Die Schweizer Drogenpolitik ist in den letzten Jahrzehnten kontinuierlich pragmatischer geworden. Die moralische Herausforderung des Leidens in den 1980er Jahren hat dazu beigetragen, innovative Lösungen zu finden und eine wegweisende Politik zu entwickeln, die als Viersäulenpolitik bekannt geworden ist. Doch heute scheint sich die Schweizer Drogenpolitik auf ihren Lorbeeren auszuruhen. Es mangelt an Lösungen für anstehende Probleme, wie die nach wie vor fehlende Regulierung des Cannabiskonsums oder auch die zunehmende multiple Abhängigkeit von verschiedenen Substanzen.

Die soziologische Perspektive auf Innovation zeichnet sich dadurch aus, dass nicht Techniken oder Maschinen, sondern die Rezeptionsfähigkeit für Veränderungen in Gesellschaften analysiert werden. Es ist zwar durchaus möglich, dass eine neue Technologie an einem Schreibtisch oder in einem Labor erfunden wird, doch wird diese Technologie immer nur dann zur Innovation, wenn diese auch in der Gesellschaft erwünscht, gebraucht oder umgewandelt wird. So zeigt uns Max Weber, dass eine ganz ähnliche Ausgangslage hinsichtlich des technischen Wissens, des Grads der Urbanisierung sowie einer rationalen politischen Organisation sowohl in China wie auch in Europa doch nicht die gleichen Resultate erzeugt, indem in Europa, nicht aber in China der Kapitalismus daraus entsteht. Weber erklärt diesen Unterschied mit dem religiösen Kontext bzw. mit den unterschiedlichen Wertemustern, die in Europa durch den Calvinismus oder den Protestantismus und in China durch den Konfuzianismus geprägt sind. Die religiösen Wertsysteme werden somit zur entscheidenden Differenz, die das «innovative Milieu» beeinflussen. Sowohl der Calvinismus als auch der Protestantismus fördern demnach die entbehrende Haltung und dadurch die den Kapitalismus begünstigende Logik der Investition des Gewinns. Der Konfuzianismus dagegen fördert die Investition in Prestige und soziale Positionen und ist deshalb ein Hindernis für die Entwicklung des modernen Kapitalismus (Weber [1915 –1920] 1989).

This chapter is an attempt to analyse the relationship between Migration and Health from the point of view of migrants and their environment, as well as that of the health care providers embedded in a political and institutional framework.2 Three main arguments are developed. Firstly, we describe the change in post-World War II Europe from the "healthy migrant effect" to the "unhealthy migrant effect", which secondly coincided with the change from homogenous to heterogeneous societies and in particular from the acculturation perspective to the perspective of difference sensitivity in the provision of health services. Thirdly, we want to give some insights into the differentiation of institutional and political answers to these challenges on health and migration in Europe.

Zunehmende Migrationsbewegungen haben Einschluss- und Ausschlussdynamiken politisch, wirtschaftlich und sozial auf die Tagesordnung gebracht. Eigenheit und Gemeinsamkeit wird als Herausforderung an die territoriale Identität politisch kontrovers diskutiert, wirtschaftlich als Innovationsbedingung gefordert und sozial als Verunsicherung erlebt. In diesem Text1 möchte ich der Frage nachgehen, wie solche Dynamiken entstehen und vor allem wann diese für Gesellschaften zur Herausforderung werden.

The pluralization of our society goes on, regardless of the French desire to restore the republican roots through the debate concerning the Islamic veil in the schools, regardless of Germany's attempts to impose a Leitkultur, regardless of the researchers' reflections around the political issue of what "integration" really means. This pluralization, which is unquestionably not only related to the phenomenon of migration, challenges the European national societies in three different ways. Firstly, by the constant ambivalence between the national identity discourse and the supranational construction of a European State. Secondly, by the internal differentiation of the civil society, which still only recognizes itself as a unity in the (accepted) difference. Thirdly, through the nation-state differentiations of belongings, which partially get disconnected from the territory and honoured through interlaced judicial claims, as shown by the topic of undocumented immigrants' access to health care. These dynamics of differentiation and of pluralization will here be pointed as "Americanization" of Europe, which is imposing itself "from below", while awaiting regulation "from above".

Während sich Frankreich in Kopftuchdebatten in den Schulen auf seine Wurzeln rückbesinnen möchte, Deutschland an der Operationalisierung der Leitkultur herumbastelt und sich Forscherscharen an der politisch gestellten Frage abmühen, was "Integration" bedeute, schreitet die Pluralisierung unserer Gesellschaft unbekümmert weiter. Diese Pluralisierung, die nur bedingt auf Migration zurückzuführen ist, fordert die europäischen nationalen Gesellschaften dreifach heraus. Erstens durch die ständige Ambivalenz zwischen nationalem identitären Diskurs und europäischer Staatenbildung. Zweitens durch die interne Differenzierung der Zivilgesellschaft, die sich nur noch in der (anerkannten) Differenz als Einheit erkennt. Drittens durch die sozialrechtlichen Zugehörigkeitsdifferenzierungen, die sich teilweise vom Territorium entkoppeln und über verschachtelte Rechtsansprüche eingelöst werden, wie dies beispielhaft die Diskussion um den Anspruch auf Gesundheitsleistungen von illegalen Einwanderern zeigt. Im Vortrag wird diese Differenzierungs- und Pluralisierungsdynamik als "Amerikanisierung" Europas gedeutet, die sich "von unten" durchsetzt und auf Regulierung "von oben" wartet.

Al centro di questa communicazione è la tensione che esiste tra Stato e mondo associativo, con l'economia che gioca anch'essa il suo ruolo. Tenterò di mostrare le grandi tendenze di trasformazione e di cambiamento che possono essere divise in microcostellazioni nazionali, (le tendenze demografiche, economiche e politiche) e in macrocostellazioni internazionali. Nel grafico illustrativo che segue si osserva la Svizzera, con le sue dinamiche nazionali, all'interno di questo approccio di micro e macro tendenze. Mi soffermerò in particolare sulle dinamiche demografiche, politiche ed economiche e, in un secondo momento, parlerò delle dinamiche internazionali, quello che succede attorno alla Svizzera e che influenza quotidianamente la nostra vita e la vita associativa.

La dynamique sociétale se caractérise par un triple processus de différenciation : Différen- ciations des modes de vie, différenciations des horizons temporels, différenciations des ac- teurs systémiques. Ces différenciations ont ébranlé les fondements de la sécurité sociale basée sur une logique assurancielle ou de couverture homogène, sans pour autant la réfor- mer dans une perspective cohérente. Les conséquences visibles de cette crise des grandes œuvres assurancielles et de redistribution sont une société des inégalités croissantes (Castells 1998), de perte de sécurité (Beck 1986) et d'augmentation de situations de précarité allant jusqu'à l'existence de «surnuméraires» (comme les appelle Castel 1995) se trouvant au-de- hors de ce système du bien-être qui s'est construit dans l'après-guerre pour maintenir une classe moyenne stable.
