Aim - based on the existing regulations and recent documents from the regulatory bodies, determine the methodology for the development of clinical guidelines (CG) that meet the requirements of the legislation in the Russian Federation.Materials and methods. We analyzed a number of regulatory and subordinate acts confirming the concept and status of the CG, in particular, the Federal Law № 323-ФЗ "On the principles of health care for citizens of the Russian Federation", Federal Law № 489-ФЗ "On the amendments to article 40 of the Federal Law "On compulsory medical insurance in the Russian Federation" and the Federal Law "On the principles of health care for citizens of the Russian Federation" concerning clinical guidelines, Order № 102н of the Ministry of Health of the Russian Federation dated 28 Feb 2019 "Regulations on the scientific and practical council of the Ministry of Health of Russia" and Order № 103н of the Ministry of Health of the Russian Federation of 28 Feb 2019 "On Approval of the procedure and terms for the development of clinical guidelines and their revision, the model format of clinical guidelines and requirements to their structure, contents and scientific validity of the information included in the clinical guidelines". In addition, we used the documents of regulatory bodies of the Russian Federation, expert opinions from medical professionals and non-profit organizations as well as leading opinions from public sources.Results. Order № 102н of the Ministry of Health of the Russian Federation secured the right of the scientific and practical council to consider and decide on the CG. Order № 103н of the Ministry of Health of the Russian Federation approved the procedure and deadlines for the development/revision of the CG; the typical format of CG; the requirements for the structure of CG, its contents and scientific validity of the information included in the CG. A step-by-step algorithm was created for the procedure and timing of CG development and update; the above documents elaborate on the requirements to the structure of CG, the contents and scientific validity of the information included. There are also comments on the development and design of each CG section (title page, table of contents, list of abbreviations, terms and definitions, brief information on the disease, diagnosis, treatment, medical rehabilitation, prevention and follow-up care, organization of medical care, additional information, criteria for assessing the quality of medical care, list of references, composition of the working group on the development and revision of the CG, methodology of the development of the CG, reference materials, algorithms for doctor's actions, patient information, assessment scales, questionnaires, and other patient assessment tools). The recently adopted requirements for CG registration are compared with the previous methodological recommendations of the regulatory bodies.Discussion. The unification of treatment strategies and the use of evidence-based reasoning in the updated CG will improve the quality of medical care and have a positive impact on the medical insurance segment and the judicial decisions. However, the presented concept of CG has attracted a number of critical comments from representatives of the medical community and medical lawyers. For example, the updated regulatory acts do not specify the deadlines and algorithms for updating the CG; these acts do not address the coordination between various medical non-profit organizations although such coordination is needed to avoid work duplication; the acts determine no specific resources required for the CG development (the latter flaw may have a negative impact on the quality and timing of the resulting CG). Some experts believe that it would be advisable to implement the system called GRADE. In addition, possible legal risks are analyzed in light of the fact that the CG are not regulatory legal documents.Conclusion. Periodic improvements in the CG renewing and approving procedure are an integral part of this large-scale process that introduces changes in the regulatory framework and clinical practice. Compulsory adherence to the CG is a due element of today's high quality medical care. ; Цель - на основании анализа нормативных актов и документов регуляторных органов определить подходы и методологию разработки клинических рекомендаций (КР), соответствующих требованиям законодательства РФ.Материалы и методы. Проанализированы нормативные и подзаконные акты, закрепляющие понятие и статус КР, в частности, Федеральный Закон № 323-ФЗ «Об основах охраны здоровья граждан в Российской Федерации», «Федеральный закон № 489-ФЗ «О внесении изменений в статью 40 Федерального закона «Об обязательном медицинском страховании в Российской Федерации» и Федеральный закон «Об основах охраны здоровья граждан в Российской Федерации» по вопросам клинических рекомендаций», Приказ Минздрава РФ от 28 февраля 2019 г. № 102н «Положение о научно-практическом совете Минздрава России» и Приказ Минздрава РФ от 28 февраля 2019 г. № 103н «Об утверждении порядка и сроков разработки клинических рекомендаций, их пересмотра, типовой формы клинических рекомендаций и требований к их структуре, составу и научной обоснованности включаемой в клинические рекомендации информации». Кроме этого, анализировались источники регуляторных органов РФ, экспертные мнения представителей медицинских профессиональных некоммерческих организаций и лидеров мнений в открытых источниках.Результаты. Приказом № 102н Минздрав РФ закрепил право рассмотрения КР за научно-практическим советом. Приказом № 103н Минздрав РФ утвердил порядок и сроки разработки/пересмотра КР; типовую форма КР; требования к структуре КР, составу и научной обоснованности включаемой в КР информации. Определен пошаговый алгоритм порядка и сроков разработки КР, обновления КР, требования к структуре КР, составу и научной обоснованности включаемой информации. Приведены детальные комментарии по разработке и оформлению каждого из разделов КР (титульный лист, оглавление, список сокращений, терминов и определений, краткая информация по заболеванию, диагностика, лечение, медицинская реабилитация, профилактика и диспансерное наблюдение, организация оказания медицинской помощи, дополнительная информация, критерии оценки качества медицинской помощи, список литературы, состав рабочей группы по разработке и пересмотру КР, методология разработки КР, справочные материалы, алгоритмы действий врача, информация для пациента, шкалы оценки, вопросники и другие оценочные инструменты состояния пациента). Приведены результаты сравнения актуальных требований к оформлению КР с предыдущими методическими рекомендациями регуляторных органов.Обсуждение. Унификация подходов к лечению, использование в КР доказательного подхода позволит повысить качество оказания медицинской помощи, оказать позитивное влияние на медицинское страхование и судебные решения. Однако концепция КР собрала ряд критических замечаний от представителей профессионального врачебного сообщества и медицинских юристов: так, в нормативных актах не регламентированы крайние сроки и алгоритм шагов при обновлении КР, координация работы медицинских профессиональных некоммерческих организаций с целью избежать дублирования процессов, не определен источник ресурсов для разработки КР, что может увеличить риски разработки КР низкого качества и срыва сроков разработки. Ряд экспертов считают, что было бы оправданно внедрить систему GRADE. Продемонстрированы и юридические риски, поскольку КР не являются нормативно-правовыми актами.Заключение. Дальнейшее совершенствование процесса разработки и утверждения КР закономерно и является неотъемлемой частью любого процесса, предусматривающего масштабные изменения нормативной базы и сложившейся практики. Последовательное внедрение в клиническую практику обязательного следования КР имеет высокие шансы позитивно повлиять на качество оказания медицинской помощи.

Objective: to justify the main directions of optimizing the medical care system for HIV-infected military personnel of the Ministry of Defense of Russia on the basis of studying the clinical, epidemiological and social characteristics of. servicemen.The materials consisted of 188 case histories of servicemen undergoing examination and treatment in the only specialized department for military medical examination and treatment of the Ministry of Defense of Russia. In the processing and analysis of data we used modern methods of descriptive statistics.Results of the study. HIV infection rate of the servicemen in Russian Federation is at a consistently low level. HIV-positive servicemen are characterized by: sexual heterosexual way of infection; high social status (income, education, housing conditions), low prevalence of associated pathology. Access of most HIV-positive military personnel to dispensary observation (76,6%) and receiving antiretroviral therapy (66,1%) in the centers for the Prevention and Control of AIDS and Infectious Diseases has already been implemented, despite the lack of a regulatory framework of the Russian Ministry of Defense. A high level of adherence to dispensary observation and receiving antiretroviral therapy (about 95%) was noted in HIV-positive military personnel. Ensuring the legitimate access of HIV-infected military personnel to antiretroviral therapy practically does not require an increase in funding. ; Цель исследования: на основании изучения клинико-эпидемиологических и социальных характеристик больных ВИЧинфекцией военнослужащих Минобороны России обосновать основные направления оптимизации системы медицинской помощи военнослужащим по контракту.Материалы составили 188 историй болезней военнослужащих по контракту, проходивших обследование и лечение в единственном специализированном отделении для освидетельствования и лечения больных с ВИЧ-инфекцией Минобороны России. При обработке и анализе данных использованы современные методы описательной статистики.Результаты исследования. Пораженность личного состава Вооруженных Сил Российской Федерации ВИЧ-инфекцией находится на стабильно низком уровне. Для ВИЧ-положительных военнослужащих характерны: половой гетеросексуальный путь инфицирования; высокий социальный статус (уровень дохода, образование, жилищные условия), низкий уровень распространенности ВИЧ-ассоциированной и сочетанной патологии. Уже реализован доступ большинства ВИЧ-позитивных военнослужащих к диспансерному наблюдению (76,6%) и получению антиретровирусной терапии (66,1%) в центрах по профилактике и борьбе со СПИД и инфекционными заболеваниями, несмотря на отсутствие разрешительной нормативной базы Минобороны России. У ВИЧ-позитивных военнослужащих отмечен высокий уровень приверженности диспансерному наблюдению и приему антиретровирусной терапии (около 95%). Обеспечение легитимного доступа ВИЧ-позитивных военнослужащих к антиретровирусной терапии практически не потребует увеличения государственного финансирования.

More than 1,7 million people, 770 thousand people from HIV infection, died from tuberculosis worldwide in 2018. In accordance with the United Nations Sustainable Development Goals and the World Health Organization's strategy to fight tuberculosis, all countries need to reduce the incidence of tuberculosis by 80% and reduce the death rate from tuberculosis by 90% by 2030. According to the Joint United Nations programme on HIV/AIDS (UNAIDS) goals of «90–90–90», countries should achieve the following indicators by 2020: «90% of people living with HIV infection should know their HIV status; 90% of those who know about their HIV infection should receive treatment, and 90% of those who are being treated should not have their viral load determined». Socially significant diseases are a serious problem for the Russian Federation (RF). The RF is one of 30 countries with a high incidence of tuberculosis and ranks 3rd place in multidrug-resistant tuberculosis and 6th place in the prevalence of HIV infection. Socially significant diseases present a serious problem for the Armed Forces of the RF. 150 new cases of tuberculosis, over 50 cases of HIV infection are diagnosed every year among military personnel of the RF Armed Forces.The purpose of the study is to analyze the current epidemiological situation of tuberculosis and HIV infection in the armed forces of the Russian Federation and foreign armies, to generalize domestic and foreign experience in the organization of medical care for military personnel with these diseases.Data from reports of the medical service and a review of domestic Russian and foreign medical literature allowed us to assess TB and HIV sickness rate among military personnel in the RF and foreign states, as well as to characterize the current system of medical care for this category of patients. ; В 2018 г. во всем мире от туберкулеза умерло более 1,7 млн человек, 770 тыс. человек от ВИЧ-инфекции. В соответствии с Целями устойчивого развития Организации Объединенных Наций и стратегией Всемирной организации здравоохранения по борьбе с туберкулезом к 2030 году всем странам необходимо добиться снижения заболеваемости туберкулезом на 80% и снижения смертности от туберкулеза на 90%. В соответствии с целями Объединенной программы Организации Объединенных Наций по ВИЧ/СПИД (ЮНЭЙДС) «90–90–90» страны должны достигнуть к 2020 г. следующих показателей: «90% человек с ВИЧ-инфекцией должны знать свой ВИЧ-статус, 90% из тех, кто знает о своей ВИЧ-инфекции, получать лечение, и у 90% человек из тех, кто лечится, вирусная нагрузка не должна определяться».Для Российской Федерации (РФ) социально значимые заболевания представляют серьезную проблему. РФ входит в список 30 стран с высоким уровнем заболеваемости туберкулезом и занимает третье место по туберкулезу со множественной лекарственной устойчивостью и шестое место по распространенности ВИЧ-инфекции. Каждый год только у военнослужащих Вооруженных Сил Российской Федерации диагностируется более 150 новых случаев туберкулеза и свыше 50 случаев ВИЧ-инфекции.Лечение одного пациента с туберкулезом может обходиться Министерству обороны РФ от 300 тыс. до 1,5 млн рублей, а ежегодные расходы в военной системе здравоохранения, связанные с затратами на противотуберкулезные препараты, могут превышать десятки миллионов рублей. С учетом сложившейся демографической ситуации в мире и РФ заболеваемость туберкулезом и ВИЧ-инфекцией приобретает особенно важное значение и влияет на обороноспособность государств.Цель исследования: анализ современной эпидемиологической ситуации по туберкулезу и ВИЧ-инфекции в ВС РФ и иностранных армиях, обобщение отечественного и зарубежного опыта в организации оказания медицинской помощи военнослужащим с данными заболеваниями.Данные отчетов медицинской службы и обзор отечественной и иностранной литературы позволили оценить заболеваемость туберкулезом и ВИЧ-инфекцией военнослужащих ВС РФ и иностранных государств, а также охарактеризовать сложившуюся систему оказания медицинской помощи данной категории пациентов.
