Examining the relations between the rise of scientific psychiatry and the emergence of mental health nursing in Dutch asylums, this study analyses the social relationships of class, gender and religion that structured asylum care in the Netherlands around 1900. Drawing on archival collections of four Dutch asylums, the book highlights the gendered nature of mental health nursing politics. Seeking to model the asylum after the forceful example of the general hospital, psychiatrists introduced new somatic treatments and designed mental nurse training which aimed at creating a nursing staff skilled in somatic care. The training system, based on the projected image of the civilized, middle-class female nurse, bringing competence and compassion to the care of the mentally ill, created new opportunities for women, while at the same time restricting the role of men in nursing. Capturing the contradictory realities of hospital-oriented asylum care, the book illustrates the social complexity of the care of the mentally ill and forms an important addition to the historiography on European psychiatry. - The Rise of Mental Health Nursing onderzoekt de tegenstrijdigheden in de op het ziekenhuis georiënteerde inrichtingszorg, die rond 1900 opkwam. Bovendien illustreert het boek de sociale complexiteit van de psychiatrische zorg. Op basis van archiefmateriaal uit vier Nederlandse psychiatrische inrichtingen onderzocht Geertje Boschma de sociale verbanden die de psychiatrische verpleging rond 1900 kenmerkten. De introductie van nieuwe somatische behandelingsmethoden door psychiaters creëerde destijds een vraag naar verplegend personeel dat geschoold was in somatische zorg. Het opleidingsmodel, dat (overwegend mannelijke) psychiaters ontwikkelden, was gebaseerd op het beeld van de beschaafde vrouwelijke verpleegster uit de middenklasse die competentie en compassie in de zorg verenigden. Mental deficiency nursing evolved as a distinct, yet little known field of nursing education and practice in several countries during the twentieth century. In Canada it became a separate area of nursing in only one of its provinces, Alberta, and it was tied to one large institution, called the Provincial Training School for the care of developmentally disabled children and, eventually, also adults. This paper contributes to the history of mental deficiency nursing, currently called intellectual disability nursing, an existing specialty actively practiced in the UK and Ireland for example. Alberta's program was established in 1933 and continued until 1973. I examine this history through the lens of three mental deficiency nurses' experiences of their education and work at the institution shared in an oral history interview. These nurses worked in the institution in the period after the Second World War. Their stories provide a unique perspective on a field of nursing work and education that grew increasingly controversial during the latter half of the 20th century when public debate arose over the institutionalization of people with developmental disability. Moreover, the institution's entanglement within an active eugenics policy and practice maintained in the province of Alberta until 1972 added to the controversy. Their stories provide insight in the perceptions and emotions of mental deficiency nurses, experienced and remembered some sixty years later in an interview. I examine how the nurses took up this work and how their professional identity was depicted and changed, especially as public debate arose from the 1970s onward. The stories provide a unique micro-historical lens to explore larger social and cultural tensions over eugenics, disability, care and dependency, and over mental deficiency nurses' work. Résumé Les soins infirmiers en déficience mentale ont évolué en tant que branche distincte, mais méconnue, de la formation et la pratique infirmière dans plusieurs pays au cours du 20e siècle. Au Canada, ils sont devenus une discipline séparée des soins infirmiers dans une seule province, en Alberta, et ils ont été liés à une grande institution nommée la Provincial Training School pour les soins aux enfants ayant des déficiences intellectuelles et, éventuellement, aux adultes. Cet article contribue à l'histoire des soins infirmiers en déficience mentale, à notre époque appelés soins infirmiers en déficience intellectuelle, une spécialité existante et activement pratiquée, par exemple, au Royaume-Uni et en Irlande. Le programme en Alberta a été établi en 1933 et s'est poursuivi jusqu'en 1973. J'étudie cette histoire à travers l'expérience de trois infirmières en déficience mentale par rapport à leur éducation et leur travail à l'institution, partagés lors d'une entrevue orale. Leurs histoires offrent un aperçu des perceptions et des émotions des infirmières en déficience mentale vécues et racontées dans une entrevue 60 ans plus tard. J'examine la façon dont les infirmières se sont retrouvées à faire ce travail et comment leur identité professionnelle a été dépeinte et modifiée, surtout lors du débat public dans les années 70 et après. Leurs histoires offrent un point de vue microhistorique unique pour explorer les tensions culturelles et sociales envers l'eugénisme, les handicaps, les soins et la dépendance, ainsi que le travail des infirmières en déficience mentale.

Cet article analyse le processus social connu sous le terme de désinstitutionnalisation, entre 1950 et 1980 en Alberta et en Colombie-Britannique, à partir de données démographiques sur les admissions et les sorties des hôpitaux psychiatriques. Une étude de la situation dans les deux provinces les plus à l'ouest du Canada permet d'analyser ces changements dans ces contextes régionaux. À la suite de nouveaux mécanismes de financement, d'un recadrage des soins communautaires et d'une critique croissante de la nature prétendument écrasante des grands établissements, chacun des trois principaux hôpitaux a réduit son envergure au cours des années 1950. Néanmoins, cette tendance n'a pas entraîné, au cours de cette période, une diminution du nombre global des patients hospitalisés. De fait, le nombre total des hospitalisations, et plus particulièrement celles de courte durée, a augmenté au moment où se produisait un phénomène de trans-institutionnalisation. Cette étude de cas reflète les principales tendances des soins apportés aux maladies psychiatriques de l'après-guerre et illustre les défis - sur un plan social, politique et culturel - auxquels s'est heurtée la reconstruction des soins en établissements.

The World Health Organization has identified the importance of improving the rates of breast/chest feeding for population health. Canadian health organizations have put public health resources toward breast/chest feeding support. Despite statements of purpose describing health promotional interventions to be focused on improving overall population health, many times these methods are based only upon biomedical knowledge and fail to adequately address the needs of diverse populations. Thus, in this paper we critique a Canadian policy providing clinical guidance to care providers through the application of a relational inquiry framework. We draw on the first author's experience as a Public Health Nurse delivering breast/chest feeding support within the scope of these guidelines to further illustrate the point. The results from published evidence are integrated within this critique to provide an evidence base for policy improvement recommendations to improve the social, cultural, and political components of breast/chest feeding typically overlooked in current standards.
