The legislation of Italy and the European Union requires a shift in terms of the organisation and national regulation of ethics committees and clinical trials. More generally, this affects the entire sphere of biomedical research. The first part of this article provides a brief review of the history of ethic committees in Italy. We then discuss certain considerations about the current situation and formulate some proposals in terms of the regulatory choices that must be made. There is a vital need for rules that promote efficiency, to guarantee that Italy's position remains competitive and attractive within the European Union.

After the national lockdown imposed on March 11, 2020, the Italian government has gradually resumed the suspended economic and social activities since May 4, while maintaining the closure of schools until September 14. We use a model of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) transmission to estimate the health impact of different exit strategies. The strategy adopted in Italy kept the reproduction number R(t) at values close to one until the end of September, with marginal regional differences. Based on the estimated postlockdown transmissibility, reopening of workplaces in selected industrial activities might have had a minor impact on the transmissibility. Reopening educational levels in May up to secondary schools might have influenced SARS-CoV-2 transmissibility only marginally; however, including high schools might have resulted in a marked increase of the disease burden. Earlier reopening would have resulted in disproportionately higher hospitalization incidence. Given community contacts in September, we project a large second wave associated with school reopening in the fall.

To counter the second COVID-19 wave in autumn 2020, the Italian government introduced a system of physical distancing measures organized in progressively restrictive tiers (coded as yellow, orange, and red) imposed on a regional basis according to real-time epidemiological risk assessments. We leverage the data from the Italian COVID-19 integrated surveillance system and publicly available mobility data to evaluate the impact of the three-tiered regional restriction system on human activities, SARS-CoV-2 transmissibility and hospitalization burden in Italy. The individuals' attendance to locations outside the residential settings was progressively reduced with tiers, but less than during the national lockdown against the first COVID-19 wave in the spring. The reproduction number R(t) decreased below the epidemic threshold in 85 out of 107 provinces after the introduction of the tier system, reaching average values of about 0.95-1.02 in the yellow tier, 0.80-0.93 in the orange tier and 0.74-0.83 in the red tier. We estimate that the reduced transmissibility resulted in averting about 36% of the hospitalizations between November 6 and November 25, 2020. These results are instrumental to inform public health efforts aimed at preventing future resurgence of cases.

We estimate that milder restrictions in regions at lower risk (yellow) resulted in a transmissibility reduction of about 18%, leading to a reproduction number Rt of about 0.99. Stricter measures (orange and red) led to reductions of 34% and 45% and Rt values of about 0.89 and 0.77 respectively.

L'igiene ospedaliera in Italia ha una lunga e solida storia che però richiede una necessaria riflessione per poter restare al passo con quelle che sono le innovazioni e gli sviluppi dei sistemi sanitari anche connessi a quelle che sono le attività dell'Unione Europea. A livello europeo infatti esistono diverse tradizioni nazionali che riguardo l'igiene ospedaliera e per questo motivo l'European Centre for Disease Prevention and Control (ECDC) ha adottato il termine "controllo delle infezioni e igiene ospedaliera" per poter così includere tutti i professionisti sanitari dei singoli Paesi. La nostra proposta è sia quella di modificare il termine di "igiene ospedaliera" in "igiene delle organizzazioni sanitarie" per poter focalizzare l'attenzione anche ai numerosi livelli di cura non ospedalieri, sia di adottare la seguente definizione: "tutte le attività volte a garantire, in modo appropriato, scientificamente valido ed efficiente, l'assistenza in un ambiente sicuro, confortevole e salubre per i pazienti, i caregiver e gli operatori sanitari". L'igiene ospedaliera ed i suoi professionisti sono un pilastro necessario per garantire la qualità e la sicurezza dell'assistenza sanitaria in Italia. ; Although hospital hygiene has a long history in Italy it is necessary to reflect about it because of the innovation in healthcare systems and because of the evolution due to European Union related activities. Different traditions exist in European countries about hospital hygiene and European Centre for Disease Prevention and Control (ECDC) adopted the term of "infection control and hospital hygiene" which includes all the engaged European healthcare professionals. We propose to modify hospital hygiene as "healthcare organisation hygiene" in order to focalise the attention to all care settings not only hospitals and to adopt the following definition: "all activities aimed to guarantee, in an appropriate, scientifically sound and efficient way, that structures and processes support healthcare practices in a safe comfortable and healthy environment both for patients, caregivers and healthcare workers". Hospital hygiene and its professionals, besides the long tradition, still remain a relevant pillar in guaranteeing quality and safety of healthcare in Italy.
