"Mental disorders clearly are among the major challenges of global health causing many concerns for health systems, health experts, and health policy makers. The present study was conducted to evaluate the mental health program in comprehensive health centers and health houses under the coverage of Ilam University of Medical Sciences in the West of Iran. The present study was conducted based on the reports obtained from the mental health program in 2019. The data were recorded into registration and evaluation forms of mental disorders with the codes 102-19-03 and 102-19-01 and then entered into the Excel software. The prevalence per 100 and 100,000 people, as well as their 95% confidence intervals were calculated using Formula 1 ROUND (Number of cases*100000/population) and Formula 2 ROUND (Prevalence ±1.96*SQRT (Prevalence *(1- Prevalence)/population). The results showed that the prevalence of depression was 19,100 [95% CI: 18999-19201] 19100), and the prevalence of anxiety was 304,66 [95% CI: 20348-30584] per 100,000 population. The highest prevalence among mental disorders was related to depression and anxiety. Considering the high average score of mental health status in people over 65 years old, it is necessary to implement measures and interventions to improve these individuals' mental condition and upgrade their mental health."

"In this descriptive-analytical study, 110 nurses working in educational hospitals of the Ilam University of Medical Sciences were included. Participants were selected using a random sampling method. The data collection tools were the demographic information form, the moral intelligence questionnaire of Lenik and Kiel (2005), and the standard clinical competency assessment questionnaire (CIRN). Data were analyzed using SPSS software, version 21. Descriptive and inferential statistical tests were used to analyze the data. The mean ± SD score of nurses' moral intelligence and clinical competency was estimated at 157.59 ± 26.60 and 136.65 ± 46.08, respectively. Fifty-one percent of participants had low clinical competency, and 46% had high clinical competency. There was a significant association between moral intelligence and clinical competency (p<0.05). "

"Improving the mental health of nurses and investigating the factors affecting it leads to the improvement of job performance and the quality of nursing care. Aim: The present study aimed to determine the relationship between mental health, life satisfaction, and job satisfaction of nurses in Ilam city in 2022. The present correlational study was carried out on 160 nurses working in educational hospitals of Ilam city and was selected by stratified random sampling in 2022. A four-part questionnaire was used: the first part included a question on demographic information. Minnesota Satisfaction Questionnaire (MSQ) (1977), Diener et al.'s Satisfaction with Life Scale (SWLS) (1985), and Derogatis et al.'s Symptom Checklist-90 (SCL-9) (1973) were included in the second, third and the fourth parts, respectively. Resultsshowed no significant relationship between the total score of mental health and the total score of job satisfaction. There was a significant relationship between the total job satisfaction score and life satisfaction score (r=0.267, p=0.001). There was also an inverse and significant relationship between the total mental health score and the life satisfaction score (r=0.294, p=0.001). The collected data was analyzed using descriptive tests (frequency, mean percentage, and standard deviation) and non-parametric Spearman and Kruskal-Walli's correlation tests in SPSS ver. 22. Pvalue<0.05 was considered as the significance level in all tests. Job satisfaction is effective in creating motivation, efficiency, responsibility, and social accountability in the personnel of any organization. Job dissatisfaction can lead to complications such as job burnout, illness, absenteeism, the job left, leaving many nursing jobs, and the reluctance of the young workforce to attend the wards. Therefore, it is necessary to make further efforts in this field by healthcare policymakers. "

"Clinical competence means a person's ability to use techniques on time, to communicate effectively with others, and to benefit from the knowledge and clinical reasoning ability when responding to different clinical situations in such a way that a person can use these skills at all times. The aim of the study is the relationship between clinical competence and ethical reasoning and the factors affecting it in nurses working in therapeutic educational centers of Ilam. Methods: In this descriptive-analytical study, 110 nurses working in three therapeutic educational centers in Ilam city were randomly selected. Demographic information forms, standard nursing clinical competence questionnaire (NCCQ), and nursing dilemma test (NDT) were used to collect data. The results also showed a positive and significant relationship between age, level of education, years of work experience, employment status, and occupational role with clinical competence and ethical reasoning (P<0.05). On the other hand, there was an inverse and significant relationship between the workplace and clinical competence and ethical reasoning (P<0.05). Data analysis was carried out using Descriptive statistics (frequency, percentage, mean and standard deviation) and analytical tests (Pearson correlation tests and analysis of variance) in SPSS. P-value< 0.05 was considered as the significance level. Using the results of the present study, nursing education planners and managers can periodically organize workshops on nursing ethics and nursing problems in hospitals and universities. Moreover, clinical competence and ethical reasoning should be taken into account when training nursing personnel in university classrooms and in-service courses "

Background: Pain is a common problem afflicting humanity. Back pain is a musculoskeletal disorder with a high prevalence; this pain may occur to anyone at least once during their lifetime. Aim: the present study was performed to investigate the effect of the use of methylprednisolone pulse on the pain status of patients suffering from back pain in Ilam city. Methods: This study is a clinical trial conducted as double-blind in 2021 in Ilam city with pretest and post-test groups plus a control group. The patients referred with vertebral disc herniation diagnosis suffering from diffuse pain in the right leg were assigned into test and control groups. A methylprednisolone 500 mg IV would be administered to the patient as pulse inside DW 500cc serum. The extent of pain was compared before and after the intervention at one, two, three, and seven days later. Placebo was applied for the control group patients. The data were analyzed by SPSS 16 as well as based on descriptive and analytical tests. Results: According to the findings, no statistically significant difference was observed between the demographic characteristics of patients receiving methylprednisolone and patients in the control group (p 0.05). Also, the result showed the comparison of pain scores of patients in the two groups. According to the findings, the M (SD) pain score in the experimental group was 6.59 (0.88) before the intervention and 1.76 (0.69) in the week after the intervention, which was statistically significant (P=0.000). Also, the changes in the control group's score before intervention were 6.82 (0.71) and after the intervention was 6.77 (1.12) (P>0.05). Conclusions: Considering the effect of methylprednisolone on mitigating the pain of patients suffering from back pain, it is suggested to use injective methylprednisolone for reducing the pain of patients suffering from back pain

Background: One of the chronic diseases that causes various challenges for patients is spinal cord injury. On the other hand, apart from the problems that patients and caregivers of patients with SCI experience, the spread and prevalence of Covid-19 disease has been effective as an effective risk factor for abuse. Objectives: the aim of the present study is to determine the abuse prevalence and its effective risk factors among SCI patients in Ilam City during the Covid-19 outbreak. Methods: This cross-sectional descriptive study, population included all SCI patients in Ilam, included in the present study. Moreover, researcher-made forms and questionnaires whose validity and reliability were confirmed, including the demographic characteristics form, perceived abuse researcher-made questionnaire for SCI patients, were used. The perceived abuse questionnaire for SCI patients was a researcher-made questionnaire that was designed using library studies, interviewing patients and determining abuse instances. This instrument consists of 20 questions that are answered using Yes (score 1) and No (score 0). After data collection, statistical analyzes were performed. Data analyzed with spss16 software. Results: According to the results, the mean ± SD of the perceived abuse score by caregivers and patients was 8.48 ± 2.31 and 42.45% during the Covid-19 pandemic, respectively. Also, the mean ± SD of patients' age was 62.86 ±19.15 and the patient perceived abuse increased with increasing age (P = 0.000, F = 27.42). Also, the possible abuse score was 0 to 20 that was divided into three categories: low (20, 20.4%), moderate (76, 76.5%) and high (2, 2%). also, significantly higher perceived abuse score among women and patients with a history of more than 10 years of SCI, but there was no significant relationship between abuse prevalence with income and marital status (P>0.05). With regard to age and abuse, results also showed an increase in perceived abuse score with increasing age. Conclusions: necessary to take the necessary measures and preventions in this regard. Also, the prevalence of Covide-19-related abuse in patients was hight, which is statistically important and significant. For this reason, while proposing preventive action in this field

"Background and Objectives: Sulfur mustard is a functional alkylating chemical warfare agent that gives rise to appalling lung injury. In people with pulmonary diseases, including asthma, the complication of respirational dynamics is reduced. However, the complexity of breathing patterns in lung injury caused by chemical agents is not clear. In the current study, the outcome of 2-chloroethyl ethyl sulfide (CEES), and mustard analogue, upon breathing pattern of rats without or with treatment were reviewed. Methods: The interbeat interval (IBI) and respiratory volume (RV) data have been acquired from spontaneous respiration rats with lung injury by CEES using a whole-body plethysmograph. We calculated mean and coefficient of variation, alpha exponent derived from detrended fluctuation analysis (DFA), and sample entropy of IBI and RV. Finding: Entropy examination of respiratory variation displayed reduced inconsistency (less complication) in the breathing pattern of this rat model of lung injury. The mustard analogue also led to increased lung inflammation in damaged rats. However, treatment by NAC and dexamethasone had a compelling impact on the complication of the breathing rhythm and lung inflammation of rats with lung injury. Conclusion: Our findings show that inflammation could be the possible origin of respiratory dynamics shifting apart from the normal variation in CEES-induced lung injury"

Introduction: Complication after inguinal hernia repair is one of the challenging problems after inguinal hernia repair. This study aimed to compare the complications of three common surgical methods in inguinal hernia repair, Bassini, Lichtenstein, and Preperitoneal methods (with anterior approach). Methods: In this prospective study, patients who underwent inguinal hernia repair surgery in Baqiyatallah Hospital from June 2019 to May 2020 were included. Patients were allocated in three groups: Bassini, Lichtenstein, and Preperitoneal (with anterior approach), based on surgeons' opinions. Postoperative pain, hematoma and seroma, wound infection, recurrence, and also we assed postoperative satisfaction after surgery. Results: The mean age of patients was 49.90 ± 16.44 years, and 92.5% of patients were male. Of the total patients, 6.8% had an infection and there was no significant difference in the distribution of infection in the groups (P=0.230). The mean of pain in all patients was 2.37 ± 1.10. There was no significant difference between the three groups in the mean of pain (P = 0.253). There was no significant difference in the distribution of recurrence in the groups (P=0.356). There was no significant difference in the distribution of testicular swelling in the groups (P=0.421). No postoperative hematoma and seroma were reported in the groups. The mean of satisfaction in all patients was 93.61±19.63. There was no significant difference between the three groups in the mean of satisfaction (P = 0.156). Conclusion: The results showed that infection, pain, and recurrence rate in the Preperitoneal group were lower than the other groups, although this difference was not significant. Also, testicular swelling, hematoma, and seroma as well as patients' satisfaction were similar between groups.

Abstract: Purpose: This study is carried out to evaluate the diagnostic value of using common features of computed tomography (CT) imaging in COVID-19 disease, and to assess the relationship between blood oxygen saturation level and severity of CT findings. Materials and Methods: In this retrospective study, the chest CT of 173 test-confirmed COVID-19 patients have been evaluated to determine the patterns of involvement in multiple phases of illness. Then, the correlation between the severity of lung involvement and oxygen saturation levels has been assessed. Results: The chest CT results show that 87.6% of patients had GGO, which was the most common pattern in our findings. 83.8% of patients had bilateral lung involvement with the dominant multifocal and peripheral distribution. peribronchovascular involvement was also a common finding in our study (47.2 %). we found predominantly peribronchovascular view in 3 patients (1.7%), pleural effusion in 4 patients (2.3%), lymphadenopathy in 10 patients (5.8%), the tree in the bud in one patient (0.6%), and nodules in 4 patients (2.3%). We also found that GGO is the most common pattern during the early phase of the disease (97.4% of early phase cases). However, in the intermediate and late phases, consolidation and crazy paving patterns are more common. Moreover, our findings indicate that there is a significant relationship between oxygen saturation level and Total Severity Score, with the exclusion of the young adult patients (20-40 years). Conclusion: Relying on chest CT-scan findings apart from the oxygen saturation level is sufficient for the diagnosis and management of patients with COVID-19 pneumonia. Introduction: Osteoarthritis is considered as the most common joint disease. Sulfur mustard (SM) is a toxic compound, which was applied as a chemical warfare agent by Iraq against Iran veterans during 1980-1988. Chemical veterans suffer from chronic implications caused by sulfur mustard that not only adversely affect their quality of life, but also restrict their working abilities and their participation in social activities. This study aims to consider the prevalence of osteoarthritis disorders among Iranian SM-exposed veterans. Methods: In this cross-sectional study, 248 Iranian SM exposed patients were evaluated from 2013 to 2014. Veterans filled a special questionnaire for COPCORD studies and then general and specific rheumatology examinations were applied under the supervision of internists and rheumatologists. The obtained data from 245 filled questionnaires were analyzed by SPSS software and the results were reported as a percentage or mean ±SD. Results: Bone and joint pain (72%), knee joint pain (52.5%), Joint stiffness (49.2%), and shoulder joint pain (45.8%) were the most common type of complications in these patients. Osteoarthritis in at least one joint was the most common type of osteoarthritis in veterans. 69.5% of veterans had osteoarthritis in at least one joint. Knee osteoarthritis was the secondmost common type of osteoarthritis, which has been found in 58.4% of all veterans. The prevalence of neck osteoarthritis in all veterans was 23.9%. Conclusion: The total prevalence of musculoskeletal disorders among Iranian veterans was significantly higher than the average of society.

" Background: Mental disorders, especially in children, can be manifested in different ways, which is of great importance to them, so the purpose of this evaluation was to evaluate the changes in psychiatric symptoms in children with ADHD. Method: This evaluation was performed as a descriptive-analytic study on 141 children with ADHD who were followed up for 6 months. At the beginning of the study and the third and sixth months after, patients' symptoms were assessed with the CBCL Behavior Checklist and the ADHD rating scale questionnaire, as well as the course of illness and association with other disorders. Finally, the data were entered into SPSS statistical software and analyzed. Results: The mean age of the children was 5.89±0.81 years and the prevalence of males was 68.8%. Among the two treatment groups, only the mean score below the ADHD-RS questionnaire score in the non-Ritalin group at the end of the third and sixth months was significantly lower than the Ritalin group. During the study in both treatment groups, the overall ADHD-RS questionnaire and both subscales of attention deficit hyperactivity disorder (ADHD) had a significant decrease from baseline to the end of the sixth month. In relation to the CBCL behavioral checklist, the mean score of the total checklist except for anxiety/depression in the non-Ritalin group and the somatic complaints subscale in the Ritalin group decreased significantly from baseline to the end of the sixth month. Conclusion: Based on the results observed in this evaluation, Ritalin had a significant effect on the symptoms of patients with ADHD, although these symptoms improved in both groups, however, further studies are needed"

"Introduction: Following the crisis of coronavirus outbreak in numerous regions of the world, this epidemic enhanced promptly in most countries, including Iran, and sent a host of patients in need of medical services to hospitals. This study aimed to investigate the challenges of operational management of a reference military hospital during the pandemic COVID-19. Methods: This research is a qualitative study with the content analysis method with a deductive and guided approach. Data collection was conducted through a semi-structured interview method and utilizing a questionnaire designed based on the management functions of the Hospital Incident Command System (HICS). The targeted sampling method and participants in this study included senior-middle managers working in organizational positions of the reference hospital in 2020 employed during the organizational position in Covid-19 Pandemic. Classifications and coding were performed employing MAXQDA 2018 software. Results: The findings of this study were explored in the form of strengths, weaknesses, and challenges. Weaknesses included taking surprise, early and non-missionary entry into the COVID-19 crisis, lack of appropriate depot, instant management, deficiencies in the training of patients and staff, poor functioning of the crisis management system, deficiencies in monitoring, and poor provision of psychological support. The strengths were the devoted and altruistic presence of the medical staff and managers, the financial support of the headquarters, the effective field research of the management team of the organization, and the proper management of the volunteer forces. The most significant challenge was the poor coordination with the relevant organizations, including the headquarters, the Defense Organization, and the Ministry of Health. Conclusions: The findings of this study demonstrate that in the peak period of the COVID-19 pandemic to promote the operational management of the hospital, sticking to the general policies of the organization, structured coordination with internal and external organizations, organized management of equipment and human resources, generating a robust documentation structure and decision-making based on the evidence, effective and practical use of the crisis management system, systematic training of staff and patients, prioritizing the safety of patients and staff, organized dissemination of information and systematic monitoring as well as evaluation are the most significant points to deal with the epidemic crisis. "

Recently, respiratory viral infections have witnessed substantial diagnostic evolution, with the advent of emergent pathogenic agents and the improvement of new diagnostic approaches. Acute viral respiratory infection (ARVI) commonly causes illness and fatality, particularly in pediatric patients. Adenovirus has the highest prevalence in upper and lower respiratory tract infections (RTIs) amongst respiratory pathogens of importance. The purpose of this research was to introduce two effective techniques for the detection of adenovirus respiratory infection in Human samples, cell culture, and real-time PCR. Samples from patients aged less than ten years were obtained from Baqiyatallah (as) Hospital and cultured on HEK cells after preparation. After observing the cytopathic effects of the virus, a molecular test (Real-time PCR) with the help of specific primers was done on samples taken from the cell culture. Both assays detected viruses in clinical samples, and results of cell culture and real-time PCR confirmed infection with adenovirus in samples. According to our findings, simultaneous use of tests (cell culture and PCR) based test real-time PCR is a somewhat reliable method for detecting adenovirus in clinical specimens.

Background and Aim: The prevalence of high-risk behaviors is one of the most serious issues threatening the health of young people. The first step in preventing youth risk-taking is to recognize the problem and its dimensions. The purpose of this study was to investigate high-risk behaviors among students at Tehran's universities. Methods: This cross-sectional descriptive-analytical study was conducted from March to August 2019. The sample set was 4,000 students. Data was collected by multi-stage sampling at eight universities in Tehran. Demographic information and standard youth-risk questionnaires were used. SPSS 19 software was used to perform an independent t-test, Pearson ANOVA, and linear regression at a significance level of 0.05 on the questionnaire data collected. Results: The mean of the total tendency to high-risk behavior was 130.34±40.37. Significant risks identified were driving (27.3±8.33), violence (18.86±6.4), smoking (18.86±7.68), drugs (22.47±8.83), alcohol (24.10±8.57), and high-risk sexual behavior (18.47±7.49). High-risk behaviors were more common in men, single people, and people living in dormitories (P<0.05). Age and gender were the best-correlated predictors of students' high-risk behaviors. Conclusions: Results show that the tendency to high-risk behaviors in students is a worrying situation. as, at younger ages, the tendency to engage in high-risk behaviors is greater. Among young men and in dormitory life, students are more prone to high-risk behaviors. So, more monitoring in dormitories, especially male dormitories, and better planning and education to reduce high-risk behaviors among students, is necessary
