This book is about innovation, big data, and data science seen from a business perspective. Big data is a buzzword nowadays, and there is a growing necessity within practitioners to understand better the phenomenon, starting from a clear stated definition. This book aims to be a starting reading for executives who want (and need) to keep the pace with the technological breakthrough introduced by new analytical techniques and piles of data. Common myths about big data will be explained, and a series of different strategic approaches will be provided. By browsing the book, it will be possible to learn how to implement a big data strategy and how to use a maturity framework to monitor the progress of the data science team, as well as how to move forward from one stage to the next. Crucial challenges related to big data will be discussed, where some of them are more general - such as ethics, privacy, and ownership - while others concern more specific business situations (e.g., initial public offering, growth strategies, etc.). The important matter of selecting the right skills and people for an effective team will be extensively explained, and practical ways to recognize them and understanding their personalities will be provided. Finally, few relevant technological future trends will be acknowledged (i.e., IoT, Artificial intelligence, blockchain, etc.), especially for their close relation with the increasing amount of data and our ability to analyse them faster and more effectively.

Background and introduction: Resident medical training following medical school is a period of great importance in the instruction and education of young physicians, but also the first step into the labour market for doctors. Unfortunately, the long educational curricula as well as the low economic remuneration render medical training attractive only in some European countries: often low salaries accompany endless weekly working hours with a wide range of differences among the European countries. The aim of this study was to analyse the different economic conditions for resident trainees by reporting the different salaries and the weekly burden of working hours, and also comparing the different costs of living in eight European countries and in Israel. Materials and methods: A questionnaire was sent to resident medical doctors working in large university hospitals in eight European countries (Denmark, France, Germany, Greece, Italy, Spain, Switzerland and the UK) and Israel, and data on the monthly salary, number of weekly working hours and general satisfaction were collected. Purchase power parity (PPP) in US dollar (USD) adjustment was calculated according to the latest Organisation for Economic Co-operation and Development (OECD) tables. Results: Among the different countries, working hours per week ranged from 37 to 56. The net pay report had a median value of 2,000€ per month. The net monthly wage ranged between 1,000 and 3,000€. Power purchase parity in USD-corrected salaries varied from 1,388.80 (Greece) to 5,788.30 (UK), mean 2,562.30. Conclusions: Taking into account PPP-adjusted wages, France, Greece and Italy are below the median continental values. The trend of migration of medical trainees to countries where the economic situation is more favourable seems reasonable. Because of both the high salary and the language, the UK represents the most attractive training destination.

The objectives of the survey were to explore any changes in the adherence to the three main COVID-19 preventive measures (social distancing, washing hands, wearing face-masks) among 795 individuals who received the COVID-19 vaccine booster dose in Italy and to identify the predictors associated. The concern of contracting COVID-19 before the vaccination, after the primary COVID-19 vaccine series, and after the booster dose resulted with a mean value of 7.7, 4.6, and 4.2, respectively. Females, those who had a lower mean self-perceived health status, who perceived COVID-19 as a very serious health problem, who decided to receive the booster dose because they perceived to be at risk of getting COVID-19, and who expressed interest in acquiring more information regarding the COVID-19 vaccine booster dose were more likely to report a higher concern of contracting COVID-19 after the booster dose. Over two-thirds were willing to respect the three main COVID-19 preventive measures following the booster dose. The multinomial logistic regression analysis showed that those who had used all three main preventive measures before and after the second or single dose were less likely to perceive COVID-19 as a serious illness. Those who had used them irregularly were less likely to have used official government organizations and scientific journals as sources of information. This survey provided an understanding regarding the adherence to COVID-19 preventive measures that may help to target policy interventions needed to reduce the spread of SARS-CoV-2.

PURPOSE: To analyse structural and non-structural modifications of acute stroke care pathways undertaken at healthcare institutions across the regions of Italy due to the coronavirus disease 2019 (COVID-19) pandemic. METHODS: Research on National decrees specific for the pandemic was carried out. The stroke pathways of four Italian regions from North to South, such as Lombardy, Veneto, Lazio and Campania, were analysed before and after the pandemic outbreak. FINDINGS: On 29 February 2020, the Italian Minister of Health issued national guidelines on how to address the COVID-19 emergency. Stroke management was affected and required changes, basically resulting in the need to prioritise the ongoing COVID-19 emergency. In the most affected regions, the closure of departments and hospitals led to a complete reorganisation of previously functioning stroke networks. With the closure of several Stroke Units and Stroke Centres, the transportation time to hospital lengthened significantly, especially for the outlying populations. DISCUSSION: The COVID-19 pandemic outbreak has been spreading rapidly in Italy and placing an overwhelming burden on healthcare systems. In response to this, political and healthcare decision-makers worked together to develop and implement efforts to sustain the national healthcare system while fighting the pandemic. Stroke care pathways changed during the pandemic and different organisational models were applied in the most affected regions. CONCLUSIONS: Stroke treatment pathways will need to be redesigned so to guarantee that severe and acute disease patients do not lose their rights to the access and delivery of care during the COVID-19 pandemics.

Background and Purpose—Systemic thrombolysis represents the main proven therapy for acute ischemic stroke, but safe treatment is reported only in well-established stroke units. To extend the use of tissue plasminogen activator (tPA) treatment in primary care hospitals on isolated areas through telemedic was the purpose of specific initiatives in southern Umbria, Italy. Methods—The stroke center of Foligno established a telestroke network to provide consultations for three local hospitals in southern Umbria. The telemedic system consists of a digital network that includes a two-way video conference system and imaging sharing. The main network hospital established specialized stroke wards/teams in which qualified teams treat acute stroke patients. Physicians in these hospitals are able to contact the stroke centers 24 h per day. Quality data are available to support the safe implementation of the stroke procedures. Those available from governmental authorities and local datasets are volume of hospitalization, in-hospital mortality, 30-days mortality, and discharge setting. Objective of the study was to assess the annual hospitalization volume in both the hub and spoke hospitals for ischemic stroke and appraise the performance of the network after the introduction of the telestroke system. Results—A total of 225 systemic thrombolyses were performed in time period indicated above all hospitals. In the main spoke hospital, 41 procedures were performed after teleconsultations were made available. The thrombolysis rate in the hub hospital ranged between 10% in 2016 and 20% in 2019, while in the spoke hospital was below 5% in 2016 and raised to 15% in 2019. The statistically significant difference, in the number of procedures, between hub and spoke in the beginning of the observation time disappeared after introduction of the telestroke network.
