Drowning is a significant risk in fishing communities in Lake Victoria, East Africa. In the southern part of the lake, within the United Republic of Tanzania (URT), the incidence of fatal drowning is estimated at 217 people per 100,000 person-years, far exceeding the estimated national average incidence (Sarassett et al. 2019; Whitworth et al. 2019). This study aimed to identify major stakeholders and potential approaches to drowning reduction advocacy for small-scale and artisanal fishers in southern Lake Victoria. Four key research questions were the focus of the study: a. Who are the main individual and institutional actors affecting or affected by changes in drowning policies or practices related to fishers in southern Lake Victoria? b. What is the existing policy and regulatory framework governing safety on the lake for fishers in southern Lake Victoria? c. Why have efforts to increase lake safety among fishers in southern Lake Victoria been successful or unsuccessful? d. How could current and potential stakeholders become more engaged in advocacy and agenda-setting to reduce fisher drowning in southern Lake Victoria? Research methods consisted of: • Review of 18 national acts, rules, regulations, policies and plans and 20 international agreements and voluntary guidance documents that are relevant to fisher water safety and drowning reduction in southern Lake Victoria, Tanzania; • 31 semi-structured in-depth interviews primarily conducted with government officers who work directly with the fisheries industry (i.e. shipping/transport, fisheries, or police) from the local to national levels; and • 8 focus group discussions primarily conducted with fishers and fishing community leaders in four villages in southern Lake Victoria. Results: Currently the main water safety stakeholders affecting or affected by drowning-related policies and practices in southern Lake Victoria are: small-scale fishers and their families; boat owners and builders; Beach Management Units (BMUs) and other local leaders; and Tanzania Shipping Agencies Corporation (TASAC), fisheries, and marine police officers; and the East African Community (EAC), e.g. the Lake Victoria Basin Commission (LVBC). Strengths of the national policy and regulatory framework are: active collaboration between the fisheries, transport, and police sectors; the meteorology sector is becoming stronger; the fisheries and shipping sectors generally have clear policies, rules, and regulations; the health and labour sectors have broadly relevant policies; and the youth sector has a specifically relevant policy. Weaknesses and gaps are that there is no national water safety plan or strategy, as recommended by the World Health Organization (WHO 2017; WHO 2018); few national documents directly address water safety among fishers on inland waters; and there is little attention to fisher water safety education and training needs. • Strengths of the international policy and regulatory framework include the planned LVMCT Project, the ratified SADC protocols, and the detailed, voluntary guidance on fisher water safety from international bodies within which the URT is a member. • Weaknesses and gaps are that many of the international fisheries documents do not address small-scale fishers in inland waters, and very few relevant international Conventions have been ratified by the URT.

BACKGROUND: Prevalence of childhood overweight and obesity in low- and middle-income countries is on the rise. We focused on multiple factors which could influence body mass index. METHODS: A cross sectional school-based study was conducted in Moshi, Tanzania. Primary school children aged 9-11 years were recruited from 20 schools through a multistage sampling technique. Questionnaires were used to collect information on physical activity and diet by food frequency questionnaire. Height and weight measurements were taken and body mass index z scores for age and sex (BMIZ) calculated using the WHO AnthroPlus. Children were considered thin if BMIZ was 1 SD. Information on school policies and environment was obtained from headteachers. Correlates of overweight and obesity were examined using a multinomial multilevel logistic regression. RESULTS: A total of 1170 primary school children, of whom 636 (54%) were girls, were recruited from 20 schools. The prevalence of overweight and obesity was 15% overall (overweight 9% and obesity 6%) and most prevalent in urban areas (23%) and in private schools (24%). Moreover, thinness was found to be (10%) overall, most prevalent in rural areas (13%) and in government schools (14%). At school level, residing in urban (adjusted relative risk ratio [aRRR] 3.76; 95% confidence interval [CI] 2.49,5.68) and being in private school (aRRR 4.08; 95% CI 2.66,6.25) were associated with a higher risk of overweight and obesity while availability of playgrounds in schools (aRRR 0.68; 95% CI 0.47, 0.97) was associated with a lower risk of overweight and obesity. At home level, availability of sugary drinks (aRRR 1.52; 95% CI 1.01,2.28) was associated with a higher risk of overweight and obesity. CONCLUSION: Overweight and obesity are common in private schools and in urban settings. Efforts should be taken to ensure availability of playgrounds in schools and encouraging children to engage in physical activities.

Multiple outcomes reflecting different aspects of routine care are a common phenomenon in health care research. A common approach of handling such outcomes is multiple univariate analyses, an approach which does not allow for answering research questions pertaining to joint inference. In this study, we sought to study associations among nine pediatric pneumonia care outcomes spanning assessment, diagnosis and treatment domains of care, while circumventing the computational challenge posed by their clustered and high-dimensional nature and incompletely recorded covariates. We analyzed data from a cluster randomized trial conducted in 12 Kenyan hospitals. There were varying degrees of missingness in the covariates of interest, and these were multiply imputed using latent normal joint modeling. We used the pairwise joint modeling strategy to fit a correlated random effects joint model for the nine outcomes. This entailed fitting 36 bivariate generalized linear mixed models and deriving inference for the joint model using pseudo-likelihood theory. We also analyzed the nine outcomes separately before and after multiple imputation. We observed joint effects of patient-, clinician- and hospital-level factors on pneumonia care indicators before and after multiple imputation of missing covariates. In both pairwise joint modeling and separate univariate analysis methods, enhanced audit and feedback improved documentation and adherence to recommended clinical guidelines over time in six and five pneumonia care indicators, respectively. Additionally, multiple imputation improved precision of parameter estimates compared to complete case analysis. INTRODUCTION: Poor menstrual health negatively impacts psychosocial and physical health, and subsequently leads to poor school outcomes, but the effort to improve adolescent girls' menstrual health in Tanzania remains fragmented. This study aimed to develop and pilot a scalable, comprehensive menstrual, sexual and reproductive health (MSRH) intervention within Tanzanian government structures to improve MSRH practices and perceptions and the overall school climate to ensure the psychosocial well-being and optimal school participation and performance among secondary schoolgirls. METHODS AND ANALYSIS: This study will be conducted in three phases. Phase I will be a formative research to iteratively refine an existing puberty and menstrual health intervention, and to collaboratively design strategies to embed the intervention into government structures thereby promoting scalability. In Phase II, we will pilot and evaluate the refined intervention and implementation strategies using a mixed-methods design to assess (1) feasibility, acceptability and sustainability; and (2) effect on MSRH practices and perceptions and the overall school climate. In Phase III, we will synthesise the research findings in collaboration with the national, regional and district government and non-government stakeholders. ETHICS AND DISSEMINATION: This pilot study will provide evidence-based recommendations for a comprehensive, complex menstrual and puberty intervention within secondary schools in Tanzania that can be further tested for broader effectiveness across a larger population. Papers, policy briefs and both regional/international conference presentations are planned to reach academic and non-academic groups. Abstract Background It is increasingly appreciated that the interpretation of health systems research studies is greatly facilitated by detailed descriptions of study context and the process of intervention. We have undertaken an 18-month hospital-based intervention study in Kenya aiming to improve care for admitted children and newborn infants. Here we describe the baseline characteristics of the eight hospitals as environments receiving the intervention, as well as the general and local health system context and its evolution over the 18 months. Methods Hospital characteristics were assessed using previously developed tools assessing the broad structure, process, and outcome of health service provision for children and newborns. Major health system or policy developments over the period of the intervention at a national level were documented prospectively by monitoring government policy announcements, the media, and through informal contacts with policy makers. At the hospital level, a structured, open questionnaire was used in face-to-face meetings with senior hospital staff every six months to identify major local developments that might influence implementation. These data provide an essential background for those seeking to understand the generalisability of reports describing the intervention's effects, and whether the intervention plausibly resulted in these effects. Results Hospitals had only modest capacity, in terms of infrastructure, equipment, supplies, and human resources available to provide high-quality care at baseline. For example, hospitals were lacking between 30 to 56% of items considered necessary for the provision of care to the seriously ill child or newborn. An increase in spending on hospital renovations, attempts to introduce performance contracts for health workers, and post-election violence were recorded as examples of national level factors that might influence implementation success generally. Examples of factors that might influence success locally included frequent and sometimes numerous staff changes, movements of senior departmental or administrative staff, and the presence of local 'donor' partners with alternative priorities. Conclusion The effectiveness of interventions delivered at hospital level over periods realistically required to achieve change may be influenced by a wide variety of factors at national and local levels. We have demonstrated how dynamic such contexts are, and therefore the need to consider context when interpreting an .
