Despite mounting evidence of the seriousness of climate change, the problem remains a low policy priority for most countries. Yet action is urgently needed. Emissions-trading regimes, which do too little to cap pollution, must be revised. And any new strategies must be customized to the particular needs and means of those states, developed and developing alike, that will have to implement them.

The obligation of accession countries of Central & Eastern Europe to approximate & implement European Union (EU) environmental directives has forced the environment on the agenda where it might otherwise be eclipsed by other issues. The result might be improvements over time in the media & subject areas in which the EU has spoken if small, inexperienced, & underfunded environmental ministries can manage the tremendous costs & institutional skills that these directives impose. EU accession might diminish the power of nongovernmental organizations (NGOs) in some respects & enhance it in others. The environmental agenda is likely to be set in Brussels, not in response to domestic "consumer-driven" demand in the foreseeable future. NGOs will have less flexibility to introduce environmental initiatives that are inconsistent with EU demands. But EU membership creates opportunities to police governments' progress implementing the directives. Background: Mental health can help explain how social inequalities impact on health. Many current public health challenges are shaped by social, economic and environmental conditions that take a mental toll on society. Purpose: This article describes a conceptual framework illustrating the psychosocial pathways that link the wider conditions to health behaviours and outcomes. It draws out implications of this framework for mental health practice that aim to support policy and decision-making on future action to reduce health inequalities and presents practical examples of what can be done. Methods: This article expands on a report commissioned by Public Health England. A narrative review and synthesis of relevant evidence built on existing research by the Institute of Health Equity. A conceptual framework was developed and a consultation exercise with stakeholders helped to revise and illustrate it with practice examples. Conclusions: The field of mental health has much to contribute to prevention, not just of mental illness but also of physical health conditions and reduction of inequalities in life expectancy and healthy life expectancy, especially through collaborative public health action.

The article examines the convergences and contrasts between social epidemiology, social medicine, and human rights approaches toward advancing global health and health equity. The first section describes the goals and work of the WHO Commission on Social Determinants of Health. The second section discusses the role of human rights in the Commission's work. The third section evaluates, from the perspective of social epidemiology, two rights-based approaches to advancing health and health equity as compared to a view that focuses more broadly on social justice. The concluding section identifies four areas where social epidemiologists, practitioners of social medicine, and health and human rights advocates can and must work together in order to make progress on health and health equity. This paper draws lessons from a review of primary health care services in Windhoek, the capital of Namibia, undertaken by a regional health management team. The review was carried out because of perceived increases in workload and inadequate staffing levels, arising from the rapid expansion of the city associated with inward migration. A survey of the utilization of government clinics was used to develop a more equitable allocation of primary health care services between localities. (DSE/DÜI)
