Cost-effectiveness of treating first-episode psychosis: five year follow-up results from an Italian early intervention programme
Aim: Early intervention programmes are expected to result in the reduction of illness severity in patients with schizophrenia, and contain healthcare costs by reducing hospital admissions and improving the social functioning of patients. This study aimed to investigate the costeffectiveness of treatment in an early intervention programme in comparison to standard care. Methods: Retrospective analysis of data prospectively recorded in an urban area (Milan, Italy). Twentythree patients from an early intervention programme and 23 patients from standard care with first-episode psychosis were evaluated on their use of services over a 5-year period. The Health of the Nation Outcome Scale was used to measure clinical status. Results: Significant changes with respect to initial assessment were recorded on the Health of the Nation Outcome Scale,with larger effect sizes in the early intervention programme than in the standard care group. Consequently, the cost-effectiveness ratio per reduced score of severity was lower in the early intervention programme than in standard care (€ 4802 vs. € 9871), with an incremental costeffectiveness ratio, or net saving of € -1204 for every incremental reduced score of severity. Over time, greater recourse to hospital and residential facilities to obtain comparable improvement in symptoms resulted in a steady cost increase for the patients in standard care. Conclusions: Allocation of funds to specialized early intervention programmes is the best alternative, as it can save costs by reducing the use of hospitals and residential facilities, and may produce net savings of costs in the long term.

Protocolo cognitivo de gerenciamento e controle do estresse ocupacional: um estudo de caso

Este é um estudo de caso desenvolvido em 2017 numa clínica universitária, e teve como objetivo analisar um caso quanto à eficácia de um protocolo de atendimento individual, de base cognitiva, no tratamento, controle e gerenciamento do estresse ocupacional. Participou do estudo um homem de 30 anos, casado, que apresentava estresse em quase exaustão. Foi realizada uma entrevista de anamnese e avaliação por meio de inventários para identificação do nível de estresse e percepção do contexto de trabalho. Na sequência, foi aplicado um protocolo de gerenciamento do estresse organizado em oito encontros estruturados e baseados na terapia cognitiva. Os resultados demonstraram melhora do quadro de estresse e de percepção geral da realidade, apontando para a eficácia do protocolo, passível de ser aplicado em programas focais para controle de estresse ocupacional.

O Panorama Moral e Social no Fazer Docente em Campo Grande-MS e suas Escolhas Éticas: Dilemas e Implicações

Resumo Este trabalho discute os problemas éticos cuja contextualização abrange as vivências de seus autores, em suas práticas pedagógicas, mais especificamente, os contextos escolares de escolas públicas, nos quais se colocam diariamente os professores, diante do número crescente de profissionais da educação que se ausentam do trabalho amparados em atestados médicos ou laudos psiquiátricos, em razão de adoecimento no trabalho docente, em seu contexto escolar. O objetivo geral foi analisar os problemas éticos enfrentados por professores da rede de educação básica pública, bem como suas consequências para professores e para a educação como um todo. Como hipótese de tal projeto se supõe que o professor não está preparado para gerenciar os conflitos éticos decorrentes da sua prática docente. A metodologia utilizada foi da pesquisa qualitativa em educação sobre a observação sistemática da participação dos professores em palestras utilizadas como oficinas para a Rede Municipal de Ensino de Campo Grande, MS em maio de 2019, nas quais foram aplicados questionários sobre os problemas éticos com os que eles têm se defrontado. Os resultados foram coletados a partir das respostas aos questionários. As análises das respostas obtidas nessa abordagem permitiram alcançar percepções, impressões e intuições fundamentais para a conclusão de caráter subjetivo.

AbstractThis work discusses on the ethical problems whose contextualization encompasses the experiences of their authors, in their pedagogical practices, more specifically, the public schools contexts, in which teachers are placed daily, in view of the growing number of education professionals who are absent from education. work supported by medical certificates or psychiatric reports, due to illness in the teaching work, in their school context. The general objective was to analyze the ethical problems faced by teachers in the public basic education network, as well as their consequences for teachers and for education as a whole. As a hypothesis of such a project, it is assumed that the teacher is not prepared to manage the ethical conflicts arising from his or her teaching practice. The methodology used was qualitative research in education on the systematic observation of the teachers' participation in lectures used as workshops for the Municipal Education Network of Campo Grande, MS in May 2019, in which questionnaires about ethical problems were applied with them which they have been facing. Results were collected from the responses to the questionnaires. The analysis of the responses obtained in this approach allowed to reach fundamental perceptions, impressions and intuitions for the conclusion of a subjective character.

This measurement is based on the observation of events with an imbalance of transverse energy in excess of 130 GeV and a single photon in the absolute pseudorapidity range vertical bar eta vertical bar nugamma production cross section is measured to be 21.1 +/- 4.2(stat.)+/- 4.3(syst.)+/- 0.5(lum.)fb, which agrees with the standard model prediction of 21.9 +/- 1.1 fb. The results are combined with the CMS measurement of Z gamma production in the l(+)l(-)gamma final state (where l is an electron or a muon) to yield the most stringent limits to date on triple gauge boson couplings. vertical bar h(3)(Z)vertical bar < 2.7 x 10(-3), vertical bar h(4)(Z)vertical bar < 1.3 x 10(-5) for ZZ gamma and vertical bar h(3)(gamma)vertical bar < 2.9 x 10(-3), vertical bar h(4)(gamma)vertical bar < 1.5 x 10(-5) for Z gamma gamma couplings.

This prediction is tested by measuring the mass difference between the top quark and antiquark (Delta m(t) = m(t) - m((t) over bar)) that are produced in pp collisions at a center-of-mass energy of 8 TeV, using events with a muon or an electron and at least four jets in the final state. The analysis is based on data corresponding to an integrated luminosity of 19.6 fb(-1) collected by the CMS experiment at the LHC, and yields a value of Delta m(t) = 0.15 0.19 (stat) +/- 0.09(syst) GeV, which is consistent with the SM expectation. Charged pions, kaons, and protons in the transverse-momentum range -1.7 and laboratory rapidity are identified via their energy loss in the silicon tracker. The average increases with particle mass and the charged multiplicity of the event. The increase of the average with charged multiplicity is greater for heavier hadrons. Comparisons to Monte Carlo event generators reveal that Epos Lhc, which incorporates additional hydrodynamic evolution of the created system, is able to reproduce most of the data features, unlike Hijing and Ampt. The spectra and integrated yields are also compared to those measured in pp and PbPb collisions at various energies. The average transverse momentum and particle ratio measurements indicate that particle production at LHC energies is strongly correlated with event particle multiplicity.

The datasets correspond to integrated luminosities of about 31 nb(-1) (pPb) and 5.4 pb(-1) (pp), collected in 2013 by the CMS experiment at the LHC. Upsilons that decay into muons are reconstructed within the rapidity interval |y (CM)| < 1.93 in the nucleon-nucleon centre-of-mass frame. Their production is studied as a function of two measures of event activity, namely the charged-particle multiplicity measured in the pseudorapidity interval |eta| < 2.4, and the sum of transverse energy deposited at forward pseudorapidity, 4.0 < |eta| < 5.2. The cross sections normalized by their event activity integrated values, (nS)/aEuro(nS)aEuro parts per thousand, are found to rise with both measures of the event activity in pp and pPb. In both collision systems, the ratios of the excited to the ground state cross sections, (nS)/ (1S), are found to decrease with the charged-particle multiplicity, while as a function of the transverse energy the variation is less pronounced. The event activity integrated double ratios, [(nS)/ (1S)](pPb) /[(nS)/ (1S)](pp), are also measured and found to be 0.83 +/- 0.05 (stat.) +/- 0.05 (syst.) and 0.71 +/- 0.08 (stat.) +/- 0.09 (syst.) for (2S) and (3S), respectively.

The data sample, selected on the basis of the dimuon decay mode of the J/psi, corresponds to an integrated luminosity of 5.2 fb(-1). Fitting the structure to an S-wave relativistic Breit-Wigner lineshape above a three-body phase-space nonresonant component gives a signal statistical significance exceeding five standard deviations. The fitted mass and width values are m = 4148.0 +/- 2.4 (stat.) +/- 6.3 (syst.) MeV and Gamma = 28(-11)(+15) (stat.) +/- 19 (syst.) MeV, respectively.
