Background: Community engagement and participation has played a critical role in successful disease control and elimination campaigns in many countries. Despite this, its benefits for malaria control and elimination are yet to be fully realized. This may be due to a limited understanding of the influences on participation in developing countries as well as inadequate investment in infrastructure and resources to support sustainable community participation. This paper reports the findings of an atypical systematic review of 60 years of literature in order to arrive at a more comprehensive awareness of the constructs of participation for communicable disease control and elimination and provide guidance for the current malaria elimination campaign. Methods: Evidence derived from quantitative research was considered both independently and collectively with qualitative research papers and case reports. All papers included in the review were systematically coded using a pre-determined qualitative coding matrix that identified influences on community participation at the individual, household, community and government/civil society levels. Colour coding was also carried out to reflect the key primary health care period in which community participation programmes originated. These processes allowed exhaustive content analysis and synthesis of data in an attempt to realize conceptual development beyond that able to be achieved by individual empirical studies or case reports. Results: Of the 60 papers meeting the selection criteria, only four studies attempted to determine the effect of community participation on disease transmission. Due to inherent differences in their design, interventions and outcome measures, results could not be compared. Development of effective policy responses to address complex public health problems can be challenged by a lack of clarity about the interaction of risk factors driving the problem, differing views of stakeholders on the most appropriate and effective intervention approaches, a lack of evidence to support commonly implemented and acceptable intervention approaches, and a lack of acceptance of effective interventions. Consequently, political considerations, community advocacy and industry lobbying can contribute to a hotly contested debate about the most appropriate course of action; this can hinder consensus and give rise to policy resistance. The problem of alcohol misuse and its associated harms in New South Wales (NSW), Australia, provides a relevant example of such challenges. Dynamic simulation modelling is increasingly being valued by the health sector as a robust tool to support decision making to address complex problems. It allows policy makers to ask 'what-if' questions and test the potential impacts of different policy scenarios over time, before solutions are implemented in the real world. Participatory approaches to modelling enable researchers, policy makers, program planners, practitioners and consumer representatives to collaborate with expert modellers to ensure that models are transparent, incorporate diverse evidence and perspectives, are better aligned to the decision-support needs of policy makers, and can facilitate consensus building for action. This paper outlines a procedure for embedding stakeholder engagement and consensus building in the development of dynamic simulation models that can guide the development of effective, coordinated and acceptable policy responses to complex public health problems, such as alcohol-related harms in NSW.

Background: Early diagnosis and prompt effective case management are important components of any malaria elimination strategy. Tafea Province, Vanuatu has a rich history of traditional practices and beliefs, which have been integrated with missionary efforts and the introduction of modern constructions of health. Gaining a detailed knowledge of community perceptions of malarial symptomatology and treatment-seeking behaviours is essential in guiding effective community participation strategies for malaria control and elimination. Method: An ethnographic study involving nine focus group discussions (FGD), 12 key informant interviews (KII) and seven participatory workshops were carried out on Tanna Island, Vanuatu. Villages in areas of high and low malaria transmission risk were selected. Four ni-Vanuatu research officers, including two from Tanna, were trained and employed to conduct the research. Data underwent thematic analysis to examine treatment-seeking behaviour and community perceptions of malaria. Results: Malaria was perceived to be a serious, but relatively new condition, and in most communities, identified as being apparent only after independence in 1980. Severe fever in the presence of other key symptoms triggered a diagnosis of malaria by individuals. Use of traditional or home practices was common: perceived vulnerability of patient and previous experience with malaria impacted on the time taken to seek treatment at a health facility. Barriers to health care access and reasons for delay in care-seeking included the availability of health worker and poor community infrastructure. Conclusion: Due to programme success of achieving low malaria transmission, Tafea province has been identified for elimination of malaria by 2012 in the Government of Vanuatu Malaria Action Plans (MAP). BACKGROUND AND AIM: Evaluations of alcohol policy changes demonstrate that restriction of trading hours of both 'on' and 'off' license venues can be an effective means of reducing rates of alcohol-related harm. Despite this, the effects of different trading hour policy options over time, accounting for different contexts and demographic characteristics, and the common co-occurrence of other harm reduction strategies in trading hour policy initiatives are difficult to estimate. The aim of this study was to use dynamic simulation modelling to compare estimated impacts over time of a range of trading hour policy options on various indicators of acute alcohol related harm. METHODS: An agent-based model of alcohol consumption in New South Wales, Australia was developed using existing research evidence, analysis of available data, and a structured approach to incorporating expert opinion. Five policy scenarios were simulated, including restrictions to trading hours of on-license venues and extensions to trading hours of bottle shops. The impact of the scenarios on four measures of alcohol-related harm were considered: total acute harms; alcohol-related violence; emergency department presentations; and hospitalisations. RESULTS: Simulation of a 3am (rather than 5am) closing time resulted in an estimated 12.3 ± 2.4% reduction in total acute alcohol-related harms, a 7.9 ± 0.8% reduction in violence, an 11.9 ± 2.1% reduction in ED presentations, and a 9.5 ± 1.8% reduction in hospitalisations. Further reductions were achieved simulating a 1am closing time including a 17.5 ± 1.1% reduction in alcohol-related violence. Simulated extensions to bottle shop trading hours resulted in increases in rates of all four measures of harm, although most of the effects came from increasing operating hours from 10pm to 11pm. CONCLUSIONS: An agent-based simulation model suggests that restricting trading hours of licensed venues reduces rates of alcohol-related harm and extending trading hours of bottle shops increases rates of alcohol-related harm. The model can estimate the effects of a range of policy options.

Cystic echinococcosis (CE) is endemic among the human population of Xiji County, Ningxia Hui Autonomous Region, China, where the prevalence is estimated to be between 2.2% and 3.6%. Government-run sheep abattoirs in Xiji County have closed in recent years and, as a consequence, slaughter is carried out mostly at rural market places. The market place in Xinglong Township, Xiji County, is home to an increasing number of stray dogs and the lack of government control over slaughter practices potentially favours Echinococcus granulosus transmission. A survey of sheep, goats and cattle reared in Xiji County was conducted in Xinglong Market and Xinglong Township to determine prevalence and transmission dynamics of E. granulosus infection. The liver and lungs of all livestock aged one year and older were examined macroscopically post mortem; visual examination and palpation of organs determined overall prevalence of E. granulosus. Cysts consistent in appearance with E. granulosus were observed in 2/184 sheep (prevalence 1.0%) and 1/55 of the cattle examined (prevalence 1.8%); 0/13 goats were found to be infected. However, microscopic examination of these suspected cysts failed to confirm these samples as E. granulosus, giving a prevalence of confirmed infection of zero percent in all three species. The prevalence of liver fluke was 61.3% in sheep and 12.7% in cattle with a significant difference between males and females (p ≤ 0.001). Considering the high prevalence of echinococcosis in the local human population, the absence of CE observed among commercially slaughtered livestock was surprising. To assist with proactive and effective responses to the global COVID-19 crisis, the scientific community has been rapidly deploying our most advanced analytic tools to model the dynamics of disease transmission based on existing (albeit imperfect) knowledge, data, and available human and material resources. The multifactorial, multilevel influences on transmission dynamics and the disease's pervasive impact at the individual, community, and global levels have required the use of the analytic techniques of complex systems science, namely, systems modeling and simulation, to forecast the trajectory of the disease under different conditions, to quantify uncertainty, and to inform effective responses (1–3). These methods have been deployed by infectious disease epidemiologists for over a century (4), maturing into a robust interdisciplinary field intersecting mathematics, computational epidemiology, ecology, evolutionary biology, immunology, behavioral science, and public health (5). As a result, there have been numerous advances that have informed policies to control infectious diseases, facilitate epidemic and bioterrorism preparedness, and provide governments with critical tools for managing complexity and weighing alternative responses in the midst of the confusion of an evolving crisis (6–14). The field's commitment to achieving rapid response capability in the face of changing conditions has led to advances in rapid assessment of the impact of the pandemic, and data assimilation methods that combine theory with empirical observations in a continuous knowledge feedback process facilitating continuous hypothesis development, testing, and refinement in the service of more effective decision making (15–19).

Background: The Australian Government's Pacific Malaria Initiative (PacMI) is supporting the National Malaria Program in both Solomon Islands and Vanuatu, complementing assistance from the Global Fund for AIDS, Tuberculosis and Malaria (GFATM). Two remote island groups - Tafea Province, Vanuatu and Temotu Province, Solomon Islands have been selected by the governments of both countries as possible malaria elimination areas. To provide information on the prevalence and distribution of the disease within these island groups, malariometric surveys were conducted during the wet seasons of 2008. Methods: In Tafea Province, a school-based survey was conducted which included the 2-12 y age group, while in Temotu a village based all-ages survey was conducted. An effort was made to sample villages or schools from a wide an area as possible on all islands. Diagnosis was initially based on Giemsa stained blood slides followed by molecular analysis using polymerase chain reaction (PCR). Results: In Tafea Province, 73% (5238/7150) of children (2-12 y) were surveyed and in Temotu Province, in the all-ages survey, 50.2% (8742/17410) of the provincial population participated in the survey. In both Vanuatu and Solomon Islands malariometric surveys of their southern-most islands in 2008 showed relatively low over-all malaria parasite prevalence (2 to 3%). Other features of malaria in these island groups were low parasitaemia, low gametocyte carriage rates, low spleen rates, low malaria associated morbidity, a high incidence of asymptomatic infections, and a predominance of Plasmodium vivax over Plasmodium falciparum. Conclusion: For various reasons malaria rates are declining in these provinces providing a favourable situation for local malaria elimination. This will be advanced using mass distribution of bed nets and selective indoor residual spraying, the introduction of rapid diagnostic tests and artemisinin combination therapy, and intensive case detection and surveillance. It is as yet uncertain whether malaria parasites can themselves be sustainably eliminated from entire Melanesian islands, where they have previously been endemic. Key issues on the road to malaria elimination will be continued community involvement, improved field diagnostic methods and elimination of residual P. vivax parasites from the liver of asymptomatic persons.
