Valid and precise quantification of clinical variables is essential for appropriate interpretation to inform healthcare decision making. The outcomes produced from different measurement procedures and instruments, purporting to quantify the same measurand, should be directly comparable. This ensures the appropriate application and widespread adoption of clinical research findings. Metrology provides a framework for the development of a common language of reference measurement systems, which have the potential to improve the accuracy and comparability of patients' results.However, the practices, procedures and instruments used in social measurement are currently excluded from any formal metrological framework. In this paper, we build on previous arguments, and propose a new international body to bring together metrology, psychometrics, philosophy, and clinical management to support the global comparability and equivalence of measurement results in patient centred outcome measurement to improve healthcare.

AbstractThere is an increasing commitment to "Personal Recovery" as the desired outcome for mental health rehabilitation, yet there is little agreement about how to measure it. The purpose of this study was to develop a method of measuring recovery in community-dwelling people with mental health concerns. We describe a four-phase process, informed by guidelines for (patient-reported outcome measure) PROM development described by the Scientific Advisory Committee of the Medical Outcomes Trust, the Food and Drug Administration, and the International Society for Quality of Life Research, on how to quantify recovery in mental health care. The approach includes an iterative mixed methods process, guided by Classical Test Theory (CTT) and Rasch Measurement Theory (RMT), to develop the Canadian Personal Recovery Outcome Measure (C-PROM) for adults receiving mental health services. In Phases 1 and 2, 40 new items were generated by people with schizophrenia. In Phase 3, psychometric analysis and cognitive interviewing suggested that the item set be reduced to 30 items. Phase 4 (n = 575) showed good overall fit of the C-PROM items to the Rasch model (χ2 = 163, df = 130, p = .05), no item misfit, high reliability (rp = 0.92), an ordered response scale structure, high correlation between logit and scale scores (0.92) and no item bias for gender, age, or diagnosis. This study provided evidence for the C-PROM as a measure of personal recovery for people with mental health concerns. The measurement model underpinning this set of items has potential to support clinical relevance of scale scores, advancing an evidence-based approach to mental health rehabilitation practice and outcomes.

Foundry is a province-wide network of integrated health and social service centres for young people aged 12–24 in British Columbia (BC), Canada. Online resources and virtual care broaden Foundry's reach. Its online platform – foundrybc.ca – offers information and resources on topics such as mental health, sexual wellness, life skills, and other content suggested by youth and young adults. The COVID-19 pandemic has presented significant and unique challenges to the youth and their families/caregivers served by Foundry. Disruptions to school, access to essential healthcare services such as counselling, familial financial security and related consequences has left young people with heightened anxiety. The Foundry team mobilized to respond to these extenuating circumstances and support BC youth and their families/caregivers during this hard time through three goals: (1) to amplify (and translate for young people and their families/caregivers) key messages released by government to support public health responses to the COVID-19 pandemic; (2) to develop content that supports the needs of young people and their families/caregivers that existed before COVID-19 and are likely to be exacerbated as a result of this pandemic; and (3) to develop and host opportunities through social media and website articles to engage young people and their families/caregivers by creating a sense of community and promoting togetherness and social connection during the COVID-19 pandemic. Each goal and plan integrated the leadership, feedback and needs of youth and their families through engagement with Foundry's provincial youth and family advisory committees. INTRODUCTION: Bystanders to drug overdoses often avoid or delay calling 9–1-1 and cite fear of police involvement as a main reason. In 2017, the Good Samaritan Drug Overdose Act (GSDOA) was enacted by the Canadian government to provide people present at an overdose with legal protection from charges for simple drug possession, and conditions stemming from simple possession. Few studies have taken a multi-methods approach to evaluating the GSDOA. We used quantitative surveys and qualitative interviews to explore awareness, understanding, and perceptions of the GSDOA in people at risk of witnessing an overdose. METHODS: Quantitative cross-sectional surveys and qualitative telephone interviews were conducted with adults and youth at risk of witnessing an overdose across British Columbia. Cross-sectional survey participants were recruited at 19 Take Home Naloxone sites and online through Foundry. Multivariable logistic regression models were constructed hierarchically to determine factors associated with GSDOA awareness. Telephone interview participants were recruited by research assistants with lived/living experience of substance use. Deductive and inductive thematic analyses were conducted to identify major themes. RESULTS: Overall, 52.7% (n = 296) of the quantitative study sample (N = 453) reported being aware of the GSDOA. In multivariable analysis, cellphone possession (adjusted odds ratio [AOR] = 2.19; 95% confidence interval [CI] 1.36, 3.54) and having recently witnessed an opioid overdose (AOR = 2.34; 95% CI 1.45, 3.80) were positively associated with GSDOA awareness. Young adults (25 – 34 years) were more likely to be aware of the Act (AOR = 2.10; 95% CI 1.11, 3.98) compared to youth (16–24 years). Qualitative interviews (N = 42) revealed that many overestimated the protections offered by the GSDOA. To increase awareness and knowledge of the Act among youth, participants recommended adding the GSDOA to school curricula and using social media. Word of mouth was suggested to reach adults.
