Introduction. Nowadays medical online services are evolving rapidly and become more and more popular. Telemedicine is developed as a part of the Russian government program "The medical system 4P". However, doctor-patient online communication has rarely been the subject of research in modern sociology. Nevertheless, online-services are valuable resource for researchers to study and analyze information that is difficult to obtain in other ways.Methodology and sources. The paper is devoted to the research of doctor-patient discursive practices on the internet medical advice forums by the example of "Health.mail.ru". The general method of research is critical discourse analysis, based on the theory and methodology of N. Fairclough, which gives an opportunity to make suggestions about social practices using text analysis and discursive practices analysis. A non-reactive strategy was used in the empirical research. During the research 28 800 messages, which include 14 400 messages from patients and 14 400 messages from doctors, were analyzed. Data analysis was performed in the "R" program.Results and discussion. As a result, some assumptions about social practices and discursive practices on the online forum were formulated. The main patient's goal on online forums is avoiding face-to-face interaction with doctors in a hospital. One of doctor's goals is promoting their services. Interaction is carried out in a form of request from the patient and expert assessment from the doctor in response. Usually, patients send their messages anonymously. Doctors, by contrast, emphasize their identity. It helps them to maintain their image and to legitimate their expertise. Doctor's and patient's discourses are using special medical terminology, and they are not interpreting it. It demands from the patients being experts.Conclusion. Online communication is a new type of doctor-patient communication, which is more democratic, more complementary and more impersonal than face-to-face communication. Usually doctor-patient online communication on the internet medical advice forums does not involve thorough analysis of individual patient's situation and maintaining sustainable interpersonal relationships.

В настоящее время стремительно развиваются и набирают популярность различные онлайн-сервисы медицинской направленности. В рамках реализации государственной программы «Медицинская система 4П» развивается телемедицина. Несмотря на то, что современная социология на данный момент не часто изучает онлайн-взаимодействие врачей и пациентов, интернет-площадки являются ценным ресурсом для исследователей, позволяющим изучить и проанализировать информацию, которую трудно получить другими способами.Методология и источники. Статья содержит результаты исследования дискурсивных практик взаимодействия врачей и пациентов на медицинских онлайн-форумах на примере платформы «Health.mail.ru». В качестве теоретико-методологической рамки была использована трехмерная модель анализа дискурса Н. Феркло, позволяющая на основе изучения текстовых сообщений делать предположения о социальных практиках. Исследование реализовано в нереактивной стратегии. Эмпирическую базу составили 28 800 сообщений (по 14 400 сообщений врачей и пациентов), размещенных на форуме «Health.mail.ru». Данные были обработаны и проанализированы в программе «R».Результаты и обсуждение. По результатам исследования были сформулированы выводы о социальных и дискурсивных практиках участников онлайн-ресурса. Так, пациенты обращаются с запросом на форум с целью заменить онлайнкоммуникацией очное посещение врача, а для врачей одной из задач выступает продвижение своих услуг. Взаимодействие происходит в формате «запрос – экспертная оценка». Если пациенты стремятся сохранить свою анонимность, то медики, напротив, подчеркивают свою идентичность, что не только выступает способом поддержания имиджа, но и обеспечивает легитимность экспертизы. Дискурсы врачей и пациентов содержат специальную медицинскую терминологию, которая не расшифровывается, что требует от пациента выступать в роли компетентного субъекта взаимодействия.Заключение. Онлайн-консультирование представляет собой новый тип взаимоотношений между врачом и пациентом, подразумевающий более демократичную и более симметричную, но в то же время более безличную коммуникацию без глубокого анализа индивидуальной ситуации пациента и формирования устойчивых межличностных связей.
Epilepsy is a common disease of the nervous system that requires long-term (often lifelong) use of drugs that allow many people with this diagnosis to lead an active social life with no risk of disability or life-threatening situations. Despite the existing normative acts regulating the provision of free medicines to epilepsy patients, there are still many problems associated with the compliance with the current legislation (Constitution of the Russian Federation) and the availability of free medicines for patients with epilepsy. In this article, we tried to cover the main norms of the current legislation regulating the issues of providing patients with epilepsy with free medicines; this can be helpful for both physicians and patients in overcoming bureaucratic barriers when confronting medical officials in defending the rights of patients. ; Эпилепсия – распространенное заболевание нервной системы, требующее многолетнего, нередко пожизненного применения лекарственных препаратов, что позволяет многим людям с таким диагнозом вести активную социальную жизнь, избегая угрозы инвалидизации или жизнеугрожающих ситуаций. Несмотря на имеющиеся нормативно-правовые акты, достаточно полно регламентирующие вопросы обеспечения пациентов с диагнозом «эпилепсия» бесплатными лекарственными препаратами, остается много вопросов в части соответствия действующего законодательства заявленным в Конституции РФ нормам, а также в части доступности лицам с эпилепсией бесплатных лекарственных препаратов в рамках, обозначенных законодательством. В этой статье мы постарались отразить основные нормы действующего законодательства, регламентирующие вопросы обеспечения пациентов с эпилепсией бесплатными лекарственными препаратами, знакомство с которыми поможет всем заинтересованным лицам (врачам и пациентам) в преодолении бюрократических барьеров при противостоянии с чиновниками от медицины в отстаивании законных прав пациентов.
