Im Mittelpunkt der Untersuchung stehen die innerrussische Diskussion um die Dritte-Welt-Problematik sowie deren konzeptionelle Auslegung im Rahmen eines breiteren Diskurses um die russische Identität. Die Frage nach dem russischen Identitätsdiskurs als Grundlage für ein Sonderverhältnis zur Dritten Welt macht vor allem die Bedeutung der russischen Diskussion um Eurasismus und Tiersmondisierung für die Debatte um die außenpolitische Orientierung des Landes deutlich. Vor allem mit der Abwendung von einer einseitigen Westorientierung seit 1993 ist in der russischen Außenpolitik eine Wende zur eurasischen Ausrichtung festzustellen. Gründe für ein neues Interesse Rußlands an der Dritten Welt sind die Sehnsucht eines Großteils der politischen Elite nach der Größe und Omnipräsenz der Sowjetzeit, wirtschaftliche Erwägungen, das Problem der transkaukasischen und zentralasiatischen GUS-Staaten sowie das Interesse an internationalen Modernisierungsmodellen. Für die zweite Hälfte der neunziger Jahre ist eine aktivere Rolle Rußlands in der Dritten Welt zu erwarten. (ICE2)

Nach den Transformationen 1989 bis 1991 wurde Europa mit dem überwunden geglaubten Problem der "nationalen Frage" überrascht, obwohl alle Unabhängigkeitsbewegungen in Osteuropa und der UdSSR Ende der 80er Jahre mit der Idee des Nationalstaates verknüpft waren. Einzige Ausnahme bildete Rußland, das heute auf der schwierigen Suche nach einer neuen Identität sei. Ausgehend von diesen Überlegungen skizziert die Autorin historische Wurzeln der russischen Staatsbildung, erläutert das Verhältnis der demokratischen sozialen Bewegungen zur "russischen Frage" (Aufbau eines russischen Nationalstaates etc.). Daran anknüpfend skizziert sie konkurrierende Paradigmen der russischen Nationalidee (fundamentalistisch konservativer russischer Nationalismus, Eurasismus etc.). Auf der Suche nach einer neuen russischen Identität wende man sich vor allem den "ewigen" russischen Werten zu, alte Ideen- und Wertsysteme dominieren die heutige Diskussion, heißt ein Fazit. (rk)

Le rôle séculaire de puissance impériale de la Russie, qu'elle a perdu après l'éclatement de l'URSS, reste au cœur des préoccupations de la plus grande partie de l'équipe dirigeante russe. A ce titre, elle considère la « défection » de l'Ukraine comme un événement inacceptable, auquel il faudra remédier d'une manière ou d'une autre. Cependant, s'il se trouve des forces politiques issues de l'ancienne nomenklatura, en particulier en Ukraine centrale et orientale, qui espèrent pouvoir retrouver leurs positions politiques et sociales d'antan par le biais d'un rapprochement avec la Russie et qui, par conséquent, préconisent la « réunification » avec Moscou comme l'unique moyen de surmonter les difficultés actuelles, la grande majorité des responsables politiques du jeune Etat perçoivent au contraire les velléités impériales qui se font de plus en plus jour en Russie comme une menace immédiate pour la nation ukrainienne.

From mid-1990 to mid-1991, the Soviet Union witnessed increasing clashes between democrats and conservatives inside the Party and the rising pressure of daims for independence from ail the Soviet Republics. The USSR, which had previously played off concessions — in terms of its security policy and economie reforms — against real political support and massive économie aid from the West, imposed a turning point through the « 9 + 1 » treaty in order to save the cohesion of the Fédération. Negotiations with the republics then differed from the methods used during the winter of 1990-1991. Although the existing System was challenged, no clear indication was given of the way things could swing. A definitive transformation perhaps, but one whose consequences on international stability are unknown.

The article is devoted to the impact of healthcare reform on the situation in the industry and the requirements for managers of medical organizations. On the data from the expert survey, participants of which were physicians with practical experience in the field of health management; economists studying the work of the public sector; lawyers involved in the protection of patients' rights, etc., it provides characteristics of today's financial, logistical support and staffing of medical institutions, their relationships with founders, insurance companies and patients; there are given experts' opinions on who should be the leaders of medical organizations — managers or clinicians, how to create their personnel reserve, what should be changed in the system of professional development and retraining of medical personnel. It is shown that the working conditions of medical organizations have radically changed, and the amount of attracted extra-budgetary funds has become an important indicator of their effectiveness. At the same time, the institutional context, in which they function, is highly controversial, devoid of a number of fundamentally important elements, and is not balanced in terms of the rights and obligations of various actors involved in the provision of medical care. This significantly complicates the activity of the management of medical institutions, generates a «conflict of interests» among them, which ultimately negatively affects patients. According to experts, today an experienced manager should be at the head of the medical institution, but — with the obligatory good knowledge and understanding of the specifics of the medical industry. Since medical institutions are now managed by medics, their training in economics and management within the system of continuing professional education becomes very important.
