Background: To achieve effective antiretroviral therapy (ART) outcomes, adherence to an antiretroviral regimen and a good immunometabolic response are essential. Food insecurity can act as a real barrier to adherence to both of these factors. Many people living with human immunodeficiency virus (PLHIV) treated with ART in the Democratic Republic of the Congo (DRC) are faced with nutritional challenges. A significant proportion are affected by under nutrition, which frequently leads to therapeutic failure. Some HIV care facilities recommend supplementation with Moringa oleifera (M.O.) Lam. leaf powder to combat marginal and major nutritional deficiencies. This study aims to assess the impact of M.O. Lam. leaf powder supplementation compared to nutritional counseling on the nutritional and immune status of PLHIV treated with ART. Methods: A single-blind randomized control trial was carried out from May to September 2013 at an outpatient clinic for HIV-infected patients in Kinshasa (DRC). Sixty adult patients who were at stable HIV/AIDS clinical staging 2, 3 or 4 according to the World Health Organization (WHO), and were undergoing ART were recruited. After random allocation, 30 patients in the Moringa intervention group (MG) received the M.O. Lam. leaf powder daily over 6 months, and 30 in the control group (CG) received nutritional counseling over the same period. Changes in the body mass index (BMI) were measured monthly and biological parameters were measured upon admission and at the end of the study for the patients in both groups. Results: The two study groups were similar in terms of long-term nutritional exposure, sociodemographic, socioeconomic, clinical, and biological features. At 6 months follow-up, patients in the MG exhibited a significantly greater increase in BMI and albumin levels than those in the CG. For ethnobotanists, aspects such as the influence of a plant's accessibility on its frequency of use, consensus among traditional therapists over the relationships between plants and diseases and the degree of fidelity to a plant for a given category of ailments are essential to assess a medicinal tradition, but difficult to interpret. We explored these aspects through a study based on semi-structured interviews with 88 traditional therapists from the Batwa, Havu, Shi and Tembo communities near the montane forests of the Kahuzi-Biega National Park, in South Kivu Province in eastern DRC. Our survey showed that 77 plant species are used to treat pathologies grouped into 18 categories of ailments, the most frequent being digestive disorders and infections. Leaves and bark are the parts most frequently used, usually in recipes involving a single plant and mainly prepared as aqueous solutions. Although there is a positive correlation between the accessibility of plants and the frequency of their use (H = 17.64; p < 0.001), some less accessible forest plants have very high use frequencies. Consensus between traditional therapists is high overall, especially for musculo-skeletal disorders (= 0.83) and infections (= 0.80). Although the degree of fidelity to a plant for a given category of ailments is low overall, it is always high for at least one plant in all categories. Our results, although exploratory, suggest that certain medicinal traditions are well rooted in the region's local communities. This assumes a good local knowledge of medicinal plants, intergenerational transmission of that knowledge and a degree of cooperation between traditional therapists. Description of the subject. Several ethnobotanical studies have demonstrated links between traditional medicine practices and the ethnicity and geographical location of healers, while many others have concluded the opposite. This study deals with the typology of traditional healers in the mountain region of Kahuzi-Biega. Objectives. The goal is to understand whether the typology of traditional healers is related to their inter-ethnic and inter-zonal differences, based on diseases treated and plants used. Method. Ethnobotanical surveys were conducted using the "PSSVV" method. This involved 88 traditional healers recognized as "specialists" in 33 villages adjacent to the forest of Kahuzi-Biega, in DR Congo. Multivariate analysis (clustering, ordination, Mantel test, IndVal) were applied to establish typologies of traditional healers. Results. Multivariate analyses showed that ethnicity and geographical location did not explain the practices and knowledge of healers. However, by using the IndVal method, differences were observed in their degree of specialization. Non-specialized healers (70%) could be distinguished from specialized healers (30%). Two clear groups of specialists emerged; those who treat bone trauma and those who treat obstetric-gynecological complaints. The Mantel correlation test revealed a positive association (r = 0.134, p < 0.05) between the "healers-plants" and "healers-diseases" matrices. This indicates that healers who treat similar diseases use similar herbs. Both typologies have shown their preferences for forest species (81%), especially trees (51%). Conclusions. This exploratory study suggests that traditional healers are characterized based on their specializations. Ethnopharmacological relevance: Studies on traditional Chinese medicine (TCM), like those of other systems of traditional medicine (TM), are very variable in their quality, content and focus, resulting in issues around their acceptability to the global scientific community. In an attempt to address these issues, an European Union funded FP7 consortium, composed of both Chinese and European scientists and named "Good practice in traditional Chinese medicine" (GP-TCM), has devised a series of guidelines and technical notes to facilitate good practice in collecting, assessing and publishing TCM literature as well as highlighting the scope of information that should be in future publications on TMs. This paper summarises these guidelines, together with what has been learned through GP-TCM collaborations, focusing on some common problems and proposing solutions. The recommendations also provide a template for the evaluation of other types of traditional medicine such as Ayurveda, Kampo and Unani. Materials and methods: GP-TCM provided a means by which experts in different areas relating to TCM were able to collaborate in forming a literature review good practice panel which operated through e-mail exchanges, teleconferences and focused discussions at annual meetings. The panel involved coordinators and representatives of each GP-TCM work package (WP) with the latter managing the testing and refining of such guidelines within the context of their respective WPs and providing feedback. Results: A Good Practice Handbook for Scientific Publications on TCM was drafted during the three years of the consortium, showing the value of such networks. A "deliverable - central questions - labour division" model had been established to guide the literature evaluation studies of each WP. The model investigated various scoring systems and their ability to provide consistent and reliable semi-quantitative assessments of the literature, notably in respect of the botanical ingredients involved and the scientific quality of the work ...

Abstract Traditional Chinese medicine (TCM) is an integral part of mainstream medicine in China. Due to its worldwide use, potential impact on healthcare and opportunities for new drug development, TCM is also of great international interest. Recently, a new era for modernisation of TCM was launched with the successful completion of the Good Practice in Traditional Chinese Medicine Research in the Post-genomic Era (GP-TCM) project, the European Union's Seventh Framework Programme (FP7) coordination action on TCM research. This 3.5-year project that involved inputs from over 200 scientists resulted in the production of 20 editorials and in-depth reviews on different aspects of TCM that were published in a special issue of Journal of Ethnopharmacology (2012; volume 140, issue 3). In this narrative review, we aim to summarise the findings of the FP7 GP-TCM project and highlight the relevance of TCM to modern medicine within a historical and international context. Advances in TCM research since the 1950s can be characterised into three phases: Phase I (1950s-1970s) was fundamental for developing TCM higher education, research and hospital networks in China; Phase II (1980s-2000s) was critical for developing legal, economic and scientific foundations and international networks for TCM; and Phase III (2011 onwards) is concentrating on consolidating the scientific basis and clinical practice of TCM through interdisciplinary, interregional and intersectoral collaborations. Taking into account the quality and safety requirements newly imposed by a globalised market, we especially highlight the scientific evidence behind TCM, update the most important milestones and pitfalls, and propose integrity , integration and innovation as key principles for further modernisation of TCM. These principles will serve as foundations for further research and development of TCM, and for its future integration into tomorrow's medicine.

Conclusion Good quality, peer-reviewed literature is crucial in maintaining the integrity and the reputation of the herbal scientific community and promoting good research in TCM. These guidelines provide a clear starting point for this important endeavour. They also provide a platform for adaptation, as appropriate, to other systems of traditional medicine.
